1.Animal study results of a novel designed transcatheter mitral valve replacement system

Objective:To preliminarily assess the biocompatibility and durability of the TruDelta TM transcatheter mitral valve replacement (TMVR) system. Method:Six adult sheep were divided into 3 groups based on the duration of follow-up: 30 days ( n=1), 90 days ( n=3) and 180 days ( n=2). The TruDelta TM TMVR system was implanted through a transapical approach under transesophageal echocardiographic guidance. The operability of the TMVR system was evaluated using an instrument performance evaluation scale (consisting of 39 items), with scores ranging from 1 (worst) to 10 (best) assigned by the operator. Echocardiography was conducted preoperatively, immediately after surgery, and at 30, 90, and 180 days post-implantation. At the last follow-up time point, the intervention mitral valve membrane and major organs were dissected for observation. The artificial valves were taken for hematoxylin eosin (HE) staining and observed under a scanning electron microscope. Result:All six procedures were successfully completed using 29S size TruDelta TM TMVR device. At the final follow-up, echocardiogram demonstrated good valve function without obvious paravalvular leakage, with a transvalvular gradient of (7.8±3.2) mmHg (1 mmHg=0.133 kPa) and a mitral valve orifice area of (1.8±0.2) cm 2. Autopsy findings revealed no structural valve failure and almost complete endothelialization (>75%) with 90 to 180 days. Both HE staining and scanning electron microscopy confirmed optimal endothelialization of the valve stent. Conclusion:The preclinical animal study indicates that the TruDelta TM device exhibits favorable biocompatibility and durability.
2.Prognostic factors for endovascular treatment of acute anterior circulation vessel occlusion with large-core infarction

Objective To investigate the potential beneficial predictors for endovascular treatment(EVT)in case of acute anterior circulation large vessel occlusion with large-core infarction.Methods We made a retrospective inclusion of 92 patients with anterior circulation large vessel occlusion and core infarct volume ranging from 50 to 100 mL,who underwent EVT at Zhangzhou Municipal Hospital of Fujian Province from March 2018 to February 2021.All the patients were confirmed as anterior circulation large infarction and large infarct volume using computed tomography(CT),computed tomography angiography(CTA),and computed tomography perfusion(CTP)before EVT.All clinical and imaging data were collected to observe the postoperative recurrence rate,incidence of symptomatic intracranial hemorrhage(sICH),and functional prognosis 90 days.Then the patients were divided into favorable outcomes group(mRS≤3)and unfavorable outcomes group(mRS>3)based on Rankin scores 90 days post-stroke.Univariate and multivariate Logistic analysis were conducted to analyze factors influencing the patients'prognosis.By combining the results of multivariate analysis,we constructed receiver operating characteristic(ROC)curves and identified the cut-off value to evaluate the predictive value of age for post-vascular treatment prognosis.Results Among the included patients,the postoperative revascularization rate(mTIICI≥2b)was 95%(88 cases),the rate of favorable outcomes at 90 days postoperatively(mRS≤3)was 32.61%(30 cases),the incidence of sICH was 13%,and the mortality rate was 31.5%.Compared with the unfavorable outcomes group,the favorable outcomes group had a younger age and a higher proportion of males.Multivariate analysis indicated that older age was an independent risk factor for adverse outcomes following endovascular treatment(OR=4.97,95%CI:1.78-13.90,P=0.002).The ROC curve indicated that the area under the curve was maximized at the age of 72.5 years(AUC=0.763,95%CI:0.661-0.864,P<0.001).Its sensitivity and specificity was 0.565 and 0.833,respectively.Conclusion Age is an independent predictor of the prognosis of acute procirculatory large core stroke.Patients with large core infarction older than 72.5 years may not benefit from endovascular therapy,which needs to be confirmed by a multicenter large sample prospective randomized controlled trial.
3.Research on prediction model of protein thermostability integrating graph embedding and network topology features.

Protein structure determines function, and structural information is critical for predicting protein thermostability. This study proposes a novel method for protein thermostability prediction by integrating graph embedding features and network topological features. By constructing residue interaction networks (RINs) to characterize protein structures, we calculated network topological features and utilize deep neural networks (DNN) to mine inherent characteristics. Using DeepWalk and Node2vec algorithms, we obtained node embeddings and extracted graph embedding features through a TopN strategy combined with bidirectional long short-term memory (BiLSTM) networks. Additionally, we introduced the Doc2vec algorithm to replace the Word2vec module in graph embedding algorithms, generating graph embedding feature vector encodings. By employing an attention mechanism to fuse graph embedding features with network topological features, we constructed a high-precision prediction model, achieving 87.85% prediction accuracy on a bacterial protein dataset. Furthermore, we analyzed the differences in the contributions of network topological features in the model and the differences among various graph embedding methods, and found that the combination of DeepWalk features with Doc2vec and all topological features was crucial for the identification of thermostable proteins. This study provides a practical and effective new method for protein thermostability prediction, and at the same time offers theoretical guidance for exploring protein diversity, discovering new thermostable proteins, and the intelligent modification of mesophilic proteins.

6.Application of self-monitoring of blood glucose information management platform based on medical consortium in community patients with type 2 diabetes

Objective To study the effect of self-monitoring of blood glucose information management platform based on the medical consortium on the level of blood glucose control by improving the implementation rate of self-monitoring of blood glucose prescriptions for type 2 diabetes patients in the community.Methods A convenient sampling method was used to select 121 patients with type 2 diabetes from April 2021 to April 2023 who were enrolled in a community of a tertiary hospital medical consortium in Beijing.They were randomly divided into 61 patients in an experimental group and 60 patients in a control group.The control group received routine blood glucose monitoring and guidance education,while the experimental group was managed by a self-monitoring of blood glucose information management platform based on the medical consortium with nurses as the main body.Glycated hemoglobin,glucose test,fasting blood sugar,blood glucose 2 hours after meal,and the completion of self blood glucose monitoring prescriptions were evaluated and analyzed in the first week and the twelfth week of the 2 groups.Results 61 cases in the experimental group and 58 cases in the control group completed the follow-up,and the patients who completed the follow-up were analyzed.There was no statistically significant difference in glycated hemoglobin between the 2 groups at baseline(P=0.117).After 12 weeks,the reduction standard-reaching rate in glycated hemoglobin in the experimental group was significantly different from that in the control group(P=0.014).The number of self-monitoring of fasting blood glucose and the total number of other time periods in experimental group were significantly higher than those in the control group(P<0.001);the implementation rate of self-monitoring prescription in the experimental group was better than that in the control group(P<0.001).Conclusion A self blood glucose monitoring information platform was built to manage community type 2 diabetes patients,so that patients can master and implement the personalized blood glucose monitoring prescription formulated by the medical union team with nurses as the main body,and it is conducive to improving the frequency of blood glucose monitoring and the level of blood glucose control of diabetes patients.
7.A scope review of self-management intervention research in patients with hematopoietic stem cell transplantation

Objective To review the research of self-management intervention in patients with hematopoietic stem cell transplantation,clarify the intervention elements,so as to provide references for clinical practice and future related research.Methods According to the scoping review framework,a literature search was performed in the PubMed,Embase,CINAHL,Web of Science,Cochrane Library,CNKI,Wan fang Database,VIP,CBMdisc for all the studies on self-management intervention of patients with hematopoietic stem cell transplantation.The retrieval time was from the establishment of the database to December,2022.There were 2 researchers who extracted and analyzed the data of the included literature.Results A total of 12 articles were included,including 3 randomized controlled trials,8 quasi-experimental studies,and 1 mixed study.6 studies were conducted with interventions based on the theoretical basis or nursing model such as self-management oriented 5A nursing model,continuous nursing model,problem-based learning theory.The intervention methods of 12 studies included nurse-led self-management guidance,mobile applications,multidisciplinary collaboration,and graphic cards.The intervention time was mostly 1~3 months.The outcome indicators included feasibility evaluation indicators such as operability,ease of use,and user satisfaction of the application,and effect evaluation indicators such as self-management ability,quality of life,and medication compliance of patients.The results showed that self-management interventions could improve patient self-management ability,quality of life,medication compliance,and improve their psychological and nutritional status.Conclusion The research on self-management intervention in patients with hematopoietic stem cell transplantation is still in the development stage,and its positive effect has been preliminarily verified.Medical staff should strengthen the self-management evaluation of patients with hematopoietic stem cell transplantation,play the role of multidisciplinary team,use electronic health intervention methods,and formulate multi-level,scientific and effective self-management intervention programs.
8.Curative effect of synchronous brain bionic electrical stimulation in hyperbaric oxygen chamber on patients with sudden deafness and the hemorheological indictors’ changes

Objective:To explore the curative effect of synchronous bionic electrical stimulation in hyperbaric oxygen chamber on patients with sudden deafness(SD)and the changes in hemorheological indicators.Methods:A total of 128 SD outpatients and inpatients treated at the Hyperbaric Oxygen Department of the Sixth Medical Center of the Chinese PLA General Hospital from June 2020 to June 2021 were randomly divided into study group(70 cases treated with synchronous brain bionic electrical stimulator in hyperbaric oxygen chamber)and control group(58 cases given conventional hyperbaric oxygen therapy),respectively. The curative effects of the two groups were evaluated,and the hemorheological indicators,such as the whole blood(high-shear and low-shear)viscosity,plasma viscosity,and red blood cell aggregation,were analyzed by the automatic biochemical analyzer before and after treatment.Results:After two courses of treatments,the overall effective rate of the study group(88.57%)was significantly higher than that of the control group(79.31%),and the difference was statistically significant( P<0.05). After treatment,the whole blood(high-shear and low-shear)viscosities,plasma viscosity,and erythrocyte aggregation indicators of the two groups were significantly decreased compared with those before treatment( P<0.05),and the above indicators in the study group were significantly lower than those in the control group( P<0.05). Conclusions:Synchronous brain bionic electrical stimulation in the hyperbaric oxygen chamber can improve the hemorheological indicators of the SD patients,reduce their blood hypercoagulability,and improve their hearing level and treatment effect. Hence,it is worth of clinical promotion.

10.Magnetic resonance imaging changes and curative effects of early different pressure hyperbaric oxygen on moderate and severe traumatic brain injury in rats

Objective:To investigate the characteristics of magnetic resonance imaging changes and the curative effects of early different pressure hyperbaric oxygen (HBO) on moderate and severe traumatic brain injury in rats.Methods:Sixty healthy clean male adult Sprague-Dawle rats were randomly divided into four groups by random number table: traumatic brain injury group (TBI group), sham operation group (sham group. Only opening the skull, but no head strike), 0.15 MPa HBO group (giving 0.15 MPa HBO to TBI rats), and 0.25 MPa HBO group (giving 0.25 MPa HBO to TBI rats). Each group has 15 rats. All rats were subjected to modified neurological severity score (mNSS) evaluation and MRI examination on day 1 (D1), day 3 (D3), and day 7 (D7) respectively. Three regions of interest (ROI) were taken from the high-signal regions of the magnetic resonance DWI image. The apparent diffusion coefficient (ADC) of each region was measured. The ratios of the ADC of the lesion side to the ADC of the normal contralateral side, i. e. the relative ADC (rADC), were compared.Results:(1) On D1, all the rADC values of ROIs in each group were not statistically significant ( P>0.05). On D3, the rADC values of different ROIs in 0.15 MPa HBO and 0.25 MPa HBO groups were all lower than those in TBI group ( P< 0.05), but all higher than sham group ( P<0.05); there was no significant difference between 0.15 MPa HBO and 0.25 MPa HBO groups ( P>0.05). On D7, the rADC values of different ROIs in 0.15 MPa HBO and 0.25 MPa HBO groups were all lower than those of TBI group ( P< 0.05); there was no significant difference when those compared with sham group ( P>0.05), and there was no significant difference between 0.15 MPa HBO and 0.25 MPa HBO groups ( P>0.05). (2) On D1, there was no significant difference in mNSS scores among TBI group, 0.15 MPa HBO group and 0.25 MPa HBO group ( P>0.05), but the mNSS scores of TBI group, 0.15 MPa HBO group and 0.25 MPa HBO group were all significantly higher than that of sham group ( P<0.05). On D3, the mNSS scores of 0.15 MPa HBO and 0.25 MPa HBO groups were all lower than that of TBI group ( P<0.05), but all higher than that of sham group ( P<0.05); there was no significant difference between 0.15 MPa HBO group and 0.25 MPa HBO group ( P>0.05). On D7, the scores of mNSS in 0.15 MPa HBO and 0.25 MPa HBO groups were all lower than that in TBI group ( P<0.05), but all slightly higher than that in sham group ( P>0.05). Conclusion:There is definite curative effect of early HBO on moderate and severe TBI, but the pressure of HBO may not be the key factor in determining prognosis.
