1.Treatment of Idiopathic Olfactory Disorders from the Perspective of Heart and Lungs

To summarize the clinical experience of treating idiopathic olfactory disorder from the perspective of heart and lungs. It is believed that the sense of olfaction is based on the nose, rooted in the heart, functioning through the lungs, and conveyed by pectoral qi. The key pathogenesis of idiopathic olfactory disorder lies in the accumulation of pathogenic factors in the heart and lungs, blood vessel obstruction, the failure of the lungs to disperse and descend, and the loss of control of pectoral qi. In treatment, internal and external therapies could be combined. The internal therapy can correct the imbalance of the zang-fu organs, using the self-prescribed Bilong Formula (鼻聋方) to dispel wind and diffuse the lung, invigorate blood and relieving stuffy orifices; the external therapy can clear nasal obstruction, supplemented by intradermal needle embedding at three acupoints, bilateral Yingxiang (LI 20), bilateral Shangyingxiang (EX-HN 8) and Yintang (GV 29), and integrated olfactory training for comprehensive treatment.
2.A VBM study on gray matter structure alterations in patients with Alzheimer’s disease comorbid with apathy

ObjectiveTo investigate the characteristics of gray matter structure and clinical symptoms in patients with Alzheimer's disease (AD) comorbid with apathy (AD-A). MethodsThe study included 30 patients with AD-A, 30 AD disease patients without apathy (AD without apathy, AD-NA), and 30 healthy controls (HCs) matched in gender, age, and years of education. All participants underwent a comprehensive neuropsychological assessment and magnetic resonance imaging (MRI) scans. Voxel-based morphometry (VBM) was used to analyze changes in gray matter volume among the three groups. Additionally, the correlation between the identified abnormal brain regions and apathy scale scores was analyzed. ResultsThere were no statistically significant differences among the three groups in terms of age, gender, years of education, or total intracranial volume. Compared with the HCs group, both the AD-A and AD-NA groups showed significantly lower scores in cognitive function (P<0.001). The AD-A group exhibited significantly higher apathy scale scores compared with the AD-NA group (P<0.001). Compared with the AD-NA group, the AD-A group showed reduced gray matter volume in the bilateral caudate nucleus, left orbitofrontal cortex, lingual gyrus, inferior frontal gyrus, superior frontal gyrus, entorhinal cortex, right middle frontal gyrus and posterior cingulate cortex (FWE-corrected, P<0.05 for all). Compared with the HCs group, the AD-A group exhibited reduced gray matter volume in the bilateral middle temporal gyrus, left fusiform gyrus, calcarine sulcus, postcentral gyrus, right inferior frontal gyrus and supramarginal gyrus (FWE-corrected, P<0.05 for all). Compared with the HCs group, the AD-NA group showed reduced gray matter volume in the left precuneus, inferior temporal gyrus, and right inferior temporal gyrus (FWE-corrected, P<0.05 for all). In the AD-A group, changes in the gray matter volume of the left caudate nucleus (r= -0.557, P=0.002) and right middle frontal gyrus (r=-0.620, P=0.001) were negatively correlated with the apathy evaluation scale (AES) scores. ConclusionPatients in the AD-A group exhibited significant atrophy in the frontal-temporal-basal ganglia circuit, and the degree of gray matter atrophy was correlated with the severity of apathy.
3.Relationship between short video addiction and learning burnout in adolescent patients with depression: the pathways of impulsivity and coping disposition

BackgroundCurrently, the incidence of depression among teenagers is on the rise, and the related academic problems are becoming increasingly serious. Short video addiction has a negative impact on teenagers' emotional issues and academic achievements. However, few studies have explored the relationship between this addiction and the learning burnout of teenagers with depression, and even fewer have focused on the role paths of impulsivity and coping disposition in this process. ObjectiveTo explore the relationship between short video addiction and learning burnout in adolescent patients with depression, as well as the pathway of impulsivity and coping disposition, so as to provide references for the intervention of learning burnout in adolescent patients with depression. MethodsA total of 191 adolescent patients who were hospitalized at The Affiliated Brain Hospital of Guangzhou Medical University from December 2024 to April 2025 and met the diagnostic criteria for depression according to the International Classification of Diseases, tenth edition (ICD-10) were selected consecutively. The Short Video Addiction Measurement Scale (SVAMS), the Brief Barratt Impulsiveness Scale (BBIS), the Simplified Coping Style Questionnaire (SCSQ), and the Adolescent Student Burnout Inventory (ASBI) were used for assessment. Pearson correlation analysis was employed to examine the correlations of the scores of each scale. Model 6 of the SPSS macro Process 4.2 was employed to analyze the chained mediation pathway of impulsivity and coping disposition between short video addiction and learning burnout. ResultsA total of 173 cases (90.58%) of adolescent patients with depression completed the valid questionnaire survey. Correlation analysis showed that SCSQ coping disposition score was negatively correlated with the SVAMS score, the BBIS score, and the ASBI score (r=-0.282, -0.341, -0.431, P<0.01), the SVAMS score was positively correlated with the BBIS score and the ASBI score (r=0.339, 0.262, P<0.01), and the BBIS score was positively correlated with the ASBI score (r=0.486, P<0.01). The pathway analysis showed that the direct effect of short video addiction on learning burnout was not statistically significant, but the total effect and the indirect effect were statistically significant. The effect values were 0.275 (95% CI: 0.207–0.343) and 0.193 (95% CI: 0.143–0.246), respectively, with the indirect effect accounting for 70.18%. Impulsivity and coping disposition both played independent mediating roles between short video addiction and learning burnout, with effect values of 0.122 (95% CI: 0.090–0.156) and 0.054 (95% CI: 0.032–0.079), accounting for 44.36% and 19.64% of the total effect, respectively. The chained mediation effect of impulsivity and coping disposition was significant, with an effect value of 0.017 (95% CI: 0.011–0.026), accounting for 6.18% of the total effect. ConclusionAlthough short video addiction does not directly affect learning burnout in adolescent patients with depression, it may indirectly influence learning burnout through independent and chain paths of impulsivity and coping disposition. 4.Clinical efficacy of escitalopram combined with transcutaneous cervical vagus nerve stimulation therapy for patients with major depressive disorder and its effect on plasma IL-6 and IL-10 levels

BackgroundInvasive vagus nerve stimulation therapy has been approved for the adjunctive treatment of treatment-resistant depression, which may contribute to the anti-inflammatory properties of vagus nerve stimulation (VNS), whereas the efficacy of non-invasive transcutaneous cervical vagus nerve stimulation (tcVNS) in treating major depressive disorder (MDD) and its impact on plasma inflammatory factors remain unclear. ObjectiveTo observe the effect of escitaloprom combined with tcVNS on the status of depression, anxiety and sleep quality as well as the plasma levels of interleukin-6 (IL-6) and interleukin-10 (IL-10) in MDD patients, in order to provide references for the recovery and treatment of MDD patients. MethodsFrom August 21, 2019 to April 17, 2024, 45 patients who met the diagnostic criteria for MDD in the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5) were recruited from the psychosomatic outpatient clinic of the First Affiliated Hospital of Air Force Military Medical University. Subjects were divided into study group (n=23) and control group (n=22) using random number table method. All patients were treated with escitalopram. On this basis, study group added a 30-minute tcVNS therapy once a day for 4 weeks. While control group was given corresponding sham stimulation, and the duration of each stimulation lasted 30 seconds. Before and after 4 weeks of treatment, Hamilton Depression Scale-17 item (HAMD-17) was used to assess depressive symptoms, and HAMD-17 anxiety/somatization subfactor and insomnia subfactor were used to assess patients' anxiety/somatization symptoms and sleep quality. Levels of plasma IL-6 and IL-10 were measured by enzyme-linked immunosorbent assay (ELISA). ResultsThe generalized estimating equation model yielded a significant time effect for HAMD-17 total score, anxiety/somatization subfactor score and insomnia subfactor score in both groups (Wald χ2=315.226, 495.481, 82.420, P<0.01). After 4 weeks of treatment, HAMD-17 total score and anxiety/somatization subfactor score of study group were lower than those of control group, with statistically significant differences (Wald χ2=4.967, 32.543, P<0.05 or 0.01), while no statistically significant difference was found in the insomnia subfactor score between two groups (Wald χ2=0.819, P=0.366). Significant time effects were reported on plasma IL-6 and IL-10 levels in both groups (Wald χ2=21.792, 5.242, P<0.05 or 0.01). Compared with baseline data, a reduction in plasma IL-6 levels was detected in both groups (Wald χ2=22.015, 6.803, P<0.01), and an increase in plasma IL-10 levels was reported in study group (Wald χ2=5.118, P=0.024) after 4 weeks of treatment. ConclusionEscitalopram combined with tcVNS therapy is effective in improving depressive symptoms, anxiety/somatization symptoms and sleep quality in patients with MDD. Additionally, it helps reduce plasma IL-6 levels and increase IL-10 levels. 5.Evaluation of Effect of Tongnaoyin on Blood-brain Barrier Injury in Acute Ischemic Stroke Patients Based on Dynamic Contrast-enhanced Magnetic Resonance Imaging

ObjectiveTo evaluate the effects of Tongnaoyin on the blood-brain barrier status and neurological impairment in acute ischemic stroke (AIS) patients with the syndrome of phlegm-stasis blocking collaterals by dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI). MethodsA total of 63 patients diagnosed with AIS in the Jiangsu Province Hospital of Chinese Medicine from October 2022 to December 2023 were enrolled in this study. According to random number table method,the patients were assigned into a control group (32 cases) and an observation group (31 cases). The control group received conventional Western medical treatment,and the observation group took 200 mL Tongnaoyin after meals,twice a day from day 2 of admission on the basis of the treatment in the control group. After 7 days of treatment,the patients were examined by DCE-MRI. The baseline data for two groups of patients before treatment were compared. The National Institute of Health Stroke Scale (NIHSS) score and modified Rankin Scale (mRS) score were recorded before treatment and after 90 days of treatment for both groups. The rKtrans,rKep,and rVe values were obtained from the region of interest (ROI) of the infarct zone/mirror area and compared between the two groups. ResultsThere was no significant difference in the NIHSS or mRS score between the two groups before treatment. After 90 days of treatment,the NIHSS and mRS scores declined in both groups,and the observation group had lower scores than the control group (P<0.05). After treatment,the rKtrans and rVe in the observation group were lower than those in the control group (P<0.01). ConclusionCompared with conventional Western medical treatment alone,conventional Western medical treatment combined with Tongnaoyin accelerates the repair of the blood-brain barrier in AIS patients,thereby ameliorating neurological impairment after AIS to improve the prognosis.
6.Comparison of clinical characteristics and prognostic influence factors between patients with or without hypertrophic cardiomyopathy complicated by microvascular dysfunction

Objective:To discuss the clinical characteristics of the patients with hypertrophic cardiomyopathy(HCM)complicated with microcirculatory dysfunction(CMD),and to analyze the impact of concurrent CMD on the prognosis of the HCM patients.Methods:A total of 211 patients diagnosed with HCM and having complete cardiac magnetic resonance imaging(CMR)examination results from January 1,2019 to September 30,2023 were collected.They were divided into HCM complicated with CMD group(68 cases)and HCM complicated without CMD group(143 cases)based on CMR assessment.The clinical data such as age,gender,admission symptoms,and past medical history,blood test data such as troponin,electrocardiogram,echocardiography,and CMR data including abnormal Q wave,ST segment depression,inverted T wave,PR interval,QRS wavelength,corrected QT interval,ejection fraction(EF),left atrial diameter(LAD),left and right ventricular end-diastolic diameters,cardiac output,E peak,A peak,and maximum wall thickness(MWT)of the patients were compared between two groups.Logistic regression was used to analyze the clinical characteristics of the HCM patients complicated with CMD;multivariate modified Poisson regression was used to analyze the risk factors for major adverse cardiovascular events(MACE)in the HCM patients.Results:Compared with HCM complicated without CMD group,the percentage of palpitation patients in HCM complicated with CMD group was significantly increased(P<0.05),the percentage of tachycardia episode patients was significantly increased(P<0.05),the troponin level was significantly increased(P<0.05),and the percentage with a history of hypertension patients was significantly decreased(P<0.05).Compared with HCM complicated without CMD group,the percentage of abnormal Q wave on electrocardiogram in the patients in HCM complicated with CMD group were significantly increased(P<0.05),the percentage of inverted T wave and the EF of the patients was significantly decreased(P<0.05),the LAD was significantly increased(P<0.05),and the MWT was significantly increased(P<0.05).The multivariate Logistic regression analysis results showed that increased LAD(OR=1.05,95%CI:1.00-1.11,P=0.048)and increased MWT(OR=1.11,95%CI:1.03-1.19,P=0.007)were the risk factors for concurrent CMD in the HCM patients;history of hypertension(OR=0.40,95%CI:0.20-0.80,P=0.010)was a protective factor for concurrent CMD in the HCM patients.The average follow-up time in this study was 20.5 months.A total of 27 patients experienced MACE,with an overall incidence of 12.80%,including 12 patients in HCM complicated with CMD group and 15 patients in HCM complicated without CMD group.The multivariate modified Poisson regression analysis results showed that history of diabetes(RR=2.34,95%CI:1.09-5.06,P=0.030),history of arrhythmia(RR)=4.00,95%CI:1.82-8.83,P=0.001),and decreased ejection fraction(RR=0.96,95%CI:0.94-0.99,P=0.001)were risk factors for MACE in the HCM patients.Conclusion:The HCM patients complicated with CMD have unique clinical characteristics,including higher symptom burden,left atrial enlargement,myocardial hypertrophy,and increased troponin levels.Concurrent CMD does not increase the short-term risk of adverse events;diabetes,arrhythmia,and decreased EF are key risk factors for prognosis;early intervention and complication management for HCM complicated with CMD patients may improve the long-term prognosis of the HCM patients.
7.Efficacy of direct versus double-balloon occlusion techniques in endoscopic ultrasound-guided gastroenterostomy for gastric outlet obstruction: a retrospective cohort study (with video)

Objective:To compare the clinical efficacy of direct versus double-balloon occlusion in endoscopic ultrasound-guided gastroenterostomy (EUS-GE) for benign and malignant gastric outlet obstruction (GOO).Methods:Clinical data of patients with GOO who underwent EUS-GE at Nanjing Drum Tower Hospital between April 2017 and July 2024 were analyzed in a retrospectively cohort study. The patients were divided into the direct technique group ( n=36) and the double-balloon occlusion technique group ( n=105). The technical success rate, clinical success rate, procedure time, postoperative stay, stent replacement rate, and incidence of adverse events were compared between the two groups. Results:The technical success rates of the two groups were comparable, 97.2% (35/36) and 94.3% (99/105) ( χ2=0.065, P=0.798), so were the clinical success rates, 94.4% (34/36) and 86.7% (91/105) ( χ2=0.932, P=0.334). However, the direct technique group demonstrated significantly shorter procedure time and postoperative stay compared to the double-balloon occlusion group [33.4 (23.2, 42.3) min VS 43.4 (31.7, 63.1) min, Z=-3.057, P=0.002; 4.0 (3.00, 5.75) days VS 6.0 (5.00, 9.00) days, Z=-4.031, P<0.001]. Adverse event rates [11.1% (4/36) VS 11.4% (12/105), χ2<0.001, P=1.000] and stent replacement rates [5.6% (2/36) VS 9.5% (10/105), χ2=0.152, P=0.696] showed no significant differences. Conclusion:Both EUS-GE techniques achieve comparable efficacy and safety for GOO. However, the direct technique showed significant advantages over the double-balloon occlusion technique in terms of shorter procedure time and reduced postoperative hospital stay.

Post-stroke spasticity is a series of symptoms after stroke,such as hand and foot urgency,unflexion and extension of muscles,etc.In order to deeply understand the cognition of post-stroke spasticity of ancient Chinese physicians and comb out their therapeutic thoughts,this study took the General Catalogue of Chinese Ancient Books of Traditional Chinese Medicine as a bibliographic reference,all the ancient Chinese literature on spasms after stroke was retrieved manually and by computer,and then sorted and analyzed,and classified them by longitudinal time,and extracted the description about post-stroke spasticity,including medical classics,prescriptions,clinical evidence,medical records and so on.And this paper verified and summarized the etiology,pathogenesis,functional and indications and prescription characteristics of spasticity after stroke,in order to deeply understand systematic theories and treatment ideas of the ancient medical practitioners in the bud,development and mature stages of their understanding of spasticity after stroke,and provide the theoretical basis for the later doctors to understand this disease and the modern clinical treatment of traditional Chinese medicine.
9.Research progress of anti-leucine-rich glioma-inactivated 1 protein antibody-associated encephalitis

Anti-leucine-rich glioma-inactivated 1 protein (LGI1) antibody-associated encephalitis is an autoimmune encephalitis mediated by LGI1 antibodies, which can occur in both adults and children. Its common clinical manifestations include epileptic seizures, cognitive and psychiatric disorders; rare symptoms include sleep disorders and autonomic disorders; and its characteristic manifestations are faciobrachial dystonic seizures and refractory hyponatremia. Since anti-LGI1 antibody-associated encephalitis is relatively rare in clinical practice, this article reviews the disease in terms of etiology and pathogenesis, clinical manifestations, auxiliary examinations, diagnosis and differential diagnosis, treatment, recurrence and prognosis. It aims to improve clinicians′ understanding of this disease, provide references for its early diagnosis and treatment, and thereby improve patients′ prognosis.
10.Effects of Split Formulas of Biyuan Heji on Paranasal Sinus Mucosal Inflammation in ARS Rats Based on the Pathogenesis of"Wind-Cold Transforming into Lung-Heat"

OBJECTIVE To investigate the effects of the split formulas of Biyuan Heji(BYHJ)on paranasal sinus mucosal in-flammation in rats with acute rhinosinusitis(ARS)based on the pathogenesis of"wind-cold transforming into lung-heat".METHODS Unilateral nasal cavity occlusion combined with nasal dripping of Staphylococcus aureus were performed to establish a rat model of ARS.SD rats were randomly divided into blank,model,BYHJ(wind-cold removal+lung-heat removal),lung-heat removal,wind-cold removal,and positive drug groups,with 6 rats in each group.The rats were treated with the corresponding drugs for 7 d and then the samples were collected.HE staining was used to observe the pathological changes of rat paranasal sinus mucosa tissues,ELISA was employed to determine the levels of interleukin-1β(IL-1β),IL-6,IL-8,IL-9,IL-10,and IL-12 in serum,immunohistochemis-try(IHC)was adopted to measure the protein expression of tumor necrosis factor-α(TNF-α)and intercellular adhesion molecule(ICAM-1)in paranasal sinus mucosa tissues,and Western blot was used to detect the protein expression of phosphorylated p38 mito-gen-activated protein kinase(p38 MAPK),nuclear transcription factor-κB p50(NF-κB p50),and NF-κB p65 in paranasal sinus mucosa tissues.RESULTS The acute sinusitis rat inflammation model was successfully established.Compared with the model group,the water drinking,diet eating,and body weight of rats in the BYHJ group,wind-cold removal,lung-heat removal,and positive drug groups were significantly improved,the aggregation of inflammatory cells in the paranasal sinus mucosal tissue was reduced,and the levels of IL-1β,IL-6,IL-8,IL-9,and IL-12 in the serum were significantly reduced(P<0.01),IL-10 content significantly in-creased(P<0.01),the protein expression of TNF-α,ICAM-1,p38 MAPK,NF-κB p50,and NF-κB p65 in the paranasal sinus mucosa was significantly decreased(P<0.01).The comparison between various traditional Chinese medicine groups showed that the decrease of IL-1β,IL-6,IL-8,IL-9,IL-12,TNF-α,ICAM-1,p38 MAPK,NF-κB p50,and NF-κB p65 and the increase of IL-10 in the BYHJ group were better than those in the split formula groups(P<0.01),and the lung-heat removal group was better than the wind-cold removal group(P<0.01).CONCLUSION BYHJ and its split formulas can effectively inhibit the inflammatory response in rats with ARS.
