1.Early diagnosis and treatment of a case of gastrointestinal perforation and candidemia caused by gastrointestinal mycosis

Fungi are widely present in the human environment and generally non-pathogenic.However,they may invade tissues and cause disease when host immunity is compromised or conditions conducive to fungal proliferation arise.A case of a 70-year-old woman with acute gastrointestinal perforation and secondary abdominal infection,admitted to the department of intensive care medicine(ICU)of the Affiliated Hospital of Qingdao University on August 17,2021 was enrolled.Following emergency exploratory laparotomy with gastric perforation repair,partial small bowel resection,peritoneal lavage drainage,and jejunal feeding jejunostomy,the patient received imipenem and cilastatin sodium antimicrobial therapy.Despite these interventions,clinical deterioration ensued with septic shock and multiple organ dysfunction syndrome.Prompt initiation of antifungal therapy alongside existing anti-infective treatment,anti-shock management,continuous renal replacement therapy(CRRT),and invasive mechanical ventilation led to rapid clinical improvement.Notably,blood cultures obtained at admission revealed Candida albicans after 5 days in ICU,while pathological examination of gastric pyloric perforation margins(8 days post-admission)demonstrated abundant Candida and small bowel ulcer sections showed extensive Mucor infiltration.This case highlights critical lessons in ICU management:Invasive fungal infection should be strongly suspected when gastrointestinal perforation patients deteriorate despite adequate surgical intervention and empirical antibacterial therapy;Timely fungal diagnostic workup should be initiated,utilizing validated fungal infection risk assessment tools to guide rapid diagnosis;Early empirical antifungal therapy proves crucial for improving outcomes in such critical scenarios.
2.Mediating effect of sleep quality on the relationship between serum vitamin D3 level and depressive symptoms in community-dwelling elderly people with mild cognitive impairment

Objective:To explore the mediating effect of sleep quality on the relationship between serum vitamin D3 level and depressive symptoms in community-dwelling elderly people with mild cognitive impairment(MCI).Methods:A total of 480 elderly people with MCI meeting inclusion criteria in a community health service center from Jan 2023 to Dec 2023 were selected as the research objects.The general information questionnaire,mini-mental state examination(MMSE),activity of daily living scale(ADL),Center for Epidemiological Studies Depression Scale(CES-D)and Pittsburgh sleep quality index(PSQI)were used to investigate the subjects.Serum vitamin D3 level was detected within 1 week.Spearson correlation analysis was used to explore the correlation between sleep quality,serum vitamin D3 level and depressive symptoms.The mediating effect model was tested by Model 4 in the Process plug-in.Results:Among the 480 MCI elderly,the average score of depression was(12.28±4.75),the average serum vitamin D3 level was(25.81±7.09)ng/ml,and the average score of sleep quality was(7.17±2.71).Depression severity was positively correlated with sleep quality score(r=0.294,P<0.01),and negatively correlated with serum vitamin D3 level(r=-0.237,P<0.01).Serum vitamin D3 level was negatively correlated with sleep quality score(r=-0.225,P<0.01).Bootstrap analysis showed that the mediating effect of sleep quality between serum vitamin D3 level and depression level was-0.064(95%CI:-0.115--0.023),accounting for 26.57%of the total effect.Conclusion:Sleep quality partially mediates the relationship between serum vitamin D3 level and depressive symptoms in the elderly with MCI in the community.
3.Current status of cleaning and disinfection of medical bedding items in public medical institutions of Liangshan ethnic minority area

OBJECTIVE To investigate the current status of cleaning and disinfection of medical bedding items in public medical institutions of Liangshan ethnic minority area and analyze the risk of infections so as to provide ba-ses for optimizing the infection control strategies in the ethnic minority area.METHODS On the basis of question-naire survey,onsite sampling method and bacterial isolation and identification technique,a cross-sectional survey was conducted for the current status of cleaning and disinfection of medical bedding items in 15 different grades of public medical institutions of Liangshan ethnic minority area by stratified random sampling method from Sep.2021 to Sep.2023.RESULTS A total of 66 valid questionnaire were recycled,and 540 bedding item samples were col-lected.Totally 93.33%of the medical institutions relied on laundries for centralized cleaning of the medical bed-ding items.The implementation rates of management items such as policy formulation(100.00%)and pre-job training were remarkably higher in the tertiary hospitals than in the secondary and primary hospitals(allP<0.05).There were no significant differences in the qualified rates of surveillance of microbiological indexes for the medical bedding items among the medical institutions,the qualified rate of surveillance of microbiological inde-xes was highest(71.30%)in the general hospitals and was lowest(61.11%)in the infectious diseases specialized hospital.The qualified rate of surveillance of microbiological indexes was lowest(3.33%)in pediatrics department or internal medicine department,and it raised most remarkably(80.00%)after the disinfection,and there was significant difference(x2=36.274,P<0.001).CONCLUSIONS The current status of disinfection of the medical bedding items is not optimistic in the medical institutions of Liangshan ethnic minority area.There are the prob-lems such as imperfect system,insufficient equipment and absence of personnel training.It is necessary to com-plete the execution and supervision system for cleaning and disinfection standards,increase the cleaning facilities,promote the use of antibacterial textile and bring in smart management system so as to improve the cleaning and disinfection quality and reduce the risk of hospital-associated infections.

9.Survey on the knowledge, attitude, and practices of breastfeeding among doctors and nurses in the neonatal intensive care unit of Qianxinan Prefecture, Guizhou Province

Objective:To understand the breastfeeding situation in the neonatal intensive care units (NICUs) in Qianxinan Prefecture, Guizhou Province, and to assess the knowledge, attitudes, and practices of doctors and nurses regarding breastfeeding, aiming to provide foundational data for improving breastfeeding quality.Methods:A questionnaire was developed to survey the knowledge, attitudes, and practices related to breastfeeding in NICUs. The questionnaire was divided into three dimensions: knowledge (seven items, total score of 7), attitudes (nine items, total score of 45), and practices (seven items, total score of 35). Lower scores indicated weaker recognition of breastfeeding. Additionally, five items were included to identify the most influential factors affecting breastfeeding. From November 25 to November 30, 2023, a survey was conducted among doctors and nurses with professional qualifications who had worked in the neonatal departments of nine hospitals in Qianxinan Prefecture for at least one year. Independent sample t-tests and Chi-square tests were used to compare the scores of doctors and nurses from different levels of hospitals and within the same level of hospitals across the three dimensions. Results:(1) Among the nine hospitals, three were tertiary grade A hospitals (referred to as "tertiary hospitals"), with 95.6% (43/45) of the doctors and 96.5% (110/114) of the nurses participating in the survey. Six were secondary grade A hospitals (referred to as "secondary hospitals"), with 95.0% (38/40) of the doctors and 97.6% (83/85) of the nurses participating. (2) All nine hospitals were baby-friendly hospitals and all had breastfeeding promotional materials. Six hospitals had NICUs that promoted breastfeeding, with an average NICU breastfeeding rate of 25.8% across the prefecture between year 2021 to 2023. (3) The proportion of doctors who had received breastfeeding training was higher than that of nurses within the same level of hospitals [tertiary hospitals: 69.8% (30/43) vs. 40.0% (44/110), χ 2=10.97, P=0.001; secondary hospitals: 47.4% (18/38) vs. 24.1% (20/83), χ 2=6.55, P=0.010], although the overall training rates were low. (4) In tertiary hospitals, doctors scored higher than nurses in the attitude dimension [(35.35±4.75) vs. (33.18±5.60) scores, t=-2.03, P=0.044] and also in the practice dimension [(26.98±3.00) vs. (25.60±3.75) scores, t=-2.17, P=0.032]. In secondary hospitals, the total knowledge dimension score of doctors was higher than that of nurses [(4.92±1.44) vs. (4.20±1.45) scores, t=-2.52, P=0.013]. In tertiary hospitals, the total scores for attitude and practice dimensions of doctors were higher than those of doctors in secondary hospitals, and the total scores for knowledge, attitude, and practice dimensions of nurses were higher than those of nurses in secondary hospitals (all P<0.05). (5) In the knowledge dimension, the lowest scoring item of doctors in the tertiary hospitals was "Breastfeeding is possible for maternal hepatitis B newborns after receiving vaccines and immunoglobulin"; the lowest scoring item of nurses in the tertiary hospital, and doctors and nurses in the secondary hospitals was "The duration of breastfeeding has a greater impact on neonatal outcomes". In the attitude dimension, the lowest scoring item for doctors and nurses in both tertiary and secondary hospitals was "You think the breastfeeding process is more troublesome than feeding preterm formula". In the practice dimension, the lowest scoring item of the doctors and nurses in the tertiary hospitals was "Your hospital had enough breastfeeding knowledge training", while for the doctors and nurses in the secondary hospitals were "You have more opportunities to participate in various breastfeeding-related training" and "Breast feeding should be started as soon as possible when the infant is stable after active treatment", respectively. (6) The most influential factors affecting breastfeeding were: lack of cooperation from parents (50.0%, 137/274), relative insufficient human resources for doctors and nurses (21.9%, 60/274), and the absence or poor implementation of breastfeeding management policies (18.3%, 50/274), etc. Conclusions:The breastfeeding rate in NICU of county-level hospitals is relatively low, and medical staff, especially nurses, have insufficient knowledge about breastfeeding. It is necessary to strengthen various breastfeeding training for medical staff to enhance their understanding of NICU breastfeeding.
10.Effect of acacetin on lipopolysaccharide induced apoptosis of dental pulp cells by regulating the HMGB1/TLR4 signaling pathway

Objective To investigate the effect of acacetin(ACE)on the apoptosis of dental pulp cells induced by lipopolysaccharide(LPS)by regulating the high mobility histone 1(HMGB1)/Toll-like receptor 4(TLR4)signaling pathway.Methods Third generation dental pulp cells were isolated,cloned and purified from pulp of five patients with healthy teeth extracted from orthodontics and impaction.Cell morphology was observed and identification of stromal cell surface antigen1(STRO1)by immunofluorescence,and epidermal antigens CD90,CD105,CD34 and CD45 were identified by flow cytometry.MTT was applied to identify the effect of ACE on the activity of primary dental pulp cells(DPSCs).DPSCs were divided into the control group(0 μmol/L ACE culture),the LPS group(1 ng/L LPS treatment),the L-ACE group(added 20 μmol/L ACE on the basis of LPS group),the H-ACE group(added 40 μmol/L ACE on the basis of LPS group),the pcDNA-NC group(transfected pcDNA-NC plasmid on the basis of H-ACE group)and the HMGB group(transfected pcDNA-HMGB on the basis of H-ACE group).Cell proliferation was detected by CCK-8.Clone formation number was detected by clone formation assay.Western blot experiments were applied to detect the expression of HMGB1/TLR4 pathway,apoptosis and inflammation related proteins.ELISA assay was applied to detect levels of interleukin(IL-)-1β,IL-4 and tumor necrosis factor-α(TNF-α)in cell supernatant.Results DPSCs were successfully isolated and identified based on cell morphology,positive expression of STRO1,positive expression of CD90 and CD105,and negative identification of CD34 and CD45.Compared with the control group,cell proliferation activity,clonal cell count,B-cell lymphoma-2(Bcl-2)and IL-4 levels were decreased in the LPS group,while apoptosis rate,the pathway related proteins HMGB1,TLR4,apoptosis related proteins aspartate-specific cysteine protease(Caspase)-3,Caspase-7,Bcl-2 associated X protein(Bax),and inflammatory factors IL-1β,TNF-α were increased in the LPS group(P<0.05).Compared with the LPS group,cell proliferation activity,Bcl-2 and IL-4 were increased successively in the L-ACE group and the H-ACE group,and apoptosis rate,pathway-related proteins HMGB1,TLR4,Caspase-3,Caspase-7 and Bax,and inflammatory factors IL-1β,TNF-α were reduced successively(P<0.05).Overexpression of HMGB reversed the effects of H-ACE on cell proliferation,apoptosis,expression of related proteins and inflammatory factors(P<0.05).Conclusion ACE inhibits LPS induced apoptosis of dental pulp cells,and it may be achieved by inhibiting the HMGB1/TLR4 signaling pathway.
