1.Key Contributors to Fatigue in Disaster Responders: Analysis of the 2024 Noto Peninsula Earthquake

Background:
The well-being of disaster responders is just as important as that of disaster victims as maintaining responders’ health is essential for the effective disaster response. This study aimed to identify the key factors contributing to fatigue in disaster responders.
Methods:
This cross-sectional study analyzed data from disaster responders deployed during the 2024 Noto Peninsula Earthquake in Ishikawa prefecture, Japan. Data were collected using the health management version of the Japanese Surveillance in Post-Extreme Emergencies and Disasters app from January to March 2024. Associations between contributing factors including problems in the work environment, type of work activity, and occupation, and self-reported fatigue score were examined using the t-tests, analysis of variance, and multivariable regression analysis.
Results:
We analyzed 15,067 data entries from disaster responders. Fatigue levels among disaster responders were generally low, with 73.7% of scores between 1 and 3 during the first phase and 82.3% during the second phase. Fatigue was significantly higher in the early response phase (β = 0.32, 95% confidence interval [CI]: 0.27—0.37) and among logisticians (β = 0.23, 95% CI: 0.18—0.28) and Health Emergency Operations Center staff (β = 0.20, 95% CI: 0.14—0.25) than among nonlogisticians and non —Health Emergency Operations Center staff members. Problems in the work environment had the largest effects: inability to take meals and breaks increased fatigue by 1.82 points (95% CI: 1.56—2.07), and unclear tasks or command structures by 0.82 points (95% CI: 0.57—1.06).
Conclusion
This study underscores the importance of establishing a structured command system, strengthening responder training and improving working conditions to safeguard responder well-being.
2.Thrombolysis for Bileaflet Valve Thrombosis.

Between January 1981 and December 1996, we performed valve replacement in 281 patients using bileaflet prosthetic valves in mitral and/or tricuspid positions. Thrombosed valve were seen in 10 patients (7 in mitral, 3 in tricuspid positions). In 5 patients, coumadin had been stopped for several reasons (pacemaker implantation, melena, drug allergy), but in the other 5 patients, anticoagulation was within the therapeutic range at the time of presentation. For thrombolytic therapy urokinase or tissue plasminogen activator were used. The treatment was successful in 5 patients (4 mitral, 1 tricuspid), and unsuccessful in 5 patients (3 mitral, 2 tricuspid). Three of the 5 unsuccessful patients were treated surgically (3 with re-mitral valve replacement, 1 with thrombectomy). Prompt surgical treatment can be used as the first line of therapy for thrombosed valves. Thrombolytic therapy may be useful in some cases of bileaflet valve thrombosis without critical hemodynamic collapse. Doppler echocardiographic assessment of increasing peak velocity and pressure half time is useful for detecting thrombosed valves.
3.Complications and Prognoses of Patients Treated for Stanford Type B Aortic Dissection.

In the past 11 years, we treated 41 patients with Stanford type B aortic dissection. Principally, medical therapy was carried out and surgery was performed only when complications related to the dissection occurred. Twenty two patients (53.7%) had complications, including 5 (12%) with peripheral limb ischemia, 3 (7%) with rupture, 13 (32%) with dilatation of the aorta, 4 (10%) with extension of dissection (type A dissection). Seventeen patients received surgery including palliative operation. Among 41 patients, 3 died due to aortic rupture and 2 died at surgery for type A dissection, while 4 of them had developed proximal extension of the dissection. The 5-year survival rate for all patients was 86.7±6.6%. Long term survival will improve in patients with Stanford type B aortic dissection when the operative mortality for type A dissection is reduced and sound management policies are developed.
4.PLSVC as a Pitfall of Retrograde Cardioplegia.

Retrograde cardioplegia is now an alternative or adjunctive method used worldwide as a cardiac protection during open heart surgery. However, its use involves some limitation. We operated on a patient suffering from aortic stenosis associated with PLSVC. During the operation on this patient for aortic valve replacement, retrograde infusion of cardioplegic solution could not be performed because the coronary sinus was excessively dilated and prevented the balloon from occluding it. Other anomalous lesion of the coronary sinus make the retrograde infusion of the cardioplegic solution difficult and these must always be kept in mind when cardioplegia is infused from the coronary sinus.

From January of 1987 to December 1992, twelve patients (7 males and 5 females, mean age, 52.8 years) underwent emergency surgery for DeBakey type I acute aortic dissection. The surgical procedure was resection of the initial intimal tear and replacement of the ascending aorta (four patients underwent hemiarch replacement). Operative mortality was 41.7% (5/12). Three died in the operating room due to heart failure (2) and uncontrollable bleeding (1). Another two early deaths resulted from extension of the residual false lumen. All surviving patients each had a patent double-channeled aorta and aneurysmal dilatation of the false lumen was noted in 3 patients. There were two late deaths, one due to rupture of the residual false lumen and the other, to stroke during re-operation for enlargement of the residual false lumen. It is apparent from these results that in type I acute aortic dissection extensive operation such as total arch replacement is necessary.
