1.Influencing factors of work alienation among Chinese nurses: A meta-analysis

Background Work alienation is a subjectively negative psychological state characterized by detachment from one's job and its environment. Due to heavy workloads, high-intensity tasks, low compensation, as well as pressures related to promotion, nurses are particularly susceptible to work alienation. Objective To meta-analyze the primary factors affecting work alienation of nurses in China, providing an evidence-based foundation for improving nursing management and nurses' physical and mental health. Methods A systematic search was conducted across multiple databases, including PubMed, Embase, the Cochrane Library, Web of Science, CNKI, VIP, Wanfang Data, and DBM, for cross-sectional studies on work alienation and its influencing factors among nurses in China published from inception to December 2024. Two researchers independently conducted literature screening, data extraction, and risk of bias assessment for the included studies. Meta-analysis was performed using Stata 15.0 and RevMan 5.4 software. Pooled effect sizes were calculated using either fixed-effects or random-effects models based on the magnitude of heterogeneity. Results A total of 27 studies involving 13 028 nurses were included in the meta-analysis, from which 11 influencing factors of work alienation were identified. The pooled score for work alienation among nurses in China was 34.79 (95%CI: 32.56, 37.01). Subgroup analysis revealed that the emergency department reported the highest score (36.81, 95%CI: 31.64, 41.98), followed by the intensive care unit (34.99, 95%CI: 32.27, 37.72) and internal medicine department (33.90, 95%CI: 29.10, 38.71). Significant factors influencing work alienation included length of work experience [standardized mean difference (SMD)=0.32, 95%CI: 0.02, 0.61], professional title (SMD=0.30, 95%CI: 0.16, 0.44), monthly income (SMD=0.38, 95%CI: 0.14, 0.62), psychological capital (SMD=–0.41, 95%CI:–0.52, –0.29), work stress (SMD=0.54, 95%CI: 0.49, 0.58), attitude toward aggression and violence management (SMD=–0.38, 95%CI:–0.44, –0.32), and organizational climate (SMD=–0.47, 95%CI: –0.57, –0.36) (P <0.05). Sensitivity analysis confirmed the robustness of the meta-analysis results, and no significant publication bias was detected. Conclusion Nurses in China exhibit a high level of work alienation, particularly in high-acuity settings like emergency and intensive care unit departments compared to general departments. Shorter work experience (<5 years), lower professional title, lower monthly income (<5 000 CNY), and greater work stress are positively correlated with work alienation. In contrast, psychological capital, proactive attitudes toward managing aggression and violence, and a supportive organizational climate are negatively correlated. These findings indicate a need for nursing managers to develop targeted strategies based on these multifaceted factors to mitigate alienation among nursing staff.
2.Application of red blood cell apheresis in a case of juvenile polycythemia vera

Objective: To retrospectively investigate the clinical efficacy, safety, and management experience of a patient diagnosed with polycythemia vera (PV) at the age of 3 who underwent regular erythrocytapheresis for 7 years starting at the age of 10. Methods: Treatment was initiated when the patient's hematocrit was≥50%. The goal was to reduce the hematocrit to below 40%; the removal volume was calculated as 10%-15% of the total red blood cell mass, and an equal volume of normal saline was supplemented during the procedure. Pre-treatment precautions included avoiding fasting and high-fat diets. Post-treatment measures included lying flat for 30 minutes, reducing strenuous exercise for 2–3 days, and rechecking complete blood count 2–5 days after the procedure. During the procedure, adverse reactions such as palpitations, dyspnea, hypotension, and convulsions were monitored. Follow-up was conducted after 3 days to assess any delayed adverse effects. Results: As of August 2025, the patient had undergone a total of 16 erythrocytapheresis sessions. The average volume ranged from 212 to 500 mL. The frequency was 1 to 4 times annually. Post-procedure, hemoglobin (Hb) and hematocrit (Hct) were effectively controlled. Conclusion: Continuous erythrocytapheresis can effectively control Hb and Hct levels in PV patients, preventing adverse effects from the primary disease. Erythrocytapheresis demonstrates efficacy with minimal adverse reactions and can be used as a routine therapeutic technique for PV patients.
3.Correlation Between Traditional Chinese Medicine Constitution and Body Composition Including Visceral Fat in Overweight and Obese Populations

Objective To investigate the relationship between traditional Chinese medicine(TCM)constitution and body composition such as visceral fat in overweight and obese individuals.Methods A total of 320 overweight/obese patients who visited the Preventive Treatment Center of Foshan Hospital of Traditional Chinese Medicine between September 15,2023,and September 14,2024,were selected as study subjects.Among them,135 were classified into the overweight group and 185 into the obese group.Data were collected using the TCM Constitution Questionnaire and a body composition analyzer(InBody570).The differences in TCM constitution distribution between the overweight and obese groups,as well as between genders,were compared.The correlation between TCM constitution types and body composition parameters was analyzed.Logistic regression analysis was employed to identify risk factors for biased constitutions in the overweight/obese population.Results Among the 320 overweight/obese patients,phlegm-damp constitution(56.25%)and damp-heat constitution(40.31%)were the most predominant biased constitutions.The proportions of blood stasis constitution,qi depression constitution,and inherited special constitution in females were significantly higher than those in males(P<0.05 or P<0.01),with the risk of qi depression constitution in females being 6.028 times higher than that in males(P<0.01).Yang deficiency constitution was positively correlated with protein content but negatively correlated with skeletal muscle mass and body fat mass(P<0.05).The proportion of blood stasis constitution in the overweight group was higher than that in the obese group(P<0.01).In overweight/obese individuals with blood stasis constitution,the risk of excessive visceral fat was 2.658 times as high as those without excessive visceral fat.Blood stasis constitution was positively correlated with body fat mass,bone mineral content,intracellular water,and skeletal muscle mass index(SMI),but was negatively correlated with body mass index(BMI)and body cell mass(P<0.05 or P<0.01).Coclusion For overweight and obese populations,special attention should be paid to the management of visceral fat in individuals with blood stasis constitution and to emotional intervention in females.A staged and precise prevention and treatment strategy should be developed by integrating TCM constitution and body composition indicators.
4.Analysis of risk factors for diaphragmatic dysfunction after cardiovascular surgery with extracorporeal circulation: A retrospective cohort study

Objective To clarify the risk factors of diaphragmatic dysfunction (DD) after cardiac surgery with extracorporeal circulation. Methods A retrospective analysis was conducted on the data of patients who underwent cardiac surgery with extracorporeal circulation in the Department of Cardiovascular Surgery of Peking University People's Hospital from January 2023 to March 2024. Patients were divided into two groups according to the results of bedside diaphragm ultrasound: a DD group and a control group. The preoperative, intraoperative, and postoperative indicators of the patients were compared and analyzed, and independent risk factors for DD were screened using multivariate logistic regression analysis. Results A total of 281 patients were included, with 32 patients in the DD group, including 23 males and 9 females, with an average age of (64.0±13.5) years. There were 249 patients in the control group, including 189 males and 60 females, with an average age of (58.0±11.2) years. The body mass index of the DD group was lower than that of the control group [(18.4±1.5) kg/m2 vs. (21.9±1.8) kg/m2, P=0.004], and the prevalence of hypertension, chronic obstructive pulmonary disease, heart failure, and renal insufficiency was higher in the DD group (P<0.05). There was no statistical difference in intraoperative indicators (operation method, extracorporeal circulation time, aortic clamping time, and intraoperative nasopharyngeal temperature) between the two groups (P>0.05). In terms of postoperative aspects, the peak postoperative blood glucose in the DD group was significantly higher than that in the control group (P=0.001), and the proportion of patients requiring continuous renal replacement therapy was significantly higher than that in the control group (P=0.001). The postoperative reintubation rate, tracheotomy rate, mechanical ventilation time, and intensive care unit stay time in the DD group were higher or longer than those in the control group (P<0.05). Multivariate logistic regression analysis showed that low body mass index [OR=0.72, 95%CI (0.41, 0.88), P=0.011], preoperative dialysis [OR=2.51, 95%CI (1.89, 4.14), P=0.027], low left ventricular ejection fraction [OR=0.88, 95%CI (0.71, 0.93), P=0.046], and postoperative hyperglycemia [OR=3.27, 95%CI (2.58, 5.32), P=0.009] were independent risk factors for DD. Conclusion The incidence of DD is relatively high after cardiac surgery, and low body mass index, preoperative renal insufficiency requiring dialysis, low left ventricular ejection fraction, and postoperative hyperglycemia are risk factors for DD.

Clinical practice guidelines represent the best recommendations for patient care. They are developed through systematically reviewing currently available clinical evidence and weighing the relative benefits and risks of various interventions. However, clinical practice guidelines have to go through a long translation cycle from development and revision to clinical promotion and application, facing problems such as scattered distribution, high duplication rate, and low actual utilization. At present, the clinical practice guideline information platform can directly or indirectly solve the problems related to the lengthy revision cycles, decentralized dissemination and limited application of clinical practice guidelines. Therefore, this paper systematically examines different types of clinical practice guideline information platforms and investigates their corresponding challenges and emerging trends in platform design, data integration, and practical implementation, with the aim of clarifying the current status of this field and providing valuable reference for future research on clinical practice guideline information platforms.
6.Salvianolic acid B promotes mitochondrial homeostasis and improves cardiac function in mice with ischemia-reperfusion injury by inhibiting Sirt1 protein degradation.

OBJECTIVES:
To investigate the molecular mechanism by which salvianolic acid B (Sal-B) modulates mitochondrial functional homeostasis and alleviates myocardial ischemia-reperfusion (I/R) injury in mice.
METHODS:
Mouse cardiomyocyte HL-1 cells were pretreated with 5 μmol/L Sal-B with or without sh-Sirt1 transfection before exposure to hypoxia-reoxygenation (HR), and the changes in ATP production, mitochondrial superoxide activity, substrate oxidation level were evaluated. In the animal experiment, 36 C57BL/6J mice were randomized into 3 groups (n=12) for sham operation or ligation of the left anterior coronary artery to induce myocardial I/R injury with or without intravenous injection of Sal-B+I/R (50 mg/kg). In the rescue experiment, 60 adult C57BL/6J mice were randomized into 5 groups (n=12): sham-operated group, myocardial I/R group, Sal-B+I/R group, I/R+Sal-B+Sirt1fl/fl group, and I/R+Sal-B+cKO-Sirt1 group. Myocardial injury was evaluated with HE staining, and cardiac function was assessed by measurement of the ejection fraction and fractional shortening using echocardiography.
RESULTS:
In HL-1 cells with HR injury, Sal-B pretreatment significantly increased cellular ATP production, reduced mitochondrial superoxide anion levels, and enhanced oxygen consumption level. In the mouse models of myocardial I/R injury, Sal-B pretreatment markedly ameliorated I/R-induced structural disarray of the cardiac myocytes and improved cardiac ejection. Cycloheximide chase with Western blotting and ubiquitination assays after Sirt1-IP showed that Sal-B significantly inhibited Sirt1 degradation in HL-1 cells. Sirt1 knock-down reversed Sal-B-induced increases in ATP production, reduction in superoxide, and elevation of OCR in HL-1 cells. Cardiomyocyte-specific Sirt1 knockout obviously reversed Sal-B-mediated improvement in cardiac ejection function and myocardial structure damage in mice with myocardial I/R injury.
CONCLUSIONS
Sal-B promotes mitochondrial functional homeostasis in cardiomyocytes with HR injury and improves cardiac function in mice after myocardial I/R by inhibiting Sirt1 protein degradation.

7.Correlation of neutrophil-to-lymphocyte ratio, lymphocyte-to-monocyte ratio, and C-reactive protein with the efficacy and prognosis of immunotherapy in patients with advanced non-small cell lung cancer

Objective:To investigate the relationship between the neutrophil- to-lymphocyte ratio (NLR), lymphocyte-to-monocyte ratio (LMR), C-reactive protein (CRP), and the efficacy of immunotherapy in patients with advanced non-small cell lung cancer (NSCLC).Methods:The clinical data of 47 patients with NSCLC who received immunotherapy at The First Affiliated Hospital of Anhui University of Science and Technology from December 2021 to May 2023 were retrospectively analyzed. Based on the duration of immunotherapy, patients with a duration of more than 1 year were classified as having a good immune response, while those with a duration of less than 1 year were classified as having a poor immune response. The clinical pathological characteristics of patients with good and poor immune responses were compared. The cutoff values for NLR, LMR, and CRP were calculated using receiver operating characteristic curves, and patients were grouped based on these cutoff values. The predictive probabilities of different combinations were compared. Univariate and multivariate Cox analyses were performed to identify factors affecting patient survival.Results:Significant differences were observed in the distribution of therapy lines (1 st-line vs. 2 nd-line treatment), NLR, LMR, and CRP levels between patients with good immune response and those with poor immune responses (all P < 0.05). The area under the curve (AUC) for NLR was 0.763 [95% CI: (0.608, 0.918)], the AUC for LMR was 0.715 [95% CI: (0.544, 0.875)], and the AUC for CRP was 0.697 [95% CI: (0.540, 0.853)]. To assess the diagnostic value of combined indicators in predicting the efficacy of immunotherapy in NSCLC, different indicators were combined, resulting in the variables NLR + LMR, NLR + CRP, LMR + CRP, and NLR + LMR + CRP. Receiver Operating Characteristic curves were plotted based on the probabilities. The combination of NLR + LMR + CRP showed the best predictive performance, with an AUC of 0.897 [95% CI: (0.806, 0.988)]. Univariate and multivariate Cox analyses indicated that LMR [ HR: 0.428; 95% CI: (0.213, 0.858), P = 0.017] and the distribution of treatment lines [ HR: 1.815; 95% CI: (1.005, 3.642), P = 0.033] were important independent prognostic factors for progression-free survival. Conclusions:NLR, LMR, and CRP are correlated with immunotherapy efficacy in patients with NSCLC and provide predictive value. LMR and treatment line are independent prognostic factors for progression-free survival.
8.Epidemiological characteristics of injury deaths in local residents in Nanjing, 2009-2023

Objective:To understand the epidemiological characteristics of injury deaths in local residents in Nanjing from 2009 to 2023, and provide evidence for the development of injury prevention and control strategies.Methods:The injury mortality data in Nanjing from 2009 to 2023 were analyzed based on the death cause surveillance system. In the recorded 33 542 injury death cases, 19 906 (59.35%) were men, and 13 636 (40.65%) were women. The crude mortality rate, age-standardized mortality rate, age-specific mortality rate, cause-eliminated life expectancy (CELE), potential gains in life expectancy (PGLEs) and life loss rate, were calculated. Joinpoint 5.0 software was used to estimate the average annual percentage change (AAPC) and its 95% CI to assess temporal trends of injury deaths. Results:In Nanjing, the crude injury mortality rate showed an upward trend (AAPC=2.11%), while the age-standardized mortality rate exhibited a downward trend (AAPC=-1.27%) from 2009 to 2023. The ranking of injury deaths in all causes of death declined from the 4 th in 2009 to the 6 th in 2023. The crude and age-standardized injury mortality rates in men were consistently higher than those in women. The primary cause of injury deaths was fall (31.42%). Drowning was the primary cause of injury deaths in age group 0-14 years (35.94%), while traffic accident was the primary cause in age group 15-64 years. For residents aged ≥65 years, fall was the primary cause of injury deaths. From 2009 to 2023, the CELE (AAPC=0.61%, 95% CI: 0.34%-0.89%, P<0.05) and the PGLEs (AAPC=1.73%, 95% CI: 0.21%-3.29%, P<0.05) showed increasing trends. The PGLEs and life loss rate due to injury were consistently higher in men than in women, but the AAPC of PGLEs and life loss rate was higher in women. Conclusions:From 2009 to 2023, the age-standardized injury mortality rate decreased, but the life loss due to injury deaths showed an upward trend in Nanjing, indicating that injury still has non-negligible negative impact on life expectancy.
9.Correlation between serum cytokine expression and clinical characteristics in patients with autoimmune retinopathy

Objective:To investigate the expressions of serum cytokines in patients with autoimmune retinopathy (AIR), and their association with disease diagnosis as well as clinical features.Methods:A prospective case-control study was conducted.A total of 90 eyes of 45 AIR patients were consecutively collected in Beijing Tongren Hospital from September 2018 to December 2023.Additionally, age-matched 43 controls (86 eyes) were enrolled, consisting of 21 patients (42 eyes) with retinitis pigmentosa as a disease control group and 22 healthy subjects (44 eyes) as a normal control group.The main clinical outcome measures included best-corrected visual acuity (BCVA), mean deviation (MD) of visual field, central retinal thickness and the maximal response amplitude and implicit time of full-field electroretinography (ff-ERG). A total of 21 serum cytokines were detected by Luminex multiple cytokines assay or ELISA.Differences in serum cytokine concentrations of among groups, the correlation between cytokine levels and clinical outcomes, and the contribution of cytokines to the diagnosis of AIR were analyzed.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Beijing Tongren Hospital (No.TRECKY2018-048). Written informed consent was obtained from each subject.Results:In AIR patients, the expression levels of Th1-type cytokines or receptors (including interferon-γ[IFN-γ], interleukin [IL]-8, C-X-C motif chemokine ligand [CXCL]9, and CXCL10) and Th17-type cytokines (including IL-17 and IL-6) were elevated.There were statistically significant overall differences in pro-inflammatory cytokines/chemokines (IL-6, IL-8, CXCL10, IL-10, IFN-γ, IL-17, C-X3-C motif chemokine ligand 1 [CX3CL]1, CXCL9) among the three groups ( H=10.823, 10.816, 9.633, 10.103, 23.670, 16.493, 9.050, 9.253; all P<0.05). Compared with the disease control group and the healthy control group, the IFN-γ level was significantly increased in the AIR group (both P=0.001). Compared with the healthy control group, the serum levels of IL-6, IL-8, CXCL10, IL-10, IL-17, and CXCL9 in the AIR group were significantly elevated (all P<0.05). The serum level of CX3CL1 was significantly higher in the AIR group than in the disease control group ( P=0.039). The serum level of IFN-γ was significantly higher in the AIR group than in the healthy control and disease control groups, but no association with AIR diagnosis was found ( OR=1.402, 95% CI: 0.710-2.870, P=0.245). Multilevel mixed-effects regression modeling analysis showed that the serum levels of IL-8 and CXCL9 in AIR patients were positively correlated with LogMAR BCVA ( β=0.028, P=0.033; β=0.023, P=0.003). The C-C motif chemokine ligand 11 level was positively correlated with implicit time of ERG a-wave ( β=13.950, P<0.001). The tumor necrosis factor-α level was positively correlated with MD value of visual field ( β=6.310, P=0.002). Granulocyte-macrophage colony-stimulating factor was negatively correlated with the amplitude of ERG a-wave and granulocyte colony-stimulating factor was negatively correlated with ERG b-wave amplitude ( β=-152.700, P<0.001; β=-14.790, P=0.003). Conclusions:Serum Th1 and Th17-type cytokines/chemokines may be involved in the pathogenesis of AIR and are related to disease severity.
10.The Digital and Intelligent Development of Acupuncture and Moxibustion in China:Discussion Based on Literature

Our acupuncture-moxibustion(Acu-Moxi)field has entered an era of digital and intelligent transformation,yet systematic and in-depth development remains limited.This article aims to provide an overview and reflection on the digital and intelligent progress of Acu-Moxi in China,offering systematic insights for the comprehensive development of this field.Through a retrieval from CNKI,VIP,and Wanfang databases,along with a discussion of relevant policies,case studies,achievements,and updates,the study evaluated the current advancements in the digitization and intelligence of Acu-Moxi,also discussing the current advantages and challenges.Findings revealed an overall upward trend in publications related to digital-intelligent technologies in Acu-Moxi,key forms including robotics,acupuncture point digitization,smart acupuncture devices,and clinical decision-making models,highlighting the empowering potential of digital intelligence across the discipline.Recently,publications of the Acu-Moxi digitization has decreased sharply,and the number of Acu-Moxi research about digital and intelligent transformation has increased.In conclusion,the field has undergone a transition from comprehensive digital accumulation to increasing intelligence,moving toward integrated digital-intelligent development.However,the internal development of digital and intelligent Acu-Moxi remains uneven.To achieve a harmonious intelligent Acu-Moxi ecosystem,it is crucial to leverage opportunities of discipline and times for systematic and balanced improvement.
