1.Effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparo-scopic cholecystectomy

OBJECTIVE To investigate the effects of subanesthetic dose of esketamine on postoperative anxiety and recovery in patients undergoing laparoscopic cholecystectomy. METHODS A total of 200 patients scheduled for laparoscopic cholecystectomy at Suzhou Ninth Hospital Affiliated to Soochow University from January 2023 to December 2024 were randomly assigned to control group (n=100) and observation group (n=100). One minute before the initiation of anesthesia, patients in the control group received intravenous injections of Propofol emulsion injection, Sufentanil citrate injection, and Succinylcholine chloride injection. On this basis, patients in the observation group received an intravenous injection of Esketamine hydrochloride injection. The anxiety status of patients in both groups was compared, along with their general intraoperative conditions (including sufentanil dosage, duration of pneumoperitoneum, operative time, anesthesia time, and extubation time), postoperative recovery, incidence of adverse reactions, and the need for dezocine rescue analgesia. Heart rate and mean arterial pressure, entropy index (state entropy and response entropy), inflammatory marker levels [interleukin-6 (IL-6) and C-reactive protein (CRP)], numerical rating scale (NRS) for pain intensity were compared between the two groups at different time points. RESULTS No significant differences were found between the two groups in pneumoperitoneum duration, operative time, anesthesia time,extubation time, incidence of postoperative dry mouth, entropy index or length of stay in the post-anesthesia care unit (P>0.05). Compared with the control group, the observation group showed significantly lower postoperative STAI-S scores, reduced intraoperative sufentanil consumption, decreased incidence of postoperative nausea, vomiting, and shivering, the need for dezocine rescue analgesia, as well as lower plasma IL-6 and CRP levels at 24 h after surgery, and NRS (P<0.05). The heart rate and mean arterial pressure of patients in the observation group at the start of surgery, end of surgery, and during extubation were all significantly higher than those in the control group (P<0.05). CONCLUSIONS Subanesthetic dose of esketamine can effectively alleviate postoperative anxiety, reduce intraoperative opioid consumption, suppress postoperative inflammatory response, relieve postoperative pain, and promote recovery in patients undergoing laparoscopic cholecystectomy.
2.Expert consensus on perioperative nursing care for myasthenia gravis patients undergoing thymectomy

Myasthenia gravis is an autoimmune disorder characterized by impaired neuromuscular transmission. Thymectomy is one of the therapeutic options for acetylcholine receptor antibody-positive myasthenia gravis patients. The quality of perioperative care is directly associated with surgical safety and patient outcomes. However, there is currently a lack of specialized nursing consensus or guidelines specifically addressing the care of these patients domestically or internationally. To promote the standardization and normalization of perioperative nursing care for myasthenia gravis patients undergoing thymectomy and to ensure treatment efficacy, a panel of 57 experts from relevant fields was convened. Based on evidence-based medicine and clinical practice experience, discussions were held on various aspects including condition assessment, nutritional support, medication management, and airway care, resulting in a consensus with 18 final recommendations by using the Delphi method through two rounds of expert consultation. This consensus aims to provide a scientific reference for the perioperative nursing care of myasthenia gravis patients undergoing thymectomy.
3.Skeleton Binding Protein 1 of Plasmodium berghei Influences Deformability and Cytoskeletal Ultrastructure of Infected Erythrocyte

ObjectiveThe malaria parasites remodel the host erythrocyte structure by exporting parasite proteins that interact with the membrane skeleton proteins of red blood cells (RBCs), facilitating their intracellular survival and pathogenicity. Skeleton-binding protein 1 (SBP1) is a conserved exported protein across Plasmodium species. In Plasmodium falciparum, SBP1 has been reported to interact with erythrocyte membrane skeleton proteins 4.1R and spectrin, while its contribution to erythrocyte remodeling and parasite virulence in Plasmodium berghei (Pb) remains unclear. This study aims to determine whether PbSBP1 associates with the host cytoskeletal protein 4.1R and to investigate its role in the remodeling of host RBCs and the pathogenicity of Plasmodium berghei. MethodsIn Plasmodium berghei, the relationship between PbSBP1 and the erythrocyte cytoskeletal protein 4.1R was examined using co-immunoprecipitation. A Pbsbp1 gene knockout mutant of Plasmodium berghei (Pbsbp1∆) was generated based on the principle of double crossover homologous recombination. The deformability of erythrocytes infected with Pbsbp1∆ parasites was assessed using microfluidic methods. Microchannels with an array of cylindrical pillars were used to detect modifications in infected RBC deformability. The infected RBCs were squashed between the rows and recovered between the columns and the transit velocity (μm/s) of infected RBCs travelling through the microchannel was recorded. The component of the erythrocyte membrane skeleton junctional complex, tropomodulin (TMOD), was fluorescently labeled, and the cytoskeletal network of infected erythrocytes was imaged using super-resolution stochastic optical reconstruction microscopy (STORM) to analyze ultrastructural changes in the cytoskeleton of wild-type (WT) and Pbsbp1∆-infected erythrocytes. Actin-based junctional complexes were displayed as individual clusters by the labeled TMOD in the STORM images, and the cluster densities and distances between adjacent clusters of infected RBCs were calculated. Additionally, rodent malaria models (BALB/c mice) and experimental cerebral malaria models (C57BL/6 mice) were employed to monitor the growth of Pbsbp1∆ and WT parasites during the intraerythrocytic stage and their capacity to induce cerebral malaria in mice. ResultsPbSBP1 may participate in the remodeling of infected erythrocytes through direct or indirect interaction with the erythrocyte cytoskeletal protein 4.1R. Microfluidic assays revealed that the deformability of erythrocytes infected with Pbsbp1∆ parasites was significantly enhanced compared to those infected with WT parasites. STORM imaging further demonstrated that the ultrastructure of the erythrocyte cytoskeleton in Pbsbp1∆-infected cells was altered relative to that in WT-infected erythrocytes. The distances between nearest neighbors of clusters had a tendency to increase while the cluster densities were decreased in Pbsbp1∆-infected RBCs compared to WT-infected RBCs. Subsequent phenotypic analysis indicated that the growth rate of Pbsbp1∆ parasites during the intraerythrocytic stage was significantly slower than that of WT parasites, and their ability to induce cerebral malaria in mice was also attenuated. These findings suggest that PbSBP1 is involved in the remodeling of the erythrocyte membrane skeleton, likely through its direct or indirect interaction with protein 4.1R, thereby regulating the deformability of infected erythrocytes and influencing the pathogenicity of the blood-stage parasites. ConclusionThis study establishes a role for PbSBP1 in host erythrocyte remodeling and parasite virulence, providing new research strategies for the prevention and treatment of malaria.
4.First Stage Ultrasonic Indicator-Based Nomogram Model for Predicting Vaginal Delivery in Nulliparous Women

Purpose To explore factors influencing vaginal delivery during the first stage of labor using intrapartum ultrasound and to construct predictive models for delivery decision-making.Materials and Methods A total of 473 nulliparous women admitted to Heping Hospital,Northern Theater General Hospital from July to December 2021 were prospectively enrolled as the training set.Clinical data on admission and fetal biometric parameters(biparietal diameter,femur length,head circumference and abdominal circumference)measured within one week before delivery were collected.Ultrasound assessments of fetal position,angle of progression(AOP)and head-perineum distance(HPD)were performed during the first stage of labor.The latent phase group(n=255)was subdivided into vaginal delivery group(n=186)and cesarean section group(n=69);the active phase group(n=218)was divided into vaginal delivery group(n=168)and cesarean section group(n=50).The associations between fetal position,AOP,HPD and vaginal delivery were analyzed,and predictive models were constructed for the latent phase(model 1)and active phase(model 2).A validation set of 547 women from January to September 2022 was used to evaluate model performance via area under the curve(AUC),calibration curves and decision curve analysis.Results In the latent phase,multivariate regression identified maternal height(OR=3.970,P=0.002),pre-pregnancy body mass index(OR=0.893,P=0.036),labor onset type(OR=2.415,P=0.045),neonatal birth weight(OR=3.728,P=0.002),AOP(OR=11.649,P<0.001)and HPD(OR=4.240,P=0.004)as significant predictors.The training and validation sets showed AUCs of 0.917 and 0.869,respectively.Goodness-of-fit tests indicated excellent model calibration(χ2=3.437,P=0.904;χ2=10.877,P=0.209).Decision curve analysis demonstrated strong clinical utility.For the active phase,significant predictors included maternal height(OR=6.532,P<0.001),neonatal birth weight(OR=11.890,P<0.001),fetal position(OR=4.600,P=0.003),AOP(OR=7.229,P<0.001)and HPD(OR=4.722,P=0.005).AUCs were 0.943(training)and 0.906(validation),with good calibration(χ2=4.340,P=0.740;χ2=9.836,P=0.277)and clinical applicability.Conclusion First stage ultrasound assessment of fetal position,AOP and HPD correlates with delivery outcomes.The developed nomogram models combining these parameters with clinical factors provide valuable guidance for delivery decision-making.
5.Sodium lactate modulates TLR4/NF-κB signaling pathway for treatment of right heart failure

Aim To investigate the effects of sodium lactate(NALA)on right heart failure induced by monocrotaline(MCT)-induced pulmonary arterial hy-pertension in rats and to reveal the underlying mecha-nisms.Methods Forty male Sprague-Dawley(SD)rats were randomly allocated into four groups,with ten rats in each group,namely,MCT group,NALA group,and NALA+MCT group;the MCT and NALA+MCT groups were administered a single intraperito-neal injection of MCT at 60 mg·kg-1 to induce pul-monary hypertension,and one week later,the NALA and NALA+MCT groups received intraperitoneal in-jections of NALA at 0.1 g·kg-1(once a day,for 5 weeks),while the CON and MCT groups received e-qual volumes of physiological saline(once a day,for 5 weeks);right heart function was assessed using echo-cardiography,right ventricular and pulmonary artery remodeling were evaluated via histopathological sec-tions,and the expression levels of ANP,BNP,and in-flammatory factors were measured by ELISA,along with assessments of oxidative stress levels,Western blot detection of the expression levels of proteins in the TLR4/NF-κB signaling pathway.Results Compared to the CON group,the MCT group exhibited increased RVSP and RVHI,decreased right heart function,in-creased collagen fiber deposition,and elevated oxida-tive stress and inflammatory factor expression,and the expression levels of proteins in the TLR4/NF-κB signa-ling pathway increased(P<0.05);compared to the MCT group,the NALA+MCT group showed reduced RVSP and RVHI,improved right heart function,atten-uated pulmonary vascular remodeling,decreased ex-pression of ANP,BNP,inflammatory factors,and H2O2,along with increased antioxidant enzyme expres-sion,and the expression levels of proteins in the TLR4/NF-κB signaling pathway decreased(P<0.05).Conclusion NALA can inhibit right ventric-ular remodeling in rats with pulmonary hypertension,and the underlying mechanism may involve the allevia-tion of inflammatory responses and oxidative stress through the inhibition of the TLR4/NF-κB signaling pathway.
6.Chemical constituents from Commelina communis

AIM To study the chemical constituents from Commelina communis L.METHODS The 95%ethanol extract from C.Communis was isolated and purified by activated charcoal,silica gel,Sephadex LH-20,and HPLC,then the structures of obtained compounds were identified by physicochemical properties and spectral data.RESULTS Seventeen compounds were isolated and identified as p-hydroxyl ethyl cinnamate(1),p-hydroxybenzaldehyde(2),vanillin(3),4-hydroxy-2,3-dimethyl-2-nonen-4-olide(4),hemeratrol A(5),chakyunglupulin B(6),chakyunglupulin A(7),2-(2-hydroxyethyl)-3-methylfumaric acid(8),N-cis-feruloyl tyramine(9),N-trans-coumaroyltyramine(10),5,6,7,3',4',5'-hexamethoxyflavone(11),N-trans-sinapoyltyramine(12),dihydro-feruloyltyramine(13),N-trans-feruloyltyramine(14),2-phenylethanol-β-D-glucoside(15),quercetin-3-O-β-D-glucoside(16),and isorhamnetin-3-O-β-D-glucopyranoside(17).CONCLUSION Compounds 4-8,10 and 11 are isolated from Commelina genus for the first time,and 1,9,12-15 are first isolated from this plant.
7.Bioinformatics and experimental validation reveal the core genes of neutrophil extracellular traps in chronic obstructive pulmonary disease

Objective:Bioinformatics and experimental validation reveal the core genes of neutrophil extracellular traps(NETs)in chronic obstructive pulmonary disease(COPD).Methods:Chip data GSE8545,GSE10006,GSE11906,GSE19407 and GSE20257 were downloaded from GEO database,and the common differential expression genes(DEGs)of the first 4 datas were ob-tained through intersection and enrichment analysis were performed.Neutrophil extracellular traps differential expression genes(NETs-DEGs)were obtained by overlapping the NETs-related genes and the common DEGs,and GSE20257 was used to verify the NETs-DEGs and evaluate the diagnostic efficacy,and enrichment analysis was performed and mRNA expression level was verified in lung tissue of mice emphysema model.Results:A total of 149 common DEGs were acquired.DO analysis mainly concentrated in lung disease and COPD.GO functional annotation mainly involved in olefin metabolism,cytohormone metabolism,endoplasmic reticulum lumen,signaling receptors and oxidative stress activities.KEGG enrichment analysis mainly involved in cytochrome P450 metabo-lism,chemical carcinogenic reactive oxygen species,steroid hormone biosynthesis and phagosome,etc.GSE20257 dataset verifica-tion showed that NETs-DEGs TLR7 and CYP4F3 were up-regulated,and AUC values were 0.750 and 0.947.GO and KEGG mainly focused on fat-soluble vitamin catabolic signaling pathways,phagosomes,arachidonic acid monooxygenase activity,pattern recogni-tion receptors,arachidonic acid metabolism,Toll-like receptor signaling pathways and formation of NETs.Compared with air exposure group,mRNA expression levels of TLR7 and CYP4F3 in lung tissue of emphysema mice induced by cigarette smoke exposure were sig-nificantly increased(P<0.05).Conclusion:TLR7 and CYP4F3 may be the core genes in formation and release of NETs during the pathogenesis of COPD,which are expected to become the targets of immunotherapy for COPD.
8.Relationship between sleep quality,serum S100 calcium-binding protein β subunit level,soluble CD40 ligand level and the severity of cerebral small vessel disease

Objective To explore the relationship between sleep quality,serum S100 calcium-binding proteinβ subunit(S100β)level,soluble CD40 ligand(sCD40L)level and the severity of cerebral small vessel disease.Methods According to the total imaging load score,60 cerebral small vessel disease patients were divided into severe disease group(22 cases)and mild-to-moderate disease group(38 cases).The general clinical data,Pittsburgh sleep quality index(PSQI),Epworth sleepiness scale(ESS)score,insomnia severity index(ISI)score and the levels of S100β and sCD40L were compared between the two groups.Binary Logistic regression analysis was used to evaluate the correlation between the difference index between the groups and the severity of the disease in patients with cerebral small vessel disease.Results The rate of hypertension in severe disease group was significantly higher than that in mild-to-moderate disease group(P<0.05).The ESS and ISI scores in severe disease group were significantly higher than those in mild-to-moderate disease group(all P<0.001).The levels of serum S100β and sCD40L in severe disease group were significantly higher than those in mild-to-moderate disease group(all P<0.001).Univariate Logistic regression analysis showed that hypertension,ESS score,ISI score,S100β level and sCD40L level were potentially correlated with the severity of cerebral small vessel disease(all P<0.05).Multivariate Logistic regression analysis showed that ESS score,ISI score,S100β level and sCD40L level were closely related to the severity of cerebral small vessel disease(all P<0.05).Conclusions The severity of sleepiness and insomnia and the levels of serum S100β and sCD40L in patients with cerebral small vessel disease are closely related to the severity of the disease.Paying attention to the sleep status and the body's inflammatory state of patients is conducive to a more accurate assessment of the severity of cerebral small vessel disease.
9.Characteristics of T lymphocyte subsets and its relationship with tumor-related death in patients with non-small cell lung cancer after radiofrequency ablation

Objective:To investigate the characteristics of T lymphocyte subsets and its relationship with tumor-related death in patients with non-small cell lung cancer(NSCLC)after radiofrequency ablation(RFA).Methods:A total of 105 patients with NSCLC who were treated with RFA in the First People's Hospital of Shuangliu District and West China Hospital of Sichuan University from August 2017 to August 2020 were selected.According to the tumor-related death within 3 years of postoperative follow-up,patients were divided into survival group(n=45)and death group(n=60).Changes of T lymphocyte subsets CD3+T,CD4+T,CD8+T and CD4+T/CD8+T in patients before and after RFA treatment were analyzed.Clinical data and T lymphocyte subsets before and 1 day,7 days and 14 days after RFA treatment were compared between the two groups.Multivariate Cox proportional hazard regression analysis was used to analyze the factors affecting tumor-related death and the predictive value of T lymphocyte subsets at 7 days after operation.Con-struct the prediction model and evaluate its stability.Results:Levels of CD3+T,CD4+T and CD4+T/CD8+T on the 7th and 14th day after operation were significantly lower than those before RFA treatment,while CD8+T was increased significantly(P<0.05).CD3+T,CD4+T and CD4+T/CD8+T of two groups at the 7th and 14th after operation were significantly lower than those before RFA treatment,while CD8+T was significantly higher(P<0.05).CD3+T,CD4+T and CD4+T/CD8+T in death group on the 7th and 14th day after opera-tion were significantly lower than those in survival group,while CD8+T was significantly higher than that in survival group(P<0.05).Tumor diameter≥3 cm,TNM stage Ⅲ~Ⅳ,and the increase of CD8+T on the 7th day after operation were the risk factors of tumor-related death,while postoperative adjuvant therapy,and the increase of CD3+T,CD4+T and CD4+T/CD8+T on the 7th day after operation were protective factors(P<0.05).After adjusting the covariates,CD3+T,CD4+T,CD8+T and CD4+T/CD8+T on the 7th day after operation were still independent predictors of tumor-related death(P<0.05).The stability of the prediction model was good.Conclusion:Charac-teristics of T lymphocyte subsets on the 7th day after RFA treatment were related to tumor-related death in patients with NSCLC.
