1.Effects of Kidney-Tonifying and Blood-Activating Acupuncture on Mitochondrial Membrane Potential and SIRT1/PGC-1α Axis in Hippocampal Tissue of SAMP8 Mice

ObjectiveTo explore the potential mechanisms of kidney-tonifying and blood-activating acupuncture for Alzheimer's disease. MethodsMale SAMP8 mice were randomly divided into a model group, acupuncture group, non-acupoint group, and donepezi group, with 10 mice in each group, and 10 SAMR1 mice as normal group. The acupuncture group received acupuncture at Baihui (GV 20), Xuehai (SP 10), Shenshu (BL 23), and Geshu (BL 17). Xuehai (SP 10), Shenshu (BL 23), and Geshu (BL 17) were stimulated on the left side first and then on the right side alternately, once a day. The non-acupoint group received acupuncture at fixed bilateral non-meridian, non-acupoint points under the ribs, once a day. The model and normal groups underwent equivalent handling and restraint stress without acupuncture. The donepezi group received 1 mg/kg donepezil via gastric gavage daily. All groups were treated for 4 weeks. After treatment, Morris water maze tests (to record orientation sailing latency, number of traverses through the platform quadrant) and open field (to record distance travelled) were used to evaluate learning and memory abilities; hippocampal neuronal damage was analyzed via HE and Nissl staining; mitochondrial membrane potential and reactive oxygen species (ROS) were measured using assay kits; Western blot and qRT-PCR were performed to detect silencing information regulator 1 (SIRT1), peroxisome proliferator-activated receptor gamma coactivator 1-alpha (PGC-1α), and mRNA expression levels. ResultsCompared with the normal group, mice in the model group and non-acupoint group showed elevated orientation sailing latency and relative multiplicity of ROS in hippocampal tissues, and reduced number of traverses through the platform quadrant, distance of movement in the open-field experiment, number of Nissl-staining-positive cells in the hippocampal tissues, mitochondrial membrane potential, and protein levels and mRNA expression of SIRT1, PGC-1α (P<0.01); HE staining showed that the hippocampal tissues of the mice was loosely arranged, with reduced number of neurons and vacuolar degeneration; Nissl staining showed that pyramidal neurons in the hippocampal region were not neatly arranged, and the number of Nissl bodies in the cytoplasm was less and the staining was lighter. Compared with the model group, mice in the acupuncture group and donepezil group had lower orientation sailing latency and relative multiplicity of ROS in the hippocampal tissue, higher number of traverses through the platform quadrant, distance of movement in the open-field experiment, number of Nissl-stained positive cells in the hippocampal tissue, mitochondrial membrane potential, and protein levels and mRNA expression of SIRT1 and PGC-1α (P<0.01), and HE staining and Nissl staining showed significant improvement in hippocampal histopathological damage. Compared with the donepezil group, the orientation sailing latency shortened in the acupuncture group of mice (P<0.01). ConclusionKidney-tonifying and blood-activating acupuncture method can alleviate the SIRT1/PGC-1α signalling pathway in the hippocampal tissue and improve the mitochondrial function, thus alleviating the neuronal damage, which is one of the possible mechanisms for its treatment of Alzheimer's disease.
2.Association between age at first sexual intercourse and gynecologic malignant tumors: a Mendelian randomization study

Objective:
To examine the casual association between age at first sexual intercourse and gynecologic malignant tumors using Mendelian randomization (MR) approach.
Methods:
The single nucleotide polymorphisms (SNPs) associated with age at first sexual intercourse were obtained from a meta-analysis of genome-wide association study (GWAS), and the SNPs related to gynecologic malignant tumors (ovarian cancer, endometrial cancer, cervical cancer), and their subtypes were sourced from the IEU OpenGWAS database. Using age at first sexual intercourse as the exposure and gynecologic malignant tumors as the outcome, a MR analysis was performed with the inverse-variance weighted (IVW) method. Heterogeneity was assessed using Cochran's Q test, horizontal pleiotropy was evaluated using MR-Egger regression and MR-PRESSO test, and bias was examined using funnel plots.
Results:
The Mendelian randomization analysis demonstrated that younger age at first sexual intercourse was significantly associated with an increased risk of low-grade serous ovarian carcinoma (OR=0.553, 95%CI: 0.335-0.911), cervical cancer (OR=0.674, 95%CI: 0.466-0.974), endometrial cancer (OR=0.854, 95%CI: 0.730-0.999), and endometrioid carcinoma (OR=0.830, 95%CI: 0.690-0.998). No statistical association was found between the age at first sexual intercourse and ovarian cancer, high-grade serous ovarian cancer, mucinous ovarian cancer, endometrioid ovarian cancer, or non-endometrioid ovarian cancer (all P>0.05). Sensitivity analysis showed no evidence of undetected instrumental variable heterogeneity or horizontal pleiotropy (all P>0.05), and the funnel plot indicated no presence of bias.
Conclusion
Younger age at first sexual intercourse may be associated with an increased risk of certain gynecologic malignant tumors, highlighting the need to strengthen adolescent sex education.
3.A case report of premature ovarian insufficiency caused by a novel FANCL mutation(c.1033G>A)and in vitro functional validation

Objective To investigate the characteristics of a novel FANCL mutation identified in a patient with premature ovarian insufficiency(POI)and to explore its potential functional impacts in vitro.Methods A novel FANCL heterozygous mutation c.1033G>A(p.Glu345Lys)was screened in a patient with POI using whole exome sequencing(WES),which was found to be inherited from a mother who had undergone early menopause.The authenticity of the mutation was identified by Sanger sequencing and the conserved nature of the mutation site was predicted by software.Overexpressing FANCL mutant and wildtype plasmids were constructed and transiently transfected into HEK293T cell lines,and the effect of the mutation was detected by qPCR,immunofluorescence and Western blot.Results The mutation site of FANCL was located within the Ring domain of FANCL,which was highly conserved across multiple species.The mutant showed no significant change in mRNA expression level,while the protein expression level was significantly down-regulated.In vitro cellular experiments further revealed that the mutation leads to decreased expression levels by reducing protein stability.Conclusion A FANCL c.1033G>A mutation was found and it may cause disease in the POI patient due to decreased protein stability.
4.Prenatal ultrasound diagnosis of fetal hypoplastic left heart syndrome with left ventricular enlargement:a case report

The pregnant woman was 30 years old,G2P0.This singleton pregnancy at 22 weeks of gestation was screened for second-trimester ultrasound malformations,suggesting fetal aortic valve atresia,aortic stenosis with reverse blood flow,mitral valve atresia,and markedly enlarged left ventricle,which was considered for the diagnosis of hypoplastic left heart syndrome(HLHS).The pregnancy was terminated at our hospital and subsequently underwent genetic testing with results of heterozygous variants in the NOTCH1 gene,which can cause aortic valve disease type 1.The findings of the fetal autopsy were aortic valve atresia,mitral valve widening and thickening,and left ventricular enlargement with myocardial infarction.This report focuses on the ultrasound characteristics of HLHS with left ventricular enlargement and its hemodynamic changes in order to improve clinicians'understanding of the progressive changes in the disease phenotype of HLHS.
5.Study on the correlation between serum APRIL,PLA2R-Ab,and 25-(OH)D3 levels and the severity and prognosis of primary membranous nephropathy

Objective Investigating the correlation between fluctuations in proliferation-inducing ligand(APRIL),M-type phospholipase A2 receptor antibody(PLA2R Ab),and 25-hydroxyvitamin D3[25-(OH)D3]levels and their impact on the severity and prognosis of primary membranous nephropathy(PMN).Methods A prospective study design was employed,wherein 100 confirmed PMN patients from Xingtai People's Hospital were recruited as the PMN group,and 100 healthy volunteers served as the control group.The levels of APRIL,PLA2R Ab,and 25-(OH)D3 were compared between the two groups of participants,stratified by PMN disease stage and treatment outcomes.A simple linear correlation analysis was conducted to evaluate the correlation between APRIL,PLA2R Ab,and 25-(OH)D3 with renal function indicators.Additionally,a multiple regression model was utilized to analyze the associations between these indicators and patient treatment outcomes as well as prognosis.Results The levels of APRIL and PLA2R Ab in the MN group were significantly higher than those in the control group,whereas the levels of 25-(OH)D3 were significantly lower than those in the control group(P<0.05).Among 100 patients with PMN,there were 20 in stage I,42 in stage Ⅱ,34 in stage Ⅲ,and 4 in stage Ⅳ.The levels of APRIL and PLA2R Ab in stage Ⅲ+Ⅳ patients were significantly higher than those in stage Ⅰ+Ⅱ patients,while the level of 25-(OH)D3 was significantly lower in stage Ⅲ+Ⅳ patients compared to stage Ⅰ+Ⅱ patients(P<0.05).In PMN patients,serum APRIL and PLA2R-Ab levels were negatively correlated with urea nitrogen(BUN),creatinine(Scr),and 24-h urinary protein(P<0.05).Additionally,APRIL and PLA2R-Ab levels were positively correlated with total protein(TP)and albumin(ALB)(P<0.05),while serum 25-(OH)D3 levels were negatively correlated with BUN,Scr,and 24-h urinary protein(P<0.05).After treatment,42 patients achieved complete remission,while 58 patients did not meet the remission criteria.Serum APRIL and PLA2R-Ab levels in the remis-sion group were significantly lower than those in the non-remission group both before treatment and after 12 months of treatment.Furthermore,serum 25-(OH)D3 levels in the remission group were significantly higher than those in the non-remission group both before treatment and after 12 months of treatment(P<0.05).Conclusions Elevated levels of serum APRIL and PLA2R antibodies,which contribute to immune dysfunction,are closely asso-ciated with the onset and severity of PMN.Renal impairment leads to a substantial reduction in serum 25-(OH)D3 levels.Collectively,these three indicators serve as critical markers for the occurrence,progression,and prognosis of PMN.
6.Risk factors for postoperative recurrence in patients with chronic sinusitis and nasal polyps treated by en-doscopic sinus surgery

Objective To analyze the risk factors for postoperative recurrence in patients with chronic sinusitis and nasal polyps(CRSwNP)treated by endoscopic sinus surgery(ESS),and to construct a predictive model.Methods A retrospective study was conducted on 203 patients with CRSwNP who underwent ESS in the hospital from March 2022 to February 2023.These patients were divided into a recurrence group(n=43)and an non-recurrence group(n=160)based on whether they experienced recurrence after surgery.Clinical data were collected and analyzed using univariate analysis to identify significant differences.Lasso regression was used to screen potential influencing factors.Multivariate logistic regression was employed to analyze the risk factors.A nomogram was constructed for postoperative recurrence model,and the receiver operating characteristic(ROC)curve and calibration curve were used to evaluate the model.Results Single-factor analysis showed that disease duration,Lund-Mackay CT score,SNOT-22 score,EOS,ECP,total IgE,CRP,IL-5,and IL-1β in the recurrence group after ESS were higher than those in the non-recurrence group(P<0.05),while the UPSIT score,CD3+,CD4+,CD8+,and CD4+/CD8+were lower(P<0.05);the proportion of patients with allergic rhinitis,asthma,no preoperative glucocorticoid treatment,and surgery time≥2 hours in the recurrence group after ESS was higher than that in the non-recurrence group(P<0.05).Multivariate logistic regression analysis model showed that disease duration(OR=1.389,95%CI:1.094~1.763,P=0.007),combined asthma(OR=2.997,95%CI:1.065~8.432,P=0.038),Lund-Mackay CT score(OR=1.156,95%CI:1.027~1.301,P=0.016),EOS(OR=1.540,95%CI:1.249~1.898,P<0.001),total IgE(OR=1.005,95%CI:1.000~1.009,P=0.041),and IL-5(OR=1.165,95%CI:1.078~1.260,P<0.001)were risk factors for ESS recurrence.Based on multivariate logistic regression analysis nomogram,the area under the ROC curve(AUC)was 0.9057;the sensitivity 76.74%;the specificity 87.5%;the average absolute error(MAE)of the calibration curve 0.03;the mean square error(MSE)0.00157,and the absolute error at the 0.9 percentile 0.065.Conclusions Disease course,the presence of asthma,Lund-Mackay CT score,EOS,total IgE,and IL-5 are all risk factors for postoperative recurrence in patients with CRSwNP.Moreover,the relevant nomogram model can be used as a reliable tool for assessing the risk of postoperative recurrence in CRSwNP.
7.Yiqi-Yangyin-Quyu prescription attenuates skeletal muscle injury and related metabolic disorders in mice with Sj?gren syndrome by targeting Hadhb

AIM:The aim of this study was to explore the effects of Yiqi-Yangyin-Quyu prescription(YP)on skeletal muscle injury and related metabolic disorders in mice with Sj?gren syndrome(SS),and to clarify the role of hy-droxyacyl-CoA dehydrogenase trifunctional multienzyme complex subunit beta(Hadhb)in mediating the effect of YP on skeletal muscle in SS.METHODS:The SS mice underwent YP treatment for 8 weeks.The morphological changes of the submandibular gland and muscle tissue were examined using hematoxylin-eosin staining.The mitochondrial status in mus-cle tissue was assessed through transmission electron microscopy.Additionally,combined transcriptome and proteome se-quencing was conducted on skeletal muscle samples.The omics sequencing results were validated by RT-qPCR.Immuno-fluorescence was used to confirm the levels of key proteins involved in the P53/peroxisome proliferator-activated receptor gamma(PPARG)signaling pathway.Immunohistochemistry and Western blot were employed to determine the levels of Hadhb key targets.RESULTS:Combined transcriptome and proteome analysis identified 1 523 differentially expressed genes(DEGs)and 182 differentially expressed proteins(DEPs)between the muscle tissue of SS mice(model group)and that of control animals(ICR group),12 of which showed co-differential expression at both transcriptomic and proteomic levels.Compared with model group,1 232 genes and 432 proteins were found to be differentially expressed in the muscle tissue of the mice in YP group.Among these,23 exhibited co-differential expression at both mRNA and protein levels.Gene Ontology(GO)analysis showed that the DEGs and DEPs between ICR and model groups were mainly involved in ener-gy metabolism and fatty acid oxidation,while the DEGs and DEPs between YP and model groups were primarily associated with sarcomere tissue and actin structure.Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment analysis indi-cated that the DEGs and DEPs between ICR and model groups and between model and YP groups were enriched in the com-plement and coagulation cascade and lipid and pyruvate metabolism.The RT-qPCR validation results were consistent with those of the transcriptome analysis.Furthermore,the protein expression of the tumor suppressor P53 was significantly de-creased in YP group compared with model group,whereas that of PPARG was significantly increased.Western blot analy-sis showed that compared with ICR group,Hadhb protein expression was significantly decreased in model group,whereas the opposite trend was detected in YP group.CONCLUSION:The SS-related skeletal muscle damage is closely related to amino acid metabolism disorder and fatty acid degradation.Treatment with YP modulates innate immune defenses,lipid metabolism and energy metabolism in SS,and Hadhb is the key target of YP in SS-related skeletal muscle.
8.Risk factors for adverse outcomes in elderly patients with nonvalvular atrial fibrillation after percuta-neous left atrial appendage closure combined with radiofrequency ablation

Objective:To investigate the risk factors for adverse outcomes in elderly patients with nonvalvular atrial fibrilla-tion(AF)after percutaneous left atrial appendage closure(LAAC)combined with radiofrequency ablation(RFA).Meth-ods:A total of 199 elderly AF patients who admitted in Department of Cardiology,Yuhuangding Hospital between Jan 1st,2019 and Aug 17th,2023 and underwent LAAC combined RFA were enrolled.The patients were divided into case group(n=78)and control group(n=121)according to presence of atrial arrhythmia and/or cerebral infarction(CI)within one year after operation.Baseline clinical data,laboratory indexes and color Doppler ultrasound indexes were compared be-tween the two groups,and multivariate Logistic regression was used to analyze the risk factors for postoperative atrial ar-rhythmia and/or CI.Receiver operating characteristic(ROC)curve was constructed to analyze the predictive efficacy of risk factors for atrial arrhythmia and/or CI after operation.Results:Univariate analysis indicated that compared with pa-tients in control group,those in case group had significant higher proportions of preoperative persistent atrial fibrillation(52.60% vs.37.20%),old myocardial infarction(OMI)(11.50%vs.1.70%)and left atrial diameter(LAD)[(45.47±6.90)mm vs.(43.34±6.64)mm](P<0.05 or<0.01).Multivariate Logistic regression analysis indicated history of OMI(OR=8.736,95%CI 1.772~43.069,P=0.008)and LAD(OR=1.053,95%CI 1.006~1.102,P=0.027)were independent risk factors of adverse outcomes in elderly AF patients after LAAC+RFA.ROC analysis indicated that predic-tive efficacy of combined detection of OMI and LAD(AUC=0.663,95%CI 0.593~0.728)for adverse outcomes within 1 year in elderly AF patients after LAAC+RFA was significantly better than those of OMI(AUC=0.549,95%CI 0.477~0.620)and LAD(AUC=0.602,95%CI 0.531~0.671)alone(Z=3.045,2.312,P=0.002,0.021).Conclusion:His-tory of old myocardial infarction and left atrial diameter are independent risk factors for atrial arrhythmia and/or cerebral infarction within 1 year after percutaneous left atrial appendage closure combined with radiofrequency ablation in elderly patients with atrial fibrillation,and the combination has good predictive performance.

10.Endoplasmic reticulum membrane remodeling by targeting reticulon-4 induces pyroptosis to facilitate antitumor immune.

Pyroptosis is an identified programmed cell death that has been highly linked to endoplasmic reticulum (ER) dynamics. However, the crucial proteins for modulating dynamic ER membrane curvature change that trigger pyroptosis are currently not well understood. In this study, a biotin-labeled chemical probe of potent pyroptosis inducer α-mangostin (α-MG) was synthesized. Through protein microarray analysis, reticulon-4 (RTN4/Nogo), a crucial regulator of ER membrane curvature, was identified as a target of α-MG. We observed that chemically induced proteasome degradation of RTN4 by α-MG through recruiting E3 ligase UBR5 significantly enhances the pyroptosis phenotype in cancer cells. Interestingly, the downregulation of RTN4 expression significantly facilitated a dynamic remodeling of ER membrane curvature through a transition from tubules to sheets, consequently leading to rapid fusion of the ER with the cell plasma membrane. In particular, the ER-to-plasma membrane fusion process is supported by the observed translocation of several crucial ER markers to the "bubble" structures of pyroptotic cells. Furthermore, α-MG-induced RTN4 knockdown leads to pyruvate kinase M2 (PKM2)-dependent conventional caspase-3/gasdermin E (GSDME) cleavages for pyroptosis progression. In vivo, we observed that chemical or genetic RTN4 knockdown significantly inhibited cancer cells growth, which further exhibited an antitumor immune response with anti-programmed death-1 (anti-PD-1). In translational research, RTN4 high expression was closely correlated with the tumor metastasis and death of patients. Taken together, RTN4 plays a fundamental role in inducing pyroptosis through the modulation of ER membrane curvature remodeling, thus representing a prospective druggable target for anticancer immunotherapy.
