1.Effects of metronidazole and vulvar cleaning with 0.1% benzalkonium bromide solution on vaginal environment, etiology, and interleukin-6 and Toll-like receptor 2 levels in lavage fluid of patients with bacterial vaginosis

Objective:To investigate the effects of metronidazole and vulvar cleaning with 0.1% benzalkonium bromide solution on vaginal environment, etiology, and interleukin-6 and Toll-like receptor 2 levels in lavage fluid of patients with bacterial vaginosis.Methods:A total of 114 patients with bacterial vaginosis who were admitted to Jinan 2 nd People's Hospital from February 2021 to February 2024 were included in this prospective study. These patients were randomly divided into a control group and an observation group, with 57 patients in each group, following a 1:1 ratio. The control group received treatment with metronidazole, while the observation group received metronidazole in combination with vulvar cleaning using a 0.1% benzalkonium bromide solution, for a total of 14 days. The clinical efficacy was evaluated by comparing the vaginal environment (including vaginal pH, cleanliness, and microbial density), microbiological outcomes (lactobacilli detection rate and pathogenic bacteria detection rate), and interleukin-6 and Toll-like receptor 2 levels in the vaginal lavage fluid between the two groups before and after treatment. Adverse drug reactions were recorded. Patients were followed up for 3 months after treatment to record recurrence rates. Results:The total response rate in the observation group was significantly higher than that in the control group [92.98% (53/57) vs. 78.95% (45/57), χ2 = 4.65 P < 0.05]. After treatment, improvements of vaginal pH, vaginal cleanliness, and bacterial density were greater in the observation group than those in the control group ( χ2 = 4.61, Z = 4.92, 4.29, all P < 0.05). After treatment, the detection rate of lactobacilli in the observation group was significantly higher than that in the control group [63.16% (36/57) vs. 42.11% (24/57), χ2 = 5.07, P < 0.05]. The detection rate of pathogenic bacteria in the observation group was significantly lower than that in the control group [5.26% (3/57) vs. 17.54% (10/57), χ2 = 4.25, P < 0.05]. After treatment, lavage fluid levels of interleukin-6 [(8.70 ± 2.13) ng/L vs. (13.52 ± 3.36) ng/L] and Toll-like receptor 2 [(5.06 ± 1.34) ng/L vs. (7.84 ± 1.22) ng/L] in the observation group were significantly lower than those in the control group ( t = 9.15, 11.58, both P < 0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups ( χ2 = 1.37, P > 0.05). After 3 months of treatment, the recurrence rate of bacterial vaginosis in the observation group was significantly lower than that in the control group [4.55% (1/22) vs. 27.78% (5/18), χ2 = 4.19, P < 0.05]. Conclusions:The combination of metronidazole and vulvar cleaning with a 0.1% benzalkonium chloride solution is effective for treating bacterial vaginosis. This approach helps regulate the vaginal environment, increases the levels of lactobacilli, reduces the levels of pathogenic bacteria, and decreases the levels of interleukin-6 and Toll-like receptor 2 in the lavage fluid. Additionally, it decreases the risk of recurrence of bacterial vaginosis and is highly safe.

Objective:To investigate the phenotypes of Rubinstein-Taybi syndrome (RSTS) caused by variants in the CREBBP or EP300 gene, and the correlation between genotype and phenotype.Methods:This case series study was performed on pediatric patients who were referred to the Children′s Hospital of Capital Institute of Pediatrics between January 2013 and July 2022. Both point variant and copy number deletion in CREBBP or EP300 gene were detected by whole exome sequencing, chromosomal microarray analysis, or copy number variation sequencing (CNV-seq). The variant categories were summarized and phenotype numbers were re-visited for RSTS patients. Based on variant types, the patients were divided into different groups (point variant or copy number deletion, EP300 or CREBBP point variant, and loss of function or missense variant). Phenotype counts between different groups were compared using the rank-sum test of two independent samples.Results:A total of 21 RSTS patients were recruited, including 12 males and 9 females, with ages ranging from 1 month to 14 years and 2 months. Among them, 67% (14/21) had point variants, and 33% (7/21) had copy number deletions. Out of these, 20 variants (95%) were de novo. Among 20 patients finishing phenotype count during re-visit, 95% (19/20) of the patients exhibited developmental delays before the age of 2 years. Additionally, 80% (16/20) of the patients had distinctive facial features. Considering phenotype count, no statistically significant difference was found between point variant (14 cases) and copy number deletion (6 cases) (5.0 (3.0, 7.0) vs. 5.0 (2.5, 5.3), Z=0.75, P=0.452), CREBBP (10 cases) and EP300 gene (4 cases) point variant (5.0 (3.8, 7.0) vs. 4.0 (2.0, 6.0), Z=1.14, P=0.253), and loss of function (9 cases) and missense (5 cases) variant (6.0 (4.5, 7.0) vs. 3.0 (2.5, 5.5), Z=1.54, P=0.121). Conclusions:Patients with RSTS primarily exhibit developmental delays in early childhood. Specific facial features serve as suggested signs of genetic testing. However, no significant genotype-phenotype correlation is found.
6.Exploring the Effect of Ganmai Dazhao Tang on Intestinal Inflammation in ADHD Model Rats Based on Network Pharmacology

Objective Based on network pharmacology,protein mechanism,and experimental validation,this study aims to explore the role of Ganmai Dazao Tang in treating intestinal inflammation in children with attention deficit hyperactivity disorder(ADHD)model rats.Methods Screening the relevant targets and pathways for the effect of Ganmai Dazhao Tang on intestinal inflammation in ADHD model rats using network pharmacology methods,and verifying the affinity of the main components and targets through molecular docking.Forty spontaneously hypertensive rats(SHR)were randomly divided into model group,methylphenidate group,and low,medium,and high dose groups of Ganmai Dazao Tang,with 8 rats in each group.Additionally,8 Wistar Kyoto rats(WKY)were set as the normal group.Rats in each group were treated daily by gavage for 3 weeks.Observing the pathological condition of the small intestine using HE staining technology,and detecting interleukin-1 using enzyme-linked immunosorbent assay(ELISA)and Western blot methods β(IL-1β),tumor necrosis factor α(TNF-α),Expression of the oncogene Fos(FOS).Results Network pharmacology screened 117 chemical components and predicted 248 effective targets.Molecular docking results showed that Isovitexin β-Ingredients such as beta carotene,kaempferol,luteolin,and naringenin have strong affinity with targets such as AKT1,FOS,VEGFA,and TNF.Animal test results show that Ganmai Dazhao Tang can improve the core symptoms of ADHD,protect normal intestinal function and status,and downregulate related IL-1 β,TNF-α,FOS factor content.Conclusion Ganmai Dazao Tang can alleviate the core symptoms of ADHD in SHR rats by protecting normal intestinal function and downregulating inflammatory factors.

8.Follow-up study on newly diagnosed occupational pneumoconiosis patients in Guizhou Province, 2006-2021

Objective To analyze the survival status, distribution characteristics and social security of newly diagnosed occupational pneumoconiosis patients in Guizhou Province from 2006 to 2021. Methods The newly diagnosed pneumoconiosis cases reported in Guizhou Province from 2006 to 2021 were collected from the “Occupational Diseases and Hazards Monitoring Information System” under “National Health Insurance Disease Prevention and Control Information System”. Telephone or face-to-face surveys were conducted to investigate these patients. Results A total of 12 413 newly diagnosed pneumoconiosis patients were reported in Guizhou Province from 2006 to 2021, with 11 192 cases included in the follow-up, and 10 631 cases were followed-up successfully. According to the follow-up study, 10 565 cases (accounting for 99.4%) were survived, and 66 cases (accounting for 0.6%) died. The number of pneumoconiosis showed an increasing trend followed by a sharp decline from 2006 to 2021, reaching its peak in 2016. The main industry of the newly pneumoconiosis cases was concentrated in the mining industry (accounting for 90.6%). The top three regions with surviving cases in the follow-up were Bijie City, Zunyi City, and Qiannan Prefecture, accounting for 34.0%, 25.4% and 12.0%, respectively. The main types of pneumoconiosis were coal workers' pneumoconiosis and silicosis, accounting for 67.1% and 30.8%, respectively. Most cases were in the age group of 50 to <60 years old, accounting for 59.1%, and the majority of the workers had worked in dusty environments for 5 to <25 years, accounting for 91.5%. In terms of social security, about 89.5% of cases were in the basic medical insurance for urban and rural residents which was the most popular social security. The employer's compensation rate was 67.7%, and the work-related injury insurance participation rate was 51.5%. Conclusion Pneumoconiosis cases in Guizhou Province exhibit significant regional disparities. It has a high concentration in the industry, a younger age profile, and limited social security coverage. It is necessary to strengthen the special management of dust and industry supervision in mining industry, intensify follow-up work for pneumoconiosis cases, reinforce the construction of pneumoconiosis rehabilitation station in key regions, and improve the quality of life of pneumoconiosis cases.
9.Analyzing the occupational health literacy and its influencing factors among occupational populations in Guizhou Province, 2022

Objective To analyze the awareness of occupational health literacy (OHL) and its influencing factors among occupational population in key industries of Guizhou Province in 2022. Methods A total of 11 153 workers from eight key industries in 88 counties and districts of Guizhou Province was selected as the research subjects in 2022 using a stratified cluster sampling method. The OHL levels were surveyed using the Occupational Health Literacy Questionnaire of National Key Populations. Results The overall OHL level of the research subjects in 2022 was 57.7%. The OHL levels of basic knowledge of occupational health protection, healthy lifestyle and behavior, legal knowledge of occupational health, and basic skills of occupational health protection were 79.6%, 69.7%, 60.0%, and 42.0%, respectively. The overall OHL level of the tertiary industry population was higher than that of the secondary industry (59.8% vs 54.9%, P<0.01). The overall OHL levels of occupational population in the eight key industries from high to low were medical and health, electric heating water supply, environmental health, transportation, non-metallic mineral products, express delivery/take-out,education and coal mining, with the overall OHL level of 82.5%, 64.2%, 64.0%, 55.9%, 52.8%, 48.8%, 46.1% and 44.7%, respectively (P<0.01). The results of logistic regression analysis showed that gender, age, ethnicity, marital status, educational level, personal monthly income, length of employment, and industry category were independent influencing factors of OHL levels (all P<0.05). The OHL level of females was higher than that of males (P<0.01). The older age, higher educational level, and higher personal monthly income were associated with higher OHL levels in workers (all P<0.01). The OHL level of Han population was higher than that of Miao and other ethnic groups (all P<0.01). The OHL level of unmarried population was higher than that of married and widowed/divorced population (all P<0.01). The OHL level of workers with less than 3.0 years of employment was lower than that of workers with 3.0 - < 6.0 and 6.0 - < 10.0 years of employment (all P<0.01). The OHL level of workers in the tertiary industry was higher than that in the secondary industry (P<0.01). Conclusion The OHL level of occupational population in Guizhou Province needs to be further improved. Special attention should be paid to the industry of coal mining and selection, education, express delivery/take-out, and occupational population in ethnic minority areas, low educated, low-income, and newly employed occupational population.
10.The chemical reprogramming of unipotent adult germ cells towards authentic pluripotency and de novo establishment of imprinting.

Although somatic cells can be reprogrammed to pluripotent stem cells (PSCs) with pure chemicals, authentic pluripotency of chemically induced pluripotent stem cells (CiPSCs) has never been achieved through tetraploid complementation assay. Spontaneous reprogramming of spermatogonial stem cells (SSCs) was another non-transgenic way to obtain PSCs, but this process lacks mechanistic explanation. Here, we reconstructed the trajectory of mouse SSC reprogramming and developed a five-chemical combination, boosting the reprogramming efficiency by nearly 80- to 100-folds. More importantly, chemical induced germline-derived PSCs (5C-gPSCs), but not gPSCs and chemical induced pluripotent stem cells, had authentic pluripotency, as determined by tetraploid complementation. Mechanistically, SSCs traversed through an inverted pathway of in vivo germ cell development, exhibiting the expression signatures and DNA methylation dynamics from spermatogonia to primordial germ cells and further to epiblasts. Besides, SSC-specific imprinting control regions switched from biallelic methylated states to monoallelic methylated states by imprinting demethylation and then re-methylation on one of the two alleles in 5C-gPSCs, which was apparently distinct with the imprinting reprogramming in vivo as DNA methylation simultaneously occurred on both alleles. Our work sheds light on the unique regulatory network underpinning SSC reprogramming, providing insights to understand generic mechanisms for cell-fate decision and epigenetic-related disorders in regenerative medicine.
