Objective:To investigate the association between plasma anti-Müllerian hormone(AMH) levels and ischemic stroke. Methods:In this case-control study, 93 ischemic stroke patients were randomly selected as the case group from a study on the prevention and treatment of metabolic syndrome, which was conducted in 2018-2019 in Changshu, Jiangsu Province, while 372 nonischemic stroke patients were selected as the control group according to the principle of 1∶4 matching. An enzyme-linked immunosorbent assay was used to measure plasma AMH levels. The conditional logistic regression model and restricted cubic spline were used to analyze the relationship between AMH levels and ischemic stroke. Results:A total of 465 subjects with an average age of (68.7±7.4)years were included in this study, of whom 215(46.2%)were men and 250(53.8%)were women.According to our conditional Logistic regression analysis, the risk of ischemic stroke was reduced by 44% for every unit increase in the log-AMH level( OR=0.56, 95% CI: 0.37-0.85)in the overall population after multivariate adjustment.Compared with the tertile with the lowest AMH level, the risk of ischemic stroke in the tertile with the highest AMH level decreased significantly( OR=0.37, 95% CI: 0.19-0.69). When subgrouped by sex, the tertiles with the highest AMH levels were associated with a 66% lower risk of ischemic stroke in men( OR=0.34, 95% CI: 0.13-0.88)and a 64% lower risk of ischemic stroke in women( OR=0.36, 95% CI: 0.15-0.87), compared with the tertiles with the lowest AMH levels. The results of restricted cubic spline analysis showed that there was a linear dose-response relationship between plasma AMH levels and ischemic stroke both in the general population and in male or female population( Pvalues for linear trends were 0.0002, 0.008 and 0.007, respectively). Conclusions:Higher plasma AMH levels decrease the risk of ischemic stroke with a dose-response pattern.

Journal of Modern Urology 2023;28(7):591-596
【Objective】 To investigate the effects of preoperative ureteroscopy (URS) on the intravesical recurrence (IVR) in patients with upper tract urothelial carcinoma (UTUC) after radical nephroureterectomy (RNU). 【Methods】 The clinical data of 241 UTUC patients treated during May 2012 and Jan.2020 in the Second Hospital of Laozhou University were retrospectively analyzed. The patients were divided into URS before RNU group (URS group) and non-URS before RNU group (non-URS group). The cumulative IVR rate, progression-free survival (PFS) and overall survival (OS) after RNU were compared, and the survival curve was drawn. Cox proportional hazards models were used to assess risk factors affecting IVR. 【Results】 Of the 241 patients, 64 (26.6%) were included in the URS group and 177 (73.4%) in the non-URS group. In the URS group, 49 underwent biopsy and 15 did not. All patients were followed up for a median of 44 (3 to 122) months, with a median time to recurrence of 12 (3 to 56) months. IVR occurred in 18 patients (28.1%) in the URS group and 25 (14.1%) in the non-URS group. Kaplan-Meier survival analysis showed that the cumulative IVR rate was higher in the URS group than in the non-URS group (all P<0.05), regardless of whether patients had a history of bladder cancer (BC) or not, while PFS was lower in the URS group than in the non-URS group (P=0.007). Cox multivariate regression analysis showed that URS (P=0.031) and complicated renal pelvis tumor and ureteral tumor (P=0.004) were independent risk factors for IVR. 【Conclusion】 Preoperative URS increases the incidence of IVR in patients with UTUC, and routine preoperative use of URS is not recommended.

Objective:To investigate the risk factors of multi-drug resistant organism(MDRO)infection in orthopedic trauma patients. Methods:A retrospective case series study was made on clinical data of 95 orthopedic patients with in-hospital wound infection admitted to West China Hospital of Sichuan University from January 2020 to December 2020,including 71 males and 24 females at age of 14-70 years[(42.6 ± 13.8)years]. MDRO infection occurred in 38 patients(DRO group),while not in 57 patients(non-MDRO group). Etiological characteristics of wound infections were observed. Univariate analysis was used to compare variables between the two groups, including baseline data(gender, age, length of hospital stay, chronic comorbidity)and injury as well as hospitalization information(transfer from another hospital, emergency admission, time from injury to first treatment, open injury, combination with fracture, wound contamination, depth of wound,ICU stay, number of operations, cumulative operation time, type of incision, length of hospital stay before diagnosis of infection, type of antibiotics used, usage of hormones/immunosuppressive agents, level of hemoglobin/serum albumin/blood glucose on admission). Binary Logistic regression was used to analyze independent risk factors for MDRO infection. Results:A total of 119 pathogenic bacteria were detected on the wound, including 21 gram-positive cocci(18%),91 gram-negative bacilli(76%)and 7 others(6%). Top five strains were Acinetobacter baumannii,Enterobacter cloacae,Escherichia coli,Staphylococcus aureus and Pseudomonas aeruginosa. Univariate analysis showed the two groups had significant differences in terms of length of hospital stay, wound contamination,ICU stay, length of hospital stay before diagnosis of infection, type of antibiotics used and levels of hemoglobin and serum albumin on admission( P < 0.05),but not in gender, age, hypertension, diabetes, transfer from another hospital and emergency admission( P > 0.05). Multivariate Logistic regression analysis showed MDRD infection was obviously correlated with the long hospital stay( OR = 1.033, 95%CI 1.005-1.061, P < 0.05),time from injury to first treatment over 6 hours( OR = 4.282, 95%CI 1.174-15.616, P < 0.05),types of antibiotics used ≥ 3( OR = 7.486, 95%CI 2.451-22.863, P < 0.05)and low level of hemoglobin on admission( OR = 0.973, 95%CI 0.962-0.985, P < 0.05). Conclusions:For orthopedic trauma patients,MDRO infection is independently associated with long hospital stay, time from injury to first treatment, type of antibiotics use and level of hemoglobin on admission, suggesting that decreased length of hospitalization, shortened time of first wound treatment, individualized and rational usage of antibiotics and timely correction of anemia are important measures to prevent MDRO infection.

Journal of Central South University(Medical Sciences) 2020;45(6):701-708
OBJECTIVES:
Lymph node metastasis affects the initial treatment strategy for cervical cancer and is hard to be diagnosed in clinical practice. This paper aims to explore the relationship between calcium-binding A9 (S100A9) and lymph node metastasis (LNM) in cervical cancer, and to determine the predictive value of S100A9 for LNM in cervical cancer.
METHODS:
We performed a retrospective cohort study and collected the pathological data, follow-up data, and paraffin tissue samples of 99 patients with cervical cancer who underwent modified extensive or extensive hysterectomy plus pelvic lymphadenectomy at the Department of Gynecology, Xiangya Hospital, Central South University from January 2013 to December 2018. Immunohistochemistry was used to detect the expression of S100A9 in cervical cancer tissues, and the correlation between S100A9 expression and LNM of cervical cancer, or clinicopathological characteristics were analyzed. The receiver operating characteristic (ROC) curve was used to establish a predictive model for LNM of cervical cancer, and Chi-square test of four-grid table was used to evaluate the diagnostic value of S100A9 for LNM in cervical cancer.
RESULTS:
The expression of S100A9 was significantly correlated with LNM. The S100A9 immunohistochemical semi-quantitative score of the LNM group was significantly higher than that in the non-lymph node metastasis group (<0.001). Moreover, the expression of S100A9 was significantly correlated with histological type, stromal invasion, lymphatic vessel invasion, or LNM (<0.05). The cut-off of the ROC curve for predicting LNM was 5, with the Youden index of 0.649 and the area under the ROC curve of 0.863. The disease-free survival and overall survival in the S100A9 positive group were significantly shorter than those in the negative group (<0.05). S100A9 alone had a sensitivity of 71.4%, a specificity of 91.5%, and an accuracy of 85.1% for diagnosing LNM. Imaging had a sensitivity of 32.1%, a specificity of 74.6%, and an accuracy of 60.9%. Combination of S100A9 with image examination in parallel test had a sensitivity of 85.7%, a specificity of 71.2%, and an accuracy of 75.9%, while combination of S100A9 and image examination in serial test had a sensitivity of 17.9%, a specificity of 98.3%, and an accuracy of 72.4%.
CONCLUSIONS
S100A9 may be associated with LNM in cervical cancer. S100A9 shows a promising perspective in predicting LNM in cervical cancer. Combination of S100A9 and image examination in serial test has a high specificity for LNM.
