Objective To compare the mid- and long-term clinical results of mitral valve plasty (MVP) and mitral valve replacement (MVR) in the treatment of functional mitral regurgitation (FMR). Methods Patients with FMR who underwent surgical treatment in the Department of Cardiovascular Surgery of the General Hospital of Northern Theater Command from 2012 to 2021 were collected. The patients who underwent MVP were divided into a MVP group, and those who underwent MVR into a MVR group. The clinical data and mid-term follow-up efficacy of two groups were compared. Results Finally 236 patients were included. There were 100 patients in the MVP group, including 53 males and 47 females, with an average age of (61.80±8.03) years. There were 136 patients in the MVR group, including 72 males and 64 females, with an average age of (61.29±8.97) years. There was no statistical difference in baseline data between the two groups (P>0.05). There was no statistical difference between the two groups in the extracorporeal circulation time, aortic occlusion time, postoperative hospital and ICU stay, intraoperative blood loss, or hospitalization death (P>0.05), but the time of mechanical ventilation in the MVP group was significantly shorter than that in the MVR group (P=0.022). The total follow-up rate was 100.0%, the longest follow-up was 10 years, and the average follow-up time was (3.60±2.55) years. There were statistical differences in the left atrial diameter, left ventricular end-diastolic diameter, left ventricular end-systolic diameter and cardiac function between the two groups compared with those before surgery (P<0.05). The postoperative left ventricular ejection fraction in the MVP group was statistically higher than that before surgery (P=0.002), but there was no statistical difference in the MVR group before and after surgery (P=0.658). The left atrial diameter in the MVP group was reduced compared with the MVR group (P=0.026). The recurrence rate of mitral regurgitation in the MVP group was higher than that in the MVR group, and the difference was statistically significant (10.0% vs. 1.5%, P=0.003). There were 14 deaths in the MVP group and 19 in the MVR group. The cumulative survival rate (P=0.605) and cardiovascular events-free survival rate (P=0.875) were not statistically significant between the two groups by Kaplan-Meier survival analysis. Conclusion The safety, and mid- and long-term clinical efficacy of MVP in the treatment of FMR patients are better than MVR, and the left atrial and left ventricular diameters are statistically reduced, and cardiac function is statistically improved. However, the surgeon needs to be well aware of the indications for the MVP procedure to reduce the rate of mitral regurgitation recurrence.

China Pharmacy 2025;36(13):1655-1660
OBJECTIVE To analyze the characteristics and influencing factors of patient deaths caused by non-ionic iodine contrast media and provide recommendations for safe medication, thereby providing references for clinical application. METHODS Reports of fatal cases associated with non-ionic iodine contrast media were retrieved from databases including CNKI, Wanfang, VIP, PubMed, Cochrane Library, Web of Science, Embase, and SpringerLink. Statistical analysis was performed to examine the characteristics and influencing factors of these fatalities. RESULTS A total of 43 deaths caused by non-ionic iodine contrast media were retrieved, including 21 males (48.84%) and 22 females (51.16%). The majority of deaths occurred in 32 patients aged 51-85 years old (74.42%). The original diseases were mostly cardiovascular and cerebrovascular diseases (30.23%) and digestive system diseases (30.23%), and only 2 cases had a clear allergy history (4.65%). Among the lethal drugs, iohexol (37.21%) and iopromide (25.58%) accounted for the highest proportions. The main causes of death were anaphylactic shock (51.16%), cardiac arrest (11.63%), and pulmonary edema (11.63%); 48.84% of the patients’ ADR occurred within 30 minutes, and 62.79% of the patients died on the first day. It mainly involved the circulatory system, nervous system and respiratory system, and the main manifestations were breathing difficulties, low or undetectable blood pressure, vomiting, etc. CONCLUSIONS The lethal drugs of non-ionic iodine contrast media are mainly iohexol and iopromide. The adverse reactions occur quickly. Clinically, it is necessary to focus on monitoring the vital signs within 30 minutes after medication, and timely treatment of symptoms such as dyspnea and abnormal blood pressure to reduce the risk of death.

A 48-year-old female patient was treated with hydroxychloroquine sulfate and methotrexate for rheumatoid arthritis. After two weeks, tender erythema and papules appeared on the trunk and limbs, accompanied by fever.Skin biopsy was suggestive of neutrophilic infiltration in the superficial mesodermal layer of the dermis. After hydroxychloroquine sulfate withdrawal and 18 days of methylprednisolone sodium succinate and halometasone cream treatment, she improved and was diagnosed with acute febrile neutrophilic dermatosis.By correlation analysis, the causal relationship between acute febrile neutrophilic dermatosis and hydroxychloroquine sulfate was evaluated as"most likely",which had not been reported in China. The pathogenesis of hydroxychloroquine sulfate-induced acute febrile neutrophilic dermatosis may be related to elevated granulocyte-colony stimulating factor levels, abnormal cytokine regulation, and genetic susceptibility.Discontinuation of the suspected drug and application of glucocorticoids are currently the most effective treatment.

Objective:This paper explores the predictive value of indicators of cardiopulmonary exercise test in the degree of coronary artery disease in patients with coronary heart disease. It provides an objective basis for predicting the degree of coronary artery disease by non-invasive detection methods in patients with coronary heart disease, and provides objective guidance for formulating exercise prescriptions for patients with coronary heart disease. Method:Fifty-three patients with coronary heart disease who were treated in the Affiliated Cardiovascular Hospi-tal of Kunming Medical University from March 2020 to May 2022 were selected to complete coronary angiogra-phy and cardiopulmonary exercise test.According to the coronary angiography results and the SYNTAX score, the patients were divided into low group(SYNTAX≤22 points)with 30 cases and medium-high group(SYN-TAX>22 points)with 23 cases. The general information of the patients such as gender, age,BMI and comor-bidities were collected, and the indicators of cardiopulmonary exercise test were collected. Result:Compared with the medium-high group, maximum oxygen uptake(VO2max),maximum oxygen uptake as a percentage of the predicted value(VO2max%P),maximum metabolic equivalent(METSmax),maximum heart rate(HRmax)and maximum heart rate as a percentage of predicted value(HR%P)of the low group were high-er,and the difference between the two groups was statistically significant with p-value less than 0.05.Com-pared with the medium-high group, the heart rate reserve(HRR)in the low group was lower, and the differ-ence between the two groups was statistically significant with p-value less than 0.05.The SYNTAX score was negatively correlated with VO2max,VO2max%P,maximum oxygen uptake in kilograms(VO2max/kg),METSmax,HRmax and HR%P,and positively correlated with HRR with P-value less than 0.05.HRR has the highest sensitivity in predicting coronary heart disease. Conclusion:The low group with low SYNTAX scores and mild coronary lesions has higher VO2max,VO2max%P,VO2max/kg,METSmax,HRmax and HR%P values, suggesting that the patients in this group have better cardiopulmo-nary reserve function and better exercise capacity. The high group with severe coronary artery disease has high-er HRR,and the gap between the maximum heart rate and the target heart rate during exercise is larger, which means it is more difficult for patients in the high group to reach the target heart rate.

HDAC(histone deacetylase)is a class of epigenetic modifying enzymes that can deacetylate proteins by altering the acetylation status of histones in the nucleus, regulating promoter activation levels, and thereby affecting downstream gene expression. However, expression changes of HDACs during endo-thelial differentiation are still unclear. This study used a three-stage method to induce human induced pluripotent stem cells(hiPSCs)to differentiate into endothelial cells, and qRT-PCR was used to detect the expression changes of class I HDAC(HDAC1,2)and class Ⅱ HDAC(HDAC4,5)genes.It was found that HDAC5 exhibits significant expression changes during endothelial differentiation. It is downreg-ulated by 90%during the mesodermal differentiation stage(P<0.01),upregulated by 3.7-fold during the vascular precursor stage(P<0.01),and subsequently downregulated by 70%during the late stage of endothelial differentiation(P<0.01).Immunoblotting experiments further confirmed that HDAC5 under-goes periodic expression changes during endothelial differentiation.Mechanistic studies have shown that HDAC5 downregulation during the differentiation stage of the mesoderm is mediated by Wnt signaling.CHIR99021 treatment and overexpression of Wnt3a can activate the Wnt signaling pathway, leading to HDAC5 downregulation.Inhibiting the Wnt signaling pathway through IWP-2 promotes the recovery of HDAC5 expression. In addition,it was found that HDAC5 is mainly localized in the nucleus, and IWP-2 restores HDAC5 expression, but it remains in the cytoplasm.Further research suggests that downregu-lation of HDAC5 during mesodermal differentiation may contribute to the expression of the mesodermal marker BraT.Treatment with the HDAC inhibitor BML210 can promote early mesodermal differentiation, interfere with endothelial differentiation of vascular precursor cells, and enhance late-stage endothelial differentiation. In conclusion,HDAC5 displays a stage-specific expression during endothelial differentia-tion,and Wnt signaling activation is the main mechanism regulating the downregulation of HDAC5 during the mesoderm stage.
