1.Effects and mechanism of total alkaloids of Corydalis Rhizoma on the regulation of cuproptosis in rats with diabetic cardiomyopathy

OBJECTIVE To investigate the effects and mechanism of total alkaloids of Corydalis Rhizoma (TAC) on the regulation of cuproptosis in rats with diabetic cardiomyopathy (DCM) based on silence information regulator 1(Sirt1)/tumor protein 53(P53)signaling pathway. METHODS DCM rat model was induced by high-fat and high-sugar diet and intraperitoneal injection of streptozotocin. Thirty-two model rats were randomly divided into model group, TAC low-dose, medium-dose and high-dose groups (7, 10.5, 14 mg/kg), with 8 rats in each group. An additional 8 rats were assigned to normal control group. Related drugs or normal saline were administered intragastrically in each group, once a day, for 4 weeks. After the last medication, the fasting blood glucose (FBG) levels of the rats were measured. The levels of myocardial creatine kinase (CK), creatine kinase isoenzyme (CK-MB), and lactate dehydrogenase (LDH) in serum and myocardial tissue of rats were all detected. The pathological morphology, fibrosis degree, and Cu2+ deposition of myocardial tissue in rats were observed. The levels of Cu2+ and glutathione (GSH) in myocardial tissue, the expressions of Sirt1/P53 signaling pathway-related proteins [Sirt1, P53, solute carrier family 7 membrane 11 (SLC7A11)], and iron-sulfur cluster-related proteins [ferredoxin 1 (FDX1), lipoic acid synthetase (LIAS), aconitase 2 (ACO2), NADH-ubiquinone oxidoreductase core subunit S8 (NDUFS8), dihydrolipoamide acetyltransferase (DLAT), dihydrolipoamide succinyltransferase (DLST)], and heat shock protein 70 (HSP70) were all determined. RESULTS Compared with normal control group, the model group exhibited significantly elevated levels of FBG, CK, CK-MB and LDH in both serum and myocardial tissue, as well as increased 2+ levels of Cu in myocardial tissue and the expression of P53 and HSP70 proteins (P<0.05); the level of GSH and the expression levels of Sirt1, SLC7A11, FDX1, LIAS, ACO2, NDUFS8, DLAT, and DLST proteins in myocardial tissue were all significantly decreased (P<0.05); the myocardial tissue exhibited severe pathological damage, with numerous inflammatory cell infiltrations and significant fibrosis, as well as increased deposition of Cu2+. Compared with model group, most of the above quantitative indicators in rats were significantly reversed in TAC groups (P<0.05); the pathological damage to the myocardial tissue was alleviated, with reduced fibrosis and Cu2+ deposition. CONCLUSIONS TAC can ameliorate DCM in rats, and its mechanism of action may be related to activating the activity of the Sirt1/P53 signaling pathway, promoting the chelation of GSH with Cu2+, and inhibiting cuproptosis of cardiomyocyte.
2.Labial protuberances of anterior alveolar bone in orthodontic treatment: a case report and literature review

Objective:
This study aims to explore the influencing factors, formation mechanisms, and treatment methods of labial protuberance in the anterior maxilla during orthodontic treatment, providing a reference for clinical practice.
Method:
This study reports a case where the absence of upper anterior teeth 11 and 21, and the retraction tilting movement of teeth 12 and 22, resulted in labial protuberance and gingival hyperplasia. Alveolar osteoplasty and gingivoplasty were performed. The specific changes in the alveolar bone during the retraction of the anterior teeth and the characteristics of its remodeling were analyzed. Combined with relevant literature, the factors influencing the formation of labial protuberance in orthodontic patients, mechanisms, and methods for prevention and treatment were summarized.
Results:
After periodental surgery follow-up for 6 months, the gingival color and shape of teeth 12 and 22 were good, the labial alveolar bone was normal, and the overall condition was stable. A review of the literature showed that labial protuberance is more common in adult orthodontic patients, and the distance (>4 mm) and speed of retraction of anterior teeth are related to its formation, with the main mechanism likely being differential remodeling of the alveolar bone. In adult patients, the number of active osteoblasts and osteoclasts in the alveolar bone decreases, along with a reduction in metabolic activity and overall cellular activity, which diminishes the reactivity of the alveolar bone. After treatment of anterior teeth retraction, there is insufficient labial bone resorption. Moreover, the lack of mechanical stress-mediated periodontal ligament in the interdental space leads to reduced bone remodeling stimulation in this area, resulting in thickening of the labial alveolar bone of the upper anterior teeth. The remodeling rates of cortical and trabecular bone differ, with active trabecular bone proliferation near the tooth root surface and slow cortical bone resorption near the outer surface, which ultimately results in increased bone thickness at the labial cervical region. Specific case analysis indicates that the retraction distance of the upper anterior teeth in this case was about 6 mm. The alveolar bone at the missing sites of teeth 11 and 21, lacking periodontal ligament stimulation, showed less remodeling and absorption, likely appearing as hyperplasia. The prevention of labial bone protrusion mainly involves controlling the speed and distance of retraction of anterior teeth. Smaller labial protuberances generally do not require treatment, but those affecting function and aesthetics can be addressed with periodontal alveolar osteoplasty.
Conclusion
After the retraction of anterior teeth in orthodontics, a prominent, hard bone protuberance on the labial side can sometimes occur, which may be due to differential remodeling efficiency in different regions of the alveolar bone. For bone protuberance that influences aesthetics or function, periodontal alveolar osteoplasty can be a reliable option.

6.All-trans Retinoic Acid Regulates Erythroid Differentiation of K562 Cells via Epigenetic Mechanisms

All-trans retinoic acid(ATRA)is able to induce promyelocytic differentiation effectively.However,its role in the process of erythroid differentiation remains unclear.To investigate the role of AT-RA in the process of erythroid differentiation and its epigenetic regulatory mechanism,we established an induced leukemia cell K562 model in this study.Firstly,hemin was used to induce the differentiation of K562 cells into erythroid cells.The results of flow cytometry showed that ATRA affected the lineage changes of cells during erythroid differentiation and blocked the process of cell differentiation.After AT-RA treatment of differentiating cells,the expression level of erythroid differentiation-related genes de-creased.Through chromatin conformational capture(3C),formaldehyde-assisted separation of regulatory elements(FAIRE),chromatin immunoprecipitation(ChIP)techniques,the epigenetic mechanism was explored and it was found that after ATRA treatment of cells,the chromatin accessibility within the β-glo-bin family gene locus decreased,and the frequency of interaction between the locus control region(LCR)and its target gene promoter decreased.The decrease in the chromatin accessibility of the gene locus led to a decrease in the enrichment frequency of erythroid-related transcription factors GATA binding protein 1(GATA1),LIM domain binding 1(LDB1),LIM domain only 2(LMO2),and BHLH transcription factor 1(TAL1)at the promoter regions of the LCR and the gene locus of the globin family.The above results indicate that the ATRA treatment of differentiating cells leads to a decrease in the chromatin acces-sibility of erythroid differentiation-related genes,and a more closed chromatin structure hinders the bind-ing of LCR-recruiting transcription factors to the promoter regions of genes,thereby further repressing the expression of β-globin family genes.This dynamic process elucidates the epigenetic mechanism of ATRA in regulating erythroid differentiation.
7.Clinical characteristics and effects of maternal glucocorticoid treatment during pregnancy in cases with different fetal congenital pulmonary airway malformation volume ratios

Objective:To investigate the clinical characteristics of cases with different congenital pulmonary airway malformations (CPAM) volume ratios (CVR) and the effect of maternal glucocorticoid treatment during pregnancy on CPAM.Methods:A retrospective study was conducted on 56 singleton pregnant women with fetal CPAM diagnosed prenatally in the Department of Obstetrics and Gynecology at Nanjing Drum Tower Hospital, Affiliated Hospital of Medical School, Nanjing University, from September 2020 to May 2023. Among these, three cases received maternal glucocorticoid treatment during pregnancy and clinical conditions were reported in detail. Statistical analyses were performed using independent sample t-tests, non-parametric tests, Chi-square tests, or Fisher's exact test. Results:(1) General information: The average age of the 56 pregnant women with CPAM fetuses was (32.0±0.7) years. All fetuses had unilateral lesions, with 25 cases (44.6%) on the left side. Types Ⅰ, Ⅱ, and Ⅲ CPAM accounted for 5.4% (3/56), 50.0% (28/56), and 44.6% (25/56), respectively. Fetal hydrops occurred in two cases, and the maximum CVR during the fetal period for the other 54 non-hydropic fetuses was 0.79±0.66. (2) The CVR threshold for the risk of fetal hydrops was set as the mean maximum CVR of non-hydropic CPAM fetuses plus 2 standard deviations (0.79+2×0.66=2.1). The subjects were divided into two groups based on the maximum CVR during the fetal period: CVR≤2.0 group ( n=50) and CVR>2.0 group ( n=6). Comparison between the CVR>2.0 group and CVR≤2.0 group: The CVR>2.0 group had significantly higher rates of fetal hydrops [2/6 vs. 0.0% (0/50), Fisher's exact test], mediastinal shift [5/6 vs. 32.0% (16/50), χ 2=4.03], polyhydramnios [6/6 vs. 4.0% (2/50), Fisher's exact test], and postnatal surgery [4/5 vs. 22.2% (10/45), continuity correction χ 2=4.86] (all P<0.05). None of the fetuses with CVR≤2.0 had hydrops or received intrauterine intervention. The overall live birth rate was 89.3% (50/56). (3) Maternal glucocorticoid treatment during pregnancy: three of six fetuses with CVR>2.0 were treated with maternal glucocorticoid during pregnancy, and all were delivered alive at term after the intervention with resolution of edema and/or reduction in mass size. Two of them were treated with postnatal thoracoscopic surgery and were followed up to 5 and 14 months of age, respectively, with no abnormalities in feeding and development; the other was not treated surgically until 3 months of age, with no respiratory-related symptoms and no abnormalities in feeding and development. Conclusions:Prenatal ultrasound indicating CVR>2.0 is associated with increased rates of fetal hydrops, mediastinal shift, and polyhydramnios. Maternal glucocorticoid treatment during pregnancy may lead to favorable pregnancy outcomes for these CPAM fetuses.
8.Effectiveness and prognosis of improved domestic neonatal ureteral stents for intrauterine treatment of fetal chylothorax

Objective:To investigate the effectiveness and prognosis of using improved domestic neonatal ureteral stents (referred to as improved double-J stents) for thoraco-amniotic shunting (TAS) in treating fetal chylothorax.Methods:A retrospective analysis was conducted on the clinical data of 21 cases of fetal chylothorax treated with TAS using improved double-J stents at Nanjing Drum Tower Hospital, Nanjing University Medical School from April 1, 2018, to September 30, 2023. Surgical complications and perinatal outcomes were summarized, and the development of surviving infants in five domains (communication, gross motor, fine motor, problem-solving, and personal-social) was assessed using the Ages and Stages Questionnaires-Third Edition (ASQ-3). Descriptive statistical analysis was used. Results:(1) The median gestational age at prenatal diagnosis was 28.7 weeks (27.3-30.4 weeks), with 85.7% (18/21) of cases complicated by fetal hydrops, 90.5% (19/21) by polyhydramnios, and 85.7% (18/21) by bilateral pleural effusion. (2) The median gestational age at the first TAS was 30.9 weeks (29.7-32.7 weeks). Of the 21 cases, 10 required repeat stent placement due to dislodgement or blockage, with a total of 49 stent placements. The dislodgement rate within 7 days was 24.5% (12/49), and the blockage rate was 16.3% (8/49). The rate of premature rupture of membranes within one week post-stent placement was 9.5% (2/21), with an overall preterm premature rupture of membranes rate of 28.6% (6/21). The median interval from the first TAS to delivery was 30.0 d (19.8-40.0 d). Of the 21 cases, three opted for selective termination of pregnancy; the remaining 18 cases resulted in live births, with a median gestational age at delivery of 35.6 weeks (34.1-37.1 weeks), and three neonatal deaths. The overall neonatal survival rate was 15/18. Surviving infants were followed up to a median age of 30 months (7-48 months), with 13 showing normal development and two scoring below the ASQ-3 threshold.Conclusion:The improved double-J stent can be used for TAS in the treatment of fetal chylothorax, with generally favorable outcomes.
9.Modification and implementation of digital operating room based on hospital LAN

10.Analysis of 41 cases of non-metastatic Ewing's sarcoma in children

Objective To summarize the clinical characteristics,treatment outcomes,and prognostic factors of children with non-metastatic Ewing's sarcoma(ES).Methods A retrospective analysis was conducted on the clinical data of 41 children with non-metastatic ES diagnosed and treated at the Shanghai Children's Medical Center,Shanghai Jiao Tong University School of Medicine from January 2010 to December 2018.All patients underwent chemotherapy based on the RMS-2009 protocol of the center,and local treatment such as surgery and/or radiotherapy was performed according to risk grouping.The Kaplan-Meier method was used to calculate the overall survival(OS)and event-free survival(EFS)rates.Univariate prognostic analysis was performed using the log-rank test,and multivariate analysis was conducted with Cox regression.Results Of the 41 children,21 were male and 20 were female.The median age at diagnosis was 7.7 years(range:1.2-14.6 years).The median follow-up time for patients with event-free survival was 68.1 months(range:8.1-151.7 months).As of the last follow-up,33 patients were in complete remission,and the overall 5-year EFS and OS rates were(78±6)%and(82±6)%,respectively.Univariate analysis by the log-rank test showed that a tumor diameter ≥8 cm,time from diagnosis to start of local treatment ≥16 weeks,and incomplete surgical resection were associated with poor prognosis(P<0.05).Multivariate Cox regression analysis indicated that incomplete surgical resection(HR=8.381,95%CI:1.681-41.801,P=0.010)was an independent risk factor for poor prognosis in children with ES.Secondary tumors occurred in 2 cases.Conclusions A comprehensive treatment strategy incorporating chemotherapy,surgery,and radiotherapy can improve the prognosis of children with ES.Poor prognosis is associated with an initial tumor diameter ≥8 cm,while complete surgical resection and early initiation of local treatment can improve outcomes.[Chinese Journal of Contemporary Pediatrics,2024,26(4):365-370]
