1.Mechanism of low-intensity pulsed ultrasound for promoting osteogenic differentiation of senescent bone marrow mesenchymal stem cell in mice

Objective To investigate the capacity of low-intensity pulsed ultrasound(LIPUS)for promoting osteogenic differentiation of senescent bone marrow mesenchymal stem cell(BMMSC)in mice,also the potential mechanism of involving myocardin-related transcription factor-A(MRTF-A)during this process.Methods Five aged mice received LIPUS irradiation on the left femur for eight weeks.Micro CT and HE staining were employed to evaluate bone status of bilateral femur.Bone marrow cells were isolated from mouse long bones,BMMSC were cultured,and the cells were divided into H2O2+LIPUS group,H2O2 group and control group.Mouse BMMSC were subjected to H2O2 for 3 days,followed by reactive oxygen species fluorescence staining and senescence-associated β-galactosidase staining to assess cellular senescence.Immunofluorescence and phalloidin staining were performed to examine MRTF-A distribution and cytoskeletal status.Based on H2O2-induced senescence model,MRTF-A nuclear translocation inhibitor CCG-100602 was applied,and the cells were further divided into H2O2+CCG-100602+LIPUS group,H2O2+CCG-100602 group,H2O2+LIPUS group and H2O2 group.Immunofluorescence staining was used to assess MRTF-A distribution.Alkaline phosphatase(ALP)and alizarin red staining were conducted to evaluate osteogenic differentiation capacity.Results LIPUS resulted in increased trabecular bone in the left femur of aged mice compared to the right femur,with increased bone volume to total volume and trabecular number and decreased trabecular spacing(both P<0.05).Compared with H2O2 group,H2O2+LIPUS group showed increased nuclear expression of MRTF-A and increased intracellular rigid actin filament density.The nuclear expression of MRTF-A in H2O2+CCG-100602+LIPUS group was significantly lower than that in H2O2+LIPUS group.Compared with the H2O2+CCG-100602+LIPUS group and H2O2 group,the ALP and alizarin red staining positive areas in H2O2+LIPUS group were significantly larger.Conclusion LIPUS could improve osteoporosis in aged mice.Nuclear translocation of MRTF-A was a key regulator to LIPUS for promoting osteogenic differentiation of senescent BMMSC.
2.The relationship between EBV infection, HBV reactivation and clinical features and prognosis in HBV-infected NHL patients and influencing factors of HBV reactivation

Objective:To explore the relationship between Epstein-Barr virus (EBV) infection, hepatitis B virus (HBV) reactivation and clinical features and prognosis in HBV-infected non-Hodgkin lymphoma (NHL) patients and influencing factors of HBV reactivation.Methods:A retrospective cohort study was conducted. A total of 80 NHL patients with hepatitis B surface antigen (HBsAg) positive (which was defined as HBV positive) who were admitted to the Second People's Hospital of Lianyungang and Jiangsu Province Hospital from December 2012 to October 2022 were selected. All patients were divided into EBV-positive group and EBV-negative group according to EBV DNA results, and further grouped into the HBV reactivation group and the non-reactivation group according to whether HBV were reactivated after chemotherapy. The clinical characteristics of patients among groups were compared. Multivariate logistic regression model was used to analyze the factors influencing HBV reactivation. The Kaplan-Meier method was used to evaluate the progression-free survival (PFS) and overall survival (OS) of patients, and the log-rank test was used for inter-group comparison.Results:Among NHL patients with HBV positive, 27 cases (33.8%) were EBV-positive and 29 cases (36.3%) were HBV reactivation. Compared with the EBV-negative group, the proportion of patients with Ann Arbor stage Ⅲ-Ⅳ [92.6% (25/27) vs. 66.0% (35/53)], elevated β 2-microglobulin level [88.9% (24/27) vs. 62.3% (33/53)], bone marrow involvement [40.7% (11/27) vs. 15.1% (8/53)], and HBV reactivation [51.9% (14/27) vs. 28.3% (15/53)] was higher in the EBV-positive group, and the differences were statistically significant (all P<0.05). There were no statistically significant differences in the composition of patients stratified by age, gender, pathological type, B symptom, lactate dehydrogenase level, international prognostic index score, number of extranodal involvements, liver involvement, hepatitis outbreak, prophylactic anti-HBV therapy, hepatitis B surface antibody (HBsAb), rituximab therapy, and the last chemotherapy effects between the 2 groups (all P > 0.05). Compared with the HBV non-reactivation group, the proportion of patients undergoing hepatitis outbreak [48.3% (14/29) vs. 17.6% (9/51)], not receiving prophylactic anti-HBV therapy [65.5% (19/29) vs. 39.2% (20/51)], HBsAb negative [79.3% (23/29) vs. 21.6% (11/51)], EBV positive [48.3% (14/29) vs. 25.5% (13/51)], receiving rituximab [82.8% (24/29) vs. 60.8% (31/51)] was higher in the HBV reactivation group, and the differenves were statistically significant (all P < 0.05); while there were no statistically significant differences in the composition of patients stratified by the other clinical characteristics between the 2 groups (all P > 0.05). Multivariate logistic regression analysis showed that EBV-positivity was an independent risk factor for HBV reactivation after chemotherapy in NHL patients with HBsAg positive ( OR = 7.073, 95% CI: 1.613-31.010, P = 0.009), while HBsAb positive ( OR = 0.038, 95% CI: 0.008-0.186, P < 0.001) and preventive anti-HBV therapy ( OR = 0.172, 95% CI: 0.039-0.756, P = 0.020) were independent protective factors. The last follow-up was in December 2023 and the median follow-up time was 36.5 months. There were no statistically significant differences in PFS and OS between the EBV-positive group and the EBV-negative group, HBV reactivation group and the non-reactivation group (all P > 0.05). Conclusions:Among HBV-infected NHL patients, those with concurrent EBV infection have a more advanced clinical stage and are very prone to bone marrow invasion, and they also show a higher probability of HBV reactivation; HBV reactivation may be related to whether receiving preventive anti-HBV therapy and rituximab therapy. EBV infection may increase the risk of HBV reactivation in NHL patients; EBV infection and HBV reactivation may not be relevant to the prognosis of patients.
3.Progress of PHF6 in acute T-lymphoblastic leukemia

Acute T-lymphoblastic leukemia (T-ALL) is a hematopoietic malignancy, and in recent years, with the advancement of combined chemotherapy and hematopoietic stem cell transplantation, the prognosis of T-ALL has improved significantly, but for patients with primary drug resistance or relapsed/refractory disease the prognosis is still poor. The plant homeodomain finger 6 (PHF6) is a tumor suppressor protein, it plays a pivotal role in T cell differentiation, epigenetic regulation and oncogenic pathway synergy, and its mutations and deletions are commonly associated with the development of T-lymphocytic leukemia. However, the underlying mechanism of PHF6 in the pathogenesis of T-ALL remains unclear. This article reviews the structure, function and mechanism of action of PHF6 in T-ALL, the important coexisting genes associated with the progression of T-ALL, and the research progress in targeted therapy.
4.Visual analysis of acupuncture and moxibustion treatment for cognitive impairment after stroke based on CiteSpace

The CiteSpace software was used to perform visual-based analyses of publication trends, collaborative networks among countries and institutions, author collaboration networks, and research hotspots.Results:We included 730 Chinese studies and 112 English-language studies. From 2004 to 2023, the number of publications on scalp acupuncture for PSCI showed an increasing trend. All Chinese studies were published in China, mainly by universities, and the institutional collaboration density was low (0.002 9). For English-language studies, China had the most publications (103), followed by the USA (7) and Germany (3). Chinese universities were the main publishing institutions, and the density of inter-institutional collaboration (0.183 3) was higher than that for Chinese literature. The density of the author collaboration network was 0.005 0 for Chinese studies and 0.023 8 for English-language studies. In Chinese studies, the most frequent keywords with a centrality greater than 0.1 were cognitive impairment (248 times), stroke (240 times), needling (162 times), cerebral infarction (82 times), and electroacupuncture (80 times). In English-language studies, these were stroke (40 times), cognitive impairment (32 times), acupuncture (15 times), dementia (15 times), ischemic stroke (10 times), and mechanism (7 times). For Chinese studies, the top 3 clusters focused on clinical studies of the effects of scalp acupuncture on PSCI. In English-language studies, the top 4 clusters mainly involved research on the effects and assessment methods of scalp acupuncture for PSCI. The timeline map of keywords in Chinese studies showed that before 2015, the high-frequency keywords were stroke, cognitive impairment, needling, electroacupuncture, scalp acupuncture, body acupuncture, rehabilitation, moxibustion, and clinical research. From 2015 onwards, keywords such as rat, apoptosis, inflammatory factors, mechanism research, signaling pathways, mice, and animal experiments appeared. The timeline map of keywords in English-language studies showed that before 2013, there were few accumulated keywords. After 2013, high-frequency keywords included "multi-infarction rat""computer-based cognitive training""apoptosis""electroacupuncture""mild cognitive impairment""randomized controlled trial""scalp acupuncture""post-stroke cognitive impairment""stroke rehabilitation""systematic review" and "hippocampal CA1 region". In Chinese studies, keywords with high burst intensity included cerebral infarction, acupuncture therapy, vascular, baihui (GV-20), review, learning and memory, and scalp acupuncture. In English-language studies, these included "multi-infarction rat""mild cognitive impairment""electroacupuncture""post-stroke cognitive impairment"and"systematic review".Conclusions:Research in this field has grown rapidly in recent years. Chinese studies focus mainly on the clinical effects of scalp acupuncture for PSCI, while English-language studies focus more on related mechanisms. Furthermore, both Chinese and English-language studies show good consistency in the focus of experimental research, which mainly focuses on the mechanisms of electroacupuncture in the treatment of PSCI.

7.Supervision and evaluation of medical equipment procurement agencies in public hospitals with fuzzy comprehensive evaluation method

Objective To strengthen the supervision of medical equipment procurement and construct a scientific and systematic evaluation system for procuring agency supervision,providing a reference for public hospitals to implement government procurement of medical equipment.Methods The Delphi method was employed to establish the indexes and weights of the su-pervision evaluation system.Subsequently the fuzzy comprehensive evaluation method was applied to comprehensively analyze the procuring agencies X and Y.Results The evaluation results of agencies X and Y were compared and analyzed in terms of work efficiency,business capability,organization of bid evaluation,and service quality.Additionally,the results were regularly fed back and improved.Conclusion The establishment of the supervision and evaluation system for procuring agencies is conducive to promoting the standardization and efficiency of government procurement activities of medical equipment in public hospitals and safeguarding the legitimate rights and interests of procurement participants as well as the public interests.
8.Hygroscopicity of Dry Extract Powder of Hericium Erinaceus and Eathworm Biotransformation

Objective:To confirm the relative humidity conditions of preparation of Hericium Erinaceus and Lumbricus ( HD) bio-transformation and the longest operation time under the conditions through the study on hygroscopicity of dry extract powder. Methods:The solution was prepared respectively in the environment with various humidity, and the moisture absorption rate and critical relative humidity ( CRH) of dry extract powder of HD biotransformation were measured, and then the mathematical model was built and the curves of moisture percentage vs relative humidity was drawn. The parameters of moisture absorption rate, and the relative humidity and operation time with the moisture percentage up to 5% were calculated. The analytic geometry and linear regression method were used to calculate CRH, so as to determine the air humidity control range and the conditions for the production and storage process. Results:The moisture absorption of HD dry extract powder could reach 5% when exposed to the air respectively with the relative humidity of 66. 0%, 80. 8%, 88. 2% and 99. 0% for 7. 088, 3. 953, 2. 892 and 0. 661 h. When the moisture percentage reached 5%, the rela-tive humidity and the shortest time was 50. 84% and 9. 937 h, respectively. The CRH of the dry extract powder of HD biotransforma-tion was 68. 12%. Conclusion:When the dry extract powder of HD biotransformation is under preparation, the relative humidity of en-vironment should be controlled below 50. 84% with the operation time shorter than 9. 937 h, or the relative humidity is controlled below 68. 12% with the operation time shorter than 6. 810 h. Under the above conditions, the moisture absorption rate of HD dry extract pow-der can be controlled below 5%, which does not affect the preparation.
9.Postpartum change of blood pressure and its risk factors in patients with hypertensive disorders in pregnancy.

OBJECTIVE:
To investigate the change of blood pressure in patients with hypertensive disorders in pregnancy after delivery and the risk factors.
METHODS:
In a retrospective cohort study, we collected subjects from villages and towns in Liuyang by cluster sampling method. Before enrolling in this cohort, all had established health records from January 2010 to December 2011. We collected the medical records and maternal health care manuals of this cohort as our data materials, focusing on the blood pressure records as well as related features. We compared the differences of recovery rate of postpartum blood pressure in different kinds of antenatal blood pressure groups with χ2 test. In order to explore the main factors influencing the recovery rate of blood pressure of patients with hypertensive disorders in pregnancy, we conducted univariate and multivariate analysis by χ2 test and unconditional logistic regression analysis.
RESULTS:
Among the 460 women with hypertensive disorders in pregnancy in our analysis, the recovery rate of postpartum blood pressure reached 88.7%. Multivariate analysis showed that the risk factors influencing the recovery rate of postpartum blood pressure included advanced age (OR=0.436), higher degree of hypertensive disorders in pregnancy (OR=0.436), and hypertension with simultaneously high systolic and diastolic blood pressures (OR=0.192).
CONCLUSION
For most patients with hypertensive disorders in pregnancy, the blood pressure may decrease to normal level 42 days after delivery. Women with advanced age, higher degree of hypertensive disorders in pregnancy and hypertension with simultaneously high systolic and diastolic blood pressures should be given more attention.

10.Incidence and risk factors for postpartum hemorrhage in Liuyang, China.

OBJECTIVE:
To investigate the incidence and risk factors of postpartum hemorrhage in Liuyang City, Hunan Province.
METHODS:
This was a nested case-control study. A total of 14 townships were randomly selected by cluster random sampling method from 37 townships in Liuyang City, Hunan Province. All women who got pregnant and childbirth from January 2010 to December 2011 in the above 14 townships were selected and followed up at first-trimester, second trimester, third trimester and each delivery period. Data were collected directly from maternal care manuals and the clinical records from the subjects' delivery hospitals. These whose postpartum bleeding was more than 500 mL were selected as cases, and the same amounts of normal maternal in the same cohort study whose postpartum bleeding was less than 500 mL were selected as controls. χ2 test and unconditional logistic regression analysis were applied to identify the determinants.
RESULTS:
A total of 5 933 women were included in our study. The incidence of postpartum hemorrhage was 2.22%. Unconditional logistic regression analysis showed that the risk factors for postpartum hemorrhage mainly included anemia (OR=3.940, 95% CI: 2.100-7.389), scarred uterus (OR=2.730, 95% CI: 1.198-6.220), placenta previa (OR=10.665, 95% CI: 1.142-99.566), placental factors (OR=4.332, 95% CI: 1.004-18.692), and prolonged pregnancy (OR=5.899, 95% CI: 2.602-13.376).
CONCLUSION
Postpartum hemorrhage is caused by complicated factors. Comprehensive measures should be taken to reduce postpartum hemorrhage.
