Objective To investigate the current symptom status of maintenance hemodialysis(MHD)patients, analyze the influencing factors from the perspective of multidimensional symptom burden, extract symptom groups, and construct a symptom network. Methods A total of 490 patients who received MHD at the blood purification center of a tertiary hospital in Shanghai from March to April 2025 were selected using a convenience sampling method.30 symptoms of MHD patients were investigated using a modified dialysis symptom index table.SPSS and R software were utilized to analyze the influencing factors, extract symptom groups, and construct a multidimensional symptom network. Results A total of 488 valid questionnaires were collected, with a valid questionnaire recovery rate of 99.59%.The median number of symptoms experienced by patients was 6(interquartile range:4,8),with the top 5 symptoms being dry skin(69.88%),fatigue or tiredness(56.97%),itching(56.97%),worry(50.20%),and dry mouth(46.31%).Multidimensional analysis indicated that age, duration of dialysis, education level, and complications were influencing factors of symptom burden scores across different dimensions.6 stable symptom groups were extracted, namely skin discomfort, fatigue and pain, gastrointestinal tract, sleep disorders,water-electrolyte imbalance, and uremia. The results of the symptom network analysis revealed that difficulty of falling asleep had the highest intensity centrality(rs=2.18,2.25,2.12).Conclusion The symptom association is relatively stable, and difficulty of falling asleep is the core symptom. This finding can assist clinical medical staff in achieving efficiency and precision in symptom group management.

ObjectiveTo explore the current situation of forensic ethics practice and education by designing a questionnaire on forensic ethics, with a view to exploring the path of forensic ethics education construction. MethodsA total of 667 valid questionnaires were collected using the online survey method, basically covering various regions across the country and all sub-specialties of forensic medicine. Descriptive analysis was used to analyze the relevant data. ResultsMost practitioners had relevant ethical reflections in the process of forensic practice. 69.12% of the respondents indicated that they had studied the relevant rules, but approximately half stated that there were no corresponding ethical norms or standard operating manuals. The specific behaviors violating ethics in different units were diverse. 23.04% of the respondents reported that they had encountered unethical behaviors, but only 4.9% of them reported such violations. In terms of forensic ethics education, 87.75% of the respondents believed that there were issues with the current model of forensic ethics education. Meanwhile, the respondents showed a high degree of recognition for receiving forensic ethics education, with 84.15% of respondents expressing willingness to participate in relevant courses. More than half of respondents were willing to participate in forensic ethics education during undergraduate studies, new employee training, and regular post-employment training. ConclusionCurrently, there is a problem of ethical neglect in forensic work in China. Combining ethics courses with professional courses at the practitioner training stage and providing regular training at the practice stage are effective measures to popularize forensic ethics knowledge, enhance ethical awareness, and improve the quality of practice.

Antibody humanization is critical to reduce immunogenicity and enhance efficacy in the preclinical phase of the development of therapeutic antibodies originated from animal models.Computational suggestions have long been desired, but available tools focused on immunogenicity calculation of whole antibody sequences and sequence segments, missing the individual residue sites. This study introduces Site-specific Immunogenicity for Therapeutic Antibody(SITA),a novel computational framework that predicts B-cell immunogenicity score for not only the overall antibody, but also individual residues, based on a comprehensive set of amino acid descriptors characterizing physicochemical and spatial features for antibody structures.A transfer-learning-inspired framework was purposely adopted to overcome the scarcity of Antibody-Antibody structural complexes. On an independent testing dataset derived from 13 Antibody-Antibody structural complexes,SITA successfully predicted the epitope sites for Antibody-Antibody structures with a receiver operating characteristic(ROC)-area unver the ROC curve(AUC)of 0.85 and a precision-recall(PR)-AUC of 0.305 at the residue level.Furthermore, the SITA score can significantly distinguish immunogenicity levels of whole human antibodies, therapeutic antibodies and non-human-derived antibodies.More importantly, analysis of an additional 25 thera-peutic antibodies revealed that over 70%of them were detected with decreased immunogenicity after modification compared to their parent variants.Among these, nearly 66%antibodies successfully iden-tified actual modification sites from the top five sites with the highest SITA scores, suggesting the ability of SITA scores for guide the humanization of antibody.Overall, these findings highlight the potential of SITA in optimizing immunogenicity assessments during the process of therapeutic antibody design.

Objective:To observe the effect of Wenyang Huazhuo recipe (WYHZ) on pregnancy outcomes in patients with unexplained recurrent spontaneous abortion (URSA).Methods:A retrospective cohort study was used and 240 cases of pregnancy URSA patients with spleen kidney yang deficiency syndrome at the Reproductive Center, First Teaching Hospital of Tianjin University of Traditional Chinese Medicine and Department of Gynaecology and Obstetrics, General Hospital of Tianjin Medial University from November 2018 to March 2022 were recruited. The patients were assigned to WYHZ group (120 cases) and control group (120 cases) based on whether accepting oral WYHZ from 3 months before pregnancy to 8-12 weeks after pregnancy regularly. Pregnancy outcomes, perinatal period outcomes and obstetric complications were compared between the two groups. Multivariate logistic regression were used to analyze the effect of WYHZ on pregnancy outcome. Results:The live birth rate and the clinical pregnancy rate in WYHZ group [86.67% (104/120), 90.00% (108/120)] were higher than those in control group [71.67% (86/120), P=0.004; 75.00% (90/120), P=0.002]. The early abortion rate in WYHZ group [10.08% (12/119)] was lower than that in control group [25.86% (30/116), P=0.002], the differences were statistically significant. There were no significant differences in perinatal outcome and incidence rate of obstetric complications between the two groups (all P>0.05). Multivariate logistic regression analysis showed that the application of WYHZ significantly improved the live birth rate ( OR=8.818, 95% CI: 3.556-21.871, P<0.001) and sustained pregnancy rate ( OR=11.261, 95% CI: 4.262-29.751, P<0.001) in URSA patients. Conclusion:WYHZ using before and during pregnancy can significantly improve the live birth rate and the clinical pregnancy rate of URSA patients with syndrome of yang deficiency of spleen and kidney, and improve their pregnancy outcomes.

Objective:To observe the impact of a home-based intervention based on video feedback on the social, cognitive and fine motor skills of children with autism spectrum disorder (ASD).Methods:Fifty children with ASD were randomly divided into a control group and an observation group, each of 25. In addition to conventional rehabilitation training, the control group received routine comprehensive family intervention, while the observation group′s family intervention used video feedback. Before and after 6 months, all of the children′s social, cognitive and fine motor skills were assessed using the Psychoeducational Profile instrument (PEP-3), an aberrant behavior checklist (ABC), the Childhood Autism Rating Scale (CARS), an autism treatment evaluation checklist (ATEC) and a neuropsychological development scale (CNDS) for children up to 6 years old. Results:Before the treatment there was no significant difference between the 2 groups in any of the measurements. Afterward, significant improvement was observed in all of the measurements in both groups. After the treatment, however, the experimental group′s average social interaction, cognition and personal self-care scores on the PEP-3 scale, as well as their average fine motor skill, adaptive capacity, language use and personal social ability were all significantly better than the control group′s averages. At the same time, the experimental group′s average ABC, CARS, communication, sensation, social ability and behavior results were significantly better. Conclusions:Video feedback can effectively improve family interventions stimulating the social and cognitive abilities of children with ASD, as well as their fine motor skills.
