Congenital Adrenal Hyperplasia and Turner Syndrome are not very rare diseases. However, their
combination may be confounding. Presented here is a case of a 54 year old nulligravid, with
primary amenorrhea, short stature, absent breast development, hirsutism, signs of virilization,
and clitoromegaly who came in due to hypogastric pain and an enlarging palpable hypogastric
mass. Diagnostic procedures and surgical management are discussed.

2.Association of metabolic syndrome with different phenotypes of polycystic ovarian syndrome among Filipino women in a tertiary hospital: A retrospective cohort study

Introduction:
Polycystic ovarian syndrome (PCOS) is a common endocrinopathy affecting women during reproductive age. Women affected by PCOS generally have a higher risk of developing Metabolic syndrome (MetS). MetS on each phenotype of PCOS reflects some phenotypes with worse metabolic profiles and a higher risk of developing long-term complications in women with PCOS.
Objective:
To determine the association of MetS with different phenotypes of PCOS among Filipino women in a tertiary hospital.
Materials and Methods:
This is a retrospective cohort study of 154 women in a tertiary hospital, both private and service divisions
Results:
A total of 154 patients with PCOS were analyzed in this study: 67 (43.51%) Phenotype A, 25 (16.23%) Phenotype B, 3 (1.95%) Phenotype C, and 59 (38.31%) phenotype D. The prevalence of MetS in PCOS was 69.48%, with no significant difference statistically between phenotypes. MetS was most prevalent in Phenotype A (74.63%) and least prevalent in phenotype D (62.71%). Among Filipino women with PCOS, Phenotype A had a 2.5 times increased risk of developing MetS compared to Phenotype D.
Conclusion
Phenotype A is the most common phenotype and has the highest prevalence in developing metabolic changes. Increasing body mass index and age played significant roles in elevating the risk of developing MetS. Early detection of MetS in all phenotypes of PCOS can aid in preventing the development of long-term complications such as cardiovascular disease and diabetes mellitus type II.

3.The use of luteal phase support in In-vitro fertilization: Experience of a private infertility center in 2005 - 2008.

Objectives: To describe the experience of the Center for Reproductive Medicine in the use of luteal phase support protocols in in-vitro fertilization (2005 - 2008).Design: A retrospective cohort study of couples undergoing in vitro fertilization using the different modalities of luteal phase support from 2005 to 2008 at CRM.Main outcome measure(s): clinical pregnancy rate. miscarriage rate and live birthrate.Result(s): From 2005 - 2008, 94 patients underwent in vitro fertilization.The center achieved a 36% clinical pregnancy rate, and 27.6% live birthrate. The most common luteal phase used was a combination of progesterone, estradiol and aspirin which achieved a 39% clinical pregnancy rate and a 28% live birthrate. The luteal phase support protocol which obtained the highest clinical pregnancy rate of 50% was that of progesterone alone which resulted in 50% live birthrate. But it did not reach clinical significance as compared to the other protocols (P>0.05). The use of the intramuscular (IM) progesterone showed a significant P value, with an odds ratio of 3.83, suggesting that it can increase the likehood of clinical pregnancy by as much as 4x.Conclusion(s): The most common luteal phase support protocol used was a combinatiion of progesterone and aspirin. Progesterone alone protocol achieved the highest clinical pregnancy rate of 50% which all resulted in live birht. Among the different types and routes of progesterone used, only the IM progesterone was shown to be clinically significant in increasing the pregnancy rate.
