1.Relationship between four unconventional lipid indicators and mild cognitive impairment in middle-aged and elderly patients with type 2 diabetes mellitus

Objective:To investigate the relationship of plasma residual cholesterol (RC), atherogenic index (AI), triglyceride (TG) to high-density lipoprotein cholesterol (HDL-C) ratio (THR) and low-density lipoprotein cholesterol (LDL-C) to HDL-C ratio (LHR) with mild cognitive impairment(MCI) in middle-aged and elderly patients with type 2 diabetes mellitus (T2DM).Methods:This was a cross-sectional study. A total of 318 T2DM patients aged 50-80 years admitted to the general practice ward of Tiantan Hospital from October 2023 to May 2024 were enrolled. The relevant clinical data were collected, including patient basic information, glycated hemoglobin, lipid test results, and cognitive level was evaluated using the Mini-Mental State Examination (MMSE) form. According to MMSE scores, the patients were divided into the MCI group ( n=102) and the normal cognitive (NC) group ( n=216). Results:Compared with the NC group, patients in MCI group had a older age, longer diabetes duration, lower education level, higher HbA1c, and higher plasma TG levels ( P<0.05). The levels of RC, AI, LHR, and THR in the MCI group were significantly higher than those in the NC group ( P<0.001). Multivariate logistic regression analysis showed that RC ( OR=1.981, P=0.045), AI ( OR=1.657, P=0.043), LHR ( OR=4.360, P=0.001), and THR ( OR=2.908, P=0.002) were independently associated with mild cognitive impairment in T2DM patients. Age-stratified analysis revealed that among T2DM patients aged 60 to 69 years, all four plasma lipid parameters were associated with the risk of MCI in T2DM patients ( P<0.05). Conclusion:Plasma levels of RC, AI, LHR and THR are significantly associated with MCI in middle-aged and elderly T2DM patients, while LHR and THR have a closer association with MCI.

3.Enlightment of foreign management system of rare diseases to China

Through analyses of rare diseases management of several countries and regions , successful practice has been discussed .Legislation of rare diseases should be activated as soon as possible to release specific standard of rare diseases .Accelerating development of translational medicine is a feasible approach to improve the level of diag -nosis and treatment of rare diseases .Strengthening the orientation of management policy should be put into a more significant position to promote incentives of stakeholder in this area .Both enhancing the joint collaboration of differ-ent departments and perfecting medical insurance system could be forceful accelerants to create and optimize the management system of rare diseases in China .
4.Comprehensive examination of visual electrophysiology of visual function assessment after cataract operation

Objective To study the value of comprehensive examination of visual electrophysiology in prognosis of visual function after cataract operation.Methods 315 patients with cataract(408 eyes) underwent preoperative routine examination with pattern reversal visual evoked potential(PVEP), red and blue flicker electroretinogram(ERG) and oscillatory potential (OPs). Normal control group included 162 cases (266 eyes). All of the ocular fundi were examined postoperatively and corrected visual acuities were obtained 3 months after operation. Results Postoperative corrected visions ≥0.5(Ⅰ) and <0.5(Ⅱ) in case group compared with that in control group, amplitudes of PVEP P100-wave decreased significantly and PVEP P100-wave latency times prolonged significantly in group Ⅰ and Ⅱ. There was much more prominent changes in P100-wave of group Ⅱ due to other factors influencing visual function. There wasn't significant difference of the amplitudes and the latency times of a-wave and b-wave of Red-Blue ERG in group Ⅰ and the control group preoperatively. The amplitudes of a-wave and b-wave of Red ERG decreased, the latency times of a-wave and b-wave of Red ERG prolonged, the amplitudes of a-wave and b-wave of Blue ERG decreased but latency times didn' t prolong in group Ⅱ. The value of OPs decreased in groupⅡ compared with that of the control group, which was related to the retinopathy due to blood circulation obstruction.Conclusion Combined examination of visual electrophysiology can estimate visual function thoroughly. The prognosis will be good if various examinations are normal before operation, but will not be good if PVEP P100 latency time prolong, the amplitudes of a-wave and b-wave of Red ERG decrease and the latency time prolong, the amplitudes of a-wave and b-wave of Blue ERG decrease and the value of OPs decrease.
