1.Partial nephrectomy for renal cell carcinoma in a transplanted kidney

Renal cell carcinoma in a transplanted kidney is rare, and partial nephrectomy in transplanted kidney is even more uncommon. A patient with papillary transplanted renal cell carcinoma who underwent partial nephrectomy was reported. The patient was admitted to the hospital due to the discovery of transplanted kidney mass, and the enhanced examination of CTU found a lesion in the upper pole of the transplanted kidney in the right iliac fossa, which may be renal cancer, with a diameter of about 2 cm. After open partial nephrectomy of the transplanted kidney, the postoperative pathology showed that the tumor was papillary renal cell carcinoma. The antirejection drugs were used after surgery. During the follow-up period of 16 months, there was no recurrence of transplanted kidney tumor.

3.Percutaneous Kidney-sparing Surgery for Upper Tract Urothelial Carcinoma:Report of 11 Cases

Objective To analyze the clinical efficacy of percutaneous kidney-sparing surgery(PCKSS)for the treatment of upper tract urothelial carcinoma(UTUC).Methods A retrospective analysis was conducted on clinical data and oncological characteristics of 11 cases of UTUC treated with PCKSS at our hospital from January 2018 to December 2023.After establishing a percutaneous renal working channel,a laser fiber or plasma resection device was inserted through the nephroscope to remove the tumor piece by piece along the edge of the tumor by 0.5 cm.Results All the 11 cases of PCKSS surgery were successfully completed,with 3 cases having multiple tumors.The surgical time was 75-216 min(median,150 min),and the intraoperative bleeding volume was 20-400 ml(median,150 ml).Postoperative pathological diagnosis showed 9 cases of high-grade UTUC,including 3 cases of G2 grade and 6 cases of G3 grade,with 5 cases in pTa phase and 4 cases in pT1 phase.In 2 cases with preoperative ureteroscopic biopsy showing high-grade pTa phase UTUC,no definite tumor was found during the PCKSS due to shallow sampling and obvious tissue burning.The average postoperative hospitalization time was(6.9±2.6)d.The 1 1 cases were followed up for 2-55 months(median,12 months),and there were 4 cases of recurrence,including 2 deaths,and 7 cases of recurrence free survival.Conclusions PCKSS has certain application value for treating UTUC with renal insufficiency.However,it is necessary to carefully select suitable patients before surgery,and strict and regular follow-up is required after surgery.
4.Application of spiral tracheoplasty in thyroid cancer with tracheal invasion

Objective:To explore the application of spiral tracheoplasty in the repair of large tracheal defects after the resection of trachea invaded by thyroid cancer.Methods:A retrospective analysis was performed on the clinical data of 11 patients, including 4 males and 7 females, aged from 36 to 67 years old, with large tracheal defects after tracheal resection due to thyroid papillary carcinoma invading the trachea in the Department of Otorhinolaryngology Head and Neck Surgery, Second Xiangya Hospital, Central South University from January 2019 to January 2022. The range of tracheal defects, time of tracheal reconstruction, postoperative complications and airway were recorded, and the patients were followed up for more than 24 months.Results:All patients underwent total thyroidectomy and tracheal resection and spiral reconstruction, and 2 of the cases underwent cricoid cartilage resection on the invaded side of cartilage. The tracheal defect accounted for 40%-60% of the circumference, and the lengths of the tracheal defects were 5.0 cm×7.5 cm and 6-9 tracheal rings, with 2 cases combined with partial defects of the cricoid cartilage. The reconstruction time was 30-60 min, with an average of 41.81 min. Among the 11 patients, 3 had recurrent laryngeal nerve paralysis, and 2 underwent tracheotomy. Four patients presented with hypocalcemia within one month after the operation. Followed up for 24-60 months, no tracheal stenosis occurred, the 2-year survival rate was 100%, the 2-year local control rate of the trachea was 100%, and the 2-year tumor-free survival rate was 81.8%.Conclusion:Spiral tracheoplasty is a safe and effective method that can reduce the tension at the tracheal anastomosis and expand the tracheal cavity, which can be used for tracheal reconstruction after extensive resection of trachea invaded by locally advanced thyroid cancer.
5.Reconstitution of double-negative T cells after cord blood transplantation and its predictive value for acute graft-versus-host disease

Background::With an increasing number of patients with hematological malignancies being treated with umbilical cord blood transplantation (UCBT), the correlation between immune reconstitution (IR) after UCBT and graft-versus-host disease (GVHD) has been reported successively, but reports on double-negative T (DNT) cell reconstitution and its association with acute GVHD (aGVHD) after UCBT are lacking.Methods::A population-based observational study was conducted among 131 patients with hematological malignancies who underwent single-unit UCBT as their first transplant at the Department of Hematology, the First Affiliated Hospital of USTC, between August 2018 and June 2021. IR differences were compared between the patients with and without aGVHD.Results::The absolute number of DNT cells in the healthy Chinese population was 109 (70-157)/μL, accounting for 5.82 (3.98-8.19)% of lymphocytes. DNT cells showed delayed recovery and could not reach their normal levels even one year after transplantation. Importantly, the absolute number and percentage of DNT cells were significantly higher in UCBT patients without aGVHD than in those with aGVHD within one year ( F = 4.684, P = 0.039 and F = 5.583, P = 0.026, respectively). In addition, the number of DNT cells in the first month after transplantation decreased significantly with the degree of aGVHD increased, and faster DNT cell reconstitution in the first month after UCBT was an independent protective factor for aGVHD (HR = 0.46, 95% confidence interval [CI]: 0.23-0.93; P = 0.031). Conclusions::Compared to the number of DNT cells in Chinese healthy people, the reconstitution of DNT cells in adults with hematological malignancies after UCBT was slow. In addition, the faster reconstitution of DNT cells in the early stage after transplantation was associated with a lower incidence of aGVHD.
6.Effect of azathioprine on ferroptosis in spermatocytes of mice induced by RSL3

Objective:To discuss the effect of azathioprine(AZA)on ferroptosis in spermatocytes of the mice induced by reduced glutathione(GSH)peroxidase 4 inhibitor RSL3,and to clarify the potential mechanism.Methods:The spermatogonia GC-2 cells of the mice were randomly divided into control group(no treatment),RSL3 group(treated with 10 nmol·L-1 RSL3 for 24 h),RSL3+ferroptosis inhibitor(Ferrostatin-1,Fer-1)group(treated with 10 nmol·L-1 RSL3 for 24 h+2 μmol·L-1 Fer-1 for 12 h),RSL3+low dose of AZA group(treated with 10 nmol·L-1 RSL3 for 24 h+5 μmol·L-1 AZA for 12 h),RSL3+medium dose of AZA group(treated with 10 nmol·L-1 RSL3 for 24 h+10 μmol·L-1 AZA for 12 h),and RSL3+high dose of AZA group(treated with 10 nmol L-1 RSL3 for 24 h+20 μmol·L-1 AZA for 12 h).The MTT method was used to detect the activities of the GC-2 cells in various groups after treated with different concentrations of AZA and RSL3;the GSH and GSSG levels in the GC-2 cells were detected by GSH and oxidized glutathione(GSSG)detection kits;the malondialdehyde(MDA)levels in the GC-2 cells in various groups were detected by MDA detection kit;Western blotting method was used to detect the expression levels of long-chain acyl-CoA synthetase 4(ACSL4),heme oxygenase-1(HO-1),and glutathione peroxidase 4(GPX4)proteins in the cells in various groups;immunofluorescence staining was used to detect the expressions of ACSL4 protein in the cells in various groups.Results:Compared with control group,the differences in activities of the GC-2 cells in 5,10,and 20 μmol·L-1 AZA groups had no significant differences(P>0.05),while the activities of the GC-2 cells in 30 and 40 μmol·L-1 AZA groups were significantly decreased(P<0.01);therefore,the AZA concentration was selected to be within 20 μmol·L-1.Compared with control group,the differences of the activities of the GC-2 cells in 1,5,and 10 nmol·L-1 RSL3 groups had no significant differences(P>0.05),while the activities of the GC-2 cells in 50,100,500,and 1 000 nmol·L-1 RSL3 groups were significantly decreased(P<0.05 or P<0.01);therefore,the RSL3 concentration was set to be within 10 nmol·L-1.The GSH and MDA detection kits results showed that compared with control group,the levels of GSSG and MDA in the GC-2 cells in RSL3 group were significantly increased(P<0.05),while the GSH levels were significantly decreased(P<0.05);compared with RSL3 group,the levels of GSSG and MDA in the GC-2 cells in RSL3+Fer-1 group and RSL3+AZA group were significantly decreased(P<0.01),while the GSH levels were significantly increased(P<0.01).The Western blotting results showed that compared with control group,the expression levels of ACSL4 and HO-1 proteins in the GC-2 cells in RSL3 group were significantly increased(P<0.05),while the expression level of GPX4 protein was significantly decreased(P<0.01);compared with RSL3 group,the expression levels of GPX4 protein in the GC-2 cells in RSL3+Fer-1 group and RSL3+AZA group were significantly increased(P<0.05),while the expression levels of ACSL4 and HO-1 proteins were significantly decreased(P<0.01).The immunofluorescence staining results showed that compared with control group,the expression amount of ACSL4 protein in the GC-2 cells in RSL3 group was significantly increased,and compared with RSL3 group,the expression amounts of ACSL4 protein in the cells in RSL3+Fer-1 group and RSL3+AZA group were significantly decreased.Conclusion:AZA can alleviate the ferroptosis-induced by RSL3 in spermatocytes of the mice.
7.Feasibility and efficacy of pre-management of superior laryngeal artery in endoscopic surgery for hypopharyngeal cancer

Objective:To explore the feasibility and efficacy for the dissection and ligation of the superior laryngeal artery in endoscopic surgery for hypopharyngeal cancer.Methods:Eight cadaveric heads were selected, and the laryngopharynxes were harvested. The positions of the superior laryngeal arteries entering the larynxes were dissected and observed under endoscopic vision, and their anatomical characteristics were summarized. Twenty-nine patients (all were male, aged 39-74 years old) with hypopharyngeal cancer who underwent transoral endoscopic surgery at the Department of Otorhinolaryngology Head and Neck Surgery of the Second Xiangya Hospital, Central South University from January 2018 to December 2019 were selected, and the patients were randomly divided into two groups by drawing lots, namely, the superior laryngeal artery was actively dissected and occluded during surgery in observation group ( n=15) or not in control group ( n=14). The differences in surgical time, bleeding volume, postoperative complications, and postoperative disease-free survival rate were compared between the two groups. Statistical analysis was conducted using SPSS 25.0 software. Results:The entry point of the superior laryngeal artery into the larynx was approximately at the level of the superior edge of the thyroid cartilage, and entered the larynx at the posterior one-third of the lateral wall of the pyriform fossa. The superior laryngeal artery might be determined through endoscopic exploration in all patients of observation group. The endoscopic surgery time [(40.00±7.56) minutes] and intraoperative bleeding volume [(24.00±8.28) ml] in the observation group were respectively less than those [(48.57±14.06) minutes and (42.86±15.41) ml] in the control group, and the differences were statistically significant ( t=-2.064, P=0.049; t=-4.064, P=0.001). There was no case with postoperative bleeding in the observation group, but with one case of postoperative bleeding in the control group. Total disease free survival rate was 86.2% and there was no significant difference in disease free survival rates between the two groups during a follow-up period of at least 36 months ( P=0.986). Conclusion:Dissection of the superior laryngeal artery during endoscopic surgery for hypopharyngeal cancer is feasible, and pre-management and occlusion of the superior laryngeal artery can effectively reduce intraoperative bleeding.
8.Clinicopathological features and prognosis of non-clear cell renal cell carcinoma in young patients aged 18-40 years

【Objective】 To summarize the clinicopathological features and prognosis of young patients (18-40 years old) with non-clear cell renal cell carcinoma (nccRCC) treated in a single center to provide reference for the diagnosis and treatment of similar patients. 【Methods】 Clinical data of 113 nccRCC patients treated during Jan. 2012 and Aug. 2022 were retrospectively analyzed, including 57 males (50.4%) and 56 females (49.6%). The average age of onset was (31.6±5.8) years. Among all patients, 57 had lesions (50.4%) on the left side, and 56 (49.6%) on the right side. Young patients undergoing renal cancer surgery accounted for approximately 12.4% of the total number of renal cancer patients undergoing surgery, and nccRCC accounted for 34.8% of the total number of cases. 【Results】 Minimally invasive surgery (laparoscopic or robot-assisted) was performed in 102 cases (90.3%), and open surgery in 11 cases (9.7%). Fifty-five cases (48.7%) underwent partial nephrectomy and 58 (51.3%) radical nephrectomy. Among them, 11 patients (9.7%) developed tumor thrombi. All surgeries were successful with no serious complications. The pathological types included 32 cases (28.3%) of chromophobe renal cell carcinoma, 25 cases (22.1%) of MiT family translocation renal cell carcinoma, and 20 cases (17.7%) of papillary renal cell carcinoma. The total proportion of the three pathological subtypes reached 68.1%. After 46 (2-115) months of follow-up, 8 cases (7.8%, 8/102) developed tumor metastasis and 2 died. 【Conclusion】 The nccRCC is rare in young patients. The major pathological type is chromophobe, and the major treatment method is minimally invasive surgery. Most pathological types have good long-term prognosis, while patients with tumor thrombi have a high risk of metastasis and poor prognosis.
9.A case report of bladder metastasis from pancreatic cancer

Bladder metastasis of pancreatic cancer is rare in the clinic.The male patient was diagnosed with pancreatic cancer and underwent laparoscopic pancreaticoduodenectomy. An abdominopelvic CT examination was performed 18 months after the operation and found a new bladder mass. Transurethral resection of bladder tumor was performed, and pathological examination showed that the mass was consistent with metastasis of pancreatic poorly differentiated adenocarcinoma. The patient received chemotherapy after the operation, and there was no tumor recurrence or new metastasis on PET-CT imaging after 8 months of follow-up. This case suggested the possibility of metastasis if a bladder mass was present after the surgery of pancreatic cancer.
10.Effects of Bushen Jianpi formula on farnesoid X receptor-mediated inhibition of hepatocellular carcinoma AH-130 cell-induced cachexia in rats and its underlying mechanism

Objective:To investigate the effects of Bushen Jianpi formula(BSJP)in mediating the inhibitory effects of farnesoid X receptor(FXR)on cancer cachexia(CC)induced by hepatocellular carcinoma AH-130 cells in a rat model and its underlying mechanism.Methods:A liver cancer cachexia rat model was established by intraperitoneal injection of AH-130 cells.The rats were divided into five groups:blank control,model control,CDCA(FXR agonist),BSJP,and BSJP+CDCA groups.After modelling,the rats were treated with CDCA,BSJP,or their combination for consecutive 16 days,and their body weights were measured weekly.At the end of the experiment,the rats were sacrificed,and abdominal aortic blood,feces,and brown adipose tissues from the epididymal,inguinal,and scapular regions were collected.Liquid chromatography-mass spectrometry(LC-MS)was used to detect the composition and content of bile acids in serum and feces of rats.WB and qPCR were used to detect the expression of FXR,Wnt family member 10b(Wnt10b),β-catenin,and uncoupling protein 1(UCP-1)in the ileum,brown adipose,and white adipose tissues of rats.Results:Compared with the blank control group,the body mass of the rats in the model group was significantly reduced(P<0.01);compared with the model control group,the body mass of the rats in CDCA,BSJP and BSJP+CDCA groups all increased(P<0.01).Compared with the blank control group,the epididymal,inguinal,scapular,and total brown adipose tissue mass were all elevated in the model control group(all P<0.05);compared with the model control group,the brown fat mass in the inguinal and epididymis regions of the rats decreased significantly in all treatment groups(P<0.05),but this decrease was not significant in the scapular region(P>0.05);besides,the total brown fat mass decreased notably in all treatment groups compared to the model control group(all P<0.01).LC-MS analysis showed that the composition and content of bile acids in the serum and feces of rats were altered in all groups.qPCR and WB results confirmed that,compared to the model group,BSJP and BSJP+CDCA promoted the mRNA and protein expression of FXR in the ileum,brown adipose,and white adipose tissues of rats(all P<0.05),and decreased the mRNA and protein expression of Wnt10b,β-catenin,UCP-1(all P<0.05).Conclusion:The BSJP formula can inhibit hepatocellular carcinoma cell AH-130-induced cachexia in rats,alleviating the associated body weight loss and brown adipose tissue formation.The mechanism may involve the regulation of FXR and the inhibition of the expression of Wnt10b,β-catenin,and UCP-1 in brown adipose tissue through the Wnt signaling pathway.
