ObjectiveTo investigate the therapeutic efficacy of Danggui Shaoyaosan (DSS) on renal injury in db/db mice and its impact on endoplasmic reticulum stress (ERS) in renal tissues. MethodsThirty 8-week-old male db/db mice and six db/m mice were acclimated for one week, after which urinary microalbumin and blood glucose levels were monitored to establish a diabetic kidney disease (DKD) model. The model mice were randomly divided into a model group, an irbesartan group, and three DSS treatment groups with different doses (16.77, 33.54, and 67.08 g·kg-1·d-1). A normal group was set as control. Each group was intragastrically administered with the corresponding drugs or saline for 8 weeks. After the intervention, general conditions were observed. Serum cystatin C (Cys-C), 24-hour urinary total protein (24 h-UTP), 24-hour urinary microalbumin (24 h-UMA), urinary creatinine (Ucr), and urea nitrogen (UUN) were measured. Transmission electron microscopy (TEM) was used to observe glomerular basement membrane (GBM) and ultrastructural changes of the endoplasmic reticulum (ER) in glomerular endothelial cells. Western blot, real-time fluorescence quantitative polymerase chain reaction (Real-time PCR), and immunohistochemistry were used to analyze renal tissue structure and the expression of GRP78, CHOP, and related markers. ResultsCompared with the normal group, the mice in the model group showed curled posture, sluggish response, poor fur condition, increased levels of Cys-C, 24 h-UTP, 24 h-UMA, and UUN (P<0.05), while Ucr decreased (P<0.05). The GBM was significantly thickened, with podocyte and foot process fusion. The protein expressions of GRP78, CHOP, and ATF6 were significantly upregulated (P<0.05), the mRNA levels of GRP78 and CHOP increased (P<0.05), and immunohistochemistry showed an enhanced GRP78 signal (P<0.05). After treatment, the mice exhibited improved behavior, normalized GBM and podocyte structure, improved ER morphology and markedly better biochemical indicators. Western blot, Real-time PCR, and immunohistochemistry indicated that the ERS-related markers were downregulated in the DSS treatment groups (P<0.05), suggesting alleviated ERS and improved renal function. ConclusionDSS can effectively ameliorate renal pathological damage in db/db mice, possibly by regulating ERS in glomerular endothelial cells, although the underlying signaling mechanisms require further investigation.

ObjectiveTo construct a stable monoclonal human embryonic kidney 293 (HEK293) cell line expressing recombinant human coagulation factor Ⅶ (rhFⅦ) and evaluate the expression level and procoagulant bioactivity of rhFⅦ. MethodsThe plasmid pCDNA3.1-EGFP-FⅦ was transfected into HEK293 cells to verify the effectiveness of the transfection system. The plasmid pCDNA3.1-FⅦ was transfected into HEK293 cells, and monoclonal stable transfected cell lines were selected using geneticin (G418). The transcription of the FⅦ gene was identified by reverse transcription polymerase chain reaction (RT-PCR). The expression level of rhFⅦ in the supernatant of the monoclonal stable transfected cell line was detected by sodium dodecyl sulfate-polyacrylamide gel electrophoresis and Western blot. The concentration of rhFⅦ was determined by enzyme-linked immunosorbent assay (ELISA), and the procoagulant activity of rhFⅦ was measured by human coagulation factor Ⅶ potency assay. ResultsHEK293 cells transfected with pcDNA3.1-EGFP-FⅦ showed green fluorescence, indicating that rhFⅦ was successfully expressed in the supernatant of HEK293 cells after transient transfection with pcDNA3.1-FⅦ. The monoclonal stable transfected cell line was obtained by G418 screening. RT-PCR identified that the FⅦ gene was integrated into the genome of the monoclonal stable transfected cell line. The cell viability was good as detected by Cell Counting Kit-8, and a single band of rhFⅦ was obtained by purification of the cell supernatant. The highest rhFⅦ expression was (1.27±0.09) mg/L, and the highest procoagulant activity was (380.29±13.80)%. ConclusionThe monoclonal HEK293 cell lines which can express rhFⅦ protein efficiently and stably with excellent procoagulant bioactivity is successfully screened.

With the continuous deepening of the concept of precision diagnosis and treatment for lung cancer, how to achieve higher efficiency and accuracy in the screening, diagnosis, and treatment pathways in clinical practice has become an important issue that urgently needs to be overcome. The current clinical difficulty lies in the fact that despite continuous advancements in imaging and molecular diagnostic technologies, there are still limitations in manual efficiency and subjective experience when it comes to massive data analysis and multi-scale feature extraction. Artificial intelligence (AI), especially algorithm systems based on deep learning, is an innovative technology capable of deeply empowering medical big data. This method utilizes algorithms such as convolutional neural networks, combined with radiomics, pathomics, and multi-modal data fusion analysis, demonstrating immense potential in early precise detection and benign-malignant differentiation of pulmonary nodules, digital pathological subtype recognition and non-invasive prediction of driver genes, precise 3D surgical planning and automatic delineation of radiotherapy target volumes, as well as dynamic risk warning during follow-up. This innovative technology provides a brand-new solution for realizing intelligent and individualized lung cancer diagnosis and treatment models. This consensus, based on the latest evidence from evidence-based medicine and combined with the development trends in the AI field and real-world clinical needs, was ultimately formed by gathering the consensus opinions of multidisciplinary experts in radiology, pathology, thoracic surgery, and other fields. The main content covers the application specifications of AI in the three core scenarios of lung cancer screening, diagnosis, and treatment, the technical standards for data collection and algorithm validation, as well as the ethical and regulatory challenges faced at the current stage. It aims to clarify the applicable boundaries of AI as a clinical auxiliary decision support tool, providing scientific guidance and standardized exploration directions for peers currently engaged in or planning to carry out AI-assisted clinical diagnosis, treatment, and translation of lung cancer.

ObjectiveTo investigate the value of preoperative dual-layer spectral detector CT (DLCT) parameters in predicting lymph node metastasis (LNM) of non-functional pancreatic neuroendocrine neoplasms (NF-pNENs). MethodsA retrospective study was conducted on patients with pathologically confirmed NF-pNENs who underwent preoperative contrast-enhanced DLCT examination between September 2019 and July 2024. Regions of interest (ROIs) were manually delineated on three representative slices of the solid component of the lesion, and the mean values were recorded. Conventional CT parameters, monochromatic energy images (40 keV, 70 keV, 100 keV), iodine concentration maps, and effective atomic number (Zeff) maps in the pancreatic parenchymal phase and portal venous phase were acquired. A total of 34 parameters, including clinical characteristics, imaging features, conventional CT and DLCT quantitative parameters, were analyzed. Intergroup differences were compared between the LNM and non-LNM groups. Independent predictive factors were screened using logistic regression and least absolute shrinkage and selection operator (LASSO) regression. Receiver operating characteristic (ROC) curves were generated for each predictive model, and the area under the curve (AUC) was calculated to evaluate the predictive efficacy. ResultsA total of 82 patients with NF-pNENs were enrolled, including 37 males and 45 females, with a mean age of 49.7±14.7 years. Thirty patients were performed with LNM and 52 were with non-LNM. After univariate logistic regression, LASSO regression and multivariate stepwise logistic regression analyses, body mass index (BMI), organ invasion, normalized CT value in the portal venous phase (nCTv) and normalized iodine concentration in the portal venous phase (nICv) were identified as independent predictive factors. The AUCs of the clinical-radiological qualitative model (BMI+organ invasion), conventional CT model (nCTv), DLCT model (nICv) and comprehensive model (organ invasion+nICv) were 0.815 (0.719, 0.910), 0.750 (0.642, 0.858), 0.829 (0.733, 0.925) and 0.865 (0.879, 0.953), respectively. The comprehensive model combining clinical-radiological and DLCT characteristics exhibited superior predictive performance. ConclusionDLCT parameters are more effective than conventional CT parameters in predicting LNM in NF-pNENs, enabling non-invasive preoperative prediction and assisting clinical decision-making.

ObjectiveTo explore the effects of different embolic materials for preoperative transarterial embolization (TAE) on surgical resections of sizeable (>3 cm) Shamblin Ⅱ/Ⅲ carotid body tumors (CBTs) MethodsThis study retrospectively analyzed the data of patients with Shamblin type Ⅱ/Ⅲ carotid body tumors >3 cm in size who underwent TAE at our hospital between January 2010 and August 2025. Based on the adopted embolic agents, the patients were divided into two groups: temporary embolization group (TEG) and permanent embolization group (PEG). General clinical data, periprocedural details, and postoperative outcomes were collected and compared between the two groups. ResultsA total of 50 patients with 50 lesions were included (n=25 in the TEG and n=25 in PEG group). Tumor sizes were comparable between the two groups [(41.22 vs. 42.13) mm, P=0.989]. For the TEG compared with PEG, intraoperative blood loss was (388.19 vs. 155.90) mL (P<0.001) and operative time was (254.01 vs. 162.22) mins (P=0.007). The intraoperative vascular reconstruction rate was 16% vs. 12% (P=0.684), and the complication rate was 36% vs. 12% (P=0.047) in the TEG and PEG. ConclusionsOur results indicate that the use of permanent embolic agents during TAE is optimal. These agents are superior to temporary embolic agents in terms of intraoperative blood loss, operative time, and improved surgical field visualization, which thereby facilitate the surgical resection of Shamblin type Ⅱ/Ⅲ CBTs while reducing postoperative complications.

ObjectiveTo compare the differences between clinical and environmental sources of Acinetobacter baumannii (AB) at a tertiary hospital in Guangzhou in terms of molecular epidemiological characteristics, antimicrobial resistance, disinfectant tolerance, virulence and the distribution of related genes, as well as plasmid epidemiological characteristics; and to assess the association between environmental and clinical strains and the potential transmission risks associated with environmental contamination. MethodsA total of 118 clinical and 102 environmental AB isolates were collected from a tertiary hospital in Guangzhou between 2024 and 2025. Species identification was performed using mass spectrometry and 16S rRNA sequencing; minimum inhibitory concentrations (MICs) for eight antimicrobial agents were determined using the agar dilution method; biofilm formation capacity was assessed using the crystal violet staining method; A Galleria mellonella infection model was used to evaluate the virulence; whole-genome sequencing was employed for multilocus sequence typing (MLST), phylogenetic analysis, and studies on the distribution of resistance, disinfectant resistance and virulence genes, and plasmid screening. ResultsA total of 38 known ST types were identified among the 220 AB strains, with ST2 and ST40 being the predominant lineages. Antimicrobial susceptibility testing revealed that the clinical infection group exhibited resistance to minocycline, imipenem and other antibiotics, as well as the tolerance levels to the disinfectants triclosan and benzalkonium bromide, were generally higher than those in the environmental group (P<0.001), and the detection rates of drug resistance genes such as blaOXA-23 and adeB all exceeded 70.0%. Statistical analysis confirmed that the MIC values of TCS and BAB were significantly positively correlated with various antibiotic resistance phenotypes (P<0.05). In biofilm experiments, the proportion of strong biofilm-forming strains in the environmental group (64.7%) was significantly higher than that in the clinical infection group (11.9%;χ²=69.06, df=2, P<0.000 1), and the mortality rate of the strains increased with enhanced biofilm-forming ability. Furthermore, a total of 41 plasmid lineages and 219 insertion sequences were detected; ISs such as IS1006 and IS1007 were frequently co-occurring with resistance and virulence genes including blaOXA-23 and katA. ConclusionsThe predominant lineages of AB in this tertiary hospital were ST2 and ST40. The clinical infection group generally exhibits higher levels of antibiotic resistance and tolerance to disinfectants compared to the environmental group, while biofilm-forming ability differs : the environmental group dominates in strong biofilm formation, and the clinical group dominates in weak to moderate biofilm formation. Plasmids and ISs may be involved in the co-occurrence and dissemination control strategies and for the molecular epidemiological surveillance of multidrug-resistant bacteria.

ObjectiveTo investigate whether photoreceptor-specific knockout of lanosterol synthase (Lss) induces age-related retinal degeneration and to elucidate the underlying molecular mechanisms. MethodsPhotoreceptor-specific Lss knockout mice (Lssflox/flox; Rho-Cre, CKO) models were established. Retinal structure and function were assessed at 1, 4, and 8 months of age using fundus photography, optical coherence tomography (OCT), scotopic electroretinogram (ERG) and hematoxylin and eosin (HE) staining. The mouse photoreceptor cell line 661W served as an in vitro model. Lss was knocked down via siRNA, followed by analyses including Seahorse XF mitochondrial stress test, JC-1, MitoSOX Red and DCFH-DA probes to evaluate mitochondrial function and oxidative stress, glycolytic rate assay for glycolysis ability, RT-qPCR for inflammatory factors, γH2AX immunofluorescence for double-strand DNA damage detection, and transcriptome sequencing to identify differentially expressed genes and enriched pathways. ResultsCompared with controls, 1-month-old CKO mice showed no abnormalities. Mild fundus lesions appeared at 4 months. By 8 months, CKO mice exhibited significant thinning of the outer nuclear layer, disorganization of the inner/outer segment structure, and markedly reduced amplitudes of the dark-adapted ERG a- and b-waves (P<0.05). Transcriptome analysis revealed 1 842 differentially expressed genes (826 up-regulated, 1 016 down-regulated), with Lss being the most significantly downregulated gene; these genes significantly enriched in sterol biosynthesis, oxidative phosphorylation, aerobic electron transport chain, and ATP synthesis (P<0.05). In 661W cells, knockdown of Lss significantly reduced mitochondrial basal respiration (P=0.006), ATP production (P=0.015), maximal respiration (P<0.001), and spare respiratory capacity (P<0.001), while basal glycolysis (P=0.005) and compensatory glycolysis (P=0.028) were increased. These changes were accompanied by mitochondrial membrane potential depolarization (P<0.001), elevated mitochondrial reactive oxygen species (ROS) levels (P<0.001), and increased total ROS levels (P=0.013). Additionally, the mRNA expression of inflammatory factors, including Ccl2 (P=0.004), Ccl5 (P=0.012), Cxcl1 (P<0.001), Cxcl5 (P<0.000 1), Tgfb (P=0.008), Hgf (P<0.000 1) and Mmp3 (P=0.003) was significantly upregulated. Moreover, γH2AX fluorescence intensity was increased (P=0.002). ConclusionsPhotoreceptor-specific Lss deficiency induces mitochondrial dysfunction, oxidative stress and cellular inflammation, ultimately leading to age-dependent retinal degeneration. This study provides novel mechanistic insights and potential therapeutic targets for hereditary and age-related retinal degenerative diseases.
