1.Significance of basophil levels in prognostic evaluation of intra-abdominal infection

Objective:To assess the relationship between basophil levels and mortality in patients with intra-abdominal infection.Methods:Information on patients with intraperitoneal infection admitted to the intensive care unit were extracted from the MIMIC database.A time-dependent Cox regression model was used to adjust for confounders associated with 28-day mortality.Propensity score matching(PSM)was used to balance the baseline differences be-tween groups with different basophil levels,and a restricted cube chart(RCS)was used to show the relationship between basophil count and 28-day mortality in patients with intra-abdominal infection.Results:A total of 4403 patients with intra-abdominal infection were enrolled in the MIMIC database.Patients with high basophil levels have lower mortality than those with low basophil levels.There was an L-shaped curve between basophil level and 28-day mortality,with a cut-off value of 0.47×109/L.Cox regression analysis showed that basophil levels were an independent protective factor for mortal-ity in patients with intra-abdominal infection after adjusting for potential confounders(HR=0.586,95%CI:0.443-0.769).Protective factors for death at basophil levels remained after PSM adjusted for potential confounders(HR=0.628,95%CI:0.470-0.832).Conclusion:Basophil level is an independent protective factor for mortality in patients with intra-abdominal infection,and basophil levels should be dynamically monitored to better evaluate the prognosis of patients.

3.Weka-based classification and optimization of acute lymphocytic leukemia images

Objective To propose a Weka-based method for classifying acute lymphoblastic leukemia(ALL)images,aiming to improve ALL cell classification accuracy and stability.Methods Firstly,totally 180 images were randomly selected from ALL-IDB2 subset of Acute Lymphoblastic Leukemia Image Database(ALL-IDB),including 90 images of patients and 90 images of healthy people;secondly,the image preprocessing was carried out using ImageJ software and image features were extracted such as texture,edge and shape;thirdly,image classification was implemented with four classifiers of Weka,including random forest(RF),Bayesian network(BN),J48 decision tree and sequential minimal optimization(SMO),and the key parameters of each classifier were optimized;finally,the performance of the classifiers was verified using 80 independent test images.Results Before parameter optimization,the accuracy of RF,J48 decision tree,BN and SMO classifiers was 94.3%,86.2%,83.6%and 83.0%,respectively.After optimization,the accuracy increased to 95.2%,86.3%,86.3%and 89.7%,respectively.After optimization,RF behaved the best on the independent test set with a classification accuracy of 90.0%,followed by SMO(81.3%),BN(81.3%)and J48 decision tree(75.0%).Conclusion The Weka-based ALL image classification method with a high accuracy is efficient and reliable for automated classification of ALL cell.[Chinese Medical Equipment Journal,2025,46(2):10-15]
4.Fucoidan Provokes Ferroptosis via Inhibition of the PI3K/Akt Signaling Pathway in Human Osteosarcoma 143B Cells

Fucoidan(FUC)is a natural seaweed-derived drug.Previously,our experiments have shown that FUC can significantly inhibit the cell viability of human osteosarcoma 143B cells and induce cell death,but the mechanism remains unclear.Ferroptosis,a novel form of cell death,has emerged as an important target for tumor therapy.This study aims to investigate whether FUC induces ferroptosis of 143B cells and elucidate its underlying molecular mechanisms.CCK-8 and LDH assays result showed that FUC(10,100,400 μg/mL)significantly reduced cell viability of 143B cells and induced cell death.Calce-in-AM staining,FeRhoNox-1 staining,and C11 BODIPY 581/591 staining indicated that FUC obviously increased the levels of labile iron pool(LIP),Fe2+,and lipid reactive oxygen species(Lip ROS)in 143B cells.Chemical colorimetric analysis revealed that FUC markedly decreased intracellular Glutathi-one(GSH)contents.Real-time quantitative PCR showed that FUC dramatically reduced the mRNA lev-els of ferroptosis-related factors solute carrier family 7 member 11(SLC7A11)and glutathione peroxidase 4(GPX4),while increasing the mRNA levels of prostaglandin endoperoxide synthase 2(PTGS2)and acyl-CoA synthetase long-chain family member 4(ACSL4).Western blotting analysis demonstrated that FUC significantly reduced the protein levels of SLC7A11 and GPX4,and the ratios of p-PI3K/PI3K,p-AktSer473/Akt,and p-AktThr308/Akt,but increased the protein level of ACSL4.Immunofluorescence staining showed that FUC obviously inhibited the nuclear translocation of p-AktSer473.The ferroptosis in-hibitor ferrostatin-1(Fer-1)and iron chelator deferoxamine(DFO)remarkably suppressed cell death in-duced by FUC in 143B cells.Additionally,the PI3K/Akt pathway activator 740Y-P significantly inhibi-ted FUC-induced iron overload and lipid peroxidation in 143B cells,and restored the protein levels of SLC7A11 and GPX4.In conclusion,FUC can induce ferroptosis of 143B cells by inhibiting the PI3K/Akt signaling pathway,which may be a potential target for the prevention and treatment of osteosarcoma.
5.The impact of county-level"Unified ECG Network"construction on the treatment efficiency and clinical outcomes of patients with acute ST-segment elevation myocardial infarction

Objective To investigate the impact of county-level"Unified ECG Network"construction on the treatment efficiency and clinical outcomes of patients with acute ST-segment elevation myocardial infarction(STEMI).Methods A retrospective analysis was conducted on the clinical data of STEMI patients from Beichuan County and Yanting County in Mianyang City,and Jiange County in Guangyuan City,Sichuan Province,during the 18 months before(128 cases)and 18 months after(187 cases)the establishment of the"Unified ECG Network."Differences in demographic characteristics,treatment efficiency,therapeutic methods,and clinical outcomes between the two groups were compared.Results There was no statistically significant difference in general demographic characteristics between the two groups(all P>0.05).Compared with the pre-construction group,the post-construction group showed significantly shorter times in initial ECG completion[5(3,7)min vs.6(4,8)min],initial ECG diagnosis[3(2,4)min vs.5(2,6)min],first medical contact to preliminary diagnosis[10(9,12)min vs.13(11,15)min],network hospital door-in-door-out time[21(19,23)min vs.26(23,30)min],and first medical contact to wire-crossing time[(94.82±11.87)min vs.(107.97±18.39)min](allP<0.001).The proportion of patients bypassing the emergency department and coronary care unit significantly increased(64.17%vs.32.81%,P<0.001).The proportion of patients undergoing emergency percutaneous coronary intervention significantly increased(72.73%vs.51.56%,P<0.001),while the proportions of thrombolytic therapy and non-reperfusion therapy significantly decreased(both P<0.05).Additionally,in-hospital mortality rate,Killip class≥Ⅱ proportion,incidence of major adverse cardiovascular events,and average length of hospital stay were all significantly reduced(all P<0.05).There were no statistically significant differences among the three county-level chest pain centers in terms of major treatment efficiency,therapeutic strategies,or clinical outcomes(all P>0.05).Conclusions The construction of the county-level"Unified ECG Network"can significantly improve the treatment efficiency of STEMI patients,optimize reperfusion therapy strategies,improve clinical outcomes,and demonstrate substantial clinical promotion value.
6.Application value of fluticasone propionate combined with loratadine syrup and montelukast sodium in children with asthma and allergic rhinitis

Objective:To investigate the therapeutic effects of fluticasone propionate combined with loratadine syrup and montelukast sodium on asthma and allergic rhinitis in children.Methods:A total of 119 children with asthma and allergic rhinitis, admitted to Department of Pediatrics, Dongyang Maternal and Child Health Hospital from March 2021 to March 2024, were selected for this retrospective study. Based on the different treatment methods recorded in the medical record system, the children were divided into two groups: the control group ( n = 70), which received loratadine syrup combined with montelukast sodium, and the observation group ( n = 49), which received fluticasone propionate combined with loratadine syrup and montelukast sodium. Clinical symptoms (rhinitis and asthma), serum markers (immunoglobulin E, interleukin-4, and interleukin-6), lung function (maximum peak expiratory flow rate, forced expiratory volume in the first second, forced expiratory flow at 25% of vital capacity, forced expiratory flow at 50% of vital capacity, forced expiratory flow at 75% of vital capacity), as well as adverse reactions and clinical efficacy, were observed and compared between the two groups. Results:After treatment, the symptom scores for both rhinitis and asthma in the observation group were significantly lower than those in the control group [(3.65 ± 0.88) vs. (4.55 ± 1.06), t = 4.88, P < 0.001; (2.15 ± 0.56) vs. (3.28 ± 0.89), t = 7.86, P < 0.001]. The levels of immunoglobulin E, interleukin-4, and interleukin-6 in the observation group were significantly lower than those in the control group [(0.31 ± 0.18) g/L vs. (0.42 ± 0.19) g/L, t = 3.18, P = 0.002; (56.83 ± 8.25) ng/L vs. (62.16 ± 9.94) ng/L, t = 3.08, P = 0.003; (32.48 ± 3.11) ng/L vs. (39.61 ± 4.72) ng/L, t = 9.26, P < 0.001]. The values for forced expiratory volume in the first second and forced expiratory flow at 25%, 50%, and 75% of vital capacity in the observation group were significantly higher than those in the control group [(87.18 ± 5.15)% vs. (84.36 ± 4.68)%, t = 3.10, P = 0.002; (89.35 ± 3.88)% vs. (83.25 ± 3.56)%, t = 8.86, P < 0.001; (74.36 ± 6.65)% vs. (65.25 ± 5.36)%, t = 8.26, P < 0.001; (71.68 ± 8.76)% vs. (65.35 ± 7.86)%, t = 4.12, P < 0.001; (69.24 ± 4.38)% vs. (65.35 ± 2.43)%, t = 6.20, P < 0.001]. The incidence of adverse reactions was significantly higher in the observation group than in the control group [8.16% (4/49) vs. 4.29% (3/70), χ2 = 0.60, P = 0.439]. The overall response rate was significantly higher in the observation group than in the control group [95.92% (47/49) vs. 82.86% (58/70), χ2 = 5.26, P = 0.022]. Conclusions:Fluticasone propionate combined with loratadine syrup and montelukast sodium can effectively alleviate symptoms in children with asthma and allergic rhinitis, improve serum indicators, enhance lung function, and increase clinical efficacy without substantially increasing adverse reactions, demonstrating high safety.

8.Design of new type of elastic gloves for burn scar rehabilitation

Objective To design a new type of elastic gloves for burn scar rehabilitation to solve the problems of conventional elastic gloves in pressure distribution,elasticity maintenance and absorption of sweat stains.Methods The new elastic gloves was made of non-woven fabric by spandex material,which was composed of external and internal parts.The main body of the external part was used as the primary structure of the gloves,which was provided with a sealing strip,a storage bag,a drawstring,etc.The internal part consisted of a bonding sheet,an elastic band,a fiber sheet,an absorbent sponge,some breathable holes,etc.Results The new elastic gloves designed could be used for the pressure therapy for the scars on the opisthenar,palm side,finger web and purlicue with scar proliferation inhibitted effectively,and the breathable hole and absorbent sponge contributed to the absorption of the sweat of the patient.Conclusion The new type of elastic gloves gains advantages in elasticity,wearing comfort and effectiveness of the pressure therapy for purlicue and finger web,and can be used for the pressure therapy to inhibit proliferative scarring after burns.[Chinese Medical Equipment Journal,2025,46(8):118-120]
9.Evaluation of clinical application of high value electrode in plasma surgery on the basis of real world data

Objective:To carry out differential study for clinical application of the domestic and imported electrode of plasma surgery by using real world data,so as to provide data support for clinical selection of electrode products in plasma surgery.Methods:An observational study was adopted,and a total of 384 patients who underwent adenoidectomy by using domestic electrode of plasma surgery(domestic electrode)or imported electrode of plasma surgery(imported electrode)at Children's Hospital,Zhejiang University School of Medicine between March and October 2024 were selected.The data of patients who received domestic or imported electrode were recorded,respectively.The indicators of safety and effectiveness,and economic indicator of surgeries adopted respectively two kinds of electrodes for patients were compared,and the differences of clinical application of the two kinds of electrodes were analyzed.Results:The average hospitalization cost of patients who adopted imported electrode was(11,300±2,500)CNY,which was higher than(7,600±1700)CNY,and the difference was statistically significant(t=-17.546,P<0.05).The surgical duration,postoperative heart rate,postoperative hypotension and hypertension of patients who adopted domestic electrode to conduct adenoidectomy were higher than those of patients who adopted imported electrode,while there were not significant differences between them(P>0.05).The incidences of damage and infection of eustachian cushion of patients who adopted domestic electrode to conduct adenoidectomy were higher than those of patients who adopted imported electrode,and the incidence of bleeding was lower than that of patients who adopted imported electrode,while the differences were not significant(P>0.05).Conclusion:Domestically plasma surgical electrode can effectively save patients'medical costs,which is safe and reliable in clinical use,and it can be mutually replaceable with the imported plasma surgical electrode..
