1.Association of outdoor activity time and sleep duration with screening myopia in primary school students

Objective:
To explore the association of outdoor activity time and sleep duration with screening myopia in primary school students, so as to provide strategies for myopia prevention.
Methods:
Through a convenience sampling method, a survey was conducted among 4 248 primary school students aged 7-13 years from three primary schools in Xihu District, Nanchang City, Jiangxi Province from May to July, 2023. The average daily outdoor activity time and sleep duration on both weekdays and weekends were investigated in primary school students by using a self designed questionnaire. Uncorrected visual acuity tests and non cycloplegic autorefraction were measured by professional optometrists. Inter group comparisons were conducted using the Chi square test. Logistic regression model was used to analyze the association of outdoor activity time and sleep duration with screening myopia.
Results:
The detection rate of screening myopia in primary school students was 33.6%, with the rate in boys (32.0%) lower than that in girls (35.3%), and the difference was statistically significant ( χ 2=5.11, P =0.02). The analysis results of Logistic regression showed that after adjusting for factors such as gender, grade and parental education level, both average daily outdoor activity time <2 h on both weekdays and weekends ( OR =1.27, 95% CI =1.11-1.46) and sleep duration <10 h ( OR =1.17, 95% CI =1.01- 1.35 ), as well as their combined effect ( OR =1.57, 95% CI =1.25-1.98), were associated with an increased risk of screening myopia in primary school students(all P <0.05). Subgroup analysis results indicated that compared to boys ( OR =1.46, 95% CI = 1.07 -1.99), girls( OR =1.73, 95% CI =1.22-2.44) with insufficient outdoor activity time and sleep duration had a higher risk of screening myopia(both P <0.05).
Conclusions
There is a negative correlation of outdoor activity time and sleep duration with screening myopia in primary school students. Outdoor activity time and extending sleep duration should be increased to reduce the risk of myopia in primary school students.
2.Association between exposure to entertainment screen content on mobile phones and symptoms of anxiety-depression co-morbidity among college students

Objective:
To determine the association between exposure to entertainment screen content on mobile phones and symptoms of anxiety-depression co-morbidity among college students,so as to provide evidence for mental health interventions.
Methods:
A baseline survey was conducted from April to May 2019. A total of 1 135 college students were selected from one university each in Shangrao City, Jiangxi Province and Hefei City, Anhui Province using cluster random sampling method. A follow up study was conducted in November 2019, resulting in 1 110 matched valid responses. Self rating questionnaires were used to assess the exposure of entertainment screen content. The Depression Anxiety Stress Scale-21(DASS-21) and the Patient Health Questionnaire-9 (PHQ-9) were used to evaluate the anxiety symptoms, depressive symptoms, and symptoms of anxiety-depression co-morbidity among college students. A multivariate binary Logistic regression model was constructed following initial intergroup comparisons with Chi-square test to determine the association between baseline exposure to mobile entertainment screen content and the risk of symptoms of anxiety depression co-morbidity at baseline and the 6 month follow up.
Results:
The prevalence rates of symptoms of anxiety-depression co-morbidity among college students were 25.4% and 20.6% at baseline and follow up, respectively.After adjusting for confounding factors such as gender, self rated family economic status and self rated health status, the results of multivariate binary Logistic regression analysis showed that compared with the appropriate exposure level group, the exposure of entertainment screen content on mobile phones at baseline, including frequent exposure to reading( OR =1.65,95% CI =1.14-2.39), occasional exposure to other entertainment screen content ( OR =1.46,95% CI =1.01-2.10)and frequent exposure to other entertainment screen content( OR =1.76,95% CI =1.20-2.60), increased the co-occurrence risk of symptoms of anxiety-depression co-morbidity among college students during the follow up period (all P <0.05).
Conclusion
Occasional or frequert exposure to mobile entertainment screen content can increase the risk of symptoms of anxiety depression co-morbidity among college students.
3.Differential analysis of saponins in Platycodon grandiflorus from different origins based on ultra-high performance liquid chromatography-mass spectrometry

Objective To establish a highly efficient and sensitive technical system for the identification and analysis of platycodin-type saponins, systematically compare the differences in platycodin-type saponins among Platycodon grandiflorum from different producing areas, and provide scientific references for the screening of high-quality Platycodon grandiflorum resources, authenticity evaluation, and construction of standardized quality control systems. Methods A total of 45 batches of P. grandiflorum medicinal materials from 3 producing areas (Anhui, Henan, and Jilin, with 15 batches per area) were selected as research objects. Qualitative identification and semi-quantitative analysis of saponin components were performed based on ultra-high performance liquid chromatography-quadrupole-time-of-flight tandem mass spectrometry (UHPLC-Q-TOF/MS) technology. Meanwhile, two multivariate statistical methods, principal component analysis (PCA) and partial least squares-discriminant analysis (PLS-DA), were combined to analyze the differences in platycodin-type saponins of Platycodon grandiflorus from different producing areas. Results A total of 28 saponin components were identified from Platycodon grandiflorus of the three producing areas. PCA results showed that there were minor differences in platycodin-type saponins between Henan Platycodon grandiflorus and Jilin Platycodon grandiflorus, while Anhui P. grandiflorum exhibited significant differences from both. PLS-DA further screened 15 major differential compounds. Among them, the contents of 6 components including 3''-O-acetylpolygalacin D2 and platycodin H in Anhui Platycodon grandiflorus were higher than those in Henan and Jilin Platycodon grandiflorus; platycodigenic acid A had the highest content in Jilin Platycodon grandiflorus; the contents of platycodin D3, polygalacin J, and polygalacin D were relatively higher in Henan Platycodon grandiflorus. Conclusion This study clarified the characteristic differences in core components of Platycodon grandiflorus from the three major producing areas, which provided an important theoretical basis for the screening of high-quality Platycodon grandiflorus resources, elucidation of the mechanism underlying its authenticity, and construction of a standardized quality control system.
4.Association between screening myopia and mixed exposure to air pollution among primary school students in four cities in China

Objective:
To investigate the association between screening detected myopia in primary school students and mixed exposure to air pollutants, so as to provide a scientific basis for the prevention and control of myopia in children and adolescents.
Methods:
From May to July 2023, a cluster random sampling method was used to select 15 102 primary school students in grades 3 through 6 from Hefei, Zhongshan, Shenyang, and Nanchang citys. Data were collected through questionnaires, distance vision screening, and computerized refraction. Monthly average concentration data for particulate matter (PM 10 ), fine particulate matter (PM 2. 5), NO 2 and SO 2 were used to calculate the annual average pollutant concentrations for primary school students. Quantile g computation model was employed to analyze the association between mixed exposure to air pollutants and screening detected myopia.
Results:
The differences in concentrations of the four pollutants among cities were statistically significant ( H =12 886.0, 13 285.0, 8 559.0, 13 181.0, all P <0.01). In the overall sample, there was no statistically significant association between combined exposure to air pollutants and the risk of myopia ( β=0.033, 95%CI=-0.004-0.072, P =0.08). Stratified analysis by outdoor activity time showed that among primary school students with outdoor activity time ≤1 h/d, mixed exposure to air pollutants was positively associated with the risk of myopia ( β=0.050, 95%CI=0.002-0.099, P =0.04); after stratification by city, combined exposure to air pollutants was significantly associated with the risk of myopia among primary school students in Nanchang and Shenyang [ β (95% CI )= 0.232(0.175-0.289), 0.080(0.006-0.155), both P <0.05], whereas no statistically significant association was observed among primary school students in Hefei and Zhongshan [ β (95% CI )=-0.010(-0.071-0.051), -0.032(-0.167-0.102), both P > 0.05 ].
Conclusions
Exposure to multiple air pollutants may increase the risk of myopia in primary school students, with the risk influenced by time spent outdoors and geographical location. It is recommended that outdoor activity management for children be strengthened in areas with higher air pollution levels, and that environmental factors be incorporated into comprehensive myopia prevention and control strategies.
5.MAUP Effect on Spatial Pattern of Pseudostellaria heterophylla Production Regions in China

ObjectiveTo investigate the modifiable areal unit problem (MAUP) in the spatial pattern of Pseudostellaria heterophylla production regions and reveal the impact of statistical scales on the spatial distribution characteristics of this medicinal plant species. MethodsUsing multi-source data (literature records, field surveys, and statistical data), we systematically analyzed the spatial patterns across three administrative levels (provincial, prefectural, and county scales). Spatial autocorrelation (Moran's I) analysis, high-low clustering (Getis-Ord General G), and hot/cold spot analysis (Getis-Ord Gi*) were employed. ResultsThe literature-based analysis showed that the production regions of P. heterophylla presented random distribution on the provincial scale and significant aggregation on the prefectural scale. The field survey data showed that the production regions displayed random distribution on the provincial scale but significant aggregation on both prefectural and county scales. The statistical data revealed that the production regions lacked spatial autocorrelation on the provincial scale but demonstrated significant aggregation on prefectural and county scales. ConclusionMAUP effects have substantive implications for understanding and decision-making in the arrangement of medicinal plant production regions. The county scale proves to be the most sensitive and explanatory level for analyzing the spatial pattern of P. heterophylla production regions, providing a critical foundation for habitat modeling, suitability evaluation, and ecological cultivation planning of medicinal plants.

Abstract
The issue of multimorbidity in children and adolescents is becoming increasingly prominent, but there is no consensus on the definition of multimorbidity. As research deepens, issues related to the comparability and standardization of relevant findings are gradually emerging. As a solution, a systematic review of both domestic and international research on multimorbidity is conducted, and a classification system for defining the concept of multimorbidity is proposed, offering more convenient conditions for the advancement of future research and cross study exchange.
7.Effect of anticoagulation-free veno-venous extracorporeal membrane oxygenation in patients with severe chest trauma

Objective To explore the feasibility and safety of non-anticoagulation veno-venous extracorporeal membrane oxygenation(VV-ECMO)in patients with severe chest trauma.Methods A retrospective cohort study method was used.A total of 19 patients with severe chest trauma who received VV-ECMO with a delayed anticoagulation strategy at Zhengzhou Central Hospital Affiliated to Zhengzhou University from January 2018 to October 2021 were included in the delayed anticoagulation group,and 20 patients with severe chest trauma who received VV-ECMO with a non-anticoagulation strategy from November 2021 to October 2024 were included in the non-anticoagulation group.The overall clinical characteristics of the patients were statistically analyzed,including gender,age,injury severity score(ISS),acute physiology and chronic health evaluationⅡ(APACHEⅡ),reason for VV-ECMO,use of vasoactive drugs,oxygenation index(PaO2/FiO2),and interval from injury to VV-ECMO.The primary outcomes were hemorrhagic and thrombotic complications.The secondary outcomes were blood transfusion during VV-ECMO,VV-ECMO time,mechanical ventilation time,intensive care unit(ICU)length of stay,and 28-day mortality.Results There was no significant difference in gender,age,ISS score,APACHEⅡscore,reason for VV-ECMO,use of vasoactive drugs,PaO2/FiO2,and interval from injury to VV-ECMO between the non-anticoagulation group and the delayed anticoagulation group.There was no significant difference in overall incidence of hemorrhagic and thrombotic between the two groups[incidence of hemorrhagic complications:15.0%(3/20)vs.31.6%(6/19),incidence of thrombotic:15.0%(3/20)vs.5.3%(1/19),both P>0.05].The infusion rate of 4 or more paked red blood cell(PRBC)within 24 hours during VV-ECMO in the non-anticoagulation group was significantly lower than that in the delayed anticoagulation group[5.0%(1/20)vs.31.6%(6/19),P<0.05].The amount of PRBC and platelet transfusion and the time on VV-ECMO in the non-anticoagulation group during VV-ECMO were significantly lower than those in the delayed anticoagulation group[PRBC(U):5.8±3.8 vs.8.1±3.1,platelets(U):1(0,1)vs.2(1,3),time on VV-ECMO(hours):71.55±24.37 vs.114.21±34.08,all P<0.05].There were no statistically significant differences in the amount of plasma and cryoprecipitate transfusion during VV-ECMO,mechanical ventilation time,ICU hospitalization time,and 28-day mortality between the two groups.Conclusion For patients with severe chest trauma receiving VV-ECMO withholding routine systemic anticoagulation did not result in thrombotic complications or higher mortality and required less PRBC and platelet transfusions.Non-anticoagulant VV-ECMO is safe and feasible for patients with severe chest trauma with high risk of bleeding.
8.Analysis of changes in ocular surface biology and visual quality in diabetic dry eye patients under different influencing factors

Objective To investigate the factors that influence ocular surface biology and visual acuity in individuals with diabetic dry eye(DDE)and analyze how these factors contribute to changes in visual quality.Methods Based on the disease duration,fasting blood glucose(FBG),and glycated hemoglobin(HbA1c)levels of patients with type 2 diabe-tes mellitus(T2DM),the DDE patients were divided into different groups.Logistic regression analysis was used to identify influencing factors related to ocular surface biology and visual quality in each group of DDE patients.Tear film stability was evaluated based on the tear film rupture time(BUT),Schirmer I test(SIt),and ocular surface disease index(OSDI).Lip-iview? Surface interferometers were used to measure tear film lipid layer thickness(LLT),meibomian gland loss rate(MGP),meibomian gland opening number(MGYLS),and meibomian gland secretion score(MGYSS).Wavefront aber-rometry was used to measure corneal wavefront aberration values at 4 mm and 6 mm pupil diameters.Ocular response ana-lyzer(ORA)was adopted to analyze corneal hysteresis(CH)and corneal resistance factor(CRF).Moreover,ELISA ex-periment to evaluate the trend of changes in inflammatory factors in tears.Results Logistic regression analysis revealed that T2DM duration,smoking history,FBG,HbA1c,total cholesterol(TC),triglycerides(TG),OSDI score,LLT,BUT,SIt,MGP,MGYLS,MGYSS,total higher-order aberrations,spherical aberration,coma aberration,trefoil aberration,tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insu-lin were all influencing factors for the risk of DDE(all P<0.05).As the T2DM course prolonged and FBG or HbA1 c levels rose,tear film-related indicators(LLT,BUT,and SIt)and meibomian gland-related indicators(MGYLS and MGYSS)inpa-tients gradually decreased,while OSDI scores and MGP gradually increased(all P<0.05).As the T2DM course prolonged and FBG or HbA1c levels rose,the total higher-order aberrations,spherical aberration,coma aberration,and trefoil aber-ration in DDE patients under 4 mm and 6 mm pupil diameters gradually increased;Meanwhile,best corrected visual acuity,corneal hysteresis,and corneal resistance factor gradually decreased;The contents of tumor necrosis factor-α,interleukin-6,matrix metalloproteinase-9,receptor for advanced glycation end products,and insulin in tears all gradually increased,while mucin-5AC gradually decreased(all P<0.05).Conclusion With the prolongation of T2DM duration and the in-crease of FBG or HbA1c,the ocular surface inflammatory response in DDE patients gradually worsens,corneal biological function decreases,and visual quality deteriorates.Timely systemic and local interventions are of great significance for im-proving dry eye symptoms and visual quality in DDE patients.
9.Primary ovarian mesonephric-like adenocarcinoma: a clinicopathological analysis of 17 cases

Objective:To investigate the clinicopathological characteristics, diagnosis, origin, and prognosis of primary ovarian mesonephric-like adenocarcinoma.Methods:A total of 17 cases of primary ovarian mesonephric-like adenocarcinoma diagnosed at the Obstetrics and Gynecology Hospital of Fudan University and Jiaxing Maternal and Child Health Care Hospital between January 2018 and September 2024 were included in this study. Histopathological sections were retrospectively reviewed, and clinicopathological data were systematically analyzed. Immunohistochemical analysis, molecular profiling, and clinical follow-up were performed to further characterize the cases.Results:The patients′ age was (57.1±9.3) years. Tumor involvement included 1 bilateral case, 9 left-sided cases, and 7 right-sided cases. Nine cases originated from endometrioid cysts, and 8 cases exhibited coexisting tumor components of other types. Gross examination revealed gray-yellow solid masses or solid components within cysts. Microscopically, the tumors displayed diverse architectural patterns, including papillary, glandular, cystic, tubular, and solid structures, with eosinophilic secretions within glandular lumens and mild to moderate nuclear atypia. Immunohistochemically, the tumors showed variable expression of TTF1, GATA3, CD10, and Calretinin. ER and PR were focally positive in only 2 cases, while others were negative. All cases demonstrated intact DNA mismatch repair proteins expression and wild-type p53 staining patterns. Molecular analysis performed in 10 cases identified pathogenic KRAS mutations in all tested samples. During a follow-up period of 1 to 75 months, 5 cases had recurrence, 1 patient remained alive with disease, and no disease-related death was reported.Conclusions:Ovarian mesonephric-like adenocarcinoma is an aggressive malignancy with a high potential for early recurrence and metastasis. Its frequent association with endometriosis and coexistence with other Müllerian tumors suggest a potential Müllerian origin. The tumor′s diverse morphological spectrum and common admixture with other tumor types often pose diagnostic challenges, making it difficult to distinguish from other gynecological malignancies. Therefore, accurate diagnosis of ovarian mesonephric-like adenocarcinoma is crucial for appropriate clinical management and prognostication.
10.Application and clinical outcomes of transperineal multi-modal image-fusion targeted prostate biopsy in a day-surgery model

Objective:To explore the feasibility and clinical outcomes of transperineal prostate multimodal image fusion-targeted biopsy under the day surgery model.Methods:Clinical data of 258 patients who underwent transperineal prostate biopsy at the Second Affiliated Hospital of Xi’an Jiaotong University between December 2022 and June 2024 were retrospectively analyzed. Patients were divided into two groups,and the experimental group( n = 141)underwent transperineal prostate multi-modal image-fusion targeted biopsy in the day-surgery mode,with age of(70.0 ± 8.8)years,median prostate-specific antigen(PSA)level of 11.10(7.63?17.06)ng/ml,and median Prostate Imaging Reporting and Data System(PI-RADS)score of 4(3,5). The control group( n = 117)underwent traditional transperineal systematic biopsy,with age of(69.3 ± 7.4)years,median PSA of 25.20(16.18-54.40)ng/ml,and median PI-RADS score of 4(3,5). The experimental group was given the day surgery mode:preoperatively,multiparametric magnetic resonance imaging(mpMRI)of the prostate was multimodally fused with ultrasound images,the location of target lesions in the prostate was manually mapped,and a targeted biopsy plan was developed. Intraoperatively,under ultrasound guidance,precise puncture was performed at the lesion sites,followed by systematic biopsy. After the operation,the patients were observed for 4-6 hours,and could be discharged if there were no obvious abnormalities,with the total hospitalization time within 24 hours. For the control group,the conventional biopsy mode was used. Intraoperatively,under ultrasound guidance,the standard 12-core transperineal systematic biopsy protocol was adopted,and sampling was conducted according to the anatomical regions of the prostate(base,midportion,apex,transition zone,and peripheral zone)to cover the entire gland. Patients in this group required routine hospitalization,with a hospital stay of 3-5 days. Operative time,intraoperative pain Numerical Rating Scale(NRS)score,complications within one week postoperatively,treatment costs,overall prostate cancer detection rate,and clinically significant prostate cancer(csPCa,defined as Gleason score ≥ 7 or pathological stage ≥ T 2b)detection rate were compared between the two groups. Results:All procedures were successfully completed without special incidents. The operative time was(13.49 ± 2.00)min in the experimental group and(13.05 ± 2.89)min in the control group,showing no significant difference( P > 0.05). The intraoperative pain NRS scores were(3.01 ± 1.17)and(3.10 ± 1.25)in the experimental and control groups,respectively,with no significant difference( P > 0.05). Pathological examination revealed that the overall prostate cancer detection rates were 46.8%(66/141)in the experimental group and 42.7%(50/117)in the control group,while the csPCa detection rates were 35.5%(50/141)and 31.6%(37/117),respectively. The differences were not statistically significant( P > 0.05). The complication rate was 6.4%(9/141)in the experimental group(including 2 cases of acute urinary retention,3 hematuria,3 fever,and 1 sepsis)and 6.0%(7/117)in the control group(including 3 acute urinary retention,1 hematuria,2 fever,and 1 sepsis),with no significant difference( P > 0.05). All complications improved after symptomatic treatment. The treatment costs were(4 063.25 ± 67.26)yuan in the experimental group and(5 185.14 ± 469.15)yuan in the control group,demonstrating a statistically significant difference( P < 0.05). Conclusions:Transperineal prostate multimodal image fusion-targeted biopsy can be safely performed under the day surgery model,offering advantages including a relatively high detection rate for csPCa,low complication rate,and better cost-effectiveness.
