2.Comparison of the efficacy of anatomical resection versus hepatic parenchymal preservation preference in patients with solitary small hepatocellular carcinoma and cirrhosis: a multicenter retrospective study

Objective:To investigate the efficacy of anatomical resection (AR) in the early stages of treating solitary hepatocellular carcinoma (HCC) combined with liver cirrhosis with a diameter of ≤5 cm in comparison to different surgical methods of preferential hepatic parenchymal preservation (non-anatomical liver resection, NAR).Methods:The clinical data of 1 390 cases with solitary HCC combined with liver cirrhosis at an early stage who underwent liver resection at Mengchao Hepatobiliary Hospital of Fujian Medical University and six other medical centers from September 2013 to May 2019 were retrospectively analyzed. Patients were divided into the AR group (486 cases) and the NAR group (904 cases) and the wide surgical margin (WSM) group (745 cases) and the narrow surgical margin (NSM) group (645 cases) according to whether they received AR and the width of the surgical margin (1 cm). The basic information of the patients, preoperative evaluation index data, and postoperative follow-up (follow-up every 3 months) were collected. The Kaplan-Meier method was used to plot the survival curve.The log-rank test was used to compare the difference in survival between the two groups. The Cox proportional hazards regression model was used to analyze the factors affecting the prognosis. Propensity score matching (PSM) was applied to reduce intergroup bias.Results:The overall survival (OS) rates for all patients at 1, 3, and 5 years were 95.5%, 79.9%, and 63.5%, respectively. The recurrence-free survival (RFS) rates were 81.5%, 59.0%, and 43.7%, respectively. There was a statistically significant difference in RFS rate between the AR group and the NAR group prior to PSM, but no statistically significant difference in OS rate (RFS rate: 47.0% vs. 41.9%, P<0.05; OS rate: 64.4% vs. 62.9%, P>0.05). The postoperative RFS rate and OS rate were significantly superior in the WSM group than those of the NSM group (RFS rate: 47.8% vs. 37.2%, P<0.001; OS rate: 69.0% vs. 57.3%, P<0.001). There was no statistically significant difference in OS rate and RFS rate between the AR group and the NAR group following PSM (RFS: 46.3% vs. 45.1%, P>0.05; OS rate: 64.0% vs. 64.3%, P>0.05).The 5-year OS and RFS rates in the WSM group were 66.8% and 60.2%, respectively. The 5-year OS and RFS rates for the NSM group were 48.7% and 41.4%, respectively, with a statistically significant difference ( P<0.05). Cox multivariate analysis indicated that serum albumin, tumor diameter, microvascular invasion, and surgical margin were independent prognostic factors affecting OS and RFS. The Child-Pugh grade and satellite lesions were independent prognostic factors affecting OS. Conclusion:Anatomical liver resection is not an independent risk factor for prognosis, but the state of the resection margin determines the prognosis of patients with solitary HCC combined with cirrhosis. Therefore, hepatic resection margins should be prioritized in such patients.

4.Effects of Platelet-Rich Plasma-Derived Exosomes on Proliferation and Migration of Tendon Stem/Progenitor Cell

Objective To investigate the effects of platelet-rich plasma-derived exosomes(PRP-Exos)on the proliferation and migration of tendon stem/progenitor cell(TSPC).Methods PRP-Exos were extracted through the combination of polymer-based precipitation and ultracentrifugation.The morphology,concentration,and particle size of PRP-Exos were identified by transmission electron microscopy and nanoparticle tracking analysis.The expression levels of surface marker proteins on PRP-Exos and platelet membrane glycoproteins were deter-mined by Western blot analysis.Rat TSPC was extracted and cultured,and the expression of surface marker mol-ecules on TSPC was detected using flow cytometry and immunofluorescence staining.The proliferation of TSPC in-fluenced by PRP-Exos was evaluated using CCK-8 assay and EdU assay.The effect of PRP-Exos on the migration of TSPC was evaluated by cell scratch assay and Transwell assay.Results The extracted PRP-Exos exhibit typi-cal saucer-like structures,with a concentration of 4.9 ×1011 particles/mL,an average particle size of(132.2±56.8)nm,and surface expression of CD9,CD63 and CD41.The extracted TSPC expressed the CD44 pro-tein.PRP-Exos can be taken up by TSPC,and after co-cultured for 48 h,concentrations of 50 and 100 μg/mL of PRP-Exos significantly promoted the proliferation of TSPC(both P<0.001),with no statistical difference be-tween the two concentrations(P=0.283).Additionally,after co-cultured for 24 h,50 μg/mL of PRP-Exos significantly promoted the migration of TSPC(P<0.001).Conclusion Under in vitro culture conditions,PRP-Exos significantly promote the proliferation and migration of rat TSPC.
5.Efficacy of concentrated growth factor in the reduction of complications after bone augmentation during surgery for odontogenic maxillary cyst

Objective:To investigate the efficacy of concentrated growth factor (CGF) in the reduction of pain and swelling after bone augmentation during surgery for odontogenic maxillary cyst.Methods:Sixteen patients with odontogenic maxillary cysts who underwent bone augmentation during surgery between June 2018 and December 2019 were included in this study. They were randomly divided into two groups ( n = 8/group). In the observation group, CGF and bone substitutes were mixed and covered with biofilm. In the control group, bone powder was used to fill the bone cavity, and artificial biofilm was applied. Postoperative swelling, pain, and scar hyperplasia were compared between the two groups. Pain was rated according to visual analogue score. Swelling was divided into three grades according to the level of swelling. Scar was compared according to vancouver scar scale. Statistical analysis was performed using SPSS 20.0 software. Results:The percentage of grade 1-3 swelling in the observation group were 62.5% (5/8), 37.5% (3/8) and 0.0% (0/8), respectively, and they were 50.0% (4/8), 37.5% (3/8), 12.5%(1/8), respectively ( Z = -1.71, P > 0.05). There was significant difference in the duration of swelling between observation and control groups [(3.8 ± 0.9) days vs. (5.8 ± 1.4) days, t = 6.88, P < 0.05]. At 3, 7 and 14 days after surgery, visual analogue score was (2.21 ± 0.25) points, (3.75 ± 0.22) points and (0.57 ± 0.13) points in the observation group and it was (3.76 ± 0.18) points, (2.38 ± 0.26) points, and (2.38 ± 0.26) points in the control group ( t = 14.23, t = 11.29, t = 17.61, all P < 0.001). At 1, 2 and 4 weeks after surgery, vancouver scar scale score was (4.26 ± 0.26) points, (1.22 ± 0.13) points and (2.47 ± 0.11) points in the observation group, and it was (6.35 ± 0.27) points, (4.47 ± 0.73) points and (2.77 ± 0.21) points in the control group ( t = 15.77, t = 2.67, t = 3.58, P < 0.001). Conclusion:CGF can promote postoperative bone defect repair and wound healing and reduce postoperative swelling, pain and scar.

7.Application of 3D printing technology in reconstruction and fixation of mxillofacial fracture

Objective:To explore the application value of 3D printing and image inversion in the reduction and fixation of jaw fracture.Methods:A retrospective analysis was conducted in 28 patiens with jaw fracture who had treated by internal fixation of jaw bone in Department of Stomatology, the Second People's Hospital of Lianyungang from January 2018 to Junr 2019.There were 18 males and 10 females.The patients were divided into two groups according to whether they used 3D printing technology to assist surgery.The observation group(15 patients) used 3D printing technology to assist surgery after obtaining 3D CT images.The 13 patients in the control group were operated only with the assistance of three-dimensional reconstruction of CT scan.The operation time, intraoperative bleeding, hospitalization time and postoperative complications were compared between the two groups.Results:All patients were followed-up for 6-12 months, with an average follow-up of 9.8 months.The observation group had significantly shorter operation time[(76.08±5.02)min] and hospital stay[(12.36±1.39)d], less intraoperative blood loss[(81.23±6.12)mL] compared with the control group[(99.12±3.18)min, (16.45±3.27)d, (130.68±3.78)mL]( t=5.324, 3.135, 9.798, all P<0.01). There were no statistically significant differences in the incidences of joint adhesion and incision infection between the two groups (all P>0.05). Conclusion:3D printing technology can achieve preoperative design, surgical simulation and internal fixation of titanium plate precise shape, effectively improve the accuracy and safety of jaw fracture reduction, greatly shorten the operation time.
8.Relationship between plasma neurodegenerative protein level and non-motor symptoms in PD patients

Objective To study the relationship between plasma neurodegenerative protein level and non-motor symptoms(NMS)in PD patients.Methods Eighty-four PD patients served as a PD group and 54age-matched persons undergoing physical examination served as a control group. The NMS of PD patients were assessed according to the HAMD scale.The plasma levels of tau,p-tau181,Aβ-42andα-syn were measured by ELISA and analyzed by Spearman correlation analysis and binary logistic regression analysis respectively.Results The FSS score and plasmaα-syn level were significantly higher while the plasma Aβ-42level was significantly lower in PD group than in control group(3.22±1.68 vs 1.89±1.16,P=0.000;320.00±64.91ng/L vs 277.78±52.75ng/L,P=0.000;267.61±77.75ng/L vs 321.80±49.41ng/L,P=0.001).No significant difference was detected in plasma tau and p-tau181levels between the two groups(P>0.05).The plasmaα-syn level was positively related with the FSS score(r=0.237,P=0.030)and was an influencing factor of FSS(OR=1.019,95%CI:1.006-1.032,P=0.004).Conclusion Plasma neurodegenerative protein level is related with NMS and plasmaα-syn level is a peripheral biomarker for fatigue in PD patients.
9.Bile acid signaling and bariatric surgery

The rapid worldwide rise in obesity rates over the past few decades imposes an urgent need to develop effective strategies for treating obesity and associated metabolic complications.Bariatric surgical pro-cedures,such as Roux-en-Y gastric bypass(RYGB)and vertical sleeve gastrectomy(VSG),currently provide the most effective treatment for obesity and type 2 diabetes(T2D),as well as for non-alcoholic steatohepatitis(NASH).However,the underlying mechanisms of the beneficial effects of bariatric surgery remain elusive.Recent studies have identified bile acids as potential signaling molecules involved in the beneficial effects of bariatric surgery.This review focuses on the most recent studies on the roles of bile acids and bile acid receptors Farnesoid X receptor(FXR)and G protein-coupled bile acid receptor 5(TGR5)in bariatric surgery.We also discuss the possibility of modulating bile acid signaling as a phar-macological therapeutic approach to treating obesity and its associated metabolic complications.
10.Clinical Observations on Needle Knife Treatment for Knee Meniscus Injury

ObjectiveTo observe andanalyze the clinical efficacy of needle knife treatment for knee meniscus injury from improving knee biomechanical balance and to provide a therapeutic basis and method for clinical practice.MethodOne hundred patients (112 knees) with knee meniscus injury meeting the inclusion criteria were allocated, in order of visits, to observation and control groups, 50 cases each. They were treated with a needle knife and an injection of sodium hyaluronate injectio into knee joint cavity, respectively. Changes in the indices were observed and assessed in the two groups by recording the plantar pressure analysis score and the knee function score before and after treatment and at follow-up. The clinical therapeutic effects were evaluated in the two groups.ResultThemaximum load, time integral difference value and load ratio around the knee joint decreased significantly in both observation and control groups after treatment and at follow-up (P<0.05). The regulating effect on knee joint force balance was better in theneedle knife group than in the control group (P<0.05). The improving effect on knee joint function was significantly better in the observation group than in the control group (P<0.01). The excellence rate was significantly higher in the observation groupthan in the control group after treatment and at follow-up (P<0.05).ConclusionNeedle knife treatment restores the function of the knee joint by loosing focal soft tissues around the knee joint, improving knee joint load imbalance and correcting the mechanical equilibrium around the knee joint in patient with knee meniscus injury. It has a definite curative effect on the disease.
