1.Screening of IgG N-glycosylation markers associated with ankylosing spondylitis

Objective:To evaluate the potential of IgG N-glycans as diagnostic biomarker for ankylosing spondylitis (AS) by comparing and analyzing the IgG N-glycan profiles with AS and healthy controls.Methods:A 1∶1 matched case-control study design was adopted, 81 AS patients who visited the Department of Rheumatology and Immunology at Taian City Central Hospital and the Second Affiliated Hospital of Shandong First Medical University between July 2020 and June 2021 were recruited. These patients were matched with 81 healthy individuals undergoing routine physical checkup. The levels of IgG N-glycosylation in human plasma were quantitatively measured using ultrahigh-performance liquid chromatography. Binomial logistic regression analysis was performed to identify IgG N-glycan biomarkers associated with AS.Results:A total of 14 primary glycans and 13 derived traits showed statistically significant differences between the AS case group and the control group. Binomial logistic regression analysis showed that glycan peak 4, agalactosylated glycans, fucosylated glycans, and fucosylated agalactosylated glycans were positively associated with AS[ OR(95% CI)=1.12(1.01, 1.42), 1.21(1.03, 1.43), 1.48(1.08, 2.03), and 1.27(1.04, 1.55); P=0.036, 0.022, 0.039, 0.020, respectively]. In terms of diagnostic performance, the single glycan GP4 exhibited the largest area under the ROC curve, with an AUC (95% CI) 0.751 (0.677, 0.826), while the combined glycan indicators (GP4+G0+F+FG0) achieved an AUC (95% CI) 0.768(0.697, 0.840). Conclusion:IgG N-glycans have the potentials to serve as candidate biomarkers for AS, and warrants further investigation.

3.Safety and Effectiveness of Peroral Cholangioscopy With ERCP in Biliary Diseases

Background:With the development of medical technology,cholangioscopy has been increasingly utilized in the diagnostic and therapeutic process of endoscopic retrograde cholangiopancreatography(ERCP).They play a significant role in the visual diagnosis and biopsy of tumors,providing more accurate and detailed information for clinicians.Aims:To evaluate the safety and effectiveness of peroral cholangioscopy combined with ERCP in biliary diseases.Methods:A total of 90 cases underwent peroral cholangioscopy and ERCP from October 2022 to October 2023 at Shanghai General Hospital,Shanghai Jiao Tong University School of Medicine were recruited retrospectively,and the clinical data were collected.The success rate,postoperative complications,and other relevant indicators were evaluated.Results:A total of 90 patients were enrolled,the preoperative diagnosis including common bile duct stones(43 cases),biliary obstruction by tumor invasion(21 cases),benign biliary stenosis(15 cases),benign ampullary tumors(6 cases),postoperative biliary stricture after liver transplantation(3 cases),primary sclerosing cholangitis(1 case),and biliary varices(1 case).The technical success rate of peroral cholangioscopy with ERCP was 100%,and the stone removal rate was 95.3%(41/43),the average operation time was(81±37)min,and the ERCP re-intervention rate within six months was 17.8%.Postoperative complications included fever(7.8%),abdominal pain(3.3%),bleeding/anemia(2.2%),and postoperative pancreatitis(2.2%).Conclusions:The combination of peroral cholangioscopy with ERCP is safe and effective for the biliary diseases with no increase in procedure-related adverse events and risks.

5. Predictive Risk Factors for Intraoperative Hypothermia During Endoscopic Retrograde Cholangiopancreatography Under General Anesthesia

Background: Unintended intraoperative hypothermia is a common complication of general anesthesia surgery, which can cause pain, coagulation dysfunction, wound infection, delayed recovery, and other adverse consequences. There are few studies related to intraoperative hypothermia during endoscopic retrograde cholangiopancreatography (ERCP). Aims: To analyze the risk factors of intraoperative hypothermia during ERCP under general anesthesia and establish a predictive model. Methods: A total of 121 patients underwent ERCP under general anesthesia from September 2021 to November 2021 at Shanghai General Hospital were recruited, and relevant clinical data were collected. Logistic regression analysis was used to screen risk factors, and a predictive model was constructed. The model was externally validated by independent datasets with ROC curve and Hosmer⁃Lemeshow goodness of fit test. Results: A total of 114 patients were enrolled in modeling group. The incidence of intraoperative hypothermia was 11.40% (13/114). There were more women in the hypothermia group (P<0.05). The temperature of entering the operating room and operating room temperature were relatively lower in the hypothermia group (P<0.05). Gender was an independent risk factor for intraoperative hypothermia in ERCP under general anesthesia (P<0.05). The predictive model constructed by using gender and temperature of entering the operating room screened by Logistic regression analysis had a good discrimination and calibration, area under the ROC curve by external validation was 0.78. Conclusions: Gender and temperature of entering the operating room can effectively predict the occurrence of intraoperative hypothermia and assist perioperative monitoring and management.
6.Analysis of prognostic factors in patients with hepatocellular carcinoma combined with portal vein tumor thrombus after surgical resection

Objective:To analyze prognostic factors of hepatocellular carcinoma (HCC) combined with portal vein tumor thrombus (PVTT) after surgical resection.Methods:The data of 98 patients with HCC combined with PVTT who underwent surgical resection in Ningbo University Affiliated Li Huili Hospital from January 2008 to June 2019 were analyzed retrospectively, including 83 males and 15 females with an average age of 53 years. The survival rate was calculated by Kaplan-Meier method and compared using log-rank test. Cox regression model was used for the multivariate analysis of the prognosis of patients.Results:The 1, 2, and 3-year overall survival rates were 75.9%, 51.5%, and 35.4%, respectively, with a median survival time of 25 months; the 1, 2, and 3-year disease-free survival rates were 35.0%, 16.1%, and 8.6%, respectively, with a median disease-free survival time of 8 months. Multivariate analysis showed that the overall survival of patients with preoperative alpha-fetoprotein (AFP) ≥400 μg/L ( HR=1.760, 95% CI: 1.079-2.873) and hepatic vein tumor thrombus (HVTT, HR=3.809, 95% CI: 1.655-8.765) was poorer after surgical resection (all P<0.05), while the survival of patients with postoperative adjuvant trans-arterial chemoembolization (TACE) ( HR=0.397, 95% CI: 0.220-0.716, P=0.002) was better. Preoperative AFP≥400 μg/L ( HR=2.339, 95% CI: 1.488-3.676) , undergoing HCC resection combined with PVTT dissection ( HR=2.038, 95% CI: 1.090-3.811), and with HVTT ( HR=2.374, 95% CI: 1.160-4.857) (all P<0.05) are independent risk factors for recurrence in patients with HCC combined with PVTT, postoperative adjuvant TACE ( HR=0.535, 95% CI: 0.307-0.933, P=0.027) is a protective factor. Conclusion:Preoperative AFP≥400 μg/L and HVTT are independent risk factors for the prognosis of patients with HCC combined with PVTT. Reasonable selection of surgical methods and postoperative adjuvant TACE may improve the prognosis of patients.
7. Analysis of the potential role of SET in chromium-induced malignant transformation cells based on quantitative proteomics

Objective:
To investigate alteration of proteins profile in malignant transformation bronchial epithelial cells(16HBE-T) induced by hexavalent chromium[(Cr(VI))] and analyze the expression level of SET protein, then to provide some new insights for the carcinogenesis mechanism of Cr(VI).
Methods:
Total protein was extracted from 16HBE cells and was alkylated and desalinated before digested into peptides. The products were labeled with Tandem Mass Tag (TMT) and identified using LC-ESI-MS/MS.
Results:
A total of 3 517 proteins were found, expression differences greater than 1.5 or less 0.67 times were to found have 185 and 201 proteins, respectively. Gene enrichment analysis revealed that differential proteins were mainly involved in autophagy, DNA damage repair, RNA processing and other biological processes. Western blot results showed the expression level of SET was significantly increased while downregulated in histone H3K18/27 acetylation and p53 protein.
Conclusion
Proteins involved in multiple biological processes altered in 16HBE-T cells and regulation mode of SET inhibiting histone H3K18/27 acetylation regulating transcriptional activity of p53 may paly an important role in Cr(VI)-association carcinogenesis.
8. Studies on the DNA damage in the transformed bronchial epithelial cells induced by hexavalent chromium

Objective:
To investigate DNA damage in the transformed human bronchial epithelial cells (16HBE) induced by hexavalent chromium (Cr6+) and further elucidate the potential carcinogenesis mechanism of Cr6+.
Methods:
16HBE were treated with different concentration of Cr6+ (0, 0.625, 1.25, 2.5 μmol/L) for 15 weeks. The malignant degrees of transformed cells were identified by the assays for anchorage-independent growth and tumorigenicity. According to the single cell gel electrophoresis (SCGE) assay, the DNA damage rate was calculated. The expression level of 53BP1 was determined by Western blot.
Results:
Chromium-treated cells could form colonies in soft agar and tumors in nude mice. Compared with the control group, colony formation efficiency of 1.25μmol/L and 2.5 μmol/L Cr6+-treated cells in soft agar showed significant increases (p<0.05) . The 2.5 μmol/L Cr6+-treated cells also formed tumors subcutaneously in nude mice. Cr6+ could cause different degree of DNA damage to 16HBE cells in a dose-dependent manner. In addition, Western blot analyses showed that 53BP1 was aberrantly down-regulated at 2.5 μmol/L dose and has no significant changes at 0.625 μmol/L and 1.25 μmol/L dose under the treatment of Cr6+.
Conclusion
The declined expression of 53BP1 may mediate Cr6+-induced DNA damage and further involved in the cell malignant transformation.
9.Effects of transcutaneous electrical acupoint stimulation assisting sodium nitroprusside controlled hypotension on Glu,ATⅡ and SOD

Objective To observe the effects of transdermal acupoint electric stimulation(TEAS)assisting sodium nitroprus-side induced controlled hypotension on serum glucose (Glu),angiotensin Ⅱ (ATⅡ)and superoxide dismutase (SOD),and to inves-tigate the protective effect of TEAS under controlled hypotension anesthesia.Methods 60 cases undergoing elective endoscopy si-nus surgery by adopting sodium nitroprusside induced controlled hypotension under general anaesthesia maintained the mean arterial pressure(MAP)in 50-60 mm Hg and were randomly and equally divided into two groups.The group Ⅰ conducted TEAS,while the group Ⅱ did not conduct TEAS.The controlled hypotension time and surgery time were recorded in the two groups;Glu and ATⅡ values were detected before anesthesia (T0 ),30 min after hypotension (T1 ),hypotension stopping(T2 );SOD was detected at T0 ,T2 ,30 min after hypotension(T3 ).Results The operation time and controlled hypotension continuous time had no statistically significant difference between the two groups(P >0.05).The Glu value in the group Ⅰ had no statistically significant difference a-mong the 3 time points,while which at T1 ,T2 was higher than that at T0 in the group Ⅱ(P <0.05),and which at T1 ,T2 in the group Ⅱ was higher than that in the group Ⅰ (P <0.05);the ATⅡ value at T1 was higher than that at T0 in the group Ⅰ (P <0.05),while which at T1 ,T2 was higher than that at T0 in the group Ⅱ(P >0.05),which at T1 ,T2 in the group Ⅱwas higher than that in the group Ⅰ (P <0.05);the SOD value at T2 was lower than that at T0 in the group Ⅰ,which at T2 ,T3 in the group Ⅱwas lower than that in the group Ⅰ(P <0.05).Conclusion TEAS assisting sodium nitroprusside controlled hypotension can better in-hibit the stress response.
10.Effect of fenofibrate on the matrix metalloprotenase-9 expression and the cultured human umbilical vein endothelial cell proliferation induced by oxidized low density lipoprotein

Objective To investigate the effect of fenofibrate on the matrix metalloprotenase-9 (MMP-9) expression and the proliferation of cultured human umbilical vein endothelial cell (HUVECs) induced by oxidized low density lipoprotein(ox-LDL-C).Methods HUVECs cultured in vitro at passage 4 to 9 were used for the experiment.They were divided into control group,ox-LDL-C group (100 mg/L) and ox-LDL-C + fenofibrate group.The cells in ox-LDL-C + fenofibrate group were firstly incubated with 10,50,100 μmol/L fenofibrate respectively for 6 h,then incubated with 100 mg/L ox-LDL-C for another 24 h.MMP-9 expression in supernatant medium was detected by enzyme-linked immuno sorbent assay (ELISA) and cell proliferation was measured by MTT assay.Results Compared with the control group,ox-LDL-C (100 mg/L) could inhibit the proliferation of HUVECs (P < 0.01) and promote the production of MMP-9 of HUVECs.Fenofibrate could inhibit the proliferation of HUVECs induced by ox-LDL-C in a dose-dependent manner (P < 0.01).Fenofibrate (10,50,100 μmol/L) could significantly stimulate the production of MMP-9 of HUVECs induced by ox-LDL-C (P < 0.01).The stimulation effect was dose-dependent.Conclusion Fenofibrate can inhibit the production of MMP-9 in HUVECs induced by ox-LDL-C,promote the proliferation of HUVECs and protect endothelial function in a dose-dependent manner,then may play the role of anti-artherosclerosis besides lipid regulation.
