Gastric cancer is a high-incidence malignancy that poses a serious threat to public health in China, ranking among the top three cancers in both incidence and mortality. The majority of patients are diagnosed at an advanced stage, resulting in limited treatment options and poor prognosis. To address key challenges in gastric cancer diagnosis and treatment, a research team led by Professor Jiafu Ji at Peking University Cancer Hospital has focused on the project "Development and Dissemination of Precision Medicine Approaches in Gastric Cancer Management". Through a series of high-quality multicenter clinical studies, the team established a set of new international standards in perioperative treatment, individua-lized drug selection, intelligent noninvasive diagnostics, and novel immunotherapy strategies. These advances have significantly improved treatment efficacy and reduced surgical trauma, achieving key technological breakthroughs in diagnosis, therapy, and mechanistic understanding, and systematically enhancing outcomes for gastric cancer patients. The project ' s findings had a broad international impact, including hosting China ' s first International Gastric Cancer Congress. Through nationwide dissemination, they have promoted the development of precision diagnosis and treatment of gastric cancer as a discipline, and led the formulation of the National Health Commission's guidelines for gastric cancer diagnosis and treatment. In recognition of its achievements, the project was awarded the First Prize of the 2024 Chinese Medical Science and Technology Award.

Objective:To conduct a scoping review on the impact of community environment on the health of older adults from the perspective of person-environment fit, so as to provide reference for the creation of targeted care environments and the construction of elderly friendly communities.Methods:Literature on the impact of community environment on the health of older adults from the perspective of person-environment fit was searched on China National Knowledge Infrastructure, WanFang Data, VIP, China Biology Medicine disc, PubMed, Embase, Web of Science, Cochrane Library, Scopus, CINAHL, PsycInfo and so on. The search period was from database establishment to December 31, 2023. Literature was screened and data was extracted based on inclusion criteria.Results:A total of 19 articles were included, and the methods of person-environment fit included compensation and consistency. Sixteen articles described how community environment promoted the health of older adults by compensating for the decline in individual abilities and socio-economic disadvantages of older adults. The poorer the individual abilities and lower the socio-economic status of older adults, the greater the impact of environmental factors. Three articles mentioned that the consistency between community environment and the needs of older adults could promote their health.Conclusions:The interaction between the individual characteristics of older adults and their surrounding environmental resources determines their health status. When building elderly friendly communities and providing care environments, consideration should be given to the individual abilities and needs of older adults, and the impact mechanism of the environment on the health of older adults can be explored from multiple perspectives.

Objective To study the montelukast sodium chewable tablets for the treatment of obstructive sleep apnea hypopnea syndrome(OSAHS)patients and treatment of tonsil and adenoid effect.Methods 84 cases of OSAHS patients in our hospital from January 2014 to August 2016 were randomly divided into the observation group and the control group,42 cases in each group.The control group was treated with conventional symptomatic treatment,on this basis,the patients in the observation group received montelukast sodium chewable tablets treatment,compared the two groups of the total efficiency,and compared before and after treatment of symptoms total score,apnea hypopneaIndex(AHI),the lowest arterial oxygen saturation(LSaO2),and adenoid and tonsil classification indexing.Results the total efficiency of observation group(90.48％)was significantly higher than the control group(69.04％,χ2=5.97,P<0.05); after treatment,the observation group of the total symptom score and AHI were significantly lower than the control group(P<0.05),LSaO2 was significantly higher than the control group(P<0.05),adenoid grade one and grade two were significantly higher than the control group(χ2=9.82,P<0.05),there was no significant difference between the one and two degrees of tonsil dividing ratio.Conclusion The clinical efficacy of montelukast sodium chewable tablets in the treatment of children with OSAHS is significant,can effectively reduce the volume of tonsil and adenoid body,and improve the clinical symptoms and sleep treatment significantly,which is worthy of clinical promotion.

Objective To study the clinical effect of phentolamine combined with furosemide therapy in children with pneumonia complicated with heart failure.Methods The subjects were 92 cases of children with pneumonia complicated with heart failure in our hospital from October 2015 to December 2016,randomly divided into control group and observation group,46 cases in each group.The control group was treated with on the basis of routine treatment,the observation group,patients received phentolamine combined with furosemide treatment.Compared two groups of the total efficiency and the symptoms disappear time,and compare the cardiac function and electrocardiogram improvement effect.Results The total efficiency of observation group(93.48％)was significantly higher than the control group(71.74％)(χ2=7.57,P<0.05); observation group of the pulmonary rales disappeared time,breath,heart failure control time and hospitalization time were significantly lower than the control group(P<0.05); after treatment,the observation group of the ESV was significantly lower than the control group(P<0.05),EDV was significantly higher than that of control group(P<0.05),electrocardiogram indicators improved significantly better than the control group(P<0.01).Conclusion The clinical curative effect of phentolamine combined with furosemide in treatment of children with pneumonia complicated with heart failure significantly,can effectively improve the cardiac structureAnd function,reduce the occurrence of abnormal ECG,high safety,worthy of clinical promotion

Objective To study the effect of low molecular weight heparin calcium combined with Bailing capsule in treatment of children with Henoch-Schonlein purpura nephritis and the efficacy of serum hs-CRP, MMP-9, APN and CysC.MethodsFrom January 2013 to December 2015, 84 children with Henoch-Schonlein purpura nephritis were enrolled in our hospital.According to the order of admission, the observation group and control group were divided into observation group and control group.The control group were given low molecular weight heparin calcium, the observation group were treated with low molecular weight heparin calcium combined with Bailing capsule for treatment.The levels of serum hs-CRP, MMP-9, APN, CysC, inflammatory factors and immune levels were observed in the two groups.ResultsAfter treatment, the clinical effective rate of the observation group was significantly higher than that of the control group[92.86% (39/42) vs 69.05% (29/42)] (P<0.05).There were no significant differences in hs-CRP, MMP-9, APN and CysC levels between the two groups.After treatment, the levels of hs-CRP, MMP-9 and CysC in the two groups were significantly higher than those before treatment (P<0.05).APN was significantly higher than that before treatment (P<0.05).Compared with the control group, the levels of hs-CRP, MMP-9 and CysC in the observation group were lower than those in the control group (P<0.05), and the APN level was lower than that in the control group (P<0.05).There were no significant differences in TNF-α, IL-6, IL-10 and IL-18 levels between the two groups before treatment.After treatment, The levels of TNF-α, IL-6 and IL-18 in the two groups were significantly lower than those before treatment (P<0.05).There were no significant differences in IgA, IgG and IgM levels between the two groups.After treatment, the levels of IgA, IgG and IgM in the two groups were significantly lower than those before treatment (P<0.05).The levels of IgA, IgG and IgM in the observation group were lower than those in the control group (P<0.05).There was no significant difference in adverse reaction rate between the two groups.ConclusionLow molecular weight heparin combined with Bailing capsule can effectively reduce hs-CRP, MMP-9, CysC level and APN level in children with Henoch-Schonlein purpura nephritis, improve clinical symptoms, good clinical efficacy and high safety.

OBJECTIVEPeriodontal tissue remodeling includes remodeling of alveolar bone, periodontal ligament, and cementum. Cementoblast plays a main role in repairing root resorption. Canonical Wnt/β-catenin signaling can promote the odontogenic differentiation in osteoblast. However, the mechanism on how the orthodontic force influences the function of cementoblast and the relationship between the canonical Wnt/β-catenin signaling and Runx2 of cementoblast are not yet known. The aim of this study is focus on this relationship.

METHODSOCCM30 cementoblasts were subjected to mechanical strain by four-point bending system with tension stress for 0, 3, 6, and 12 h. They were pretreated with different concentrations of Dikkopf-1 (DKK1) for 48 h. Western blot analysis was performed to detect the β-catenin levels in the nucleus. Runx2 mRNA was observed by real-time quantitative polymerase chain reaction (RT-PCR). OCCM30 cementoblasts were then pretreated with 150 ng · mL(-1) DKK1 for 48 h and subjected to mechanical strain by FX4000T system with tension stress for 12 h. Western blot analysis was conducted to detect the β-catenin levels in the nucleus, and Runx2 mRNA was observed by RT-PCR.

RESULTSOCCM30 cementoblasts had significantly higher Runx2 mRNA and β-catenin levels after being loaded with mechanical stress. The amount of Runx2 mRNA in OCCM30 cementoblasts was significantly decreased by DKK1. When OCCM30 cemento-blasts were pretreated with DKK1 without stress, their β-catenin level was significantly decreased by DKK1 and Wnt signaling was blocked. When they were not pretreated with stress, the β-catenin level with DKK1 was lower than that without DKK1. Without DKK1, the β-catenin level in OCCM30 cemento- blasts increased afterbeing loaded with mechanical stress. With DKK1, the β-catenin level in OCCM30 cementoblasts, which were loaded with mechanical stress, was higher than that without mechanical stress.

CONCLUSIONCementoblasts had higher Runx2 mRNA expression under mechanical stress because of the Wnt/β-catenin signaling pathway effect.
