1.Progress in research on the etiology and treatment of fertilization disorders

Infertility is an important cause of reproductive health. With the increasing development of assisted reproductive technology, millions of people choose to use assisted reproductive technology to assist pregnancy every year. However, there are still many patients in the clinic who may have fertilization disorders during the assisted pregnancy. The causes of fertilization disorders are complex and varied. Although research on fertilization disorders has progressed in recent years, the question of how to improve the fertilization rate and oocyte utilization in patients remains a focal issue. The aim of this review is to summarize the latest research progress on the possible etiology and treatment of fertilization disorders in the process of conception, so as to provide a more comprehensive theoretical reference for the clinical diagnosis and management of people with fertilization disorders.
2.Establishment and application of quality control system for medical record front page coding in a hos-pital

Objective Analyze the causes of coding issues in the inpatient medical record homepage of a hospital and propose targeted improvement measures,providing a reference basis for enhancing coding quality.Methods A total of 3 000 in-patient medical records from April 1,2021,to March 31,2022(pre-intervention group)and 2 900 records from April 1,2022,to March 31,2023(post-intervention group)were selected for analysis.Quality control statistics were performed on key indica-tors,including the overall coding accuracy rate,primary diagnosis coding,secondary diagnosis coding,primary surgical proce-dure coding,and other surgical procedure coding.Results After the implementation of intervention measures,the overall accu-racy rate of the first page coding of medical records increased from 80.77%to 86.72%(P<0.001).While,the accuracy rate for primary diagnosis coding improved from 96.90%to 98.07%(P<0.05).The secondary diagnosis coding accuracy showed significant enhancement from 87.48%to 92.26%(P<0.001),and primary surgical procedure coding accuracy rose from 96.50%to 98.34%(P<0.001).Although other surgical procedure coding demonstrated improvement,the difference was not statistically significant(P>0.05).Conclusion Through a series of measures—including standardized training for coders,es-tablishing a robust quality control system,and adopting advanced information technology—both the professionalism of coders and coding efficiency were enhanced.These interventions effectively reduced coding defects and improved the quality of medical re-cord homepage coding.

Langerhans cell sarcoma(LCS)is a rare malignant tumor derived from Langerhans cell histiocytes with unique clinicopathological,immunophenotypic,ultrastructural and molecular genetic features.Most studies on LCS are case reports,and the immunohistochemical features,biological mechanisms and molecular basis of the tumor remain to be further elucidated.A large body of literature suggests that LCS is extremely aggressive and presents many difficulties in clinical diagnosis,so it is important to accurately diagnose LCS.In this paper,we will review the clinical,patholog-ical and molecular features of LCS to provide more ideas for the definitive diagnosis of LCS and providing a basis for the selection of treatment regimens for LCS.

As a physical treatment technique, modified electro-convulsive therapy (MECT) is used to treat mental and certain neurological disorders by causing seizures with short, suitable electrical currents applied to the brain while the patient is under general anesthesia and muscle relaxants. MECT is recognized for its therapeutic efficacy and clinical safety, rendering it one of the most prevalent interventions in psychiatric care. To enhance clinical outcomes and minimize adverse effects, this consensus document delineates the indications, therapeutic parameters, therapeutic procedures, potential adverse effects, and associated management strategies for MECT. These guidelines are informed by the latest clinical research and expert consensus, integrating evidence-based medicine methodologies. The objective is to furnish clinicians with precise operational guidelines and to advance the standardization of MECT practices in clinical settings.

9.Effect of sophoridine on lipopolysaccharide-induced pulmonary alveolar epithelial cell damage

Objective To investigate the effect of sophoridine on lipopolysaccharide(LPS)induced pulmonary alveolar epithelial cell damage by regulating the high-mobility group box 1(HMGB1)-receptor for advanced glycation end product(RAGE)signal pathway.Methods A549 and HAEC Ⅱ cells cultured in vitro were randomly divided into 5 groups:control group,LPS group,LPS+sophoridine group,LPS+empty group and LPS+sophoridine+HMGB1 overexpression group.Except for the control group,cells in other groups were induced by LPS to establish the injury models and treated with sophoridine and plasmids,respectively.Cell counting kit-8(CCK-8)method and terminal-deoxynucleoitidyl transferase mediated nick end labeling staining were used to detect cell viability and apoptosis rate in each group;Western blotting was used to detect the expression of HMGB1 and RAGE proteins.Results The A549 cell viability of control group,LPS group,LPS+sophoridine group,LPS+empty group and LPS+sophoridine+HMGB1 overexpression group were(100.00±0.00)%,(56.73±8.31)%,(90.02±11.24)%,(53.26±9.15)%and(60.84±8.13)%;the cell viability of HAEC Ⅱ were(100.00±0.00)%,(50.86±7.56)%,(93.05±12.38)%,(54.10±8.42)%and(54.43±6.14)%;the apoptosis rates of A549 cells were(2.11±0.65)%,(42.46±5.20)%,(4.01±1.31)%,(44.74±4.93)%and(39.75±4.86)%;the apoptosis rates of HAEC Ⅱ cells were(2.30±0.72)%,(48.14±4.87)%,(3.83±1.23)%,(45.72±5.14)%and(44.81±5.25)%;the HMGB1 protein levels in A549 cells were 0.16±0.02,0.87±0.13,0.19±0.04,0.89±0.11 and 0.84±0.12;RAGE protein levels were 0.19±0.03,0.94±0.16,0.21±0.04,0.96±0.15 and 0.90±0.17;the HMGB1 protein levels in HAEC Ⅱ cells were 0.13±0.04,0.79±0.10,0.15±0.04,0.80±0.14 and 0.75±0.12;RAGE protein levels were 0.28±0.07,1.08±0.19,0.31±0.06,1.10±0.21 and 1.04±0.15.The above indexes:there were statistically significant differences between LPS group and control group,LPS+sophoridine group and LPS group,LPS+sophoridine+HMGB1 overexpression group and LPS+sophoridine group(all P<0.05).Conclusion Sophoritine can clear reactive oxygen species,reduce the production of inflammatory factors,enhance antioxidant enzyme activity,and inhibit LPS induced inflammation and oxidative stress in alveolar epithelial cells by reducing HMGB1-RAGE signal activity,ultimately reducing cell damage.

The concept of'harmony'is the soul of traditional Chinese culture,which has a profound impact on the formation and development of traditional Chinese medicine(TCM).TCM is rooted in traditional Chinese culture,and the mode of thinking in TCM is in line with traditional Chinese culture.Based on the harmony culture,TCM has developed a unique view of health,disease and therapeutics.From the view of the harmony culture and by combining with years of clinical experience in treating dermatosis,Chinese medical master XUAN Guo-Wei has applied the concept of'harmony'in the TCM syndrome differentiation and treatment system in clinic,and has developed the academic thoughts of harmonizing therapy for removing toxins for the diagnosis and treatment of dermatosis.The thoughts of harmonizing therapy for removing toxins includes four aspects,namely harmonizing yin and yang,harmonizing healthy qi and pathogenic qi,harmonizing water and fire(i.e.,clod and hot),and harmonizing the administration of formula and drugs,aiming to remove toxins and expel pathogens and value the harmony.The thoughts of harmonizing therapy for removing toxins will beneficial to the comprehensive understanding of the unique health-disease-therapeutics concept in TCM,and will be helpful for managing the doctor-patient relationship,which is of enlightening significance to the modern clinical practice with TCM.
