Mongolian Journal of Health Sciences 2026;92(2):134-138
Background:
Type I hypersensitivity, commonly known as an allergic reaction, is characterised by an IgE-mediated immune response triggered when foreign proteins enter the body. During allergic reactions, cytokines such as IL-4, IL-5, IL-9, and IL-13, secreted by T-helper 2 (Th2) cells, play a crucial role in activating immune effector cells like eosinophils and mast cells, which contribute significantly to airway inflammation and obstruction. Conversely, interferon-gamma (IFN-γ), produced by Th1 cells, suppresses Th2-mediated responses and plays an essential role in regulating allergic inflammation. Therefore, determining the balance of cytokines produced by Th1 and Th2 cells is crucial for understanding the mechanisms underlying allergic responses.
Aim:
We aimed to induce an allergic response in experimental animals using ovalbumin and to determine the levels of IL-4 and IFN-γ cytokines using an in-house enzyme-linked immunosorbent assay (ELISA).
Materials and Methods:
A total of 20 female C57BL/6 mice aged 6–8 weeks were selected for the experiment and randomly divided into two groups: control and experimental groups. The concentrations of IL-4 and IFN-γ cytokines in mouse serum were determined using an in-house enzyme-linked immunosorbent assay (ELISA).
Result:
Compared to the control group, the levels of IL-4 and IFN-γ cytokines in the serum of allergic model-induced C57BL/6 mice were determined to be 513.7 pg/mL and 193.2 pg/mL, respectively. A statistically significant difference was observed in IL-4 cytokine levels between the two groups (p < 0.001).
Conclusion
In the allergic model group, serum levels of IL-4 cytokine were elevated, while IFN-γ cytokine levels were decreased, suggesting a predominant Th2-type immune response.

Mongolian Journal of Health Sciences 2026;96(6):151-155
Background:
High-resolution esophageal manometry (HREM) is the reference standard for the assessment of esophageal motor function and classification of esophageal motility disorders. HREM was introduced into clinical practice in Mongolia in 2022; however, the spectrum of manometric diagnoses and factors associated with major esophageal motility disorders in this population have not been systematically characterized.
Aim:
To characterize major esophageal motility disorders among adults undergoing HREM and to identify clinical factors independently associated with these disorders.
Materials and Methods:
This retrospective observational study included 1,033 adults who underwent HREM at Intermed Hospital between September 2022 and March 2026. Manometric studies were interpreted according to the Chicago Classification version 4.0 (CCv4.0). Major esophageal motility disorders were defined as achalasia, manometric esophagogastric junction outflow obstruction (EGJOO), distal esophageal spasm, hypercontractile esophagus, and absent contractility. Multivariable logistic regression was used to identify factors independently associated with major motility disorders.
Result:
Major esophageal motility disorders were identified in 112 patients (11.2%). Compared with patients with normal motility or ineffective esophageal motility, those with major motility disorders had a significantly lower mean body mass index (BMI) (24.7±4.9 vs. 27.5±4.2 kg/m; p<0.001) and a higher prevalence of dysphagia (50.5% vs. 21.3%; p<0.001) and referral for suspected esophageal motility disorders (70.5% vs. 25.2%; p<0.001). In multivariable analysis, older age per 10-year increment (adjusted odds ratio [aOR], 1.35; 95% confidence interval [CI], 1.12–1.61), lower BMI (aOR per 1-kg/m increase, 0.90; 95% CI, 0.85–0.95), dysphagia (aOR, 1.95; 95% CI, 1.20–3.15), and referral for suspected motility disorders (aOR, 5.47; 95% CI, 3.22–9.31) were independently associated with major esophageal motility disorders.
Conclusion
1. Major esophageal motility disorders were identified in 11.2% of adults undergoing HREM in this clinical cohort.
2. Referral for suspected motility disorders was the strongest clinical correlate, while older age, lower BMI, and dysphagia were also independently associated with major motility disorders.

Mongolian Journal of Health Sciences 2026;95(5):50-56
Background:
According to the 2024 national health statistics of Mongolia, colorectal cancer accounts for 5.1% of all newly diagnosed cancers, with 74.1% detected at an advanced stage. Although population-based screening programs have recently been implemented, no studies have established a risk factor–based scoring system with an optimal cut-off value or evaluated its clinical utility in resource-limited settings in Mongolia.
Aim:
To develop a modified APCS (mAPCS) scoring system and evaluate its clinical applicability for colorectal cancer screening.
Materials and Methods:
A hospital-based prospective cross-sectional study was conducted between December 2024 and February 2026 at the outpatient clinics of the Mongolian National University of Medical Sciences and the Mongolia–Japan Hospital. A total of 447 eligible participants were enrolled. Anthropometric measurements (height and weight) were obtained, and six categories of questionnaires (demographic data, APCS risk score, behavioral factors, medical history, and medication use) were administered. Eleven participants declined colonoscopy; therefore, 436 participants underwent colonoscopic examination, with histopathological evaluation performed when indicated.
Results:
A total of 436 participants were included, of whom 282 (64.7%) were female, with a mean age of 54.8±11.5 years. Colorectal adenomas (advanced and non-advanced) and colorectal cancer were detected in 199 (45.6%) participants. Among them, 43 (21.6%) had advanced colorectal neoplasia. Multivariable regression analysis identified age ≥55 years (OR 4.8), BMI ≥28 kg/m² (OR 3.6), and a family history of colorectal cancer (OR 3.1) as independent risk factors for advanced neoplasia. The optimal cut-off value for the mAPCS score was 4, with an overall diagnostic accuracy of 41.7% (sensitivity 97.7%, specificity 35.6%).
Conclusions
1. Age ≥55 years, BMI ≥28 kg/m², and a family history of colorectal cancer were identified as independent risk factors for advanced colorectal neoplasia. At a cut-off value of ≥4, the mAPCS scoring system demonstrated high sensitivity (97.7%), low specificity (35.6%), high negative predictive value (99.2%), and low positive predictive value (14.2%). 2. Therefore, the mAPCS scoring system may serve as a useful tool for risk stratification and efficient allocation of limited resources in colorectal cancer screening.

Mongolian Journal of Health Sciences 2026;95(5):77-81
Background:
Inflammatory bowel disease (IBD) is a chronic immune-related condition caused by interactions between genetics, immune dysfunction, gut microbiota changes, and environmental factors, often presenting as persistent or recurring diarrhea. Clinically, it commonly presents with chronic diarrhea and other gastrointestinal symptoms. The global prevalence of IBD is increasing, currently affecting approximately 0.5% of the population, with projections suggesting a rise to nearly 1% by 2030. Fecal calprotectin is a neutrophil-derived S100 calcium-binding protein that is elevated in intestinal mucosal inflammation and serves as a reliable noninvasive biomarker of inflammatory activity. Due to its high stability and close association with intestinal inflammatory activity, fecal calprotectin has been established as a dependable, non-invasive marker for identifying intestinal inflammation. In clinical practice, it is extensively used to distinguish IBD from functional gastrointestinal disorders, especially irritable bowel syndrome, among patients presenting with chronic diarrhea. However, its diagnostic utility in Mongolia remains underexplored, and this study aims to determine its sensitivity and specificity in inflammatory bowel disease and compare its diagnostic performance with other inflammatory markers.
Aim:
To determine the sensitivity and specificity of fecal calprotectin in inflammatory bowel disease and to compare it with other inflammatory markers.
Materials and Methods:
A total of 109 participants aged 18-87 years with chronic diarrhea, who attended the gastroenterology outpatient clinics of the Mongolia–Japan Hospital and Intermed Hospital between December 2024 and December 2025 and provided informed consent, were enrolled in this study. Data were collected using structured questionnaires, and laboratory investigations included complete blood count, C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), and measurement of fecal calprotectin levels from stool samples. Based on fecal calprotectin results, selected participants underwent colonoscopic examination as clinically indicated.
Results:
Among the 109 participants included in the study, 17.4% were diagnosed with inflammatory bowel disease (IBD), with a mean age of 47.41±15.52 years. Fecal calprotectin levels were significantly higher in the IBD group compared to the control group (p<0.001). Fecal calprotectin demonstrated a weak positive correlation with C-reactive protein (CRP) and leukocyte count (p=0.042, p=0.06), while no significant correlation was observed with erythrocyte sedimentation rate (ESR). At a cutoff value of 250 µg/g, fecal calprotectin showed a sensitivity of 94.4%, specificity of 69.2%, positive predictive value (PPV) of 37.8%, and negative predictive value (NPV) of 98.4%. Receiver operating characteristic (ROC) curve analysis identified an optimal cutoff value of approximately 400 µg/g (AUC=0.919, Youden index=0.724), at which the sensitivity remained 94.4% and specificity increased to 78.0%. In contrast, CRP and ESR demonstrated comparatively lower diagnostic performance, with AUC values of 0.640 and 0.644, respectively.
Conclusion
This study demonstrates that fecal calprotectin has superior diagnostic performance compared to conventional inflammatory markers in differentiating inflammatory bowel disease (IBD). An optimal cutoff value of approximately 400 µg/g yielded the highest sensitivity and specificity. Therefore, fecal calprotectin may serve as a valuable tool in clinical practice for both screening and diagnostic confirmation.
5.Characterization of gastroesophageal reflux disease phenotypes using 24-hour multichannel intraluminal impedance–pH monitoring study
Ankhbayar D ; Amarjargal B ; Bira N ; ; Sarantuya G ; ;
Mongolian Journal of Health Sciences 2026;95(5):212-216
Background:
In contemporary clinical practice, gastroesophageal reflux disease (GERD) is classified into four phenotypes based on upper gastrointestinal endoscopy and 24-hour multichannel intraluminal impedance-pH monitoring: erosive reflux disease (ERD), non-erosive reflux disease (NERD), reflux hypersensitivity (RH), and functional heartburn (FH). Although the prevalence of GERD in Mongolia is high, and certain chronophysiological aspects have been investigated, there remains a significant lack of data regarding GERD phenotypic classification and physiological diagnostic evaluation using impedance-pH monitoring. This knowledge gap provided the rationale for conducting the present study.
Aim:
To determine the prevalence of GERD phenotypes among adults presenting with heartburn symptoms and to compare selected manometric and pH-impedance parameters among phenotype groups.
Materials and Methods:
This study analyzed data from 347 adults who presented to Intermed Hospital between 2022 and 2025 with heartburn symptoms and underwent 24-hour multichannel intraluminal impedance-pH monitoring. Statistical analysis was performed using SPSS version 26.
Results:
Among the study population, 193 patients (55.6%) were diagnosed with ERD, 34 (9.8%) with NERD, 73 (21.0%) with RH, and 47 (13.5%) with FH. Significant differences were observed among the groups in body mass index (BMI), esophageal acid exposure, total reflux episodes, reflux characteristics, and mean nocturnal baseline impedance (MNBI). Type III esophagogastric junction morphology was identified in 78.76% of the ERD group, 64.7% of the NERD group, 56.2% of the RH group, and 42.6% of the FH group, demonstrating statistically significant intergroup differences.
Conclusion
ERD was the predominant GERD phenotype among adults with heartburn symptoms, followed by RH, FH, and NERD. Significant differences among GERD phenotypes were observed in BMI, total reflux burden, reflux characteristics, esophageal nocturnal baseline impedance, and esophagogastric junction morphology.

Mongolian Journal of Health Sciences 2026;95(5):270-274
Background:
Liver fibrosis is the underlying pathology of chronic liver disease complications, including cirrhosis and hepatocellular carcinoma, and its early detection is essential for preventing mortality and disease progression. Liver biopsy remains the gold standard for diagnosing fibrosis; however, it is invasive, costly, limited in repeatability, and not suitable for population-level screening. Moreover, liver fibrosis is often asymptomatic for a prolonged period, necessitating reliable and accessible methods for early identification of at-risk populations. Therefore, non-invasive approaches based on routine laboratory parameters, particularly the FIB-4 index, have been widely adopted in clinical practice.
Aim:
To assess liver fibrosis using the FIB-4 index and to identify risk factors associated with indeterminate and high-risk groups among individuals participating in a national early detection program in Mongolia.
Materials and Methods:
A total of 37,961 adults who met the inclusion and exclusion criteria were selected from 447,409 individuals who participated in the national early detection screening program between 2022 and 2023. The FIB-4 index was calculated using age, aspartate aminotransferase (AST), alanine aminotransferase (ALT), and platelet count, and categorized as low (<1.3), indeterminate (1.3–2.67), and high (≥2.67) risk. Body mass index, blood pressure, fasting glucose, triglycerides, and viral hepatitis status were classified according to standard criteria. Statistical analyses were performed using SPSS version 21.0. Continuous variables were expressed as mean±standard deviation, categorical variables were compared using the χ² test, and logistic regression analysis was used to identify risk factors.
Results:
The mean age of participants was 44.96±11.31 years, with 61.3% (n=23,285) female and 38.7% (n=14,676) male. The mean FIB-4 index was 0.89±1.09. Based on FIB-4, 86.9% were classified as low risk, 11.8% as indeterminate risk, and 1.4% as high risk; overall, 13.1% (n=4,975) were in the indeterminate and high-risk groups. FIB-4 risk increased progressively with age, reaching the highest level in individuals aged ≥60 years. In multivariable logistic regression analysis, male sex (OR=1.45; 95% CI: 1.36–1.56), viral hepatitis (OR=1.85; 95% CI: 1.68–2.03), and increasing age remained independently associated with indeterminate and high risk. Underweight status and very high triglyceride levels were associated with increased risk, whereas overweight, obesity, and central obesity showed an inverse association.
Conclusions
1. The FIB-4 index is a simple, cost-effective tool for identifying individuals at risk of liver fibrosis in primary healthcare settings. Male sex, older age, and viral hepatitis were independent risk factors for indeterminate and high-risk groups. 2. Therefore, the FIB-4 index can be implemented as an initial risk stratification tool in national screening programs to identify high-risk populations for further evaluation and management.
7.The current status of occupational diseases among Railway workers
Mongolian Medical Sciences 2026;216(2):41-50
Introduction:
Occupational diseases remain a significant public health concern, especially among workers
exposed to hazardous environments such as the railway sector.
Materials and Methods:
A descriptive cross-sectional study design was employed using three official data sources,
including national and institutional registries. Data were processed using Microsoft Excel
and SPSS software. Ethical approval for this study was obtained from the Research Ethics
Committee of “Ach” Medical University.
Results:
A total of 29 occupational disease cases were registered during the study period, of which
86.1% were male workers. The highest prevalence was observed in the 40–59 age group.
Neurological disorders (51.7%) were the most common, followed by respiratory diseases
and ear, nose, and throat disorders. A sharp increase in cases was observed in 2024, likely
associated with improved detection and reporting systems.
Conclusion:
1. The relatively high prevalence of occupational diseases among male workers aged 40
59 suggests a strong association with occupational conditions and workplace risk factors,
indicating that prolonged exposure to hazardous working environments increases the risk
of developing occupational diseases.
2. The relatively frequent occurrence of occupational diseases among locomotive drivers
and repair workers is associated with physical, chemical, and ergonomic workplace risk
factors.
3. The structure of occupational diseases is predominantly characterized by neurological
disorders, respiratory diseases, ear, nose, and throat diseases, and occupational
poisoning.

Backround
Peutz–Jeghers (PJ) syndrome is a rare autosomal dominant disorder
characterized by a mucocutaneous pigmentationon on oral mucosa and
multiple hamartomatous polyps located in the digestive tract except esophagus.
PJ syndrome can be diagnosed in early childhood by a characteristic pigmentation
and family history of polyposis. However, it is often diagnosed first
as a polyp in the small intestine that causes obstruction and intussusception
and is often treated with a bowel resection. If diagnosed in young childhood,
an effective non-invasive method is to resect the polyps by tying off the blood
supply to the polyps, that is the method named ischemic polypectomy, before
they grow to the point of obstruction using a endoscopy. PJ syndrome is rare
in Mongolia, but in severe cases, small intestine polyps are treated only surgically.
Double-balloon-endoscopy (DBE) has been performed at the Mongolian-
Japanese Hospital since 2023, making it possible to diagnose and treat
the syndrome endoscopically. Our patient, a 15-year-old boy, had a mucocutaneous
pigmentation that had been previously undiagnosed and was first diagnosed
with intussusception at the age of 13. He had undergone 4 endoscopic
procedures for upper and lower gastrointestinal polyps at the National Center
for Maternal and Child Health successfully. In our hospital, we found endoscopically
multiple hamartomatous polyps of various sizes between 1-3 cm,
and a 3 mm diameter tumor that filled 3/4 of the intestinal lumen was treated
by ischemic polypectomy.
After the procedure, there were no early or late complications related to
the procedure. The child's condition improved, the main complaints subsided,
and he continues his daily life normally. However, follow-up DBE is required.

Background:
Cancer incidence and mortality are steadily increasing both
globally and in Mongolia. As these rates rise, traditional Mongolian medicine
has long utilized herbal formulas for the treatment of gastric and esophageal
cancers and precancerous conditions. One such formulation—Hot Natured
3 Herbs (HN3H)—comprises three species from the Ranunculaceae family:
Atragene sibirica L., Ranunculus repens L., and Pulsatilla bungeana L.. However,
scientific validation of its anti-tumor effects is essential. This study aimed
to investigate the effect of HN3H in a tumor-induced animal model.
Aim:
To identify the biologically active compounds of HN3H and evaluate their
effect in an experimentally induced tumor model in animals.
Materials and Methods:
The three herbs comprising HN3H—Atragene sibirica
L., Ranunculus repens L., and Pulsatilla bungeana L.—were collected
during their flowering stage (May–June) in Khishig-Undur, Bulgan province,
and dried according to official procedures. Extraction was carried out by maceration
in 96% ethanol at a 1:10 ratio. The concentrated extract was suspended
in water (1:1) and successively fractionated with dichloromethane, ethyl
acetate, butanol, chloroform, and n-hexane. The study was approved by the
Research Ethics Committee of the Mongolian National University of Medical
Sciences (Protocol №2020/03-04). A colorectal cancer model was established
by subcutaneous injection of MC-38 cells (Kerafast, USA) into C57BL/6 mice.
Immunohistochemistry was performed using CK20, CDX2, Ki67, and p53 antibodies
at 1:100 and 1:200 dilutions.
Results:
The ethanol extract of HN3H contained 2.98±0.04% total phenolics
and 2.16±0.05% total flavonoids. Body weight and tumor volume were
measured daily with three repetitions. All groups showed a time-dependent
increase in body weight. Mice in groups 1A and 1B received ethanol extract at
50 and 100 mg/kg doses; groups 2A and 2B received dichloromethane extract
at the same doses. The negative control group was administered 0.5 mg/kg
PBS orally, while the positive control group received intraperitoneal injections
of 5-fluorouracil (5FU) at 10 mg/kg twice a week. Tumor growth increased in a
time-dependent manner across groups. Compared to the negative control, tumor
volumes in four treatment groups showed statistically significant reduction
(p˂0.05), while no significant difference was observed when compared to the
positive control (p=0.08). Histological analysis revealed necrosis in all groups,
with variation in extent.
Conclusion
The ethanol extract of HN3H exhibited moderate levels of phenolic
compounds and a high concentration of flavonoids. HN3H extract inhibited
tumor progression and activated lymphocyte-predominant inflammation in
tumor tissues, indicating potential anti-tumor activity (p˂0.05).

Background:
Asthma is a chronic inflammatory disease characterized by bronchoconstriction and breathing difficulties,
which can be triggered by both allergic and non-allergic mechanisms. In allergic asthma, a hyperreactivity reaction
occurs, leading to an increase in fractional exhaled nitric oxide (FeNO) levels. Therefore, FeNO is used to differentiate
allergic and non-allergic types of asthma. In recent years, studies have been conducted to assess FeNO as an indicator of
treatment effectiveness and symptom control. Our goal is to introduce FeNO measurement in Mongolia for the first time
and utilize it for asthma diagnosis and treatment outcome evaluation.
Aim:
Study of FeNO levels in Asthma Diagnosis in Relation to Laboratory Findings and Lung Function
Materials and Methods:
This study included a total of 70 adult patients (18 years and older) with asthma who visited the
Outpatient Clinic of Mongolia-Japan Hospital and Shargaljuut Spa Clinic from May to December 2024. Based on their
allergic history, total IgE levels, and peripheral blood eosinophil count, participants were classified into allergic (>5%)
and non-allergic (<5%) asthma. The study aimed to determine the diagnostic specificity and sensitivity of FeNO levels.
Asthma control levels were assessed using the Asthma Control Test (ACT), lung function was evaluated using spirometry,
and exercise capacity was measured with the 6-minute walk test.
Results:
In a single measurement, there were no differences between the allergic and non-allergic groups in parameters
other than total IgE, peripheral blood eosinophil count, and FeNO levels. For diagnosing the allergic form of asthma, at
a FeNO level of 25 ppb, sensitivity (Se) was 0.68, specificity (Sp) was 0.30 (p=0.008); at 40 ppb, Se was 0.77, Sp was
0.31 (p=0.001); and at 50 ppb, Se was 0.74, Sp was 0.33 (p=0.001). Lung function and exercise capacity were similar in
both asthma groups.
Conclusion
The sensitivity of FeNO in distinguishing between allergic and non-allergic forms of asthma is 67-81%, and
its sensitivity tends to increase as the FeNO level rises. By distinguishing the allergic type in which inhaled corticosteroids
are more effective, the outcome of asthma treatment can be predicted in advance based on the FeNO level.
