1.PET/CT imaging of PD-1 receptor probe targeting S180 sarcoma in mice

ObjectiveTo explore the feasibility of constructing a programmed death receptor-1(PD-1) molecular probe for non-invasive micro-positron emission tomography/computed tomography (Micro-PET/CT) imaging of PD-1 protein in mouse S180 sarcoma. MethodsA transgenic PD-1 C57 S180 sarcoma mouse model was established using the S180 sarcoma cell injection. Furthermore, 124I-anti-PD-1 monoclonal antibody probe was synthesized. 18.5 MBq of the 124I-anti-PD-1 probe was injected into the tail vein of transgenic PD-1 C57 mice. Subsequently, S180 sarcoma was imaged using Micro-PET/CT. ResultsStudy successfully established a transgenic PD-1 C57 S180 sarcoma mouse model. Immunohistochemical (IHC) results showed PD-1 protein expression in S180 sarcoma. Micro-PET/CT imaging successfully visualized the PD-1 protein receptor in S180 sarcoma at different time points (20, 48, 72, and 120 h) after probe injection. ConclusionThe 124I-anti-PD-1 monoclonal antibody molecular probe successfully targets the PD-1 receptor in S180 sarcoma of transgenic PD-1 C57 mice, and presents clear Micro-PET/CT immunoassay results, thus it potentially enables the non-invasive screening of patients with PD-1 positive malignant tumors.
2.Applications and advances of artificial intelligence in embryo evaluation

With the rapid development of artificial intelligence (AI) technology, its application in embryo evaluation is becoming more and more extensive. This article reviews the main progress of AI in embryo evaluation in recent years, including image recognition, embryo quality evaluation and outcome prediction. AI models can effectively analyze large-scale embryo image data and identify morphological features, thereby improving the accuracy and efficiency of evaluation. In addition, AI can also integrate images and clinical data to provide individualized embryo evaluation strategies. While AI shows promising potential in embryo evaluation, challenges remain in model interpretability and clinical application standardization. Future study needs to explore the application potential of AI in embryo evaluation in depth to promote the development and innovation of reproductive medicine.
3.Construction and application of an airway management emergency response training program for anesthesia nurses based on a virtual simulation platform

Objective:To develop a training program for emergency airway management capabilities of anesthesia nurses based on a virtual simulation platform and to validate its effectiveness, providing a reference for airway training of anesthesia nurses.Methods:A quasi-experimental study was conducted. A total of 60 anesthesia nurses in Nanjing Drum Tower Hospital from August 2022 to August 2023 were selected as the research objects by convenience sampling method. According to the random number table method, they were divided into the control group and the experimental group, with 30 nurses in each group. The two groups had the same theoretical training. As for the operational training, the control group was taught by case combined with ordinary simulator, and the experimental group was taught by case combined with virtual simulation platform. The theoretical scores, skill operation scores, and satisfaction degree with airway management emergency response training were compared between the two groups of anesthesia nurses.Results:There were 4 males and 26 females in the control group, aged (24.37 ± 1.45) years. There were 6 males and 24 females in the experimental group, aged (24.20 ± 1.22) years.The theoretical scores and skill operation scores of the experimental group were 89.20 ± 3.99 and 90.10 ± 4.45, respectively, both higher than those of the control group, which were 84.83 ± 4.64 and 85.30 ± 5.64, showing statistically significant differences ( t=-3.91, -3.66, both P<0.05). In the self-evaluation of satisfaction degree with airway management emergency response training, the experimental group scored (16.67±1.79) for theoretical knowledge mastery, 18.37 ± 1.73 for skill operation mastery, 19.07 ± 1.17 for enthusiasm in airway training, 18.43 ± 1.48 for initiative in self-learning, and 18.00 ± 1.51 for the engagement of course design, all higher than 13.67 ± 2.17, 14.37 ± 2.34, 13.37 ± 2.63, 12.30 ± 3.51, and 12.77 ±2.71 in the control group, respectively. The differences were statistically significant ( t values were -10.83 to -5.84, all P<0.05). Conclusions:The airway management emergency training curriculum for nurse anesthetists developed based on a virtual simulation platform improved their theoretical knowledge and practical skills in airway emergency management. Additionally, satisfaction across various aspects of the training was markedly enhanced, providing a valuable reference for airway management training of nurse anesthetists and demonstrating considerable potential for clinical implementation.
4.Global analysis and forecast of the burden of infertility in 204 countries and territories from 1990 to 2019

Objective:To analyze the trend of infertility disease burden from 1990 to 2019 and predict the age-standardized prevalence rate (ASPR) and age-standardized disability-adjusted life-year (DALY) rate of male and female infertility for 2020—2029, providing a certain reference for the prevention and treatment of infertility diseases.Methods:Based on the Global Burden of Disease 2019, prevalence, ASPR, DALY rate, and age-standardized DALY rate were used to evaluate the global burden of infertility comprehensively. The estimated annual percentage change was used to describe trends in the disease burden of infertility in 21 regions worldwide. The Bayesian age period cohort model was used to predict the ASPR and age-standardized DALY rate for male and female infertility in 204 countries and regions from 2020 to 2029. Sensitivity analysis was performed using the autoregressive integrated moving average model.Results:The prevalence, DALY rate, ASPR and age-standardized DALY rate of global male infertility increased from 319.52 per 100 000,1.82 per 100 000, 2 856.53 per 100 000, and 16.19 per 100 000 in 1990 to 565.30 per 100 000, 3.22 per 100 000, 3 398.53 per 100 000, and 19.36 per 100 000 in 2019, respectively. The prevalence, DALY rate, ASPR, and age-standardized DALY rate of global female infertility increased from 656.67 per 100 000, 3.53 per 100 000, 6 036.36 per 100 000, and 32.27 per 100 000 in 1990 to 1 223.78 per 100 000, 6.59 per 10 000, 7 483.12 per 100 000, and 40.33 per 100 000 in 2019, respectively. The burden of infertility disease was the highest in men and women aged 30-34 years, and the ASPR and age-standardized DALY rates were 4 407.47 per 100 000, 25.08 per 100 000, 10 270.55 per 100 000 and 55.65 per 100 000, respectively. Only in 45-49 years of age, the prevalence of infertility (11.31 per 100 000) and DALY rate (0.06 per 100 000) in women were lower than those in men (15.68 per 100 000 and 0.08 per 100 000). In addition, the burden of infertility was the lowest in high socio-demographic index regions. Cameroon had the highest ASPR (7 652.40 per 100 000) and age-standardized DALY rate (43.94 per 100 000) for male infertility. Chinese women had the highest ASPR (20 402.30 per 100 000) and age-standardized DALY rate (106.16 per 100 000) of infertility. The forecast results show that the burden of male and female infertility diseases will increase in 204 countries and regions from 2020 to 2029.Conclusion:The burden of infertility diseases in men and women increased in 204 countries and regions from 1990 to 2019, and it is predicted that the global burden of infertility diseases will continue to rise in 2020—2029. Preliminary screening of infertility should be carried out as soon as possible, health education should be strengthened and effective prevention and treatment strategies should be formulated.
5.Current status of management of three quality control indexes for management of hospital-associated infection in medical institutions above secondary level nationwide

OBJECTIVE T o investigate the data and usage,effectiveness and existing issues of three medical quality control indicators for hospital-associated infection management(2015 Edition),including hospital-associated infec-tion incidence rate,hospital-associated infection prevalence rate and hospital-associated infection underreporting rate.METHODS Data from hospitals that participated in reporting for three consecutive years(2018-2020)prior to the survey were selected for analysis through the annual professional quality control work conducted by the Na-tional Nosocomial Infection Management and Quality Control Center.An online questionnaire-based sampling sur-vey was conducted to evaluate the usage of the aforementioned three hospital-associated infection quality control in-dicators.RESULTS The usage rates of the three indicators were above 80%in hospitals of different levels and types.For the two indicators of hospital-associated infection incidence rate and hospital-associated infection under-reporting rate,the usage rates were higher in tertiary hospitals than in secondary hospitals,and higher in general hospitals than in specialized hospitals(P<0.05).For the hospital-associated infection prevalence rate indicator,the usage rate was 96.01%in tertiary hospitals,higher than that in secondary hospitals(90.73%),with a statisti-cally significant difference(P<0.05).The usage rate of such indicator was also higher in general hospitals(92.22%)than in specialized hospitals(89.50%)(P=0.102).Regarding self-evaluatio n of the implementation ef-fectiveness of the hospital-associated infection incidence rate,statistically significant differences were found among hospitals of different levels and types(P<0.001).For self-assessment of the implementation effectiveness of the hospital-associated infection prevalence rate and hospital-associated infection underreporting rate,statistically sig-nificant differences were found among hospitals of different levels(P<0.001).CONCLUSIONS The three indica-tors,hospital-associated infection incidence rate,hospital-associated infection prevalence rate and hospital-associ-ated infection underreporting rate,have been stable overall in the past three years.There are certain differences in their usage and evaluation.Reasonable revisions should be made based on actual situations to better guide clini-cal infection control work.
6.Progress of research on epidemiology and drug resistance mechanism of Elizabethkingia anopheles infection

In recent years the detection rate of Elizabethkingia anopheles(EA)in China has shown an upward trend,making it one of the important pathogens causing hospital and community infections.EA accounts for 59.3%to 96.2%of the Elizabethkingia species,and can cause meningitis,pneumonia,bacteremia,etc.There are limited researches on its pathogenic mechanism,which mainly involves various virulence factors.EA is a mul-tidrug-resistant bacterium,with drug resistance mechanisms including biofilm formation,efflux pumps and carry-ing multiple resistance genes(such as GOB,BlaB).Currently,minocycline,piperacillin/tazobactam,trime-thoprim/sulfamethoxazole and rifampin are effective drugs for treating EA infections.Clinically,efforts are be-ing made to find the optimal antibacterial drug combination to achieve ideal therapeutic effects.This paper reviews the epidemiological characteristics,pathogenic mechanisms,drug resistance mechanisms and treatment options of EA,aiming to provide a reference for clinical prevention,control,diagnosis and treatment of EA infections.
7.Latent profile analysis of dyadic psychological capital among stroke patients and their spouses and nursing countermeasures

Objective To explore the potential categories of dyadic psychological capital and related factors among stroke patients and their spouses,in order to inform the improvement of their mental health.Methods Stroke patients and their spouses were recruited in the Neurology Department of 3 tertiary general hospitals from March,2023 to August,2023 by convenience sampling method.The general information questionnaire,Modified Rankin Scale(mRs),Psychological Capital Questionnaire,The Kessler Psychological Distress Scale(K10)and Perceived Social Support Scale(PSSS)were used for investigation.Latent profile analysis was adopted to explore the categories of dyadic psychological capital,and multiple Logistic regression analysis was used to assess the effect of various factors on different categories.Results A total of 232 returned questionnaires(out of the 235)were valid,resulting an effective response rate of 98.7%.Stroke patients and their spouses were divided into 3 categories,including dyadic low psychological capital-low self-efficacy group(6.0%),dyadic high psychological capital-low resilience group(15.9%)and dyadic medium psychological capital group(78.1%).The stroke patients'education level,mRs scores,occupational status after the illness,level of perceived social support,and spouses'working condition were the influencing factors on potential profiles of dyadic psychological capital(P<0.05).Conclusion There are heterogeneity and interdependence in the level of dyadic psychological capital of stroke patients and their spouses.Healthcare professionals should promote dyadic mental health by developing targeted interventions for stroke patients and their spouses with different psychological capital characteristics from a dyadic holistic perspective.
8.Related factors of clopidogrel resistance and its impact on prognosis in elderly patients with acute myocardial infarction

Objective To explore the risk factors of clopidogrel resistance in elderly patients with acute myocardial infarction(AMI)in order to provide personalized medication plans.Methods A prospective trial was conducted on 472 elderly AMI patients undergoing coronary stent placement in No.901 Hospital of Joint Logistics Support Force of PLA and Hangzhou Special Service Conva-lescent Center of Air Force from January 2018 to December 2022.All patients received clopidogrel for antiplatelet postoperatively,and after 7 consecutive days of administration,peripheral venous blood samples were collected to observe whether the patient had resistance to clopidogrel and to measure the concentration of active metabolites of clopidogrel and the maximum platelet aggrega-tion ratio(MARAA)of arachidonic acid induced platelets.According to MARAA,they were divid-ed into a clopidogrel resistant group(MARAA≥55%,142 cases)and a clopidogrel sensitive group of(<55%,330 cases).The clinical characteristics were compared between the two groups,and multivariate logistic regression analysis was used to identify the risk factors for clopidogrel resistance.The impact of clopidogrel resistance on major adverse cardiovascular events was ana-lyzed.Results The clopidogrel resistant group had significantly higher MARAA level,decrease in the concentration of clopidogrel active metabolites,higher BMI,and larger ratios of obesity,CYP2C19*2 mutation,diabetes,hyperlipidemia and administration of hypoglycemic drugs when compared with the clopidogrel sensitive group(P<0.01).Obesity,diabetes,hyperlipidemia and CYP2C19*2 mutation were risk factors for clopidogrel resistance in elderly AMI patients(OR=2.036,95%CI:1.282-3.235;OR=2.795,95%CI:1.803-4.334;OR=2.125,95%CI:1.355-3.331;OR=4.881,95%CI:2.807-8.486).The incidence of major adverse cardiovascular events was notably higher in the clopidogrel resistant group than the sensitive group(11.27%vs 4.55%,P<0.01).After a prediction model was constructed with R4.0.3 statistical software,330 cases randomly selected from the dataset were assigned into a training set,and the remaining 142 cases into a validation set.The AUC value of the training set and the validation set was 0.750 and 0.780,respectively.Hosmer-Lemeshow Goodness of Fit test indicated that the model had good re-liability and predictive value(Chi-square=18.907,P=0.215).Conclusion Obesity,diabetes,hy-perlipidemia and CYP2C19*2 mutation are risk factors of clopidogrel resistance in elderly AMI patients,and clopidogrel resistance may lead to poor prognosis.
9.Survey of current status of prevention and control of traditional Chinese medicine diagnosis and treatment technique-related infections in Jiangsu Province

OBJECTIVE To understand the current status of prevention and control of traditional Chinese medicine(TCM)diagnosis and treatment technique-related infections in various grades of hospitals in Jiangsu Province so as to provide bases for management departments to put forward the norms.METHODS The questionnaire was de-signed by the methods such as Delphic correspondence,the current status of prevalence,prevention and control of TCM diagnosis and treatment technique-related infections among the health care workers in the hospitals of Jiang-su Province was investigated by using convenience sampling method.RESULTS A total of 508 questionnaires were collected from 13 cities of Jiangsu Province.The results showed that the TCM treatment room(63.64%)was the most common operation site for minimally invasive TCM diagnosis and treatment techniques,the bedside(62.50%)of the patients was the major operation site for Chinese medicine enema,and the central sterile supply department(55.31%)was the preferred choice of cleaning and disinfection site for daily recycled TCM apparatu-ses.'One person used for once,airing and preparing for later use after cleaning and disinfection for once'was the major approach taken for the cleaning and disinfection of Gua Sha boards(82.45%)and TCM pots(87.50%).67.87%of the health care workers said that they wound cleaned and disinfected the medical apparatuses of dress-ing,ironing and fumigation every time after the use.The natural ventilation and ultraviolet light(50.39%)was the major method that was taken for the ventilation and disinfection of the treatment rooms for TCM operations.Quick-drying hand disinfectant and non-touch hand-washing equipment(39.37%)were the most commonly e-quipped hand hygiene facilities.CONCLUSIONS The prevention and control of TCM diagnosis and treatment tech-nique-related infections and the health care workers' capabilities for prevention and control of the infection have made remarkable progress in the various grades of hospitals in Jiangsu Province.However,there is still room for further improvement,and the standardized management and construction should be completed so as to safeguard the TCM diagnosis and treatment techniques.
10.Effects of Split Formulas of Biyuan Heji on Paranasal Sinus Mucosal Inflammation in ARS Rats Based on the Pathogenesis of"Wind-Cold Transforming into Lung-Heat"

OBJECTIVE To investigate the effects of the split formulas of Biyuan Heji(BYHJ)on paranasal sinus mucosal in-flammation in rats with acute rhinosinusitis(ARS)based on the pathogenesis of"wind-cold transforming into lung-heat".METHODS Unilateral nasal cavity occlusion combined with nasal dripping of Staphylococcus aureus were performed to establish a rat model of ARS.SD rats were randomly divided into blank,model,BYHJ(wind-cold removal+lung-heat removal),lung-heat removal,wind-cold removal,and positive drug groups,with 6 rats in each group.The rats were treated with the corresponding drugs for 7 d and then the samples were collected.HE staining was used to observe the pathological changes of rat paranasal sinus mucosa tissues,ELISA was employed to determine the levels of interleukin-1β(IL-1β),IL-6,IL-8,IL-9,IL-10,and IL-12 in serum,immunohistochemis-try(IHC)was adopted to measure the protein expression of tumor necrosis factor-α(TNF-α)and intercellular adhesion molecule(ICAM-1)in paranasal sinus mucosa tissues,and Western blot was used to detect the protein expression of phosphorylated p38 mito-gen-activated protein kinase(p38 MAPK),nuclear transcription factor-κB p50(NF-κB p50),and NF-κB p65 in paranasal sinus mucosa tissues.RESULTS The acute sinusitis rat inflammation model was successfully established.Compared with the model group,the water drinking,diet eating,and body weight of rats in the BYHJ group,wind-cold removal,lung-heat removal,and positive drug groups were significantly improved,the aggregation of inflammatory cells in the paranasal sinus mucosal tissue was reduced,and the levels of IL-1β,IL-6,IL-8,IL-9,and IL-12 in the serum were significantly reduced(P<0.01),IL-10 content significantly in-creased(P<0.01),the protein expression of TNF-α,ICAM-1,p38 MAPK,NF-κB p50,and NF-κB p65 in the paranasal sinus mucosa was significantly decreased(P<0.01).The comparison between various traditional Chinese medicine groups showed that the decrease of IL-1β,IL-6,IL-8,IL-9,IL-12,TNF-α,ICAM-1,p38 MAPK,NF-κB p50,and NF-κB p65 and the increase of IL-10 in the BYHJ group were better than those in the split formula groups(P<0.01),and the lung-heat removal group was better than the wind-cold removal group(P<0.01).CONCLUSION BYHJ and its split formulas can effectively inhibit the inflammatory response in rats with ARS.
