Oral squamous cell carcinoma (OSCC) is a common head and neck malignancy. Approximately 50% to 60% of patients with OSCC are diagnosed at a locally advanced stage (clinical staging III-IVa). Even with comprehensive and sequential treatment primarily based on surgery, the 5-year overall survival rate remains below 50%, and patients often suffer from postoperative functional impairments such as difficulties with speaking and swallowing. Programmed death receptor-1 (PD-1) inhibitors are increasingly used in the neoadjuvant treatment of locally advanced OSCC and have shown encouraging efficacy. However, clinical practice still faces key challenges, including the definition of indications, optimization of combination regimens, and standards for efficacy evaluation. Based on the latest research advances worldwide and the clinical experience of the expert group, this expert consensus systematically evaluates the application of PD-1 inhibitors in the neoadjuvant treatment of locally advanced OSCC, covering combination strategies, treatment cycles and surgical timing, efficacy assessment, use of biomarkers, management of special populations and immune related adverse events, principles for immunotherapy rechallenge, and function preservation strategies. After multiple rounds of panel discussion and through anonymous voting using the Delphi method, the following consensus statements have been formulated: 1) Neoadjuvant therapy with PD-1 inhibitors can be used preoperatively in patients with locally advanced OSCC. The preferred regimen is a PD-1 inhibitor combined with platinum based chemotherapy, administered for 2-3 cycles. 2) During the efficacy evaluation of neoadjuvant therapy, radiographic assessment should follow the dual criteria of Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1 and immune RECIST (iRECIST). After surgery, systematic pathological evaluation of both the primary lesion and regional lymph nodes is required. For combination chemotherapy regimens, PD-L1 expression and combined positive score need not be used as mandatory inclusion or exclusion criteria. 3) For special populations such as the elderly (≥ 70 years), individuals with stable HIV viral load, and carriers of chronic HBV/HCV, PD-1 inhibitors may be used cautiously under the guidance of a multidisciplinary team (MDT), with close monitoring for adverse events. 4) For patients with a poor response to neoadjuvant therapy, continuation of the original treatment regimen is not recommended; the subsequent treatment plan should be adjusted promptly after MDT assessment. Organ transplant recipients and patients with active autoimmune diseases are not recommended to receive neoadjuvant PD-1 inhibitor therapy due to the high risk of immune related activation. Rechallenge is generally not advised for patients who have experienced high risk immune related adverse events such as immune mediated myocarditis, neurotoxicity, or pneumonitis. 5) For patients with a good pathological response, individualized de escalation surgery and function preservation strategies can be explored. This consensus aims to promote the standardized, safe, and precise application of neoadjuvant PD-1 inhibitor strategies in the management of locally advanced OSCC patients.
2.Influencing factors for hypertension and abnormal electrocardiogram in workers exposed to occupational noise in Songjiang District, Shanghai

ObjectiveTo examine the prevalence of hypertension and abnormal electrocardiogram (ECG) among the noise-exposed population in Songjiang District, Shanghai, and to analyze the influencing factors for hypertension and abnormal ECG. MethodsOccupational health examination data, encompassing basic demographic information, physical examinations, audiometric testing, blood pressure measurements, and ECG assessments, were collected from 20 165 noise-exposed workers across all manufacturing enterprise in Songjiang District in 2024. Hypertension and abnormal ECG findings were identified as indicators of health impairment. Categorical data were presented as frequencies and percentages (%). Univariate chi-square tests and multivariate logistic regression analyses were employed to identify factors associated with hypertension and abnormal ECG outcomes. ResultsThe detection rates of hypertension and abnormal ECG among noise-exposed workers in the manufacturing industry in Songjiang District were 22.78% and 13.06%, respectively. Across different enterprise scales, industry categories and economic types, micro-sized enterprises (26.90%, 25.29%), furniture manufacturing enterprises (31.01%, 31.28%) and domestic-funded enterprises (23.43%, 15.57%) had the highest detection rates of hypertension and abnormal ECG. The detection rates of hypertension and abnormal ECG were relatively high among male workers, those aged ≥60 years old and those with a Body Mass Index(BMI) ≥24.0 kg·m-2. The detection rates of hypertension and abnormal ECG in workers exposed to noise combined with other hazards such as heat stress (27.47%, 16.87%) and benzene (22.22%, 23.46%) were higher than those exposed to noise alone (21.40%, 11.60%) (P<0.01). Compared to workers in the general equipment manufacturing industry, those in the automotive manufacturing industry had a higher risk of hypertension (OR=1.284, 95%CI: 1.098‒1.501), and those in the furniture manufacturing industry had a higher risk of abnormal ECG (OR=2.615, 95%CI: 2.014‒3.385). Micro and small enterprises, BMI ≥24.0 kg·m-2 and advanced age had potential associations with hypertension and abnormal ECG. In addition, compared to the group exposed to noise alone, the group with noise and other physical hazards had a higher risk of hypertension (OR=1.146, 95%CI: 1.071‒1.288) and abnormal ECG (OR=1.179, 95%CI: 1.072‒1.297). Compared to the normal group, the risks of hypertension and abnormal ECG in workers with abnormal high-frequency hearing threshold for both ears increased by 21.5% (OR=1.215, 95%CI: 1.082‒1.364) and 21.0% (OR=1.210, 95%CI: 1.006‒1.410), respectively. ConclusionDifferent types of employers (economic type, industry category, enterprise size) and individual characteristics of workers (gender, age, length of service, BMI, exposure to other physical hazards, and abnormal high-frequency hearing threshold in both ears) impacted the association between noise exposure and hypertension/ECG abnormalities. Employers should enhance the management of occupational noise. In addition to hearing impairment, attention should be paid to abnormal changes in blood pressure and ECG of workers, and corresponding intervention measures should be implemented to reduce the occurrence and development of cardiovascular diseases among workers exposed to noise.
3.Research progress on the role of amino acids in glycolipid metabolism disorders

As metabolic regulators of cellular functions,amino acids play a crucial role in maintaining glucose homeostasis,protein synthesis,reproduction,immunity,and hormone secretion.In recent years,the role of amino acid metabolism in the occurrence and development of glycolipid metabolism disorders has become a research hotspot.Glycolipid metabolism disorders represent the common core pathological mechanism of metabolic diseases such as obesity and diabetes,and also serve as risk factors for cardiovascular diseases and malignancies.This review focuses on the role of various amino acids in glycolipid metabolism disorders,elaborates the specific mechanism of amino acids in regulating the body's metabolism,and analyzes their potential roles in disease prevention,diagnosis and treatment,in order to provide reference for further research on the application of amino acids in related fields.
4.Expert consensus on aeromedical identification of vasogenic white matter hyperintensities in aircrews

White matter hyperintensities(WMH)are abnormal high-signal manifestations in white matter areas on magnetic resonance T2-weighted or fluid-attenuated inversion recovery(FLAIR)sequences,with vasogenic WMH being the most common.Its pathological mechanism is related to cerebrovascular lesions and can lead to multi-dimensional brain functional impairments in cognition,movement,and emotions.Due to their unique working conditions,aircrews are more prone to vasogenic WMH than the general population.Moreover,with the widespread use of magnetic resonance imaging in cranial disease screening for aircrews,the detection rate of vasogenic WMH in aircrews has significantly increased.However,to date,no national or academic organization has issued standardized aviation medical assessment criteria for vasogenic WMH in aircrews.Therefore,led by Xijing Hospital of Air Force Medical University and the Chinese PLA General Hospital,a multi-disciplinary expert panel involved in aviation medicine,neurology,radiology,psychology,and related fields was assembled to formulate this consensus.The document integrates evidence from a systematic review of PubMed,CNKI,Wanfang Data,and VIP databases,incorporates existing WMH guidelines,and synthesizes clinical expertise from Chinese specialists.The consensus outlines three key domains:target population,neuroimaging protocols for vasogenic WMH screening,and clinical assessment and aeromedical identification for aircrews with vasogenic WMH.The aim is to provide evidence-based support and expert recommendations for standardizing and improving the accuracy of aviation medical evaluations of vasogenic WMH in aircrews.
5.Role and mechanism of RNF8 in regulating proliferation and migration of hepatocellular carcinoma

Aim To investigate the role of RNF8 in the proliferation,invasion and migration of hepatocellular carcinoma and in the promotion of epithelial-mesenchy-mal transition(EMT);to clarify the regulatory mecha-nism of RNF8 on hepatocellular carcinoma cells.Methods Immunohistochemistry was used to detect the expression of RNF8 and RhoA in human hepatocel-lular carcinoma tissues and adjacent tissues;Western blot and RT-PCR were used to detect the expression levels of RNF8 and RhoA in human normal hepatocytes and hepatocellular carcinoma cells.RNF8 was overex-pressed in HepG2 cells,and siRNA interference was used to downregulate the expression of RNF8.The cell experimental groups were as follows:control group(Control,normal HepG2 cells),RNF8 overexpression group,RNF8 low expression group(siRNA RNF8),RNF8 overexpression+Rhosin(20 μmol·L-1,RhoA blocker)group.The cell proliferation ability was detected by CCK-8 method;the cell migration ability was detected by scratch test;the cell invasion ability was detected by Transwell test;finally,the expression levels of RNF8,RhoA,PCNA,CyclinD1,N-cadherin,vimentin,Slug,and E-cadherin proteins and mRNA were detected by Western blot and RT-PCR.Results The expression of RNF8 and RhoA in liver cancer tissues and liver cancer cells significantly increased;after RNF8 knockdown,the proliferation,migration,in-vasion and EMT of liver cancer cells were significantly inhibited,while overexpression of RNF8 significantly increased the proliferation,migration,invasion ability of liver cancer cells and promoted EMT.RhoA showed a positive correlation with knockdown and overexpression of RNF8.When RNF8 was overexpressed and RhoA blocker was given at the same time,the phenomenon of overexpression of RNF8 increasing the proliferation,mi-gration,invasion ability and promoting EMT of liver cancer cells was significantly reversed.Conclusions RNF8 can promote the proliferation,migration,invasion and EMT of liver cancer cells,and at the same time promote the expression of RhoA.RNF8 promotes the progression of hepatocellular carcinoma by regulating RhoA to promote EMT.
6.Mitochondrial dysfunction in tendinopathy:possibility of mitochondria-targeting therapy

BACKGROUND:Various clinical strategies for the treatment of tendinopathy have good short-term effects but poor long-term effects,and some studies have proven that mitochondria are closely related to the occurrence and development of tendinopathy.However,the relationship between mitochondria and tendinopathy and mitochondria-targeting therapeutic strategies for tendinopathy have not been summarized so far,which is not good for specialists and scholars in related fields to understand the recent research situation.OBJECTIVE:To review the existing clinical or preclinical original studies,in order to summarize the relationship between mitochondrial dysfunction and tendinopathy and the mitochondria-targeting methods for the treatment of tendinopathy,and to provide certain prospects for the evaluation and management of mitochondria in tendinopathy in the future.METHODS:The relevant literatures in PubMed,Web of Science,CNKI,WanFang and VIP databases were searched.The search time was from January 2009 to March 2024,and the search terms were"tendinopathy,tendon injuries,tendon,tendons,mitochondria,mitochondria dysfunction,mitochondria disease"both in English and Chinese.According to the exclusion and inclusion criteria,62 articles were finally included for review and analysis.RESULTS AND CONCLUSION:(1)In clinical tendinopathy patients or tendinopathy models,mitochondrial dysfunction is common,mainly represented by excessive production of reactive oxygen species,decreased activity of superoxide dismutase,ridge clutter and decreased number of mitochondria,which indicates that mitochondrial dysfunction will occur due to tendon injury,thus further worsening tendinopathy and forming a vicious cycle.(2)When the tendon has not been injured or tendinopathy has not yet occurred,the mitochondrial function will be affected by various internal and external factors,resulting in tendinopathy.This indicates that the normal tendon will be damaged,lesioned or even ruptured due to the abnormal function of the mitochondria.(3)Mechanical tensile stress,advanced glycosylation end products,aging and other internal and external factors are the main causes of mitochondrial dysfunction,and these factors will damage and weaken the biological activity and mechanical properties of normal tendons through molecular mechanisms such as apoptosis,inflammation and respiratory chain damage,and thereby induce tendinopathy.(4)According to molecular mechanisms,mitochondria-targeting therapies mainly include mitochondrial transfer/transplantation,transplantation,targeted antioxidants,etc.(5)This review mainly aims at clinical patients with tendinopathy or animal models with similar modeling methods,providing a reliable idea for clinical exploration of the pathogenesis of tendinopathy and targeted therapies for tendinopathy.However,the disadvantage is that the included studies are mainly animal experiments,and there is a lack of more clinical trials for verification.
7.A comparative study of depression phenotype in a tumor-bearing mouse model of breast cancer

Objective Compare the depression phenotypes of a breast cancer tumor-bearing mouse model constructed using two different method and a mouse model of breast cancer depression with clinical manifestations,as well as assess their suitability for basic research.Methods We constructed a tumor model with 4T1 breast cancer cells alone(4T1 group)and a tumor-depression composite model given chronic unpredictable mild(CUMS)(4T1+CUMS group).The experimental period was 42 d,and the body mass,tumor volume,and survival time of the mice were monitored throughout the whole process.Two depressive behavioral tests(of sucrose preference test,open field test,tail suspension test,and elevated plus maze)were performed on the 15th and 29th days,respectively.Hematoxylin-eosin(HE)staining was used to observe the pathological changes of hippocampal neurons in brain tissue sections.Results(1)Body mass:The body mass of the 4T1 group and 4T1+CUMS group began to decrease from 29 d,and the body mass of the 4T1+CUMS group was significantly lower than that of the 4T1 group and Control group at the end of the experiment(P<0.001).(2)Tumor volume:There was no significant difference in the growth rate of tumors between the two model groups throughout the experiment(P>0.05).(3)Survival time:The survival rates of the 4T1 group and 4T1+CUMS group were 100%and 60%,and the first death of mice in the 4T1+CUMS group was on the 36th day.(4)Behavior test of depression:There was no significant difference between the three groups in the first depressive behavior tests(P>0.05),and the two groups showed obvious depressive phenotypes in the second behavioral tests.The sucrose preference index and activity distance in the central area were significantly decreased in the two model groups(P<0.001),and the immobility time was significantly increased(P<0.001).(5)Pathological section of brain tissue:On pathological examination of brain tissue,we observed a reduced number of neuronal cells in the hippocampus of the 4T1 group and 4T1+CUMS group,their morphology was irregular,the arrangement between the cells was disordered and the gap was unclear,and some nucleoli were blurred.Conclusions Although the tumor-only method and the tumor with compound stress stimulation method can both be used to prepare breast cancer depression models,the tumor-only modeling method is simpler and the mortality rate after successful modeling is higher.The long window of time is convenient for subsequent drug administration and detection,and the causes of the depression phenotype are more in line with the clinical causes and manifestations.Therefore,the 4T1 model can provide a reference model for future animal experiments on breast cancer tumor-related depression.
8.Effects of total flavonoids of Dracocephalum moldavica L.on ox-LDL-induced inflammatory response of RAW264.7 macrophages via NF-κB/NLRP3 signaling pathway

AIM To study the effects of total flavonoids of Dracocephalum Moldavica L.(TFDM)on reducing the inflammatory response of RAW264.7 macrophages induced by ox-LDL via the nuclear factor κB(NF-κB)/NOD-like receptor 3(NLRP3)signaling pathway.METHODS The RAW264.7 macrophages cultured in vitro were divided into the normal group,the model group(50 μg/mL ox-LDL),the TFDM group(100 μg/mL TFDM+50 μg/mL ox-LDL),the NF-κB inhibitor group(10 μmol/L Bay11-7821+50 μg/mL ox-LDL)and the TFDM+NF-κB inhibitor group(100 μg/mL TFDM+10 μmol/L Bay11-7821+50 μg/mL ox-LDL).The cells had their viability assessed by CCK-8 method;their ROS expression detected by the ROS kit;their mRNA expressions of NF-κB p65,NLRP3,Caspase-1,IL-18 and IL-1β detected by RT-qPCR;their protein expressions of NF-κB p65,IκBα,NLRP3,pro-Caspase-1,Caspase-1,IL-18 and IL-1β by Western blot;their protein expressions of NF-κB p65 and NLRP3 detected using immunofluorescence method.RESULTS Compared with the normal group,the model group showed increased ROS expression(P<0.01);increased mRNA expressions of NF-κB p65,NLRP3,Caspase-1,IL-18 and IL-1β(P<0.05,P<0.01);decreased protein expressions of IκBα and cytoplasmic NF-κB p65(P<0.01);increased protein expressions of nuclear NF-κB p65,NLRP3,Caspase-1,IL-1 β and IL-18(P<0.01);and increased fluorescence intensity of NF-κB p65 and NLRP3(P<0.01).Compared with the model group,the groups intervened with either TFDM or TFDM+inhibitor displayed decreased ROS expression(P<0.01);the groups administrated with TFDM or NF-κB inhibitor,or TFDM+inhibitor showed decreased mRNA expressions of NF-κB p65,NLRP3,Caspase-1,IL-18 and IL-1β(P<0.05,P<0.01),increased protein expressions of IκBα and cytoplasmic NF-κB p65(P<0.05,P<0.01),decreased protein expressions of nuclear NF-κB p65,NLRP3,Caspase-1,IL-1β and IL-18(P<0.05,P<0.01),and decreased fluorescence intensity of NF-κB p65 and NLRP3(P<0.01).There existed no significant group difference between the TFDM group and the NF-κB inhibitor group(P>0.05).The TFDM+inhibitor group demonstrated decreased mRNA expressions of IL-1βand IL-18(P<0.05),increased IκBα protein expression(P<0.05),decreased protein expressions of nuclear NF-κB p65,NLRP3,Caspase-1,IL-1 β and IL-18(P<0.05),and decreased fluorescence intensity of NLRP3 protein(P<0.05).CONCLUSION TFDM can inhibit the ox-LDL-induced inflammatory response of RAW264.7 macrophages,and the mechansism may be associated with the reduced ROS expression and inflammatory factors due to the inhibited activation of the NF-κB/NLRP3 signaling pathway.
9.Outcomes of transcatheter transseptal mitral valve-in-valve replacement using Edward's SAPIEN 3 in high surgical risk patients-a multicenter study in China

Objective To evaluate the safety and efficacy of valve-in-valve transcatheter mitral valve replacement(ViV-TMVR)in patients with bioprosthetic valve degeneration who are at high surgical risk.Methods This study is a multi-center,retrospective cohort analysis of 20 consecutive patients who underwent transseptal ViV-TMVR using the Edwards SAPIEN 3 transcatheter heart valve(THV).The primary endpoints include technical success and procedural success,both defined according to the Mitral Valve Academic Research Consortium(MVARC)criteria,as well as mortality and functional change assessed based on New York Heart Association(NYHA)classification at 30-days and six months post-procedure.Clinical follow-up assessments are conducted at 30-days and six months.Results From February 2021 to October 2022,a total of 20 patients with symptoms of bioprosthetic valve degeneration were enrolled across nine sites in China.The patients had a mean age of(73.5±5.5)years,with 85.0%being females and 70.0%classified as NYHA class Ⅲ/Ⅳ.The study achieved a 100.0%technical success rate and a 90.0%procedural success rate finally.All patients remained alive during the 30-day follow-up period.However,six months post-intervention,two patients(10.0%)were re-hospitalized due to heart failure,and sadly,one of them(5.0%)died.None of the patients reported any adverse events related to ViV-TMVR during the follow-up period.Notably,there was a significant improvement in NYHA class compared to baseline(P=0.0004)at six-month follow-ups.Conclusions The transseptal ViV-TMVR technique proved to be highly successful and was associated with significant improvement in NYHA class function.These findings strongly suggest that it serves as a safe and efficient treatment alternative for high-risk patients suffering from bioprosthetic valve degeneration.
