Objective To investigate the development of the"One-Body-Two-Wings"nursing safety management model with the support of information technology,in order to enhance nursing quality and ensure patient safety.Methods A pre-post controlled design was used in this study.The cardiology department that implemented the traditional nursing management mode from September to November 2022 was used as the control group,while the cardiology department which implemented the"One-Body-Two-Wings"nursing safety management model from January to March 2023 served as the experimental group.Comparisons were made between the two different nursing management models regarding the incidence rate of adverse events per 100 discharged patients in the cardiology department,nursing quality supervision scores,and nurses' perceived intensity of decent work.Results After the implementation of"One-Body-Two-Wings"nursing safety management model,the incidence of adverse events per 100 discharged patients in the department of cardiology decreased from 3.70％to 0.64％,and nursing quality improved steadily(P＜0.01).The total scores of nurses' perceived decent work and the scores in dimensions such as work rewards,job positions,career advancement,and workplace environment were significantly higher after implementation compared to before(P＜0.01).Conclusion The""One-Body-Two-Wings"nursing safety management model demonstrates significant efficacy in reducing adverse event rates,enhancing nursing quality,ensuring patient safety,and improving nurses' perceived decent work.It is worthy of promotion.

Objective To explore the work experiences and needs of nutrition specialist nurses,so as to provide a basis in career development and management practices among the nutrition specialist nurses.Methods A method of descriptive phenomenological research was adopted in this study.Purposive sampling was used to select 12 nutrition specialist nurses who came from different regions across the country with over one year of clinical working experiences as well as had completed the specialist training and been qualified by the Nutrition Committee of Guangdong Nursing Association between January and February 2024.Semi-structured on-line interviews were conducted among the participants and the data acquired from the interviews were analysed and summarised.Themes were extracted using NVivo12 and Colaizzi's seven-step analysis from the data from interview.Results Three main themes and eight sub-themes were identified regarding the work experience of nutrition specialist nurses:growth and gains(continuously learning in nutrition therapy-related knowledge to enhance professional skills;actively practice in nutrition therapy to achieve the self-worth);sense of responsibility and mission(increased awareness of responsibility and proactive services;sense of mission to cultivate talents and promotion of the development of nutrition nursing discipline);challenges(lack of an established role in nutrition nursing,conflict in responsibilities;insufficient communication in a medical team regarding nutrition therapy;limited resource to support the development of nutrition nursing;and the professional knowledge and skills unable to fully meet the job demands).Conclusion Hospitals managers should strengthen the support and training for nutrition specialist nurses,clarify their roles,leverage their professional value and promote the career development.

Objective To investigate the effectiveness of intravenous thrombolytic therapy mode led by fast-track specialist nurses on patients with acute ischemic stroke(AIS).Methods This study involved 124 AIS patients who underwent intravenous thrombolytic therapy in the Department of Emergency of our hospital from March 2021 to February 2023.Among the patients,61 admitted between March 2021 and February 2022 received conventional AIS thrombolytic therapy were assigned to a control group.While the 63 patients who received AIS thrombolytic therapy under the specialist nurse-led intravenous thrombolytic therapy mode between March 2022 and February 2023 were assigned to an observation group.The two groups were compared in terms of the time from admission to completion of CT examination,time for signing the informed consent for thrombolytic therapy,door to needle time and percentage of DTN<60 minutes,as well as the post-thrombolysis scores according to the National Institute of Health Stroke Scale(NIHSS)and satisfaction to medical consultation.Results The observation group exhibited a significantly shorter time from admission to completion of CT examination,a shorter time for signing an informed consent for thrombolytic therapy,a shorter door to needle time and a higher percentage of DTN<60 minutes,all with significant difference in comparison with those in the control group.After thrombolysis,the NIHSS score of the observation group decreased more than that of the control group(P<0.05).The patients and their families in the observation group reported significantly higher satisfaction compared to those in the control group(both P<0.05).Conclusion The fast-track specialist nurse-led intravenous thrombolytic therapy mode demonstrates the superiority in reduction of the time from admission to completion of CT examination,time for signing an informed consent for thrombolytic therapy,door to needle time and the NIHSS scores,higher percentage of DTN<60 minutes as well as improvement of patient satisfaction.

Objective:To develop and interpret the reporting checklist for Delphi technique in clinical research papers, so as to provide guidance for such research paper reporting.Methods:On the basis of drawing on previous domestic and foreign medical paper reporting standards, guidelines, domestic paper writing standards, and clinical research paper evaluation methods, the reporting checklist for Delphi technique in clinical research papers was developed from the perspective of professionalism and standardization in critical appraisal, combining the characteristics of Delphi technique (anonymity, iteration, controlled feedback, and data statistics), and through literature review and the establishment of research groups for discussion, as well as the organization of expert focus groups.Results:The checklist was structured according to the paradigm of the paper, including the front part (titles, abstracts, ethics, references and so on), the text part (introduction, methods, results, discussion and conclusion), and other (dissemination and so on) items (including 26 list items and 44 detailed contents) .Conclusions:The development of the reporting checklist can be used to guide authors and researchers to report the entire research process clearly and completely, improving the rigor and transparency of paper reporting. This checklist can also be used by editors and reviewers to select and integrate review comments one by one, so as to improve the quality of paper review.

Objective To explore the effect of respiratory care bundle combined with management by objectives(MBO)on postoperative pulmonary function,pulmonary complications and postoperative quality of recovery of respiratory risky patients undergone gastrointestinal surgeries.Methods A total of 80 patients with respiratory risk underwent elective surgery in a ward of gastrointestinal surgery from April to November 2021 were selected.Based on the order of the surgery,48 patients with surgeries between April to July were selected into control group and received routine respiratory care,while 32 patients with surgeries between August to November were selected into trial group and received the respiratory care bundle combined with MBO.The pulmonary function,postoperative pulmonary complications and postoperative quality of recovery were analyzed and compared on the 7th postoperative day.Results There was no significant difference between two groups in preoperative general data,preoperative lung function,preoperative quality of recovery(QoR-15)and other baseline data,as well as the surgery data.On the 7th postoperative day,the decrease of pulmonary function indexes,FVC(t=3.173,P=0.002)and FEV1(t=2.883,P=0.005)in the trial group was less than that in the control group,and the degree of percutaneous oxygen saturation in the trial group was higher than that in the control group(Z=-2.454,P=0.014).There was no significant difference in the incidence of postoperative pulmonary complications.As with the QoR-15,the score of trial group was higher than that in the control group in 3 items including"moderate pain"(Z=-2.738,P=0.006),"feeling nervous or anxious"(Z=-2.593,P=0.010)and"feeling sad and depressed"(Z=-2.945,P=0.003).There was no significant difference in other items between the two groups.Conclusion The respiratory care bundle combined with MBO is beneficial to the recovery of postoperative pulmonary function and postoperative quality of recovery of gastrointestinal surgical patients with respiratory risk.

Objective To develop an expert consensus on artificial airway humidification care in critical adult patients(refer as"Consensus"hereafter)for a standardised procedure of artificial airway humidification.Methods Reviews of domestic and international literatures relevant to artificial airway humidification care in critical adult patients were conducted.Clinical experiences of medical and nursing experts were taken into consideration.Two rounds of expert consultations were conducted to finalise the Consensus.Results Various aspects were studied including concepts,targeted range of temperature and humidification,methods of humidification,selection of humidification liquid and methods for evaluation of humidification effectiveness.The Consensus was applicable to the management of humidification of artificial airway in critical adult patients.Conclusions The Consensus exhibits a certain level of scientific rigor and practical applicability.It serves as a theoretical basis and practical guide to clinical nursing personnel in the management of artificial airway humidification.

Objective To explore the humidification effects between the humidifiers Venturi high-flow oxygen therapy(HVHF)and the high-flow humidified oxygen therapy in the treatment of patients with tracheotomy after the withdrawal of ventilator,and analyse the humidification performance and effect of airway humidification on the two oxygen therapies hence to provide an objective basis for selection of a humidified oxygen therapy.Methods A total of 146 ICU patients who had tracheotomy and completely withdrawal of ventilator in a general hospital in Shenzhen from July 2020 to December 2021 were randomly divided into trial group(n=73)and control group(n=73).With identical speed of airflow,patients in the trial groups were treated with HVHF and the patients of control group were offered with high-flow humidified oxygen therapy via AIRVOTM2.Data of the two groups were compared at the time points of days 0,2,7 and 14 in terms of absolute humidity(AH),relative humidity(RH),temperature(T)),sputum viscosity,arterial partial pressure of oxygen(PaO2),arterial partial pressure of carbon dioxide(PaCO2),oxygenation index(PaO2/FIO2)and the incidence of pulmonary infection.Results In the study,total of 61 patients in the control group and 72 patients in the trial group completed the high-flow humidified oxygen therapies,due to tubing detachments in 12 and 1 patients in the two groups,respectively.Repeated-Measures ANOVA analysis showed that,in both groups,there was a time effect(P<0.05)between the absolute humidity,relative humidity,temperature of the gas,PaO2,PaCO2,and PaO2/FiO2 at different time points.PaO2 and PaO2/FiO2 in both groups showed interactions at different time points(P<0.05).PaO2 and PaO2/FiO2 in the trial group were better than those in the control group at the time points of days 2,7 and 14(P<0.05).On days 2,7 and 14,the viscosity of sputum in the intervention group was better than that in the control group,and the incidence of pulmonary infection in the trial group was significantly lower than that in the control group(P<0.05).Conclusions HVHF and AIRVOTM2 both exhibit no obvious difference in gas humidification via high-flow humidification oxygen therapy in the patients with tracheotomy after withdrawal of ventilator.However,HVHF is superior to AIRVOTM2 in terms of improving airway humidification and oxygenation as well as reducing lung infection.Therefore,it is suggested that an HVHF is preferable for high-flow humidified oxygen therapy in treating the patients with tracheotomy after the withdrawal of ventilator.
