1.Pharmacodynamic Substances and Mechanisms of Xinglou Chengqi Tang in Treating Post-stroke Complications: A Review

Stroke is the leading cause of death and disability among adults in China, and its common complications include digestive system abnormalities, cognitive impairment, depression, stroke-associated pneumonia, and hemiplegia. The combination of traditional Chinese and Western medicine has great potential in treating post-stroke complications. Xinglou Chengqitang (XLCQT) is a representative prescription of alleviating the disease in the upper part by treating the lower part. It has definite therapeutic effect and high safety. Clinically, XLCQT is often used to treat stroke and its complications. However, the quantity and quality of clinical trials of XLCQT in treating post-stroke complications need to be improved. Additionally, since the basic research is weak, the material basis and multi-target mechanism for the efficacy of this prescription are unknown. This article reviews XLCQT in terms of the pharmacodynamic basis, medicinal properties, safety evaluation, and progress in clinical research and mechanisms in treating post-stroke complications. This article summarizes 22 key active ingredients of XLCQT in treating acute stroke complicated with syndrome of phlegm heat and fu-organ excess. Among these key active ingredients, resveratrol, kaempferol, luteolin, chrysoeriol, apigenin, (+)-catechin, and adenosine have good pharmacokinetic properties and high bioavailability. The mechanisms of XLCQT in treating post-stroke complications are complex, including inflammatory response, brain-gut axis, hypothalamic-pituitary-adrenal (HPA) axis, intestinal flora, neurotrophic factors, autophagy, oxidative stress, and free radical damage. This review helps to deeply understand the pharmacodynamic basis and mechanisms of XLCQT in treating post-stroke complications and provides a theoretical basis for the clinical application of XLCQT against post-stroke complications and the development of drugs.
2.Design and inflammation-targeting efficiency assessment of an engineered liposome-based nanomedicine delivery system targeting E-selectin.

OBJECTIVES:
To develop an E-selectin-targeting nanomedicine delivery system that competitively inhibits E-selectin-neutrophil ligand binding to block neutrophil adhesion to vessels and suppress their recruitment to the lesion sites.
METHODS:
Doxorubicin hydrochloride (DOX)-loaded liposomes (IEL-Lip/DOX) conjugated with E-selectin-affinity peptide IELLQARC were developed using a post-insertion method. Two formulations [2-1P: Mol(PC): Mol(DPI)=100:1; 2-3P: 100:3] were prepared and their modification density and in vitro release characteristics were determined. Their targeting efficacy was assessed in a cell model of LPS-induced inflammation, a mouse model of acute lung injury (ALI), a rat femoral artery model of physical injury-induced inflammation, and a zebrafish model of local inflammation.
RESULTS:
The prepared IEL-Lip/DOX 2-1P and 2-3P had peptide modification densities of 4.76 and 7.57 pmoL/cm2, respectively. Compared with unmodified liposomes, IEL-Lip/DOX exhibited significantly reduced 48-h cumulative release rates at pH 5.5. In the inflammation cell model, IEL-Lip/DOX showed increased uptake by activated inflammatory endothelial cells, and 2-1P exhibited a higher trans-endothelial ability. In ALI mice, the fluorescence intensity of IEL-Lip/Cy5.5 increased significantly in lung tissues by 53.71% [Z-(2-1P)] and 93.41% [Z-(2-3P)], and 2-1P had an increased distribution by 24.19% in the inflammatory lung tissue compared to normal mouse lung tissue. In rat femoral artery models, 2-1P had greater injured/normal vessel fluorescence intensity contrast. In the zebrafish models, both 2-1P and 2-3P showed increased aggregation at the site of inflammation.
CONCLUSIONS
This E-selectin-targeting nanomedicine delivery system efficiently targets activated inflammatory endothelial cells to increase drug concentration at the inflammatory site, which sheds light on new strategies for treating neutrophil-mediated inflammatory diseases and practicing the concept of "one drug for multiple diseases".

3.Research Progress in TCM Targeted Regulating Macrophages for the Treatment of Ulcerative Colitis

Ulcerative colitis(UC)is an inflammatory disease characterized by a continuous diffuse lesion in the colorectal mucosa.Macrophages,as key players in the inflammatory microenvironment,are significantly correlated with the development of UC.Based on the TCM pathogenesis of UC,this article reviewed the current research status of targeted regulation of macrophages in the treatment of UC using TCM,summarized the biological processes related to macrophages and their role in the progression of UC,and concluded the molecular mechanism of TCM treatment of UC targeting macrophages.It found that the active components of Chinese materia medica,TCM compounds,and TCM non-pharmacological therapies mainly regulate macrophage activity by regulating macrophage polarization,autophagy,pyroptosis,metabolic reprogramming,oxidative stress,and other processes,thereby delaying the progression of UC.This article aimed to provide references for in-depth exploration of the pathogenesis of UC and the TCM prevention and treatment of UC.
4.Observation on the efficacy of Huoluo Xiaotong capsules in treating cervical spondylosis radiculopathy:a real-world study

Objective To compare the early efficacy and safety of Huoluo Xiaotong capsules and Jingtong granules in treating cervical spondylosis radiculopathy(CSR).Methods A real-world study was conducted among CSR patients who visited three tertiary-grade-A hospitals in Changsha between January 2024 and February 2025.Patients were divided into a control group(Jingtong granules,4 g,po,tid)and an observation group(Huoluo Xiaotong capsules,1.4 g,po,tid);both groups were treated for 2 weeks.Baseline characteristics were balanced with inverse probability of treatment weighting(IPTW).Numeric rating scale(NRS)scores,overall response rates,and traditional Chinese medicine(TCM)symptom scores were compared at baseline,1 week,and 2 week.Results A total of 120 patients were ultimately included,with 30 in the control group and 90 in the observation group.After IPTW adjustment,at 1 week of treatment,the clinical efficacy rate of observation group(98.85%)were significantly exceeded that of control group(80.14%)(P<0.001);at 2 weeks,the clinical efficacy rate of the two groups were both 100.00%.NRS and TCM syndrome scores showed no intergroup differences at baseline or 1 week(P>0.05),at 2 weeks,the observation group demonstrated significantly lower NRS and TCM syndrome scores versus controls(P<0.001).No adverse reactions occurred in either group.Conclusion Huoluo Xiaotong capsules demonstrate favorable short-term efficacy and safety in CSR,significantly alleviating pain and improving TCM symptoms within 2 weeks.High-quality randomized controlled trials are warranted to confirm these findings.
5.Application of"trinity teaching"model in training for medical technology skills competition for college students

Modern education emphasizes the improvement of the comprehensive quality of the educated,and medical ed-ucation has particularly high requirements for students'practical abilities.Therefore,it is necessary to continuously analyze and optimize the education work when conditions permit.This paper first introduces the"Trinity Teaching"model and the college student medical technology skills competition.Based on this,it analyzes the advantages and disadvantages of the current training for college student medical technology skills competition,the value of applying the"Trinity Teaching"model,and explores its specific application methods,including the use of group work mode and building a large-scale teaching service system to serve medical teaching work.

AIM:To investigate the antiviral effica-cy and mechanism of Qingre Xiaoyanning(QRXYN)in vivo,and provide experimental basis for their prevention and treatment of influenza A virus.METHODS:We constructed a mouse model infect-ed with influenza A H3N2 virus.To evaluate the therapeutic effect of QRXYN on influenza A virus,we measured the body weight changes,pathologi-cal changes in lung tissue,hemagglutination titer,and viral load in mouse.To evaluate the possible mechanism of QRXYN's anti influenza A virus infec-tion,we used the ELISA to measure the levels of TNF-α,IL-1β,IL-4,IFN-γ,and vascular cell adhesion molecule-1(VCAM-1)in mouse bronchoalveolar Ia-vage fluid;used flow cytometry to assess the pro-portions of macrophages(F4/80),helper T lympho-cytes(CD4+T lymphocytes),and natural killer(NK)cells in lung tissue;and used Western blotting to detect the expression of Toll-like receptor 4(TLR4),myeloid differentiation factor 88(MYD88),inhibitor of kappa B kinase-β(IKK-β),NF-kappa-B inhibitor al-pha(IκBα),and phospho-IKB alpha(p-IκBα)in lung tissue.RESULTS:Compared to the model group,both Oseltamivir and QRXYN can alleviate the se-verity of lung tissue lesions in mice,decrease the blood coagulation titer and viral load of mouse lung tissue(P<0.01),lower the levels of TNF-α,IL-4,and VCAM-1 in bronchoalveolar lavage fluid(P<0.05,P<0.01),reduce the proportion of macro-phages(P<0.05,P<0.01),and increase the propor-tion of CD4+T lymphocytes and NK cells(P<0.05,P<0.01).Additionally,oseltamivir can reduce the ex-pression of MYD88 protein in mouse lungs(P<0.05),while QRXYN can decrease the expression of IKK-β and P-IκBα proteins in mouse lungs(P<0.05).CONCLUSION:QRXYN have good in vivo antiviral ef-fects against the influenza A virus,and their mecha-nism may be related to the regulation of the immu-nologic function and NF-κB signal pathway.
