1.Analysis of liver histological characteristics and clinically related factors in patients with inactive HBsAg carriers

Objective:To analyze the liver histological characteristics and clinically related factors in inactive hepatitis B surface antigen (HBsAg) carriers (IHC), and also explore whether antiviral treatment is necessary for IHC, as defined in the 2022 version of the hepatitis B prevention and treatment guidelines.Methods:A multicenter, retrospective cohort study was conducted. Two hundred and thirty-one IHC cases who underwent liver biopsy histopathological examination in nine medical institutions, including Beijing Youan Hospital affiliated with Capital Medical University, from January 2018 to December 2023 were included. General informative data, clinical serological markers, and transient elastography (TE) examination results were collected. Patients were divided into a positive (148 cases) and a negative group (83 cases) according to the results of hepatitis B virus (HBV) DNA detection. The differences in liver pathological inflammatory activity (G) and liver fibrosis stage (S) were analyzed between the two groups to explore the correlation between liver tissue conditions and clinically related factors. Comparsions of normally distributed continwous data, skeukd continuous data, and categorical data between groups are performed using t tests, Mann-Whitney U tests and χ2 tests, respectively. Results:The age of 231 IHC cases was 43 (38, 51) years old, with 95.2% (220/231) aged ≥30 years, and males accounted for 64.9% (150/231). HBsAg and HBV DNA levels were 131.9 (20.8, 400.9) IU/mL and 94.0 (0, 448.5) IU/mL, respectively, of which 35.9% (83/231) were HBV DNA negative (<20 IU/mL). The remarkable proportions of G≥2, S≥2, and liver injury (G≥2 and/or S≥2) in liver tissue were 16.5% (38/231), 29% (67/231), and 35.9% (83/231), respectively. The S≥2 proportion was significantly higher in the HBV DNA-negative group than the positive group (42.2% vs. 21.6%, P<0.001), and it mainly occurred in the population cohort over 30 years old (44.9% vs. 31.0%, P=0.04). The liver stiffness measurement (LSM), aspartate transaminase to platelet ratio index (APRI), and platelet (PLT) were significantly higher in the S≥2 group than the S<2 group ( P<0.05). Conclusion:Clinicians can comprehensively evaluate the degree of liver fibrosis in IHC based on clinical factors such as age, PLT, APRI, and LSM, even if the liver histological results are lacking. The China 2022 version guidelines define that nearly half of IHC has histological indications for antiviral therapy, and liver biopsy and prompt treatment can be recommended.
2.Current research status of ferroptosis in Parkinson disease:A visual analysis based on CiteSpace

Objective To investigate the current research status of ferroptosis in Parkinson disease in recent years. Methods CiteSpace was used to perform bibliometric and visual analyses of the articles on the mechanism of action of ferroptosis in Parkinson disease and related prevention and treatment measures in CNKI and Web of Science (WoS). Results A total of 92 articles from CNKI and 273 articles from WoS were included in this study. There was a tendency of increase in the number of articles published in China and globally. In CNKI, the keywords of ferroptosis, Parkinson's disease, oxidative stress, and traditional Chinese medicine were cited for many times, while in WoS, activation, cell death, alpha-synuclein, and Parkinson disease were cited for many times. There were 7 clusters of keywords in CNKI and 12 clusters in WoS. The top 5 authors (teams) in terms of the number of publications in CNKI were Yang Xinling; Zhang Lijie; Li Shaodan; Hao Mengdie; and Lu Ranran, while the top 5 authors (teams) in WoS were Ayton, Scott; Zhu, Meiling; Qu, Le; Devos, David; and Xie, Junxia. The number of team members in China was lower than that in foreign countries, mainly small team cooperation. As for the number of articles published by institutional cooperation, Qingdao University published the highest number of articles in CNKI, while the University of Melbourne published the highest number of articles in WoS. Biochemistry was the subject with the highest frequency, and there were intersections between disciplines. China was the core representative of state cooperation with the highest frequency. Conclusion The research on the association between Parkinson disease and ferroptosis mainly focuses on oxidative stress, neuroprotection, traditional Chinese medicine, and neuroinflammation, among which neuroprotection and neuroinflammation are the latest trends for development. Therefore, there may be more targeted therapies for neuroinflammation and therapies that stimulate neuroprotection in the future to alleviate pain and improve the quality of life of patients.

3.The mechanism of Dange Jiecheng decoction improving alcoholic liver disease in a rat model

ObjectiveTo investigate the effect of Dange Jiecheng decoction in improving alcoholic liver disease (ALD) in rats, as well as its mechanism of action based on the tumor protein P53/glutathione peroxidase 4 (GPX4) signaling pathway. MethodsA total of 36 male Sprague-Dawley rats were randomly divided into normal group, model group, Dange Jiecheng decoction group, and compound Yiganling tablets group, with 9 rats in each group. The rats were given gradient intragastric administration of 56% alcohol to establish a model of ALD, and meanwhile, the corresponding drug was given by gavage for 12 consecutive weeks. After the experiment ended, serum and liver tissue samples were collected to measure the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and triglyceride (TG) and the levels of malondialdehyde (MDA), reactive oxygen species (ROS), glutathione (GSH), and Fe2+; HE staining was used to observe histopathological changes, and oil red O staining and Prussian blue staining were used to observe fat and iron deposition in liver tissue, respectively; RT-qPCR and Western blot were used to measure the mRNA and protein expression levels of P53, glutathione peroxidase 4 (GPX4), NADPH oxidase 1 (NOX1), prostaglandin-endoperoxide synthase 2 (PTGS2), and ferritin heavy chain 1 (FTH1) in liver tissue. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsCompared with the normal group, the model group had disordered arrangement and incomplete structure of hepatocytes with inflammatory cell infiltration and a large number of fat vacuoles, significant increases in the serum levels of AST, ALT, TG and the levels or activities of MDA, ROS, and Fe2+ in liver tissue (all P<0.01), significant increases in the mRNA and protein expression levels of P53, NOX1, and PTGS2 (all P<0.01), and significant reductions in the content of GSH in serum and the mRNA and protein expression levels of GPX4 and FTH1 in liver tissue (all P<0.01). Compared with the model group, the Dange Jiecheng decoction group and the compound Yiganling tablets group had ordered arrangement of hepatocytes with a reduction in inflammation, significant reductions in the serum levels of AST, ALT, and TG, the levels of MDA, ROS, and Fe2+ in liver tissue, and the mRNA and protein expression levels of P53, NOX1, and PTGS2 in liver tissue (all P<0.05), and significant increases in the content of GSH in serum and the mRNA and protein expression levels of GPX4 and FTH1 in liver tissue (all P<0.05), as well as an improvement in hepatic steatosis and a reduction in iron deposition (all P<0.01). ConclusionDange Jiecheng decoction can effectively improve disease progression in rats with ALD, possibly by regulating the P53/GPX4 pathway and inhibiting ferroptosis.
4.Iodine nutrition status and influencing factors of children and adolescents in Zhejiang Province in 2022

Objective:To analyze the iodine nutrition status of children and adolescents and influencing factors in Zhejiang Province, providing scientific basis for optimizing iodine deficiency disorders (IDD) prevention and control strategies.Methods:In June 2022, a multistage stratified sampling method was used to divide 16 counties (cities, districts, abbreviated as counties) in Zhejiang Province into three categories based on their geographical locations (average distance from the coastline): coastal areas (including Dinghai District, Jiaojiang District, Sanmen County, Cixi City and Lucheng District), sub-coastal areas (including Wuxing District, Haining City, Linping District, Fuyang District and Fenghua District), and inland areas(including Suichang County, Changshan County, Shengzhou City, Jindong District, Dongyang City and Yongjia County). One county was selected from each category, and one township (street) was selected from each county. Two administrative villages (neighborhood committees) were selected from each township (street). Ten households including all children and adolescents aged 6-17 in each household were selected from each administrative village (neighborhood committee). Demographic information and personal dietary characteristics were collected via questionnaires, while household salt and a random urine sample were tested for iodine level. Trend analysis was conducted using a χ 2trend test, and a multivariate logistic stepwise regression model was used to analyze the influencing factors of urinary iodine levels. Results:A total of 755 children and adolescents aged 6-17 were selected, including 387 males (51.26%) and 368 females (48.74%), with an age of (11.24 ± 3.32) years. There were 269 children and adolescents in coastal areas (35.63%) and 409 children and adolescents in urban areas (54.17%). A total of 755 household salt samples were collected, with a median salt iodine concentration of 21.80 mg/kg. These included 263 non-iodized salt samples, 38 unqualified iodized salt samples, and 454 qualified iodized salt samples. The coverage rate of iodized salt was 65.17% (492/755), and the consumption rate of qualified iodized salt was 60.13% (454/755). The distribution of salt iodine quality among children and adolescents in different geographical locations showed statistically significant differences (χ 2 = 111.95, P < 0.001), with the proportion of non-iodized salt gradually decreasing from coastal areas to inland areas (χ 2trend = 90.17, P < 0.001). A total of 755 urine samples were collected, with a median urinary iodine concentration of 186.60 μg/L. The proportions of urinary iodine < 100, 100-199, 200-299, and ≥300 μg/L were 16.95% (128/755), 37.62% (284/755), 24.37% (184/755), and 21.06% (159/755), respectively. The χ 2trend test revealed a nonlinear positive correlation between salt iodine level and urinary iodine level (χ 2regression = 21.98, P < 0.001; χ 2partial = 6.96, P < 0.001). The frequency distribution of urinary iodine in children and adolescents from different geographical locations and between urban and rural areas showed statistically significant differences (χ 2 = 29.63, 16.56, P < 0.001). Among them, the proportion of children and adolescents with urinary iodine < 100 μg/L gradually decreasing from coastal areas to inland areas (χ 2trend = 6.15, P = 0.013). The results of multivariate logistic regression analysis revealed that sub-coastal regions, inland regions, and urban-rural regions ( OR = 1.57, 1.53, 1.64, 95% CI: 1.11-2.24, 1.03-2.27, 1.17-2.32, P < 0.05) were significantly associated with urinary iodine levels in children and adolescents aged 6-17. Conclusions:In 2022, the iodine nutrition of children and adolescents in Zhejiang Province is generally suitable, but there is a risk of iodine deficiency among coastal children and adolescents. Geographic location and urban/rural areas are influencing factors on iodine nutrition status of children and adolescents in Zhejiang Province.

Objective:To explore the current status of orthopedic nurses' health science popularization information generation capacity and training needs in Beijing.Methods:This study was a cross-sectional survey. Convenience sampling was used to select 818 orthopedic nurses from 54 hospitals (48 Class Ⅲ hospitals and 6 ClassⅡ hospitals) in Beijing in April 2024 for the study. General Information Questionnaire, Self-assessment Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity, and Training Needs Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity were used to survey the study participants. Multiple linear regression was used to analyze the factors influencing orthopedic nurses' health science popularization information generation capacity. A total of 818 questionnaires were distributed and 781 valid questionnaires were recovered, with a valid recovery rate of 95.48% (781/818) .Results:The 781 orthopedic nurses had a total score of [49.00 (42.00, 57.00) ] on the Self-assessment Scale for Orthopedic Nurses' Health Science Popularization Information Generation Capacity. Multiple linear regression analysis showed that years of working experience, health status, intention to participate in health science popularization, stage of health science popularization behavior, and frequency of participation in health science popularization training were the factors influencing orthopedic nurses' health science popularization information generation capacity ( P<0.05), which explained 17.30% of the total variation. Beijing orthopedic nurses had training needs in five areas of assessing audience needs, assessing message risk, revising and improving messages, generating messages, and pretesting messages. Conclusions:The Beijing orthopedic nurses' health science popularization information generation capacity is at a medium and high level, but there is still much room for improvement. It is recommended to pay attention to the physical and mental health of orthopedic nurses, improve the work commitment, and build a long-term incentive mechanism to stimulate the enthusiasm of orthopedic nurses in health science popularization. Medical institutions or specialty societies should actively formulate training programs, build training platforms, and gradually perfect the orthopedic nurses' health science popularization training system in order to comprehensively enhance the health science popularization information generation capacity and help China's great health.

9.Research on the application of medical dialectics combined with textbook-based problem-oriented teaching in hematology internship teaching

Objective:To explore the application of medical dialectics combined with problem-oriented teaching based on textbooks in hematology internship teaching.Methods:A total of 100 undergraduate students who practiced in the Department of Hematology of the First Hospital of Jilin University from 2022 to 2023 were selected as the research subjects. Students were randomly assigned to a control group and an observation group, with 50 students in each group. The control group received traditional teaching, while the observation group received medical dialectics combined with textbook-based problem-oriented teaching. We assessed the theoretical and operational scores, classroom performance, comprehensive abilities, and teaching satisfaction of two groups of students using t-test and χ 2 test in SPSS 22.0. Results:The theoretical and operational scores of the observation group were (94.26±5.35) points and (92.68±4.72) points, respectively. The theoretical and operational scores of the control group were (86.16±5.42) points and (81.52±5.28) points, respectively. The differences between the two groups were statistically significant ( P<0.001). The recognition rates were significantly higher by students in the observation group than in the control group ( P<0.05) in terms of improving learning efficiency, self-learning ability, understanding and comprehensive analysis of diseases, problem-solving ability, language and organizational expression ability, integration of theory and practice, clinical thinking ability, and independent thinking ability. The satisfaction with teaching was higher in the observation group than in the control group ( P<0.05) in terms of teaching attitudes, teaching methods, teaching arrangements, practicality of teaching content, clear explanation of teaching theories, and outstanding teaching objectives. Conclusions:The medical dialectics combined with textbook-based problem-oriented teaching can improve the assessment scores of medical students, while helping to cultivate their comprehensive abilities and develop good clinical diagnosis and treatment thinking.
