1.Application of middle hepatic vein splitting and reconstruction technique in split liver transplantation from low-age donor livers

Objective To explore the feasibility and clinical experience of the middle hepatic vein splitting-reconstruction technique in split liver transplantation from low-age donor livers. Methods A retrospective analysis was conducted on the cases of two low-age donor livers that underwent middle hepatic vein splitting-reconstruction, which were transplanted into four child recipients at the Liver Transplantation Center of the Third Affiliated Hospital of Sun Yat-sen University from January 2017 to July 2023. The surgical and postoperative conditions were summarized and analyzed. Results Donor 1 was a 6-year-old and 4-month-old girl with a body weight of 21 kg, and the obtained donor liver weighed 496 g. After splitting, the left and right liver weights were 201 g and 280 g, and transplanted into a 9-month-old boy weighing 6.5 kg and a 9-month-old boy weighing 7.5 kg, respectively. The graft to recipient weight ratio (GRWR) was 3.09% and 3.73%, respectively. Donor 2 was a 5-year-old and 8-month-old boy with a body weight of 19 kg, and the donor liver weighed 673 g. After splitting, the left and right liver weights were 230 g and 400 g, and transplanted into a 13-month-old girl weighing 9.5 kg and a 15-month-old boy weighing 12 kg. The GRWR was 2.42% and 3.33%, respectively. Both donor livers were split ex vivo, with the middle hepatic vein being completely split in the middle and reconstructed using allogeneic iliac vein and iliac artery vascular patches. According to GRWR, none of the 4 transplant livers were reduced in volume. Among the 4 recipients, one died due to postoperative portal vein thrombosis and non-function of the transplant liver, while the other three cases recovered smoothly without early or late complications. Regular follow-up was conducted until July 31, 2023, and liver function recovered well. Conclusions Under the premise of detailed assessment of the donor liver and meticulous intraoperative operation, as well as matching with suitable child recipients, low-age donor livers may be selected for splitting. The complete splitting and reconstruction of the middle hepatic vein in the middle may effectively ensure the adequate venous return of the left and right liver and provide sufficient functional liver volume.
2.Serological and genetic analysis of a novel 27delC variant in A subgroup: a case report

[Objective] To conduct serological identification and molecular mechanism study on a ambiguous ABO blood group. [Methods] Standard serological techniques were used for the forward and reverse typing of ABO blood type. ABO gene coding and regulatory regions were analyzed by PCR after DNA extraction. Monoclonal sequencing was used to detect the haplotypes of the DNA sequence, and bioinformatics analysis was applied to predict the possible translation outcomes of the mutated DNA sequence. [Results] The sample’s red blood cells showed mixed field agglutination with anti-A, and the serum agglutinated with B cells, exhibiting serological characteristics of subtype A. Direct sequencing and monoclonal sequencing analysis of the ABO gene confirmed one allele as O02, the other had a c.27delC mutation compared with A102, which could cause the translation sequence to terminate prematurely at the 19th amino acids. Analysis and prediction suggested that the mutation might affect the function of the transferase through mechanisms such as shifting the initiation codon, altering the reading frame and affecting the splice sites. [Conclusion] This case is a rare A subtype caused by the c.27delC variation, and the impact on the glycosyltransferase may involve multiple mechanisms, which require further research and exploration.
3.Concept, design and clinical application of minimally invasive liver transplantation through laparoscopic combined upper midline incision

Objective To explore the technical process and clinical application of laparoscopic combined upper midline incision minimally invasive liver transplantation. Methods A retrospective analysis was conducted on 30 cases of laparoscopic combined upper midline incision minimally invasive liver transplantation. The cases were divided into cirrhosis group (15 cases) and liver failure group (15 cases) based on the primary disease. The surgical and postoperative conditions of the two groups were compared. Results All patients successfully underwent laparoscopic "clockwise" liver resection, with no cases of passive conversion to open surgery or intolerance to pneumoperitoneum. In 6 cases, the right lobe was relatively large, and the right hepatic ligaments could not be completely mobilized. One case required an additional reverse "L" incision during open surgery. All patients successfully completed the liver transplantation, with no major intraoperative bleeding, cardiovascular events, or other occurrences in the 30 patients. The model for end-stage liver disease (MELD) score in the cirrhosis group was lower than that in the liver failure group (P<0.001). There were no statistically significant differences between the two groups in terms of age, surgical time, blood loss, anhepatic phase, or cold ischemia time (all P>0.05). During the perioperative period, there was 1 case of hepatic artery embolism, 1 case of portal vein anastomotic stenosis, no complications of hepatic vein and inferior vena cava, and 3 cases of biliary anastomotic stenosis, all of which occurred in the liver failure group. Conclusions In strictly selected cases, the minimally invasive liver transplantation technique combining laparoscopic hepatectomy with upper midline incision for graft implantation has the advantages of smaller incisions, less bleeding, relatively easier operation, and faster postoperative recovery, which is worthy of clinical promotion and application.
4.Correlation between the serum levels of aminotransferases and liver inflammation activity in pediatric chronic hepatitis B patients: An analysis of 1 267 cases

ObjectiveTo investigate the correlation of the serum levels of aminotransferases and their ratios with liver inflammation activity in pediatric chronic hepatitis B (pCHB) patients, and to provide a basis for selecting the dominant population for treatment. MethodsThis study was conducted among 1 267 pCHB patients who were admitted to The Fifth Medical Center of Chinese PLA General Hospital from January 2010 to August 2022 and these patients did not receive antiviral therapy. The patients were analyzed in terms of demographic features, blood routine, blood biochemistry, HBV serological markers, and liver biopsy data. According to liver inflammation activity based on liver biopsy, the patients were divided into no or mild inflammation activity (G0 — G1) group and significant inflammation activity (G2 — G4) group. The serum levels of aminotransferases and their ratios were compared between groups, and their correlation with liver inflammation activity in pCHB patients was analyzed. Additionally, the patients were stratified by the age, and the relationship between serum aminotransferase levels and liver inflammation activity was analyzed in each age group. For comparison of continuous data between two groups, the independent samples t-test was used when the data were normally distributed, while the Mann-Whitney U test was used when the data were not normally distributed; the chi-square test was employed for comparison of categorical data between two groups. A Spearman’s correlation analysis was performed for correlation assessment. ResultsAmong the 1 267 pCHB patients, there were 468 (36.9%) in the G0 — G1 group and 799 (63.1%) in the G2 — G4 group, and there were significant differences between the two groups in the levels of aspartate aminotransferase (AST), alanine aminotransferase (ALT), AST/ALT ratio, gamma-glutamyl transpeptidase (GGT), total bilirubin, direct bilirubin, HBeAg quantification, low-density lipoprotein, and platelet count (PLT) (all P<0.05). The correlation analysis showed that liver inflammation activity was negatively correlated with PLT and low-density lipoprotein (both P<0.05) and was positively correlated with GGT, total bilirubin, direct bilirubin, and HBeAg titer (all P<0.05), while it was not significantly correlated with ALT, AST, and AST/ALT ratio (all P>0.05). In the 0 — 12 years group, the 13 — 18 years male group, and the 13 — 18 years female group, liver inflammation activity aggravated with the increases in the serum levels of ALT and AST, and there were significant differences between groups (all P<0.05). In the 0 — 12 years group, there was a significant difference in significant liver inflammation activity between the AST/ALT ratio >1 group and the AST/ALT ratio ≤1 group (P<0.001). Among the 1 267 patients, 447 (35.28%) had an ALT level of <2×upper limit of normal (ULN), among whom 196 (43.85%) had G≥2 liver inflammation, accounting for 15.47% of all children enrolled. ConclusionLiver inflammation activity is not significantly correlated with ALT, AST, and AST/ALT ratio in pCHB patients, suggesting that the serum levels of aminotransferases cannot truly reflect liver inflammation activity in pCHB patients with an aminotransferase level of <2×ULN. In clinical practice, liver biopsy should be performed for children with an aminotransferase level of <2×ULN to clarify whether antiviral therapy should be performed.
5.Hepatitis B incidence trends and prevention strategies over 15 years before and after (1994-2023) the establishment of a hepatitis B prevention demonstration zone in Wuwei

Objective To analyze the hepatitis B (HBV) incidence trend and intervention measures implemented over 15 years (2009-2023) following the establishment of Wuwei demonstration zone and to explore a new model suitable for hepatitis B prevention and control in rural areas of western China. Methods Data on reported hepatitis B cases (1994-2023) were collected from Wuwei City's Disease Monitoring Report Management Information System, vaccination records from the Immunization Planning Information Management System, hepatitis B maternal-to-child transmission prevention data from the Management Information System for Prevention of HIV, Syphilis, and Hepatitis B Mother-to-Child Transmission. Additionally, field epidemiological survey data, serological test results, and intervention measures were also used to analyze using descriptive epidemiological methods. Results Over the 15-year period, the average annual incidence rate of hepatitis B in the demonstration area was 200.48 per 100 000, representing a 38.76% reduction compared to the pre-construction period (1994-2008, 327.35 per 100 000). Post-establishment, the incidence decreased from 637.70 per 100 000 in 2009 to 87.45 per 100 000 in 2023, showing an average annual decline of 13.23%. In intensified intervention areas, the incidence decreased by 89.28%, and the rate stabilized in the mid-to-late stages. Among pregnant women, the HBsAg-positive rate was 4.41%, with 99.42% of newborns born to HBsAg-positive mothers receiving hepatitis B immunoglobulin immunization. The prevalence of chronic hepatitis B, liver cirrhosis, and liver cancer-related diseases decreased by 42.19%, while the mortality rate of hepatitis B patients declined from 8.69% to 2.00%. Public awareness of hepatitis B prevention and control increased from 43.48% to 83.79%. A hepatitis B prevention and control model suitable for impoverished western regions—the “Wuwei Model”—was established and validated. This model emphasizes short-course vaccination, early intervention, integration of prevention and treatment, and collaborative efforts. Conclusion The “Wuwei Model” significantly reduced the incidence and mortality of hepatitis B in the demonstration area, demonstrating effective disease control. Further validation and promotion of this model are needed. Continued efforts should focus on expanding whole-population screening for chronic hepatitis B infection, improving the management of chronic hepatitis B carriers, and increasing immunization rates among susceptible adult populations.
6.Efficacy and Safety of Fenofibric Acid in Chinese Hyperlipidemia Patients:a Randomized,Double-blinded and Placebo-controlled Clinical Trial

Objectives:Fenofibric acid is extracted from the widely used hypolipemic fenofibrate,nowadays being approved for marketing around numerous nations and regions,nonetheless not in China.Present trial evaluated the efficacy and safety in the Chinese hypertriglyceridemia population. Methods:This is a multi-center,randomized,double-blind,placebo-controlled phase Ⅲ clinical trial.Patients from 3 different cohorts,including severe hypertriglyceridemia(HTG),moderate HTG and mixed-dyslipidemia(MD),were randomized at 1:1 ratio to receive fenofibric acid 135 mg or placebo daily for 12 weeks.The primary endpoint was the percentage change of triglyceridemia(TG)from baseline at week 12.Secondary endpoints were the percentage changes of other blood lipid indexes.At the same time,the incidence of medical adverse events was observed. Results:Among the three cohorts of patients with severe HTG(n=52),moderate HTG(n=23)and MD(n=52),the TG levels in the fenofibric acid-treated group decreased by(49.12±29.19)%,(49.95±25.19)%and(49.79±19.28)%,respectively from baseline to 12 weeks,while the corresponding placebo groups decreased by(18.88±40.69)%,(8.11±29.86)%and increased by(10.42±73.04)%,respectively from baseline to 12 weeks.The differences between treatment and placebo groups were statistically significant(P<0.017 for severe HTG cohort,P<0.05 for moderate and MD cohort).The high-density lipoprotein cholesterol(HDL-C)in the fenofibric acid-treated group increased by(25.51±21.45)%,(24.55±24.73)%,and(23.60±27.38)%,and the placebo group increased by(1.91±20.42)%,(2.40±9.32)%and(7.13±19.12)%,respectively,the differences between the two groups were statistically significant(all P<0.05).In the fenofibric acid group,adverse events with incidence>5%included upper respiratory tract infection(10.9%),abdominal pain(6.3%),and increased serum creatinine levels(6.3%),rates of adverse events were similar between the two groups(P>0.05). Conclusions:Fenofibric acid can significantly reduce triglycerides and elevate HDL-C levels safely in Chinese patients with severe to moderate HTG without statin or MD patients on top of statin therapy.
7.Validity and reliability of the Positive Beliefs About Rumination Scale in Chinese college students

Objective:To evaluate the validity and reliability of Positive Beliefs About Rumination Scale(PBRS)in Chinese college students.Methods:A total of 968 college students were selected.Exploratory factor a-nalysis,criterion validity and internal consistency reliability test were performed on sample 1(n=496).The Rumi-native Responses Scale(RRS),Metacognitions Questionnaire(MCQ)and Beck Depression Inventory-Ⅱ(BDI-Ⅱ)were used as criteria for criterion validity test.Confirmatory factor analysis and Measurement invariance across gen-der were performed on sample 2(n=472).The 87 college studentsin sample 1 were retested 6 weeks later.Results:The exploratory factor analysis found 1 factor,which explained 54.39%of the total variance.The confirmatory factor analysis showed that the 1-factor model fitted well(x2/df=3.38,CFI=0.962,TLI=0.940,SRMR=0.043,RMSEA=0.071).The scores of the Positive Beliefs About Rumination Scale were positively cor-related with the scores of RRS,MCQ and BDI-Ⅱ(ICC=0.37,0.41,0.12,Ps<0.01).The Cronbach α coefficient of the Positive Beliefs About Rumination Scale was 0.89.The retest reliability(ICC)of the Positive Beliefs About Rumination Scale was 0.72.The configural,weak,strong and strict invariance of the PBRS across gender were all acceptable(△CFI,△TLI<0.01).Conclusion:The Positive Beliefs About Rumination Scale has good validity and reliability in Chinese college students.
8.Research on the cultivation path of full-time doctoral students′ scientific and technological innovation ability in a large public hospital

Objective:To analyze the influencing factors and improvement paths of the cultivation of full-time doctoral scientific and technological innovation ability in large public hospitals, and propose countermeasures and suggestions.Methods:This studyed conducted a survey and analysis of 122 doctors from Linyi People′s Hospital in Shandong Province, and completed a current situation study based on the analysis results.Results:There was no significant difference between the two groups in gender, age, degree type, professional category, discipline level, Graduate School type, job type and other indicators. There were significant differences between the two groups in scientific research topic selection ability score, project design ability score, data analysis ability score, data interpretation ability score, project approval in recent 5 years, project level, number of SCI journal papers published in recent 5 years, cumulative impact factors of SCI journal papers, and annual number of academic activities ( P<0.05). Conclusions:The hospital can improve the scientific and technological innovation ability of full-time doctors by setting up a special cultivation plan, establishing an interdisciplinary team, optimizing scientific research management services, improving the evaluation and assessment system, and improving welfare protection.
9.Exploring the Mechanism of LUO's Neiyi Prescription for the Treatment of Endometriosis Based on the HIF1A/EZH2/ANTXR2 Pathway

Objective To investigate the intervention effect and mechanism of LUO's Neiyi Prescription on the proliferation and angiogenesis of eutopic endometrium in rats with endometriosis(EMs)based on hypoxia-inducible factor 1A(HIF1A)/enhancer of zeste homolog 2(EZH2)/anthrax toxin receptor 2(ANTXR2)signaling pathway.Methods SD rats were randomly divided into sham operation group,model group,Danazol group(4.2 g·kg-1),low-dose LUO's Neiyi Prescription group and high-dose LUO's Neiyi Prescription group(15.74,31.48 g·kg-1),eight rats in each group.The rat model of EMs with qi stagnation and blood stasis syndrome was constructed by autologous endometrial transplantation combined with multi-factor intervention.Intragastric administration was given once a day for 28 consecutive days.The pathological changes of endometrial tissue were observed by HE staining.The expression levels of proliferating cell protein 67(Ki67)and platelet endothelial cell adhesion molecule 1(CD31)in endometrial tissue were detected by immunohistochemistry.The mRNA and protein expression levels of HIF1A,EZH2 and ANTXR2 in endometrial tissues were detected by qRT-PCR and Western Blot.The expression levels of YAP1,CD44 and β-catenin in endometrial tissues were detected by Western Blot.Results Compared with the sham operation group,the epithelial cells of the eutopic endometrium of the model group were thickened,the interstitial cells were arranged disorderly,and the inflammatory cells increased.The expression levels of Ki67 and CD31 in endometrial tissues were significantly increased(P<0.01),the mRNA and protein expression levels of HIF1A and ANTXR2 were significantly increased(P<0.01),while the mRNA and protein expression levels of EZH2 were significantly decreased(P<0.01),and the protein expression levels of YAP1,CD44 and β-catenin were significantly increased(P<0.01).Compared with the model group,the epithelial layer of the eutopic endometrial tissue of the rats in each administration group became thinner,the interstitial disorder and inflammatory infiltration were improved,and the levels of Ki67 and CD31 in the eutopic endometrial tissue were significantly decreased(P<0.01).The mRNA and protein expression levels of HIF1A and ANTXR2 in the endometrium of rats in the Danazol group and the high-dose LUO's Neiyi Prescription group were significantly decreased(P<0.05,P<0.01),and the mRNA and protein expression levels of EZH2 were significantly increased(P<0.05,P<0.01).The protein expression levels of YAP1,CD44 and β-catenin in endometrial tissue of rats in each administration group were significantly decreased(P<0.05,P<0.01).Conclusion LUO's Neiyi Prescription can play a role in the treatment of EMs by inhibiting the proliferation and angiogenesis of eutopic endometrial cells,and its mechanism may be related to the inhibition of HIF1A/EZH2/ANTXR2 signaling pathway.
10.Establishment of a method for detecting KRAS gene mutations based on Taqman fluorescent ARMS-PCR technology

Objective To establish a new method based on Taqman's fluorescent ARMS-PCR technique for detecting the KRAS mutations in human paraffin embedding tissue samples,which is a reference value of clinical medication.Methods Specific primers and probes were designed based on the conserved sequences of KRAS and internal reference genes(GAPDH).Taqman's fluorescent ARMS-PCR detection systems and sequencing systems were established for KRAS detection.In this paper,paraffin embedded tissue samples of 200 patients with colorectal cancer(CRC)are detected and compared with gold standard Sanger sequencing.The results of KRAS gene mutation are statistically analyzed,and the consistency of the two detection methods is analyzed.Results This method is able to detect KRAS gene mutations in human paraffin embedding tissue samples.And the results are 100%consistent with Sanger sequencing.Conclusion The method based on Taqman's fluorescent ARMS-PCR established in this research can detect KRAS mutations simply,quickly and accurately,which is valuable for clinical medication and suitable for clinical application.
