1.Differential Peripheral NLRP3 Inflammasome Expression in Patients With Parkinson’s Disease and Patients With Multiple System Atrophy

Objective:
The NOD-like receptor family pyrin domain-containing 3 (NLRP3) inflammasome has been proposed to be a downstream mediator of neuroinflammation in individuals with Parkinson’s disease (PD). However, its involvement across disease stages and related synucleinopathies, such as multiple system atrophy (MSA), remains unclear. We aimed to analyze the peripheral mRNA expression of NLRP3-related genes and cytokines across individuals with isolated REM sleep behavior disorder (iRBD), early-stage PD, late-stage PD, and MSA.
Methods:
Peripheral blood mononuclear cells (PBMCs) were collected from 151 participants: 35 healthy controls (HCs), 31 patients with iRBD, 41 patients with early-stage PD, 21 patients with late-stage PD, and 23 patients with MSA. mRNA expression was measured using quantitative real-time polymerase chain reaction. Statistical comparisons were performed using analysis of variance (ANOVA) or Welch’s ANOVA, and associations with clinical variables were analyzed through stepwise multiple linear regression.
Results:
NLRP3 expression was significantly lower in patients with iRBD (p=0.0263) and patients with early-stage PD (p= 0.0101) than in HCs. NIMA-related kinase 7 (NEK7) expression progressively decreased across the disease spectrum (HCs vs. patients with early-stage PD, p=0.0008; vs. patients with late-stage PD, p<0.0001). In contrast, apoptosis-associated speck-like protein containing a caspase recruitment domain (ASC) and caspase-1 were elevated in patients with PD, especially those in the late stages. Levels of patients with MSA resembled those of HCs but differed from those of patients with PD. Interleukin (IL)-1β and IL-18 levels were not significantly different. In patients with early-stage PD, NLRP3 expression was negatively correlated with disease duration, the Movement Disorder Society-Unified Parkinson’s Disease Rating Scale Part II score, and the cognitive score.
Conclusion
Our findings challenge the prevailing hypothesis that NLRP3 inflammasome activation directly contributes to PD pathogenesis. Instead, the observed increase in ASC and caspase-1 expression suggests the potential involvement of alternative inflammasome pathways during disease progression.
2.Large-scale salmonella outbreak associated with banh mi, Viet Nam, 2024

Objective: To investigate the cause of a foodborne outbreak that occurred in Dong Nai province, Viet Nam, in 2024, and implement control measures.
Methods: An initial investigation was conducted to confirm the outbreak, which was followed by epidemiological and environmental investigations to find the plausible causative food item. Clinical specimens and food samples were tested to identify the pathogen.
Results: A total of 547 symptomatic cases were recorded, of whom two were in severe condition requiring extracorporeal membrane oxygenation and ventilation, one of whom died. Among 99 interviewed cases, the mean incubation time was 9 hours (range 2–24 hours), with the main symptoms being fever, abdominal pain, diarrhoea and vomiting. All patients had eaten banh mi from a local bakery. Salmonella spp. were identified in food samples and clinical specimens. The bakery halted production, and the outbreak ended after 1 week.
Discussion: All the patients were exposed to only one food in common, which facilitated the investigation process. This outbreak is a reminder to small retailers and take-away shops of the importance of food safety management in preventing similar future outbreaks. All food handlers must comply with food hygiene principles, especially in hot temperatures, which boosts bacterial growth.

Objectives:
The prevalence of posttraumatic stress disorder (PTSD) has increased, particularly among individuals who have recovered from coronavirus disease 2019 (COVID-19) infection. Health literacy is considered a “social vaccine” that helps people respond effectively to the pandemic. We aimed to investigate the association between long COVID-19 and PTSD, and to examine the modifying role of health literacy in this association.
Methods:
A cross-sectional study was conducted at 18 hospitals and health centers in Vietnamfrom December 2021 to October 2022. We recruited 4,463 individuals who had recovered from COVID-19 infection for at least 4 weeks. Participants provided information about their sociodemographics, clinical parameters, health-related behaviors, health literacy (usingthe 12-item short-form health literacy scale), long COVID-19 symptoms and PTSD (Impact Event Scale-Revised score of 33 or higher). Logistic regression models were used to examine associations and interactions.
Results:
Out of the study sample, 55.9% had long COVID-19 symptoms, and 49.6% had PTSD.Individuals with long COVID-19 symptoms had a higher likelihood of PTSD (odds ratio [OR], 1.86; 95% confidence interval [CI], 1.63–2.12; p < 0.001). Higher health literacy was associated with a lower likelihood of PTSD (OR, 0.98; 95% CI, 0.97–0.99; p = 0.001). Compared to those without long COVID-19 symptoms and the lowest health literacy score, those with long COVID-19 symptoms and a 1-point health literacy increment had a 3% lower likelihood of PTSD (OR, 0.97; 95% CI, 0.96–0.99; p = 0.001).
Conclusion
Health literacy was found to be a protective factor against PTSD and modified the negative impact of long COVID-19 symptoms on PTSD.
4.Mental Health Among Healthcare Workers During the COVID-19 Pandemic in Vietnam

Objectives:
The objective of this study was to characterize mental health issues among Vietnamese healthcare workers (HCWs) and to identify related factors.
Methods:
A cross-sectional study was conducted with 990 HCWs in 2021. Their mental health status was measured using the Depression, Anxiety, and Stress Scale.
Results:
In total, 49.9%, 52.3%, and 29.8% of respondents were found to have depression, anxiety, and stress, respectively. The multivariable linear regression model revealed that factors associated with increased anxiety scores included depression scores (β, 0.45; 95% confidence interval [CI], 0.39 to 0.51) and stress scores (β, 0.46; 95% CI, 0.41 to 0.52). Factors associated with increased depression scores included being frontline HCWs (β, 0.57; 95% CI, 0.10 to 1.10), stress scores (β, 0.50; 95% CI, 0.45 to 0.56), and anxiety scores (β, 0.41; 95% CI, 0.36 to 0.47), while working experience was associated with reduced depression scores (β, -0.08; 95% CI, -0.16 to -0.01). Factors associated with increased stress scores included working experience (β, 0.08; 95% CI, 0.00 to 0.16), personal protective equipment interference with daily activities (β, 0.55; 95% CI, 0.07 to 1.00), depression scores (β, 0.54; 95% CI, 0.48 to 0.59), and anxiety scores (β, 0.45; 95% CI, 0.39 to 0.50), while age was associated with reduced stress scores (β, -0.12; 95% CI, -0.20 to -0.05).
Conclusions
Specific interventions are necessary to enhance and promote the mental health of HCWs so they can successfully cope with the circumstances of the pandemic.

MENTAL HEALTH CARE NEEDS OF STUDENTS AT HO CHI MINH CITY OPEN UNIVERSITY, VIETNAM
Abstract
Objective: Today’s life with changes in many aspects has brought people significant
advantages and conveniences, but at the same time it has also caused physical and
mental consequences for each person, especially mental health problems. Science has
had many studies showing the impact of mental health on the quality of life, work
efficiency, learning outcomes, etc. Therefore, when facing mental health problems,
individuals need help to overcome the psychological problems they are facing to have
healthier and more positive mental health. The scope of this study refers to the need
for mental health care of students at Ho Chi Minh City Open University.
Methodology: This study, based on theories of mental health, models and solutions
to support mental health care, we built a questionnaire with 05 groups of basic needs
to help students improve their physical and mental health for healthy mental health.
Specifically, the care needs for self-satisfaction; The care needs for healthy emotional
expression; the need to care for satisfaction in relationships with others; the full care
needs are able to maintain a stable psychological state in life and the care needs
for improving the ability to cope with difficulties in learning and working. After
completion, we conducted a pilot survey on a small sample to check the reliability of
the questionnaire and calibrate the scale. With an alpha coefficient of 0.98-meaning
that the scale has a high reliability coefficient.
Results: The mean score results show that all 05 groups of needs included in the study
are of interest to the subjects and all have a desire to be met. Specifically: (1) The care
needs for self-satisfaction has an mean score of 3.13 and a standard deviation of 0.73,
a high level of need on the rating scale; care needs for expressing healthy emotions
has an mean score of 3.45 and a standard deviation of 0.58, a very high level of need
on the rating scale; the care needs for satisfaction in relationships with others has
an mean score of 3.34 and a standard deviation of 0.65, a very high level of need on
the rating scale; the full care needs are able to maintain a stable psychological state
in life has an mean score of 4.42 and a standard deviation of 0.60, a very high need
level in the assessment scale and the care needs for improving the ability to cope
with difficulties in learning and working has an mean score of 3.19 and a standard
deviation of 0.74, a high need level in the assessment scale.
Conclusion: The research results show that students of Ho Chi Minh City Open
University have a very high level of mental health care needs. Students clearly show
mental health care needs in all the contents that need to be cared for. Among the
component needs of mental health care needs, the need that students pay more
attention than the remaining needs is the full care needs are able to maintain a stable
psychological state in life. The need that students care less about than the remaining

6.Vietnamese Primary School Teachers’ Perspectives on Developing Constructivist-Based Experiential Activities for Primary School Subjects

Teachers’ perspectives on teaching, learning, and education are their beliefs, attitudes, and understandings. This study examines Vietnamese teachers’ perspectives on constructivist theory, a “revolution in current educational psychology”. Because it has greatly influenced teaching and education philosophy and practice worldwide, notably general education program reform. This study uses constructivist theory to build techniques and forms for Vietnamese primary teachers through experiential activities. The north Vietnam: Hanoi Capital, Vinh Phuc City, Ninh Binh City, Cao Bang City, Lao Cai City, Bac Giang City. The average tenure of teachers is nearly 11 years. The authors used a teacher’s questionnaire from six provinces to collect data on constructing experiential activities in a constructivist primary school subject. The findings showed that primary teachers preferred experiential activities in the subject based on constructivist theory and that story-telling and conversation were the most popular teaching methods and instructional forms. Constructivist theory stresses that learners actively construct knowledge and understand via experience. Teachers that use constructivist theory to build primary school experiences will help kids at this foundational level acquire quality and capacity.
7.Sleep Quality and Poor Sleep-related Factors Among Healthcare Workers During the COVID-19 Pandemic in Vietnam

Objectives:
The coronavirus disease 2019 (COVID-19) pandemic has increased the workload of healthcare workers (HCWs), impacting their health. This study aimed to assess sleep quality using the Pittsburgh Sleep Quality Index (PSQI) and identify factors associated with poor sleep among HCWs in Vietnam during the COVID-19 pandemic.
Methods:
In this cross-sectional study, 1000 frontline HCWs were recruited from various healthcare facilities in Vietnam between October 2021 and November 2021. Data were collected using a 3-part self-administered questionnaire, which covered demographics, sleep quality, and factors related to poor sleep. Poor sleep quality was defined as a total PSQI score of 5 or higher.
Results:
Participants’ mean age was 33.20±6.81 years (range, 20.0-61.0), and 63.0% were women. The median work experience was 8.54±6.30 years. Approximately 6.3% had chronic comorbidities, such as hypertension and diabetes mellitus. About 59.5% were directly responsible for patient care and treatment, while 7.1% worked in tracing and sampling. A total of 73.8% reported poor sleep quality. Multivariate logistic regression revealed significant associations between poor sleep quality and the presence of chronic comorbidities (odds ratio [OR], 2.34; 95% confidence interval [CI], 1.17 to 5.24), being a frontline HCW directly involved in patient care and treatment (OR, 1.59; 95% CI, 1.16 to 2.16), increased working hours (OR, 1.84; 95% CI,1.37 to 2.48), and a higher frequency of encountering critically ill and dying patients (OR, 1.42; 95% CI, 1.03 to 1.95).
Conclusions
The high prevalence of poor sleep among HCWs in Vietnam during the COVID-19 pandemic was similar to that in other countries. Working conditions should be adjusted to improve sleep quality among this population.
8.Study on the effective control of postpreal blood glucose of resistant starch cakes in patients with type 2 diabetes

Background: This study was conducted on 93 volunteers with type 2 diabetes to investigate the ability of acetylated wheat starch cake containing 32.1% resistant starch to control postprandial blood glucose levels. Material and methods: The study was designed using a crossover, double-blind trial method. During each testing day, after a minimum of 12 hours of overnight fasting, each participant consumed two identical cakes containing either 80 g of acetylated wheat starch or 80 g natural wheat starch with 330ml of water within 15 minutes. Blood glucose levels were measured at baseline, 60 mins (G1), and 120 mins (G2) after ingestion. The predictive value of factors that contribute to the ability of resistant starch to control postprandial blood glucose was determined by the area under the receiver operating characteristic (ROC) curve based on the combined effect of the cake weight-to-BMI ratio (g/m²BMI) and HbA1c. Results: 60 mins and 120 mins postprandial capillary glucose levels after consuming acetylated wheat starch cake (10.4 ± 1.2 và 9.2 ± 1.2 mmol/L, respectively) were significantly lower compared with natural wheat starch cake (13.3 ± 1.8 và 11.2 ± 1.8 mmol/L, respectively) (p < 0.05). For good control of postprandial blood glucose levels, a maximum of 80 g of acetylated wheat starch can be used per serving for patients with type 2 diabetes with HbA1c ≤ 7.25 without blood glucose-lowering medication is required. Conclusion: acetylated wheat starch has better ontroled of postprandial blood glucose compared with natural wheat starch in patients with type 2 diabetes. This is very suitable in the processing of diets including resistant starch for patients with type 2 diabetes for the purpose of both supporting treatment and improving quality of life.
9.Pathological brain lesions in girls with central precocious puberty at initial diagnosis in Southern Vietnam

Purpose:
Cranial magnetic resonance imaging (MRI) is recommended to identify intracranial lesions in girls with central precocious puberty (CPP). Yet, the use of routine MRI scans in girls with CPP is still debatable, as pathological findings in girls 6 years of age or older with CPP are limited. Therefore, we aimed to identify the prevalence of brain lessons in CPP patients stratified by age group (0–2, 2–6, and 6–8 years).
Methods:
This retrospective cross-sectional study recruited 257 girls diagnosed with CPP for 6 years (2010–2016). MRI was used to detect brain abnormalities. Levels of luteinizing hormone, follicle-stimulating hormone, and sex hormones in blood samples were measured.
Results:
Most girls had no brain lesions (82.9%, n=213), and of the minor proportion of girls with CPP that exhibited brain lesions (17.1%, n=44), 32 girls had organic CPP. Pathological findings were detected in 33.3% (2 of 6) of girls aged 0–2 years, 15.6% (5 of 32) of girls aged 2–6 years, and 3.6% (8 of 219) of girls aged 6–8 years. Hypothalamic hamartoma and tumors in the pituitary stalk were the most common pathological findings. The likelihood of brain lesions decreased with age. Girls with organic CPP were more likely to be younger (6.1±2.4 vs. 7.3±1.3 years, p<0.01) than girls with idiopathic CPP.
Conclusion
Older girls appeared to have a lower prevalence of organic CPP. Clinicians should cautiously use cranial MRI for girls aged 6–8 years with CPP.
10.A New Dimeric Lignan from the Stems of Willughbeia edulis

As part of our continued study on the chemical constituents of Willughbeia edulis stems, a new dimeric lignan named edulignan (1) was isolated from its EtOAc-soluble extract. Based on NMR spectroscopic interpretation, the planar structure of 1 has been suggested to have two 2-substituted 4-chromanone subunits with different stereochemical configurations. In addition, the MS/MS analysis of the products obtained by acidcatalyzed hydrolysis of 1 was supportive of its structure. Unfornatually, the new compound 1 did not show α-glucosidase inhibitory activity with an IC 50 value > 250 μM.
