1.Research progress on traditional Chinese medicine interventions in colorectal cancer metastasis based on tumor microenvironment

Colorectal cancer has an insidious onset, and some patients have already developed local invasion or metastasis at the time of diagnosis, which affects treatment strategy selection and survival prognosis. The tumor microenvironment is not only the basic condition for tumor cell survival but also the core regulatory factor driving their local invasion and distant metastasis. This article reviewed the mechanisms by which traditional Chinese medicine regulates the tumor microenvironment to inhibit colorectal cancer metastasis. It was found that traditional Chinese medicine active ingredients such as astragalus polysaccharides, leonurine, kaempferol, etc., as well as traditional Chinese medicine formulas such as Jianpi jiedu formula, can inhibit colorectal cancer metastasis by regulating key links of the tumor microenvironment, including immunosuppression, inflammatory response, intestinal microecological disorder, abnormal angiogenesis, and extracellular matrix homeostasis imbalance. However, current related research was still mainly based on cell and animal experiments, with limited clinical evidence-based data, and mechanistic studies mostly focued on single signaling pathways. Future research should integrate multi-omics technologies and systems biology methods to systematically elucidate the key targets and action networks through which traditional Chinese medicine regulates the colorectal cancer tumor microenvironment to combat metastasis.
2.Causal relationship between Parkinson disease and the risk of mental illness: A two-sample Mendelian randomization study

Objective Observational studies have shown an association between Parkinson disease (PD) and mental illness, but further studies are needed to explore the causal relationship between them. This study aims to investigate such causal relationship using the method of two-sample Mendelian randomization (MR). Methods Related data were extracted from GWAS, and summary statistics associated with PD, depression, sleep disorders, and anxiety phenotype-variants were obtained. Single nucleotide polymorphisms (SNPs) for PD were selected as instrumental variables, and MR-PRESSO was used to exclude outliers. Inverse variance weighting was used as the main method to assess causal effect estimates, and MR Egger, weighted median, simple mode, and weighted mode were used to verify the robustness of the findings. A sensitivity analysis was used to validate the reliability of the results, including the Cochran Q test, the MR-Egger intercept test, funnel plots, and the leave-one-out method. Results A total of 21 SNPs associated with PD were identified. The MR analysis showed that PD had a causal relationship with depression (OR=0.974,95%CI 0.934‒1.015, P=0.210), sleep disorders (OR=1.056, 95%CI 0.970-1.149, P=0.211), and anxiety (OR=0.998, 95%CI 0.996‒1.001, P=0.118), with no statistical significance. Different MR analyses and sensitivity analyses showed that PD did not directly contribute to the development of mental illness. Conclusion There is no direct causal relationship between PD and depression/sleep disorders/anxiety. In clinical practice, healthcare professionals should pay attention to the mental health of patients with PD. Randomized controlled studies should be conducted in the future to further validate the results of this study.

3.Development and validation of assessment and diagnostic tools for apraxia of speech of Chinese Putonghua

ObjectiveTo develop an assessment tool for apraxia of speech (AOS) of Chinese Putonghua speakers and test its reliability and validity. MethodsThe Chinese Apraxia of Speech Assessment and Diagnostic Tool (CAADT) was developed based on the Apraxia of Speech Rating Scale 3.5, combined with the linguistic characteristics of Chinese and clinical experience. The tool consistsed of eleven items across three sections: articulation, prosody and alternating motion rates. Six experts evaluated the content validity. From November, 2024 to May, 2025, 51 patients with post-stroke AOS (experimental group) and ten patients with post-stroke aphasia without AOS (control group) were recruited from Anhui Wannan Rehabilitation Hospital (the Fifth People's Hospital of Wuhu), and tested with CAADT. Reliability was assessed using Cronbach's α coefficient, Kendall's coefficient of concordance W and Pearson correlation coefficient. Validity was evaluated using the content validity index (CVI) and Spearman correlation coefficient. Discriminative effect was analyzed using the receiver operating characteristic (ROC) curve. ResultsThe Cronbach's α coefficients for the articulation and prosody sections and the total scale were all > 0.9, while it was 0.454 for the alternating motion rates. Inter-rater reliability was good (W ≥ 0.598, P < 0.001). Test-retest reliability showed high positive correlations for the three sections and the total score between the two assessments (r ≥ 0.84, P < 0.001). The scale-level CVI was 0.95, and the item-level CVI ≥ 0.83. The Spearman correlation coefficients among the sections ranged from 0.30 to 0.70. ROC analysis revealed an area under the curve of 0.953, with a cut-off value of 11, yielding a sensitivity of 0.92 and a specificity of 0.90. ConclusionCAADT demonstrates good reliability, validity and discriminative effect, which can be used for clinical assessment and auxiliary diagnosis of Chinese Putonghua speaking patients with post-stroke AOS.
4.Construction, operational mechanism, and practical dilemmas of a community-integrated medical-nursing-rehabilitation-care service model for the elderly with chronic diseases

With the deepening of population aging and the rising prevalence of chronic diseases among the elderly in China, the traditional elderly care models are facing challenges. This paper proposed a community-integrated medical-nursing-rehabilitation-care service model, characterized by “integration, flexibility, community orientation, and demand orientation.” This model optimizes and combines various service resources such as healthcare, elderly care, rehabilitation, and nursing at the community level, providing more comprehensive and targeted services for the elderly with chronic diseases in the community. However, a series of challenges existed in dimensions such as the service integration, benefit distribution, resources and funds, as well as policies and systems, hindering the sustainable development of the community-integrated medical-nursing-rehabilitation-care service model. Future efforts should focus on constructing a more standardized collaborative system, clarifying standards for resource investment and benefit distribution, as well as strengthening policy guidance, financial support, and talent team development.
5.The Interpretation of the Connotation and Characteristics of Chinese Health Index:Integrating Cultural Connotations with Modern Health Perspectives

The Chinese health index assessment system integrates the dialectical thinking of traditional philosophy, taking the Taoist cosmology, Confucian ethical view, and the holistic perspective of traditional Chinese medicine (TCM) as its theoretical foundation. It deeply analyzes the sages' conceptions of the ideal personality and social order, as well as their ideas on mind-body cultivation, while incorporating modern health concepts from psychology, medicine. The study systematically explicates the connotation and characteristics of eight components of the Chinese health index, including harmony, dedication, compassion, happiness, peace of mind, innovation, fulfillment, and adaptability. This system establishes a multidimensional assessment framework covering physical health, psychological well-being, human-social relations, and the relationship with nature, which demonstrates high cultural adaptability and practical applicability, offering theoretical reference for assessing the mind-body health of the Chinese population and for informing health promotion policies.
6.Development and Validation of a High-Performance Liquid Chromatography-Tandem Mass Spectrometry Method for Detecting Adrenocortical Hormones and Establishment of Age-Stratified Reference Intervals in Reproductive-Aged Women from Guangxi, China

Background:
Adrenocortical hormones, particularly 11-oxygenated androgens, are pivotal in female reproductive health and fertility. Standardized detection kits and population-specific reference intervals are lacking in China, hindering related clinical applications.
Methods:
A HPLC-tandem mass spectrometry (HPLC-MS/MS) pipeline was developed, rigorously validated, and applied to simultaneously quantify corticosterone, cortisone, cortisol, 18-OH cortisol, androstenedione (A4), 11β-hydroxyandrostenedione (11-OH A4), dehydroepiandrosterone, and dehydroepiandrosterone sulfate in serum samples from 455 reproductive-aged women (18–45 yrs) in Guangxi, China. Age-dependent concentration trends were analyzed, and reference intervals stratified by age (2.5th to 97.5th percentiles) were established. Correlations with body-composition metrics, ethnicity, and the menstrual cycle were investigated.
Results:
The HPLC-MS/MS method demonstrated high precision (intra- and inter-assay CVs < 15%), accuracy, and sensitivity. All eight hormones exhibited significant age-related declines (P < 0.001 for seven hormones; P = 0.001 for 11-OH A4). Notably, 11-OH A4 levels were significantly lower in the 35–45-yr (3.05 nmol/L) and 25–34-yr (3.09 nmol/L) age groups than in the 18–24-yr (3.57 nmol/L) age group, whereas no significant difference was observed between the 35–45-yr and 25–34-yr age groups. Weak negative correlations were observed between the body mass index and corticosterone and cortisone levels, whereas ethnicity and the menstrual cycle showed no significant associations with hormone levels.
Conclusions
We developed an HPLC-MS/MS-based method for simultaneously quantifying eight adrenocortical hormones, including 11-OH A4, and defined age-specific reference intervals for reproductive-aged Chinese women. These findings advance the clinical utility of adrenocortical hormones in diagnosing and managing reproductive disorders.
7.Microcirculation and Cerebrovascular Autoregulation in Patients With Mechanical Circulatory Support Devices

Acute brain injury (ABI) affects up to one-third of patients using mechanical circulatory support (MCS). In venoarterial extracorporeal membrane oxygenation (VA ECMO), ABI incidence (11%–40%) has not improved in two decades. Conversely, improvements in left ventricular assist devices (LVADs) have reduced the incidence of stroke, although it remains a major complication (10%–30%). The failure of MCS to ensure adequate cerebral protection may impair cerebrovascular autoregulation (CVAR) and disrupt microcirculatory function affected by reduced pulsatility, endothelial injury, acute perturbations in partial pressure of arterial carbon dioxide (PaCO2), and cerebral venous congestion. Here, we review evidence demonstrating that these factors alter microcirculatory dynamics and CVAR, thereby contributing to ABI through shared mechanistic pathways. Current methods for assessing CVAR are reviewed, including invasive indices such as the pressure reactivity index (PRx) from intracranial pressure monitoring and noninvasive metrics such as the cerebral oximetry index (COx) from near-infrared spectroscopy or flow-velocity correlations from transcranial Doppler. Each method is limited by feasibility, signal artifacts, and inter-modality variability. Our review identifies three priority areas for cerebral protection in MCS: preservation of pulse pressure, cautious titration of PaCO2, and integration of CVAR-informed blood pressure management. Preliminary evidence suggests that very low pulse pressure, rapid carbon dioxide correction, and persistent microcirculatory impairment are each associated with ABI risk. Future investigations should focus on validating bedside tools to assess CVAR and microcirculatory integrity, and on determining whether physiological targets derived from these measures can improve neurological outcomes in patients using MCS.
8.Mechanisms and Clinical Research Advances of Curcumin in Modulation of Hyperlipidemia

Hyperlipidemia is a common metabolic disorder driven by dysregulated lipid metabolism, whose pathological progression involves multifaceted interactions across various signaling pathways, including disrupted cholesterol homeostasis, insulin resistance, chronic inflammation, and oxidative stress. Although existing clinical medications such as statins can effectively reduce low-density lipoprotein cholesterol through single-target inhibition, significant limitations remain in comprehensively improving reverse cholesterol transport, metabolic inflammation, and coordinated regulation of glucose and lipid metabolism. Curcumin, the primary polyphenolic active compound derived from the traditional medicinal herb turmeric, exhibits multi-dimensional pharmacological effects including modulation of lipid metabolism, improvement of insulin sensitivity, suppression of inflammatory responses, and enhancement of antioxidant defense. This review systematically integrates the multi-target regulatory network of curcumin, elucidating its key mechanisms: Balancing the low density lipoprotein receptor/proprotein convertase subtilisin/kexin type 9 (LDLR/PCSK9) signaling pathway to promote reverse cholesterol transport, activating peroxisome proliferator-activated receptor α/γ (PPARα/γ) signaling pathways to coordinate glucolipid metabolic homeostasis, remodeling the gut microbiota-bile acid axis, inhibiting nuclear factor kappa-B (NF-κB)-mediated inflammatory responses, and activating the nuclear factor erythroid 2-related factor 2/antioxidant response element (Nrf2/ARE) antioxidant signaling pathway. Furthermore, supported by clinical research data, the review validates curcumin's therapeutic potential in reducing total cholesterol (TC) and triglyceride (TG) levels, increasing high-density lipoprotein cholesterol (HDL-C), as well as improving vascular endothelial function and insulin sensitivity. The study highlights the multi-target regulatory properties of curcumin and discusses future clinical applications based on nano-delivery systems and personalized treatment strategies, providing a theoretical basis and novel insights for the prevention and treatment of hyperlipidemia and related metabolic disorders.
9.Analysis of prediction of incidence and epidemiological characteristics of hand, foot and mouth disease among children aged 0-9 years in Guangzhou, 2013-2025

Objective:
To analyze the epidemiological characteristics of hand, foot and mouth disease (HFMD) among children aged 0-9 years in Guangzhou, so as to provide reference basis for further exploring scientifically effective prevention and control measures.
Methods:
Reported cases of HFMD in children aged 0-9 years in Guangzhou City from 2013-2025 were selected from China disease prevention and control information system, and the incidence in different years was analyzed descriptively; the trends of the composition ratio and incidence in various age groups were analyzed by using the Joinpoint regression model, spatial autocorrelation analyses were performed at the district level, and the seasonal autoregressive integrated moving average model (SARIMA) was applied to predict annual incidence rate for 2026.
Results:
From 2013 to 2025, the average annual reported incidence rate of HFMD among children aged 0-9 years in Guangzhou was 2 513.98/100 000, showing a fluctuating trend. Peaks of incidence occurred from May to July and from September to October. The male to female ratio was 1.49∶1. The average annual incidence rate was 4 743.99/100 000 in the 0-1 years age group, 3 298.19/100 000 in the 2-5 years age group, and 544.22/ 100 000 in the 6-9 years age group. Joinpoint regression analysis showed a decreasing trend in the proportion of the 0-1 years age group (APC= -6.02 %, t =-3.77) and an increasing trend in the proportion of the 6-9 years age group (APC=16.39%, t =5.67) (both P < 0.05 ). No significant global spatial clustering was observed at the district level; however, local scattered clusters were identified in certain areas. Using the SARIMA(2,0,0)(2,0,0) 12 model, the predicted annual incidence rate of HFMD among children aged 0-9 years in Guangzhou in 2026 was 2 680.56/100 000.
Conclusions
From 2013 to 2025, the annual incidence rate of HFMD among children aged 0-9 years in Guangzhou exhibited a fluctuating trend; the highest incidence rate is observed in the 0-1 years age group, while the proportion of cases in the 6-9 years age group increase year by year;and local spatial clusters of abnormal distribution are identified. It is essential to enhance risk monitoring and health education in key populations and areas to mitigate the risk of infection in children.
10.Predictive value of bladder deformation index for upper urinary tract damage in neurogenic bladder patients

ObjectiveTo assess the predictive value of the bladder deformation index (BDI) in determining upper urinary tract (UUT) damage among patients with neurogenic bladder (NB). MethodsClinical data of 132 NB patients admitted to Beijing Bo'ai Hospital from January, 2015 to December, 2018 were retrospectively analyzed. Patients were divided into UUT damage group and normal UUT group according to the presence or absence of hydronephrosis. The demographics, biochemical parameters and video-urodynamics (VUDS) findings were collected, and BDI was calculated. Receiver operating characteristic (ROC) curves were utilized to evaluate the predictive capability. ResultsThere were 54 patients in UUT damage group and 33 in normal UUT group. The course of disease, creatinine level and BDI were siginificantly different between two groups (P < 0.05), while the area under the curve were 0.686, 0.836 and 0.928, respectively. ConclusionCourse of disease, creatinine level and BDI are associated with UUT damage in NB patients, and BDI demonstrates the highest sensitivity and specificity, which may play a role in diagnosis of UUT damage.
