1.Establishment and Evaluation of Diabetic Macrovascular Atherosclerosis Model with Qi and Yin Deficiency Syndrome

ObjectiveTo explore the establishment of a rat model of diabetic macrovascular atherosclerosis (DMA) with Qi and Yin deficiency syndrome induced by high-fat diet, streptozotocin (STZ), and Yin-depleting herbs, and to evaluate its biological characteristics. MethodsForty SD rats were randomly divided into a blank group (n=10) and a modeling group (n=30). Except for the blank group, rats in the model group were fed a high-fat diet for 4 weeks, followed by intraperitoneal injection of STZ (30 mg·kg-1) to establish a diabetic model. Twenty-four successfully modeled diabetic rats were randomly divided into a model group (n=7), a Qi and Yin deficiency syndrome group (n=8), and a counter-syndrome group (n=9). Except for the model group, rats received intragastric administration of Yin-depleting herbs (1.2 g·kg-1) for 8 weeks. The counter-syndrome group was further treated with Shenqi compound formula (1.69 g·kg-1) for an additional 8 weeks. General condition and body weight were recorded, and syndrome-related indicators were assessed, including precordial temperature, skin moisture content, grip strength, open-field test performance, and tongue appearance. Serum levels of vascular endothelial growth factor (VEGF), endothelin-1 (ET-1), vascular cell adhesion molecule-1 (VCAM-1), insulin-like growth factor-1 (IGF-1), and monocyte chemotactic protein-1 (MCP-1) were measured by enzyme-linked immunosorbent assay (ELISA). Fasting blood glucose, blood lipids, hemorheological parameters, and coagulation function were analyzed using an automatic biochemical analyzer. Vascular ultrasound and hematoxylin-eosin (HE) staining were used to evaluate vascular lesions. ResultsIn terms of syndrome manifestations, compared with the blank group, body weight increased rapidly during the first 5 weeks in the model, Qi and Yin deficiency, and counter-syndrome groups. After STZ injection combined with Yin-depleting herbal administration at week 5, body weight decreased significantly (P<0.01) and continued to decline until the end of the experiment. Rats exhibited decreased activity, irritability, coarse and yellowish fur with obvious shedding, polydipsia, polyphagia, frequent urination, and dry stools, which were most pronounced in the Qi and Yin deficiency group. Grip strength decreased, peak activity time occurred earlier, total distance in the open-field test was reduced, and residence time was prolonged. Precordial temperature decreased (P<0.01), while paw temperature increased (P<0.05), and skin moisture and oil content were reduced (P<0.05, P<0.01). In terms of disease-related indicators, compared with the blank group, fasting blood glucose was significantly increased (>16.7 mmol·L-1) in the model and Qi and Yin deficiency groups, and blood lipid levels were significantly elevated (P<0.05). Vascular-related factors ET-1, MCP-1, VCAM-1, and VEGF were significantly increased (P<0.05,P<0.01), while IGF-1 was significantly decreased (P<0.01). Pathological examination of the aortic valve showed valvular thickening and structural disorganization. Carotid artery examination revealed discontinuity of the intima, foam cell accumulation beneath the intima, disordered smooth muscle arrangement, and widened intercellular spaces. Compared with the model group, ET-1, MCP-1, and VEGF levels were significantly decreased in both the Qi and Yin deficiency group and the counter-syndrome group. The reductions in ET-1 and MCP-1 were more pronounced in the Qi and Yin deficiency group (P<0.01), while the decrease in VCAM-1 was more significant in the counter-syndrome group (P<0.05). Compared with the blank group, the Qi and Yin deficiency group showed significantly prolonged activated partial thromboplastin time (APTT), thrombin time (TT), and prothrombin time (PT) (P<0.01). The erythrocyte deformability index (TK), erythrocyte sedimentation rate, erythrocyte electrophoresis index, and whole blood low-shear viscosity all showed increasing trends. Vascular ultrasound revealed reduced arterial blood flow velocity, increased vascular resistance, and intimal thickening without plaque formation. The aortic intima showed no obvious overall thickening, with only occasional localized thickening and foam cell presence, and carotid artery injury was observed. ConclusionA rat model of DMA with Qi and Yin deficiency syndrome was successfully established using high-fat diet feeding combined with STZ injection and Yin-depleting herbal administration. Shenqi compound formula effectively alleviated Qi and Yin deficiency syndrome, regulated glucose and lipid metabolism, improved hemorheological and coagulation function, reduced vascular lesion severity, and demonstrated potential for early prevention and treatment of DMA.
2.Study on the changes of volatile components in Euphorbia wallichii after milk and wine processing

OBJECTIVE To systematically investigate the changes of volatile components in Euphorbia wallichii after milk and wine processing, and preliminarily elucidate the material basis for reducing toxicity. METHODS Using headspace gas chromatography-mass spectrometry technology, the volatile components in raw E. wallichii, milk-processed E. wallichii, and wine- processed E. wallichii were isolated and identified, and the relative percentage content of each component was calculated by the peak area normalization method. Combining chemometric methods such as principal component analysis and orthogonal partial least- squares discriminant analysis, changes in volatile components in samples after milk and wine processing were compared. Differential components were screened. RESULTS A total of 66 volatile components were identified from the three samples, with the types of compounds primarily comprising alkanes, olefins, heterocycles and esters, among others. A total of 39, 24 and 36 volatile components were identified from raw E. wallichii, milk-processed E. wallichii, and wine-processed E. wallichii, respectively, with 10 components common to all three preparations. Compared with raw E. wallichii, the relative percentage of other components in milk-processed E. wallichii decreased, except for alkanes and esters. The relative percentage of alkanes, olefins, aldehydes and esters in wine-processed E. wallichii increased, but the contents of heterocyclic compounds, ketones, ethers and alcohols decreased. The results of chemometric analysis showed that the volatile components of raw and processed products were significantly different. A total of 5 kinds of differential components in milk-processed products and 3 kinds of differential components in wine-processed products were screened out. Among them, the relative percentage of potential toxic components such as linalool, octanal and 3-pentanone decreased significantly after processing(P<0.05). CONCLUSIONS Milk and wine processing may exert a toxicity-reducing effect by reducing the contents of toxic components such as linalool, octanal and 3-pentanonein E. wallichii.

Background Swimming is becoming increasingly popular for its combined leisure and fitness benefits. However, polluted swimming pool water may pose various health risks. Objective:To investigate the effect of microRNA-363-5p targeted binding to THBS3 on angiotensin Ⅱ-induced apoptosis in cardiomyocytes and its molecular mechanism.Methods:A human-derived cardiomyocyte cell line(AC16)was used to establish an in vitro cardiomyocyte apoptosis model with angiotensin Ⅱ(AngⅡ),and a dual luciferase reporter assay was performed to detect the relationship between miR-363-5p and THBS3;apoptosis rate was detected by flow cytometry,and real-time fluorescence quantitative polymerase chain reaction(RT-qPCR)was performed to detect the relative expression of microR-363-5p,ATF-6mRNA,THBS3mRNA expression;Western Blot to detect the relative expression of caspase-12,caspase-3,GRP78,Bax,Bcl-2.Results:(1)Compared with the control group,the relative expression level of miR-363-5p in AngⅡ group was significantly decreased.(2)Compared with Mir-inhibitor-NC and Mir-mimics-NC groups,the apoptosis rate of miR-inhibitor and miR-mimics groups was significantly increased and decreased,the relative expression level of Bax protein was significantly increased and decreased,and the relative expression level of Bcl-2 was decreased and increased.(3)miR-363-5p targeted binding to THBS3.(4)Compared with the THBS3-OENC group,the relative expression of Bax and caspase-3 proteins was significantly higher,the relative expression of Bcl-2 protein was significantly lower,and the apoptosis rate was higher in the THBS3-OE group.(5)Compared with the negative control group,the relative expression of Bax and caspase-3 proteins in the miR-mimcs+THBS3-OE group was significantly higher,the relative expression of Bcl-2 protein was significantly lower,the apoptosis rate was significantly higher,and the cell viability was significantly lower.(6)Compared with the miR-inhibitor group,the relative expression of GRP78 and caspase-12 was decreased in the miR-inhibitor-4-PBA group,and the apoptosis rate was significantly reduced.(7)Compared with the negative control group,the apoptosis rate was elevated in the ATF-6-OE group,and the relative expression of caspase-12 was significantly increased.(8)Compared with the negative control group,miR-inhibitor+ATF-6 siRNA group showed decreased apoptosis rate and decreased relative expression of caspase-12.Conclusions:MicroRNA-363-5p is able to target binding to THBS3 to regulate myocardial apoptosis,a process that may be mediated through the endoplasmic reticulum stress ATF-6 pathway.
5.Role of peer attachment and cognitive fusion in relationship between negative life events and depressive symptoms

Objective:To examine the roles played by peer attachment and cognitive fusion in the association between negative life events and depression,as well as the differences in these relationships of variables among pri-mary and secondary school students.Methods:A total of 2 767 students(1 950 elementary school students and 817 middle school students)were selected.The Adolescent Life Events Scale,the Experiences in Close Relationships-Relationship Structures Questionnaire,the Cognitive Fusion Questionnaire,and the Patient Health Questionnaire were used to measure negative life events,peer attachment,cognitive fusion,and depression,respectively.Results:Negative life events,peer attachment,cognitive fusion and depression were positively correlated with each other(r=0.36-0.75,Ps<0.001);the mediation effects of peer attachment and cognitive fusion and their chain mediation effects were 0.04,0.30,and 0.04,respectively,accounting for 6.7%,50%,and 6.7%of the total effect.There were significant differences in the chain mediation model between primary and secondary school students(△x2=47.12,△df=6,P<0.001),and significant gender differences in the chain mediation effect among primary school students(△x2=43.72,△df=6,P<0.001).Conclusion:Negative life events influence depression in primary and secondary school students both directly and indirectly through peer attachment and cognitive fusion.Negative life e-vents have greater impact on primary school students'(particularly girls)peer attachment.Peer attachment has grea-ter impact on secondary school students' cognitive fusion.Cognitive fusion has greater impact on primary school students' depression.Peer attachment has greater impact on primary school boys' depression.
6.Benefits and challenges of reform in evidence-based obstetric nursing practice in clinical nursing:a qualitative study

Objective To investigate the benefits and challenges of the reform in evidence-based obstetric nursing practice in clinical nursing,therefore to provide a reference for initiating and keeping the reform in evidence-based nursing practices.Methods A method of descriptive phenomenological research was conducted between January and February 2024 in our hospital on 10 selected clinical nurses by purposive sampling.An outline for interviews was developed based on literature review and expert interviews,followed by semi-structured interviews with the nurses.Core themes were extracted with Colaizzi's seven-step analysis.Results Three themes and 12 sub-themes were summarised in this study.The 3 core themes that were identified among the nurses included:acceptance of evidence-based nursing practice reform(with 3 domains regarding knowledge,decision-making and action),perception of the benefits of the reform in evidence-based nursing practice(with 5 domains regarding the professionalism and scientific nature of clinical nursing,improvement of patient experience and nursing outcomes,enhancement of nursing quality and safety,promotion of innovation in on-the-job training and education,and facilitation of collaboration within the nursing team),and perception in the challenges of reform in evidence-based nursing practice(with 4 domains regarding the stress and challenges,doubts and resistance,poor cooperation between doctors and nurses,and risks in clinical decision-making).Conclusion The acceptance regarding reform in evidence-based nursing practice varies among the nurses.It is necessary to promote the acceptance,provide the targeted guidance and intervene in the factors of benefits and challenges among the nurses,hence to facilitate a smooth implementation of the reform in evidence-based nursing practice.
7.Association between cardiovascular health score based on life's essential 8 on cognitive function in older adults

Aim To investigate the association between cardiovascular health(CVH)score based on life's es-sential 8(LE8)on cognitive function and the mediation role of neutrophil-to-lymphocyte ratio(NLR)between them in ol-der adults,providing basis for the prevention and control of cognitive impairment.Methods The study population was selected from the National Health and Nutrition Examination Survey(NHANES)2011-2014 two cycles of 60 years and a-bove,demographic characteristics,medical history and other variables were collected,the participants'CVH scores were calculated according to LE8 defined by American Heart Association,and cognitive function was assessed using the digit symbol substitution test.The relationship between CVH and cognitive function was explored using a weighted linear re-gression analysis,and the dose-response relationship was analyzed using a restricted cubic spline.Moreover,mediation a-nalysis was used to explore the role of NLR in mediating the relationship between CVH score and cognitive function.Results A total of 2 510 participants were included in this study.After adjusting for relevant factors,cognitive function was positively associated with moderate CVH group score(β=3.36,95%CI:1.43~5.30)and high CVH group score(β=6.01,95%CI:2.89~9.13)compared with the low CVH group score.The LE8 single factor of diet score(β=0.04,95%CI:0.01~0.06),physical activity score(β=0.04,95%CI:0.02~0.05),tobacco exposure score(β=0.05,95%CI:0.02~0.08),blood pressure score(β=0.03,95%CI:0.01~0.05)and blood glucose score(β=0.06,95%CI:0.03~0.09)were positively correlated with cognitive function score.The results of restricted cubic splines showed that the score of CVH was linear positively correlated with the score of cognitive function.The indirect effect of CVH on cognitive function via NLR was 0.002(P>0.05),and the proportion of mediating effect was 1.29%(P>0.05),indicating the mediating effect of NLR on the relationship between CVH score and cognitive function was not signif-icant.Conclusions The CVH score based on the LE8 was positively correlated with cognitive function in older adults,underscoring the crucial role of maintaining optimal CVH levels in supporting cognitive health.However,the NLR,as an indicator of inflammation,did not exhibit a significant mediating effect between the CVH score and cognitive function.The relationship between these variables might be influenced by other mechanisms.
8.Exploration of competency-oriented standardized nutritional diagnosis and treatment training for clinical physicians

Objective:To investigate the application and effect of a competency-oriented teaching model in standardized nutritional diagnosis and treatment training for clinical physicians.Methods:A blended teaching method combining online and offline lectures was used to teach core knowledge and skills of clinical nutrition among clinical physicians by implementing a step-by-step "popularization-strengthening-deepening" strategy. The number of nutritional consultations, the improvement in nutritional diagnosis and treatment among clinical physicians, and the degree of satisfaction after training were used as assessment indices.Results:Compared with the data in 2021, the number of annual nutritional consultations was increased by 21.41% in 2022 and 53.18% in 2023. A total of 281 clinical physicians received online deepening course training, among whom 237 (84.34%) completed the online clinical nutrition knowledge test, with a mean score of (86.17±5.48) points and a pass rate of 81.86% (194/237). The online training received a satisfaction rate of 80.39%.Conclusions:The training program designed with competency-based objectives, systematic content, and diverse methodologies can significantly enhance the standardized thinking and capabilities of clinical physicians in nutritional diagnosis and treatment.
9.Effect of rhein on IgA nephropathy in rats by improving spleen immune function based on mTORC1 pathway and its mechanism

Aim To explore the mechanism of rhein in the treatment of IgA nephropathy in rats by impro-ving spleen immune function based on mTORC1 path-way.Methods Thirty-two SD rats were randomly di-vided into control group,model group(IgAN),rhein group(RH)and rapamycin group.The deposition of IgA in mesangial region and pathological changes of spleen and kidney were observed,and the ratio of Th/Ts in spleen was determined by immunohistochemical method.The protein expressions of p-mTOR,p-p70S6k,TGF-β1 and TNF-α in spleen were detected by Western blot.Results Compared with the control group,IgAN group significantly increased urinary sludge red blood cell count,24 hour urinary total pro-tein,serum creatinine,blood urea nitrogen,serum IgA,Th/Ts,p-mtor,p-P70S6K and spleen TNF-αprotein expression levels(P<0.01),and spleen TGF-β1 protein expression(P<0.05).IgA deposi-tion in mesangial area of the kidney increased signifi-cantly,and pathological changes were observed in spleen and kidney.Compared with IgAN group,urine sludge red blood cell count,serum IgA,Th/Ts,p-mTOR,p-P70S6K and TNF-α protein expression lev-els in RH and RAPA groups were significantly reduced(P<0.01).24 hour urinary total protein,blood urea nitrogen,serum creatinine and spleen TGF-β1 protein decreased(P<0.05),the deposition of IgA in the mesangial region of the kidney decreased,and the pathological changes of spleen and kidney were allevia-ted.There was no significant difference between RH group and RAPA group.Conclusions RH can allevi-ate IgA nephropathy by inhibiting the activity of m-TORC1 pathway in the spleen of IgA nephropathy rats,thereby inhibiting the differentiation of splenic lympho-cytes and reducing the secretion of IgA.
10.Effects of Aqueous Extract of Corn Silk Combined with Training on Exercise Function and Glycolipid Metabolism in Mice with Metabolic Syndrome

Objective:To investigate the effects of combination therapy with aqueous extract of corn silk(CS)and training on exercise capacity and glycolipid metabolism in mice with metabolic syndrome(MS).Methods:In this study,db/db mice were used as the animal model of MS.The mice were administered aqueous extract of CS via gavage and subjected to different intensities of training for 12 weeks(3 months).The specific experimental design was as follows:24 db/db mice were randomly divided into four groups on average:negative control group(NC),aqueous extract of CS group(CS),aqueous extract of CS+moderate-intensity training group(CS+MT),and CS aqueous extract of CS+high-intensity training group(CS+HT).The maximum running speed,forelimb grip strength,body weight and fasting blood glucose of mice were measured before and after treatment.After the intervention,oral glucose tolerance test(OGTT)and insulin tolerance test(ITT)were conducted to assess glucose metabolism,while serum triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)levels were measured to evaluate lipid metabolism.Results:After 3 months of intervention,there were significant differences in the maximum running speed and forelimb grip strength among the four groups(P<0.05).The maximum running speed and forelimb grip strength of CS group,CS+MT group and CS+HT group were higher than those of NC group(P<0.05).The CS+MT group exhibited higher forelimb grip strength,and the CS+HT group showed higher maximum running speed and forelimb grip strength compared to the CS group(P<0.05),while no significant difference was found between the CS+MT and CS+HT groups(P>0.05).Significant differences in body weight were observed among the four groups after 3 months of intervention(P<0.05).Specifically,the CS+MT and CS+HT groups exhibited significantly lower body weight compared to both the NC and CS groups(P<0.05),with the CS+MT group having the lowest body weight(P<0.05).Fasting blood glucose levels also differed significantly among the groups after 2 and 3 months of intervention(P<0.05).The CS,CS+MT,and CS+HT groups had lower fasting blood glucose levels compared to the NC group(P<0.05),with the CS+MT and CS+HT groups showing the lowest levels(P<0.05).No significant difference was found between the CS+MT and CS+HT groups(P>0.05).After 3 months of intervention,significant differences in the area under the curve(AUC)of OGTT and ITT were observed among the four groups(P<0.05).The AUC of OGTT and ITT were significantly lower in the CS,CS+MT,and CS+HT groups compared to the NC group(P<0.05).The CS+MT and CS+HT groups exhibited the lowest AUC values for both OGTT and ITT(P<0.05),with the CS+MT group showing the lowest AUC for OGTT(P<0.05).Significant differences in serum lipid levels were observed among the four groups after 3 months of intervention(P<0.05).TG,TC,and LDL-C levels were significantly lower,while HDL-C levels were higher in the CS,CS+MT,and CS+HT groups compared to the NC group(P<0.05).The CS+MT group had the lowest TG levels and the highest HDL-C levels compared to the CS+HT group(P<0.05),with no significant differences in TC and LDL-C levels between these two groups(P>0.05).Conclusion:Aqueous extract of CS combined with different intensity training can significantly improve the exercise capacity and glycolipid metabolism of MS mice and reduce body weight,especially CS combined with MT treatment is more effective in improving lipid metabolism.In addition,when combined with HT,aqueous extract of CS can also play an auxiliary role in reducing the side effects of high-intensity exercise and improving the therapeutic effect.
