1.Application and analysis of compatible platelets matching via antigen avoidance

Objective: To investigate the application of antigen avoidance pattern for compatible platelets matching. Methods: Samples from two patients with immune-mediated platelet transfusion refractoriness were screened for platelet antibodies using solid-phase red blood cell adhesion assay (SPRCA). The genotypes of HLA-A, -B loci were determined via ploymerase chain reaction sequence. The specificity of HLA class I antibodies was detected using Luminex technology. Results: Platelet antibody screening via SPRCA yielded positive results in both samples. Antibody specificity testing showed the presence of antibodies against HLA-B65, A80, B13, as well as antibodies against HLA-A11, B52, A24 respectively, with both patients exhibiting 80 kinds of positive antibodies. The antibody avoidance pattern successfully selected compatible platelets for transfusion. The bleeding symptoms of two patients were improved after compatible platelets transfusion. Conclusion: For blood stations with limited platelet gene bank resources, antibody avoidance pattern for compatible platelets matching represents an effective strategy for immune-mediated platelet transfusion refractoriness.
2.Improvement effect and mechanism of desloratadine citrate disodium in hypersensitivity pneumonitis model mice

OBJECTIVE To investigate the improvement effect and mechanism of desloratadine citrate disodium in mice with hypersensitivity pneumonitis (HP). METHODS Sixty mice were randomly divided into blank control group (normal saline), model group (normal saline), prednisone group (positive control, 20 mg/kg) and desloratadine citrate disodium low-, medium- and high-dose groups (0.5, 1, 2 mg/kg), with 10 mice in each group. Except for the blank control group, mice in other groups were intraperitoneally injected with ovalbumin (OVA) and exposed to OVA inhalation to establish the HP model. On day 22 post- modeling, mice in each group were administered the corresponding drugs or normal saline, once a day, for 11 consecutive days. After the last administration, lung function and airway hyperreactivity were assessed. The levels of interleukin-1β (IL-1β), IL-4 and IL-6 in serum as well as the levels of IL-8, IL-13 and IL-17A in bronchoalveolar lavage fluid were determined. Pathological changes in lung tissue of mice were evaluated using Masson staining. Furthermore, the expressions of fibrosis-related proteins, including transforming growth factor β1 (TGF-β1), type Ⅲ collagen (Col-Ⅲ) and fibronectin (FN) were determined in lung tissues. RESULTS Compared with the blank control group, the model group showed significant deterioration in lung function (P< 0.01), while airway resistance and serum levels of IL-1β, IL-4, IL-6 and the levels of IL-8, IL-13 and IL-17A in the bronchoalveolar lavage fluid were increased significantly (P<0.01). The lung tissues exhibited alveolar collapse, atrophy, and structural disarray, along with the formation of extensive deposits of blue collagen fibers, the percentage of positive staining increased significantly (P<0.01). Additionally, the expression levels of TGF-β1, Col-Ⅲ, and FN proteins in the lung tissues were also increased significantly (P<0.01). After intervention with desloratadine citrate disodium, the pathological changes in the lung tissues of mice in each dosage group of desloratadine citrate disodium showed varying degrees of improvement, and most of the aforementioned indicator levels were significantly reversed (P<0.05 or P<0.01). CONCLUSIONS Desloratadine citrate disodium can improve the lung function and airway hyperreactivity of HP mice, inhibit the release of inflammatory factors in serum and bronchoalveolar lavage fluid, and reduce the deposition of collagen fibers. Its mechanism of action may be related to anti-inflammatory, immunomodulatory, and antifibrotic effects.
3.Research progress on the fluorescence resonance energy transfer-based polymer micelles as drug carriers.

Polymer micelles formed by self-assembly of amphiphilic polymers are widely used in drug delivery, gene delivery and biosensors, due to their special hydrophobic core/hydrophilic shell structure and nanoscale. However, the structural stability of polymer micelles can be affected strongly by environmental factors, such as temperature, pH, shear force in the blood and interaction with non-target cells, leading to degradations and drug leakage as drug carriers. Therefore, researches on the structural integrity and in vivo distribution of micelle-based carriers are very important for evaluating their therapeutic effect and clinical feasibility. At present, fluorescence resonance energy transfer (FRET) technology has been widely used in real-time monitoring of aggregation, dissociation and distribution of polymer micelles ( in vitro and in vivo). In this review, the polymer micelles, characteristics of FRET technology, structure and properties of the FRET-polymer micelles are briefly introduced. Then, methods and mechanism for combinations of several commonly used fluorescent probes into polymer micelles structures, and progresses on the stability and distribution studies of FRET-polymer micelles ( in vitro and in vivo) as drug carriers are reviewed, and current challenges of FRET technology and future directions are discussed.

4.Penetrance estimation of PRRT2 variants in paroxysmal kinesigenic dyskinesia and infantile convulsions.

Proline-rich transmembrane protein 2 (PRRT2) is the leading cause of paroxysmal kinesigenic dyskinesia (PKD), benign familial infantile epilepsy (BFIE), and infantile convulsions with choreoathetosis (ICCA). Reduced penetrance of PRRT2 has been observed in previous studies, whereas the exact penetrance has not been evaluated well. The objective of this study was to estimate the penetrance of PRRT2 and determine its influencing factors. We screened 222 PKD index patients and their available relatives, identified 39 families with pathogenic or likely pathogenic (P/LP) PRRT2 variants via Sanger sequencing, and obtained 184 PKD/BFIE/ICCA families with P/LP PRRT2 variants from the literature. Penetrance was estimated as the proportion of affected variant carriers. PRRT2 penetrance estimate was 77.6% (95% confidence interval (CI) 74.5%-80.7%) in relatives and 74.5% (95% CI 70.2%-78.8%) in obligate carriers. In addition, we first observed that penetrance was higher in truncated than in non-truncated variants (75.8% versus 50.0%, P = 0.01), higher in Asian than in Caucasian carriers (81.5% versus 68.5%, P = 0.004), and exhibited no difference in gender or parental transmission. Our results are meaningful for genetic counseling, implying that approximately three-quarters of PRRT2 variant carriers will develop PRRT2-related disorders, with patients from Asia or carrying truncated variants at a higher risk.

5.The role of high-flow oxygen insufflation via nasal cannula in patients at high risk of re-intubation after weaning from mechanical ventilation assessed by lung ultrasound score

Objective To study the effect of high-flow oxygen insuffiation (HFOI) via nasal cannula in reduction in re-intubation rate,length of ICU stay and improvement of respiratory function in patients at high risk of re-intubation after weaning from mechanical ventilation assessed by lung ultrasound score (LUS).Methods Single center randomized(random number) clinical trial was carried out in one intensive care units in China from May 2016 to May 2017 including critically ill patients ready for planned extubation with high-risk factors for re-intubation assessed by LUS when the LUS ≥ 14 was considered to be high risk.The comparisons of the length of ICU stay,re-intubation rate in case of respiratory failure,respiratory rate pulse rate SaO2 PaO2/FiO2 of patients at 6 h,24 h and 48 hours after extubation were made between HFOI and conventional oxygen therapy (COT) group.Results During the study period,32 patients were enrolled in the study.Of them,15 were assigned in HFOI group and 17 in COT group.The length of ICU stay (8.0±2.4)days vs.(10.9±3.5) days and re-intubation rate (6.7% vs.23.5%) were significantly different between two groups (P<0.05).The respiratory rate pulse rate SaO2 and PaO2/FiO2 of patients at 6 h after ex-tubation in HFOI group were improved than those in COT group (P<0.05);and the SaO2 and PaO2/FiO2 of patients 24 h and 48 h after ex-tubation in HFOI group had much more improvement than those in COT group (P<0.05).Conclusion Among high-risk adults who assessed by lung ultrasound score,high-flow oxygen therapy could reduce re-intubation rate length of ICU stay and improve the respiratory function.High-flow oxygen therapy may offer advantages for these patients.
6.Investigation and analysis of clinical application of washed red blood cells

Objective To investigate and analyze the clinical application of washed red blood cells in Quzhou in 2014-2015 years,and to provide evidence for the rational and effective use of washed red blood cells in clinical practice.Methods the amount of washed red blood cells,the distribution of diseases and the serological detection before transfusion were analyzed retrospectively in 8 hospitals of grade two and over in Quzhou during the past 2014-2015 years.Results the amount of washed red blood cells in 2015 increased by 39.07% compared with 2014,an increase of 24.4 times the amount of red blood cells increase,increase mainly in 3 hospitals;Diseases of the blood system,malignant tumors and chronic kidney disease is the main diagnosis of transfusion cases,accounted for 63.49%,19.05%,12.70%.There were different standards for the development of serological testing items before transfusion Conclusion the advantages of washed red blood ceils gradually recognized by clinicians,but also don't rely on experience,hospitals should pay attention to the comprehensive evaluation of clinical blood transfusion and standardize the serological detection of blood transfusion department,which is the key to improve the cost performance of this component.
7.Effects of five-tone therapy on preoperative psychological reactions of patients undergoing elective surgery

ObjectiveTo explore the effects of five-tone therapy on preoperative psychological reactions of patients undergoing elective surgery.Methods Totally 120 patients were enrolled in our hospital for elective surgery. They were randomly and equally divided into a control group and an observation group according to the random number table. Patients in the control group was treated with conventional nursing intervention,while patients in the observation group received five-tone surgery on the basis of conventional nursing intervention. Their preoperative anxiety was assessed with Zung′s self-rating anxiety scale(SAS),and their systolic blood pressure (SBP),diastolic blood pressure(DBP) and heart rate(HR) at admission(T1),in postoperative conversation(T2),in the morning on the day of surgery(T3) and 15 minutes after the surgery(T4) were monitored.Results After the intervention,the SAS scores at T2 and T3 of the patients in the observation were(42.2±10.5) and(43.2±10.8), respectively,lower than those of the patients in the control group(t=2.118,2.801;P<0.05). The SBP,DBP and HR at T4 of the patients in the observation group were(122.3±12.5) mmHg,(74.1±8.9) mmHg,and(72.5±7.1) times/min,respectively,which were significantly lower than those of the patients in the control group(t=2.695,2.440, 4.40;P<0.05).Conclusions Five-tone therapy can effectively alleviate the preoperative psychological reactions of patients undergoing elective surgery and stabilize their vital signs.
8.Four types of surgery for the treatment of adenoid hypertrophy

Objective To explore the effection of four types of surgery for the treatment of adenoid hypertrophy . Methods 148 cases of with adenoid hypertrophy treated in our hospital between April 2012 and April 2015 were chose;they were randomly divided into 4 groups ,each group of 37 people .A group of patients with adenoid hypertrophy were taken traditional ade‐noidectomy curettage;Group B with nasal endoscopic adenoidectomy and cutting aspiration biopsy ;Group C by adenoidectomy shave their + residual endoscopic adenoidectomy bit cut method combined treatment ;Group D with nasal endoscopic adenoidectomy plas‐ma cutting treatment .The curative effect ,operation time ,blood loss were observed ;patients were followed‐up for half a year ,ade‐noidectomy residual rate and complications of each group were compared .Results The total effective rate of B ,C ,D three groups were significantly higher in group A patients (χ2 =7 .731 ,5 .045 ,7 .731 ,P<0 .05) ,the efficient between three groups was not sta‐tistically different (P>0 .05) .B ,C ,D three groups of operation time is significantly higher than A group of patients (t=5 .819 , 5 .829 ,2 .759 ,P<0 .05);B and C group had long operation time than group D (t=3 .555 ,3 .637 ,P<0 .05);But operation time of B and C had no significant difference between the two groups (t=0 .149 ,P>0 .149) .Bleeding of B and C group were significantly higher than group A (t=9 .305 ,4 .126 ,P<0 .05);Group D was significantly lower than A ,B ,C three group (t=8 .054 ,16 .559 , 12 .837 ,P<0 .05);Group C and group B was significantly higher than the bleeding (t=5 .739 ,P<0 .05) .Retention rate of group A is significantly higher than the other three groups (χ2 =31 .308 ,31 .308 ,24 .667 ,P<0 .05) ,the residual rate of B ,C ,B group were lower ,there was no statistically significant difference(P> 0 .05) .Complication rates between the four groups was no statistical difference(P>0 .05) .Conclusion we should choose the right means of surgical treatment according to patients condition and eco‐nomic situation to .

Cerebral ischemia is one of the most common diseases treated by acupuncture therapeutics. Recent studies indicated that acupuncture treatment by needling the conception and governor vessels had positive effects in promoting neural regeneration in patients after cerebral ischemia injury. Acupuncture intervention could continuously promote the proliferation and differentiation of the neural stem cells in the brain, obviously up-regulate expression of growth factors, accelerate angiogenesis and inhibit apoptosis. Hence, it is necessary to present an exhaustive review on the mechanisms. The present review gives a detailed description of pathological changes of cerebral ischemia and acupuncture intervention applied to the conception and governor vessels, and proposes research prospects in the future.
10.Construction of tissue-engineered skin using basic fibroblast growth factor gene transfected bone marrow mesenchymal stem cells combined with small intestinal submucosa

BACKGROUND: Studies demonstrated that small intestinal submucosa (SIS) had no immunogenicity, which can not lead to rejection following transplantation, thus, this is an ideal skin substitutes for natural skin.OBJECTIVE: Basic fibroblast growth factor (bFGF) gene was transfected into bone marrow mesenchymal stem cells (BMSCs),and combined the cells with SIS to construct tissue-engineered skin.METHODS: BMSCs were obtained from Japanese big-earad rabbits, and in vitro cultured. Then the subculturad BMSCs were transfected by pCDNA3.1 plasmid, followed by incubation on swine SIS to construct the tissue-engineered skin. The growth of cells and phenotype of BMSCs were detected by flow cytometry. In addition, the result of transfecting BMSCs with pCDNA-bFGF vector was measured by Western blot, and the structure of tissue-engineered skin was observed.RESULTS AND CONCLUSION: After passaged, BMSCs were grown quickly with Iong-fusiform shape. The cells were positive expressed CD90 and CD44, but negative expressed CD45. bFGF had been transfected into BMSCs, and stable expressed. The transfected BMSCs grew well in SIS. By this method, tissue-engineered skin can be constructed in vitro.
