Objective To investigate the association between remnant cholesterol(RC)and clinicopatho-logical parameters in patients with IgA nephropathy(IgAN),and to assess the clinical significance of RC in the progression and management of IgAN.Methods A total of 366 patients with a biopsy-proven diagnosis of IgAN were consecutively enrolled in this retrospective study.Clinical and pathological data were systematically collected,and RC was calculated.Participants were stratified into two groups according to the median RC value.Baseline characteristics were compared between these groups.The association between RC and the presence of tubular atrophy/interstitial fibrosis(T1/2 lesions)in IgAN patients was evaluated using binary logistic regression,restricted cubic spline(RCS)analysis,receiver operating characteristic(ROC)curve analysis,and subgroup analyses.Results In this study,the high-RC group exhibited significantly higher levels of body weight,body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),triglycerides(TG),total cholesterol(TC),non-high-density lipoprotein cholesterol(non-HDL-C),uric acid(UA),complement 3(C3),and 24-hour urinary protein,as well as lower levels of albumin(ALB),estimated glomerular filtration rate(eGFR),and high-density lipoprotein cholesterol(HDL-C),compared to the low-RC group.Patients with T1/2 lesions showed elevated levels of SBP,DBP,TC,non-HDL-C,UA,24-hour urinary protein,and RC,along with reduced levels of hemoglobin(Hb),ALB,and eGFR,relative to those with T0 lesions.Multivariate logistic regression analysis indicated that increased RC and decreased eGFR were independent risk factors for the presence of T1/2 lesions in patients with IgAN(P<0.05).RCS analysis revealed a linear association between RC and the likelihood of T1/2 lesions(non-linearity P=0.343).The area under the receiver operating characteristic curve(AUC)for a predictive model incorporating RC,Hb,UA,and eGFR was 0.833,indicating good discriminative ability.Subgroup analyses consistently demonstrated a significant associa-tion between RC and the risk of T1/2 lesions,with no statistically significant interactions observed across subgroups(all P>0.05).Conclusion RC can reflect the severity of both clinical and pathological manifestations in patients with IgAN,making RC monitoring a potentially valuable tool for assessing disease progression in clinical practice.

Objective:To investigate the development of health management (physical examination) institutions in 2018 and provide supporting data for the development of the industry.Methods:Using the method of questionnaire survey, the data of 348 health management institutions in 30 provinces in 2018, including institution scale (area, number of people served, income), Manpower (number of medical staff, number of medical staff with Graduate Education), scientific research ability and level (number of published papers, number of scientific research projects undertaken), appropriate technology application, etc., were obtained. Then according to the province and city where the institution is located and the property of ownership, the institutions were classified and their data were analyzed.Results:In 2018, the area of health management (physical examination) institutions was 3 000 (2 000, 4 434) m 2, the annual number of people served was 5.00 (3.37, 7.97) ×10 4, the annual income was 37.91 (23.91, 61.68) million yuan RMB, and the average cost of subjects was 788 (550, 1 046) yuan RMB. There was a positive correlation between the average cost of subjects and the local per capita GDP ( r=0.438, P<0.05). The top five provinces are Beijing, Zhejiang, Chongqing, Guangdong and Sichuan. The differences of number of people served, the annual income, the average cost of subjects, number of medical staff, number of medical staff with Graduate Education, number of scientific research projects undertaken, proportion of group examination in provincial capital city, prefecture city and county city were statistically significant (all P<0.05). There was no significant difference between the excellent independent private physical examination institutions and the excellent public physical examination institutions ( P>0.05) in the above indexes, but the number of excellent independent private physical examination institutions was relatively small ( n=28, 8.05%). Conclusions:In 2018, China′s health management (physical examination) institutions have a good development momentum, the service scale continues to expand, and the quality and efficiency continue to improve. However, the development of health management (physical examination) institutions is still uneven, and there are regional differences. Some excellent independent private health examination institutions are close to or reach the level of excellent public health examination institutions, but the number is still small.

Objective To investigate the current status, performance,challenges and demands of the health management service-related institutions in China so as to provide evidence for effective policy-making. Methods A cross-sectional study design was used in this report. The health management service-related institutions were identified based on web site searching and government business registration records. Web or mail-based questionnaire was used for data collection. Telephone interview was conducted to confirm the validity and reliability of the data. Results Of 5744 health management service-related institutions in China,585 were investigated for type of organization, personnel profile, business strategy and investment. Since 2000, the number of health management service-related institutions in China had kept increasing in an average rate of 52%. The services included physical check, health education, and disease intervention,etc. Conclusions In China,health management is still at its early stage although an increasing trend could be found and a large number of related- institutions have emerged. However, health management system in China still faces the challenge of poor professional services and insufficient healthcare providers.
