1.Exploration on the Efficacy and Mechanism of the Combination of Cryoablation and Yangfei Prescription for the Treatment of Lewis Lung Cancer Bearing Mice Based on Whole Transcriptome

Objective To investigate the efficacy and mechanism of the combination of cryoablation and Yangfei establish the Lewis lung cancer mouse model,and randomly divided into model group,cryoablation group,cryoablation+cisplatin group,cryoablation+Yangfei Prescription low-,medium-and high-dosage groups,with 5 mice in each group.Sham-operation was performed in the model group,cryoablation+cisplatin group were given intraperitoneal injection of cisplatin 3 mg/kg after cryoablation,cryoablation+Yangfei Prescription low-,medium-and high-dosage groups were given 1.64,3.28,6.56 g/kg Yangfei Prescription by gavage after cryoablation respectively,and the model group was given equal volume of normal saline by gavage for 14 consecutive days.Tumor volume and tumor mass were measured,the morphology of tumor tissue was observed by HE staining,the expression of Ki-67 protein in tumor tissue was detected by immunohistochemistry.The differentially expressed genes in tumor tissues after cryoablation combined with Yangfei Prescription intervention were analyzed by whole transcriptome sequencing,GO and KEGG pathway enrichment analysis was performed on differentially expressed genes,and the lncRNA/circNA-mediated ceRNA network was constructed.Differentially expressed genes were verified using RT-qPCR.Results The main active components of Yangfei Prescription were terpenoids,flavonoids and aldehydes.Compared with the model group,tumor volume and tumor mass decreased(P<0.05)in cryoablation+Yangfei Prescription low-,medium-and high-dosage groups and cryoablation+cisplatin group;cryoablation combined with Yangfei Prescription could destroy the structure of tumor tissue and inhibit cell proliferation(P<0.05).A total of 1 585 differentially expressed genes(1 160 mRNA,225 lncRNA,155 miRNA and 45 circRNA)of the model group and cryoablation+Yangfei Prescription high-doseage group were obtained.GO enrichment analysis mainly involved biological processes such as immune system response and cell proliferation,cellular components such as protein complex and cell junction,molecular functions such as transcription regulator activity and molecular function regulator;KEGG pathway enrichment mainly occurred in cancer-related pathways such as PI3K-Akt signaling pathway,Th17 cell differentiation,mTOR signaling pathway,FOXO signaling pathway,etc.The lncRNA-mediated ceRNA network was composed of 97 lncRNA,73 miRNA and 417 mRNA,and the circRNA-mediated ceRNA network was composed of 26 circRNA,68 miRNA and 157 mRNA.RT-qPCR validated the expressions of 15 differentially expressed genes,which was consistent with the sequencing results.Conclusion Cryoablation combined with Yangfei Prescription can inhibit the tumor growth of Lewis lung cancer bearing mice,destroy the structure of tumor tissue,inhibit cell proliferation,the mechanism may be related to the regulation of Gm38393/miRNA-136-5p/Zfp704 axis.
2.Epidemiological characteristics and immune effects of pertussis in Jiangxi Province, 2020-2023

Objective:To analyze the epidemiological characteristics and immune effects of pertussis in Jiangxi Province from 2020 to 2023 and provide a reference basis for preventing and controlling pertussis in Jiangxi Province.Methods:The annual pertussis incidence data and population data of Jiangxi Province in the same period were derived from the China Disease Prevention and Control Information System, the population data for 2020 were derived from the seventh population census data of Jiangxi Province, and pertussis cases with PV immunization history were derived from the immunization program information system of Jiangxi Province. Descriptive epidemiological methods were used to analyze pertussis epidemic characteristics and PV immunization history in Jiangxi Province from 2020 to 2023. An expert modeler was used to construct the best time series model and perform seasonal decomposition. The chi-square test and Cochran-Armitage trend test were used for statistical analysis. Excel 2013 and SPSS 27.0 software were used for statistical analysis.Results:A total of 886 cases of pertussis were reported in Jiangxi Province from 2020 to 2023, with an average annual reported incidence rate of 0.49/100 000, showing a linear upward trend ( Z=121.80, P<0.001). The Winters' additive model was optimal to fit the monthly data with stable seasonality (Ljung-Box statistic Q=10.56, P=0.78), the average number of reported cases in August was higher than the annual average of 5.12. The top three cities with reported incidence rates were Pingxiang (1.11/100 000), Yichun (1.10/100 000), and Nanchang (0.73/100 000). The highest age composition of cases was in the under 1-year-old group (373 cases,42.10%). Under 1-year-old group, 5-9-years-old group, and ≥10-years-old group all showed an increasing trend in reported incidence year by year (all P<0.001), and the reported incidence rate of under 1-year-old group increased from 12.42/100 000 in 2020 to 48.85/100 000 in 2023. A total of 545 cases (61.51%) had a history of PV immunization. Four doses PV immunization was the main ones (44.92%, 398 cases) and the median ( Q1, Q3) interval from the last vaccination to onset was 4 (1,5) years. Conclusions:The reported incidence of pertussis in Jiangxi Province increased from 2020 to 2023. The cases are mainly concentrated in the under 1-year-old group. The rates of PV vaccination in pertussis cases was low, with more number of vaccine breakthrough infection cases with 4-doses PV vaccination. It has been suggested to continue doing the following: strengthening pertussis surveillance, improving the timeliness of pertussis vaccination, implementing the fifth dose as soon as possible for children over 6-year-old, strengthening the studies of vaccine breakthrough infection case, and optimizing the population immunity strategy of pertussis.
3.Epidemiological and clinical characteristics of surveillance cases in a sentinel hospital for pertussis in Jiangxi Province in 2019

ObjectiveTo analyze the epidemiological and clinical characteristics of surveillance cases in a sentinel hospital for pertussis in Jiangxi Province in 2019, and to provide corresponding references for the prevention and control of pertussis. MethodsCase investigation of pertussis was conducted among sentinel hospital surveillance cases, collecting their basic information, epidemiological characteristics, clinical characteristics, and other information. ResultsA total of 125 pertussis surveillance cases were investigated in 2019, including 73 clinically diagnosed cases (58.40%) and 52 confirmed cases (41.60%). The age of onset was mainly concentrated in children under 5 years old (108 cases, 86.40%), with the largest number of cases in infants aged less than 1-year-old (48 cases, 38.40%). Most cases had a history of receiving pertussis vaccine before onset (110 cases, 88.00%), and the intervals between the onset date and the date of last dose of pertussis vaccine in the 1‒2 doses group were significantly shorter than that in the 3‒4 doses group (U=-5.990, P<0.001). Probable household transmission of pertussis was found in 3 cases. All cases had cough symptoms, mainly manifested as whooping cough (77 cases, 61.60%), in addition to other main clinical manifestations, such as fever (76 cases, 60.80%), vomiting (30 cases, 24.00%), conjunctival congestion (27 cases, 21.60%), and inspiratory whoop (16 cases, 12.80%). A total of 73 cases (58.40%) experienced complications, including 1 death case. All the cases had multiple medical visit experiences before this visit, with an interval of 2 (0,3) days between the onset date and the first visit date. The misdiagnosis rate at the first medical visit was 88.00% (110/125), and the misdiagnosis rate of the first visit in secondary and primary hospitals was significantly higher than that in tertiary hospitals, exhibiting a statistically significant difference (χ2=21.582, P<0.001). ConclusionThe clinical symptoms of pertussis cases are often atypical, and the first diagnosis is prone to misdiagnosis, so it’s necessary to further strengthen the early diagnosis capabilities for pertussis cases in healthcare institutions, especially in the primary healthcare institutions.
4.Factors influencing self-care ability recovery in patients with acute myocardial infarction after receiving percutaneous coronary intervention

Objective To analyze the relevant factors affecting self-care ability recovery in patients with acute myocardial infarction after receiving percutaneous coronary intervention(PCI).Methods A total of 108 patients with acute ST-segment elevation myocardial infarction,who underwent PCI at Lu'an People's Hospital of China from January 2021 to December 2023,were selected for this study.The patients were divided into normal group(postoperative self-care ability returning to normal,n=78)and disabled group(postoperative self-care ability being dysfunctional,n=30).The clinical data were compared between the two groups.Multiple linear regression analysis was used to analyze the factors affecting the recovery of postoperative self-care ability in patients with acute myocardial infarction after receiving PCI.Results In the disabled group,the mean age was(73.5±5.2)years,63.33%of patients lived in the countryside,Killip grade Ⅰ or above accounted for 16.67%of patients,lack of postoperative rehabilitation training accounted for 73.33%of patients,the Gensini score was(21.35±1.82)points,all of which were higher than those in the normal group(all P<0.05).The LVEF was(47.81±8.20)%and the MNA-HF score was(20.17±3.33)points,which were lower than those in the normal group(both P<0.05).Multiple linear regression analysis revealed that age(B=-0.171,95%CI:-0.292 to-0.049),postoperative rehabilitation training(B=-3.946,95%CI:-6.469 to-1.422),LVEF(B=0.211,95%CI:0.085-0.338),MN A-HF score(B=0.318,95%CI:0.036-0.601),and Gensini score(B=—0.907,95%CI:-2.349 to—1.465)were the factors influencing the recovery of postoperative self-care ability in patients with acute myocardial infarction after receiving PCI(P<0.05).The regression analysis model showed:R2=0.815,adjusted R2=0.802,Durbin-Watson statistic being 1.497,F=62.827,and the fit being good(P<0.05).Conclusion The elderly,lack of postoperative rehabilitation training,decreased LVEF level,low MNA-HF score,and high Gensini score are the factors influencing the recovery of postoperative self-care ability in patients with acute myocardial infarction after receiving PCI.In clinical practice,targeted intervention measures should be timely taken to ensure the recovery of patient's self-care ability and improve the quality of life.
5.Revealing potential mechanism of Danggui Buxue decoction in regulating immuno-suppression via PI3K-AKT signaling pathway using network pharmacology and in vivo experiment

Danggui Buxue decoction(DBD)is a classic prescription with immunomodulatory and hematopoietic effects.Previous studies have shown the DBD has potential to be used as an oral im-mune booster.However,its immunomodulatory effects and mechanism of action have not been thoroughly studied,especially the protective mechanism of immunomodulatory regulation in the state of immunosuppressive is still unclear.The aim of this study was to explore the protective mechanism of DBD in the immunosuppressive state using network pharmacology combined with animal experiments verification.The active components,core targets and signaling pathways of DBD in treating immunosuppression were obtained using network pharmacology tools.On this ba-sis,the active components of DBD were identified using HPLC-MS,and in vivo studies were con-ducted at the same time.The key active components of DBD obtained using network pharmacology included quercetin,kaempferol and formononetin.The core targets included TP53,RELA,TNF,AKT1,and IL-6.KEGG pathway enrichment analysis showed that phosphoinositide 3-kinase(PI3K)-protein kinase B(AKT)may play an important role in the treatment of immunosuppres-sive diseases using DBD.Molecular docking confirmed that each core target had good binding activ-ity with its corresponding compounds.Animal experiments showed that after DBD intervention,the mRNA gene and protein expression of RELA,TNF,and IL-6 in the serum was significantly down-regulated.The mRNA expression of PI3K and AKT in the ileum and PI3K protein expression were also downregulated.In conclusion,DBD exerts its role in treating immunosuppressive diseases by regulating the PI3K-AKT signaling pathway.
6.Study on Correlation between CD14+CD16+Monocytes and IgG N-Glycosyl Levels in Peripheral Blood and Disease Activity in Patients with Systemic Lupus Erythematosus

Objective To investigate the correlation between peripheral blood CD14+CD16+monocytes and IgG N-glycosyl levels and disease activity in patients with systemic lupus erythematosus(SLE).Methods A total of 109 SLE patients admitted to Xingtai Cental Hospital from August 2021 to November 2024 were retrospectively selected as the study objects.According to SLE disease activity index(SLE-DAI),the patients were divided into active group(n=52)and stable group(n=57).In addition,56 patients who underwent physical examination during the same period were selected as the control group.Clinical data of patients were collected,CD14+CD16+mononuclear cells were detected by flow cytometry(FCM),and IgG N-glycosyl levels were detected by hydrophilic interaction chromatography-mass spectrometry.Logistic regression analysis was performed to analyze the influencing factors of SLE-DAI,and multicollinearity test[variance inflation factor(VIF)]was performed for independent variables.The prediction model of disease activity was constructed.The effectiveness of the predictive model was evaluated by describing the recviver operator characteristic(ROC)curve and calculating the area under the curve(AUC)value.Hosmer-Lemeshow goodness-of-fit test predicted the calibration degree of the model.Results The levels of WBC,Hb,PLT,ALB,complement C3 and complement C4 in control group were higher than those in SLE group(t=8.917~22.171),and the levels of CRP were lower than those in SLE group(t=-17.359),with differences were statistical significance(all P<0.05).The CRP level and the proportion of CD14+CD16+mononuclear cells in the active group were higher than those in the stable group,and the differences were statistically significant(t=5.449,11.112,all P<0.05).The IgG glycosylation characteristics of galactosylation,sialylation and N-acetylglucosamine modification were lower than those in the stable group,and the differences were statistical significance(Z=-2.432~-0.158,all P<0.05).Spearman correlation analysis showed that the proportion of CD14+CD16+monocytes was significantly negatively correlated with IgG galactosylation,sialylation level and bisection N-acetylglucosamine modification(r=-0.656,-0.531,-0.608,all P<0.01).CD14+CD16+monocyte ratio was positively correlated with SLE-DIA(r=0.581,all P<0.01).IgG galactosylation,sialylation levels and bisection N-acetylglucosamine modification were negatively correlated with SLE-DIA(r=-0.645,-0.609,-0.503,all P<0.01).Logistic regression analysis showed that CRP>8.21mg/L,CD14+CD16+≥16.17%,sialylation<22.05%and isotropic N-acetylglucosamine modification<16.53%were independent risk factors for disease activity in SLE patients(Wald χ2=4.471~12.811,all P<0.05).The VIF values of the above independent variables were all less than 10.By establishing the Logistic regression prediction model and drawing the ROC curve,the AUC value for diagnosing SLE disease activity was 0.821(95%CI:0.733~0.905),the sensitivity,specificity and the Yodon index were 85.37%,75.67%,0.677,respectively.and the P values of Hosmer-Lemeshow goodness-of-fit test models were 0.568,respectively.Conclusion The proportion of CD14+CD16+monocytes in peripheral blood of SLE patients increase significantly,and the level of IgG glycosylation characteristics decrease,both of which are correlated with SLE-DIA.The predictive model constructed based on the two had a good ability to distinguish SLE-DIA from inactive state,with high sensitivity and moderate specificity conducive to early clinical recognition,and the model fitting effect is good.SLE-DIA can be evaluated more accurately.
