1.Isolation trend of Candida in a three-A hospital in Wuxi between 2021 and 2024

OBJECTIVE To investigate the distribution of Candida and predict the detection trend in the southern and northern campuses of Affiliated Hospital of Jiangnan University,Wuxi,between 2021 and 2024.METHODS A total of 27 056 patients with common Candida infections from the southern and northern campuses of Affiliated Hospital of Jiangnan University between 2021 and 2024 were selected to analyze the distribution of Candida species and predict the detection trend.RESULTS Among the 27 056 patients,there were 11 061 males and 15 995 fe-males,aged from 1 to 101 years,with a median age of 68 years.Over the past four years,the top five most com-monly detected Candida species in the hospital were Candida albicans,Candida glabrata,Candida tropicalis,Candida parapsilosis and Candida krusei.Statistically significant differences were found in infection characteris-tics among patients with C.albicans and C.glabrata in terms of gender,age,specimen source and related diseases(P<0.05).From 2021 to 2024,the number of detected cases declined in 2022 and then rebounded(P<0.001).Among the detected patients,those aged 70 and above accounted for the highest proportion.Regarding the distribution of specific diseases,the top three were vaginitis(4 176 cases,15.43%),bacterial pneumonia(1 842 cases,6.81%)and cancer(1 279 cases,4.73%).Patients with vaginitis were mainly infected with C.albicans,while patients with bacterial pneumonia were predominantly infected with C.albicans and C.glabrata.The LSTM model showed a good fit to the training set,with an root-mean-square error(RMSE)of 145.03 and an mean absolute error(MAE)of 131.19.Model predictions indicated that the number of patients with Candida in-fections in the hospital would remain low from Jan.to May 2025,which was basically consistent with actual clini-cal observations(RMSE=94.71,MAE=84.00).CONCLUSIONS The common diseases associated with Candi-da infections in the hospital include vaginitis,bacterial pneumonia and cancer.C.albicans and C.glabrata are the main pathogenic species,and the infection situation is relatively severe.The LSTM model performs well in short-term prediction and dynamic analysis of Candida detection trends.
2.Application of ARIMA in prediction of prevalence trend of carbapenem-resistant Klebsiella pneumoniae in ICU

OBJECTIVE To explore the application of autoregressive integrated moving average model(ARIMA)in prediction of prevalence trends of carbapenem-resistant Klebsiella pneumoniae(CRKP)in intensive care unit(ICU)so as to provide scientific bases for formulating prevention strategies for CRKP infection in ICU.METHODS The number of CRKP strains that were monthly isolated from the ICU patients of Jiangnan University Affiliated Hospital between Jan.2021 and Jan.2024 was collected,the duplicate samples from the same patient were excluded,and totally 555 strains of CRKP were finally enrolled in the study.The time series differencing was performed by using R statistical software,the ARIMA model was established,the parameters of the model were determined by means of autocorrelation function(ACF)and partial autocorrelation function(PACF)images.The optimal model was screened out by Akaike information criterion(AIC)and root-mean-square error(RMSD),the robustness of the residual sequences was assessed by Box-Ljung test.The data that were detected from Sep.2023 to Jan.2024 were assigned as the validation set,the prediction accuracy of the model was assessed,and the dy-namic trend of the CRKP strains from Feb.2024 to Apr.2024 was predicted.RESULTS The isolation rate of CRKP strains in ICU showed dynamic change from 2021 to 2023(x2=66.906,P=0.001),sputum and midstream urine were the major sources.The minimal AIC of the optimal ARIMA model(8,1,10)was 222.1,with RMSE 3.67.Box-Ljung(x2=0.104,P=0.746)test indicated that there was no autocorrelation among the residual se-quences.Both the actual CRKP and the predictive value firstly rose then descended from Sep.2023 to Jan.2024,and the average relative error of the model was 9.62%for prediction.The number of isolated CRKP strains predic-ted by the model might reached to the lowest point in Feb.2024 and then showed an upward trend,and it might reach to a high peak in Apr.CONCLUSION ARIMA model is effective for short-term prediction and dynamic anal-ysis of prevalence trend of CRKP strains in the ICU so as to provide theoretical basis for early warning of hospital-associated infection and take corresponding prevention measures.
3.Research progress on facial expressions for assessing emotions in dogs

Emotions are an integral part of animal welfare.Facial expressions are increasingly used as a non-invasive method for assessing the emotional state of animals.Dogs(Canis familiaris)are closely related to humans,and facial expressions are crucial for interspecies communication and emotional expression.This paper reviews the neurobiological mechanisms of facial expressions and the anatomical structure of the facial muscles and their evolution in dogs,the analogy between dog and human facial expressions,and the expression of various parts of the dog's face under different emotions.These studies demonstrate that dog's facial expressions can be used as a potential indicator of animal welfare,and that dogs can be used as a model animal for studying interspecies facial emotional communication.The use of dog's facial expressions to assess emotions will aid multidisciplinary research,including in the fields of neuroscience,psychopharmacology,animal behavior,and animal welfare.
4.Mechanism of action of baicalin on renal injury caused by Klebsiella pneumoniae of sheep origin

Based on network pharmacology,molecular docking technology and experimental valida-tion to explore the therapeutic efficacy and mechanism of action of baicalin(BC)on kidney injury caused by Klebsiella pneumoniae(KP)infection.The inhibitory activity of BC against KP was de-termined by in vitro experiments;a mouse kidney injury model was established,and the therapeu-tic effect was preliminarily verified by ophthalmoscopy and pathological histology;three pro-in-flammatory factors,namely,TNF-α,IL-10,and IL-1β,were detected by ELISA;and the cyber-pharmacology technology was utilized by PubChem,TCMSP,STRING,Cytoscape,AutoDocks and other databases and software to construct the PPI network as well as to perform GO function and KEGG enrichment analyses;and molecular docking technology was used to assess the binding ac-tivity of the drugs to the core targets and to speculate on the signaling pathways of the drug action.The results showed that BC had a better inhibitory effect on KP in the in vitro experiments;path-ological histology showed a significant therapeutic effect of BC;compared with the infected group,the content of pro-inflammatory factors TNF-α,IL-10,and IL-1β in the baicalin treatment group were significantly decreased(P≤0.05).Twenty-four core targets and 11 pathways of action were screened by network pharmacology,and BC docked stably with the acquired core targets TP53,PTGS2,MAPK1,MAPK8,TNF,BCL2,and IGF1 molecules,and it was speculated that BC might exert its antibacterial and anti-inflammatory effects through the signaling pathways of PI3K-Akt,MAPK,HIF-1,and NF-kappa B,etc.This study lays the foundation for further research on the mechanism of action of baicalin on renal injury.

9.Current work situation of out of hospital clinical research coordinators and optimization strategy

Objective To investigate the current work situation of clinical research coordinators(CRCs)and to offer proposals for optimization and improvement.Methods An online-questionnaire was used to survey the out-of-hospital CRCs assigned by Site Management Organization(SMO)in 23 clinical trial sites in Shanghai.Results CRCs were mainly female(91.09%)and 50.78%of the employees aged between 25 and 29 years old.Most CRCs(49.61%)had a nursing background.The average score of CRC professional identity was 66.45.Among the five dimensions of professional identity,the mean score of CRC occupational social cognition was the highest,while the mean score of occupational self-reflection was the lowest.The mean score of job satisfaction was 33.51,and the mean score of intrinsic satisfaction was the highest in the three dimensions of job satisfaction,and 55.18%CRCs had considered turnover.Among the 288 people considering turnover,64.24%were due to income not meeting expectations and a low chance of promotion.CRCs faced various difficulties such as difficulty in trial projects,difficulties in project execution,and weak professional skills.Conclusion Current status and career development bottleneck of out-of-hospital CRCs are clarified through the investigation.And countermeasures and suggestions are put forward from three aspects,including the state,profession and individuals in view of CRC shortage and its career development dilemma.
10.Preparation and in vitro evaluation of hemoglobin-paclitaxel dual loaded liposomes for improving tumor hypoxia resistance

【Objective】 To prepare liposomes encapsulate hemoglobin and paclitaxel(LEHP)to improve tumor hypoxia resistance. 【Methods】 LEHP were prepared by thin-film method, and the particle size, Zeta potential and polydispersity were investigated by nanoparticle size analyzer, and encapsulation efficiency was investigated by high performance liquid chromatography, and the interaction between the liposomes and tumor cells was evaluated by in vitro cell experiments. 【Results】 The optimal preparation conditions of LEHP was as follows: total phospholipid 36 mM, DPPC∶Dope∶cholesterol molar ratio 7∶2∶1, paclitaxel 3 mg, hydrated with 3 mg·mL-1 Hb-PBS for 30 min at room temperature; The average particle size was (189.17±8.22) nm, polydispersity was 0.14±0.023, paclitaxel encapsulation efficiency was (58.27±2.55)%, hemoglobin content was (0.63±0.05) mg·mL-1. In vitro cell experiments, the killing effect of LEHP was about 1.5 times that of LEP, about 1.2 times that of LEP, and ROS production was about 1.8 times that of LEP. 【Conclusion】 The preparation conditions of LEHP was optimized, and cell experiments showed that LEHP can promote tumor cell apoptosis by improving hypoxia and increasing ROS production, which is expected to provide a safe and effective new method for drug resistance caused by tumor hypoxia.
