1.Anti-neuroinflammatory Effects of 12-Dehydrogingerdione in LPS-Activated Microglia through Inhibiting Akt/IKK/NF-κB Pathway and Activating Nrf-2/HO-1 Pathway.

Ginger, one of worldwide consumed dietary spice, is not only famous as food supplements, but also believed to exert a variety of remarkable pharmacological activity as herbal remedies. In this study, a ginger constituent, 12-dehydrogingerdione (DHGD) was proven that has comparable anti-inflammatory activity with positive control 6-shogaol in inhibiting LPS-induced interleukin (IL)-6, tumor necrosis factor (TNF)-α, prostaglandin (PG) E₂, nitric oxide (NO), inducible NO synthase (iNOS) and cyclooxygenase (COX)-2, without interfering with COX-1 in cultured microglial cells. Subsequent mechanistic studies indicate that 12-DHGD may inhibit neuro-inflammation through suppressing the LPS-activated Akt/IKK/NF-κB pathway. Furthermore, 12-DHGD markedly promoted the activation of NF-E2-related factor (Nrf)-2 and heme oxygenase (HO)-1, and we demonstrated that the involvement of HO-1 on the production of pro-inflammatory mediators such as NO and TNF-α by using a HO-1 inhibitor, Zinc protoporphyrin (Znpp). These results indicate that 12-DHGD may protect against neuro-inflammation by inhibiting Akt/IKK/IκB/NF-κB pathway and promoting Nrf-2/HO-1 pathway.

2.Prognostic Threshold of Neuroendocrine Differentiation in Gastric Carcinoma: a Clinicopathological Study of 945 Cases

PURPOSE: The significance of neuroendocrine differentiation (NED) in gastric carcinoma (GC) is controversial, leading to ambiguous concepts in traditional classifications. This study aimed to determine the prognostic threshold of meaningful NED in GC and clarify its unclear features in existing classifications. MATERIALS AND METHODS: Immunohistochemical staining for synaptophysin, chromogranin A, and neural cell adhesion molecule was performed for 945 GC specimens. Survival analysis was performed using the log-rank test and univariate/multivariate models with percentages of NED (PNED) and demographic and clinicopathological parameters. RESULTS: In total, 275 (29.1%) cases were immunoreactive to at least 1 neuroendocrine (NE) marker. GC-NED was more common in the upper third of the stomach. PNED, and Borrmann's classification and tumor, lymph node, metastasis stages were independent prognostic factors. The cutoff PNED was 10%, beyond which patients had significantly worse outcomes, although the risk did not increase with higher PNED. Tumors with ≥10% NED tended to manifest as Borrmann type III lesion with mixed/diffuse morphology and poorer histological differentiation; the NE components in this population mainly grew in insulae/nests, which differed from the predominant growth pattern (glandular/acinar) in GC with <10% NED. CONCLUSIONS: GC with ≥10% NED should be classified as a distinct subtype because of its worse prognosis, and more attention should be paid to the necessity of additional therapeutics for NE components.

3.Inhibitory effect of Corynebacterium parvum cell wall on bladder cancer cells

Objective To test the inhibitory effect of Corynebacterium cell wall extract on bladder cancer cells. Methods The bladder RNA was extracted from bladder cancer rats, and concentration and purity of RNA was detected. The extracted RNA was reversely transcribed into cDNA, then the primers were designed and the Grim19 gene, β-actin gene, Stat3 gene was amplified. Finaly the PCR product was subjected to agarose gel electrophoresis. Results OD260 was 0.07, and OD260/OD280 was 2.03 in the group of Corynebacterium parvm extract(3-5# ); OD260 was 0.12, and OD260/OD280 was 2.07 in the group of MNU( 6-5#);OD260 was 0.08, and OD260/OD280 was 2.07 in the group of physiological saline(7-5#) . The results of agarose gelelectrophoresis showed that Grim19 gene and Stat3 gene was expressed highly in the bladder of rats from the group of CP cell wall extract (3-5# ). Grim19 gene was expressed lowly, while Stat3 gene was expressed highly in the bladder of rats from the group of MNU (6-5#). Grim19 gene and Stat3 gene was expressed normally in the bladder of rats from physiological saline(7-5). Conclusions The expression situation of antitumor gene Grim19 and tumor gene Stat3 in the bladder of rats was inhibited by the cell wall extract of Corynebacterium parvm, which indicates that the cell wall extract of Corynebacterium parvm has inhibitory effect on bladder cancer cells.
4.Role of Working Memory in Lexical Access: An Event-related Potential Study Based on Word-picture Matchin Tasks

Objective To observe the role of working memory in the process of language, and provide an objective evaluation for Chinese language rehabilitation.Methods From December, 2017 to June, 2018, 20 healthy subjects accepted word-picture matching tasks as the word and picture showed on the different (task 1) or same (task 2) screens, and their event-related potential of N400 were compared in matched and mismatched conditions.Results Forteen subjects were included finally. Task 1 induced N400 appeared earlier with higher amplitude and more activation in central parietal occipital region, followed larger N170 and P1 wave. There were significant differences in most cerebral regions between the two tasks in N400 amplitude difference of matched and mismatched conditions (t> 2.319, P < 0.05).Conclusion Word-picture matching tasks may involve more language-related brain regions with the intervention of working memory, that might work better in process of vocabulary. The tasks simulating the scene of Chinese language rehabilitation can be used as an objective evaluation for clinical activities.
5.Risk factors and pathogen distribution of nosocomial infection in adult patients undergoing extracorporeal membrane oxygenation after cardiac surgery

Objective To analyze the risk factors and pathogen distribution of nosocomial infection in adult patients undergoing extracorporeal membrane oxygenation (ECMO) after cardiac surgery.Methods The clinical data of 325 patients who underwent ECMO support in the Department of Adult Cardiac Surgery of Beijing Anzhen Hospital from January 2012 to December 2017 were retrospectively analyzed.There were 132 patients with nosocomial infection (infected group) and 193 patients without nosocomial infection (noninfected group).Multivariate logistic analysis was used to analyze the risk factors of nosocomial infection,and the distribution of pathogens in infected patients was also analyzed.Results Among 132 infected patients,67 cases (50.76%) had respiratory infection,40 cases (30.30%) had blood infection,15 cases (11.36%) had surgical wound infection,and 10 cases (7.58%) had urinary tract infection.The case fatality rates of the infected group and non-infected group were 61.37% (81/132) and 52.85% (102/ 193),respectively (x2 =6.356,P < 0.05).Univariate analysis showed that long operation time,application of extracorporeal circulation and long-term ECMO support were associated with nosocomial infection.Multivariate logistic regression analysis found that ECMO time (OR =5.565,95% CI =2.868-10.799,P <0.01) was an independent risk factor for nosocomial infection after ECMO treatment in adult patients after cardiac surgery.Among the 132 infected patients,364 strains of pathogens were detected,of which 233 were Gram-negative bacteria,including 79 strains of Acinetobacter Baumann (33.9%) and 32 strains of K.pneumoniae (13.73%);101 strains were Gram-positive bacteria,including Streptococcus aureus (14.85%),Micrococcus (14.85%) and catarrhal bacteria (14.85%);30 strains were fungi and Candida albicans accounted for 70% of fungi infections.Conclusion The long ECMO support time is an independent risk factor for ECMO-related hospital infections in adult patients undergoing cardiac surgery,and the pathogens are mainly Gram-negative bacteria.
6.Clinical effect of Radical TUR-BT combined with intra-arterial chemotherapy for localized muscle-invasive bladder cancer

Objective To evaluate the clinical efficacy of Radical TUR-BT combined with intra-arterial chemotherapy in the treatment of stage T2 N0 M0 localized muscle-invasive bladder cancer (MIRC).Methods The clinical data of 91 patients with T2 N0 M0 localized myometrial invasive bladder cancer admitted from January 2010 to December 2015 in the people's Hospital of Xinjiang Uyghur Autonomous Region were retrospectively analyzed,76 cases were males,15 cases were female,average age was 63.2 years old,age range 39-91 years old.Patients were divided into BPT group (n =34) and RC group (n =57) according to different surgical methods.Patients in the BPT group underwent Radical TUR-BT + intra-arterial chemotherapy,and patients in the RC group underwent radical cystectomy + urinary diversion.The median follow-up time,2-year progression-free survival,5-year disease-specific survival rate,overall survival and Karnofsky performance status (KPS) were compared between the two groups.Measurement data were expressed as mean ± standard deviation (Mean ± SD),t test was used for comparison between groups;and Chi-square test was used to compare the count date between groups.Using KaplanMeier method to depict disease-specific survival time in two groups.Quality of life evaluation using Karnofsky score,Evaluate the quality of life and health status of the two groups 3 months before and after treatment.Results The median follow-up time of BPT group and RC group were 35 months and 33 months.The 2-year progression-free rates in the BPT and RC groups were 73.5% and 75.4%,respectively,and the difference was not statistically significant (P =0.758).The 5-year disease-specific survival rates of the two groups were 72.4% and 74.8%,respectively,and the difference was not statistically significant (P =0.780).The overall survival rates of the two groups were 61.8% and 63.2%,respectively,and the difference was not statistically significant (P =0.103).There was no significant difference in preoperative KPS score between the two groups (P =0.652).The KPS score of the BPT group was significantly higher than that of the RC group at 3 months after surgery.The difference was statistically significant(P =0.034).Conclusions For patients with localized MIBC with clinical stage of T2 N0M0.Radical TUR-BT combined with intra-arterial chemotherapy has similar effects to RC.This treatment can effectively improve the prognosis,preserve the bladder,and improve the quality of life for patients.
7.Clinical features of hemophagocytic syndrome in children and significance of gene detection

Objective To explore the clinical features of hemophagocytic syndrome in children and the signifi﹣cance of gene detection. Methods TWenty-tWo pediatric patients diagnosed as hemophagocytic syndrome since 2004 clinical and laboratory criteria Were enrolled in Children's Hospital Affiliated to Zhengzhou University from January 2014 to January 2016. The clinical data of patients Were analyzed,and the genes associated With hemophagocytic syn﹣drome Were detected. The clinical biochemical indicators Were compared betWeen mutation group and non -mutation group. Results TWenty-tWo cases of patients(3 months to 12 years)Were enrolled,including 10 males and 12 fe﹣males,and the proportion of children over 5 years old accounted for the highest proportion,accounting for 50%,and all of them had fever,liver,spleen and lymph node enlargement. The main test results Were as folloWs:peripheral blood cells decreased in 6 cases( 27. 27%),hemophagocytic phenomena presented in bone marroW smears in 12 cases (54. 55%),abnormal liver function in 18 cases(81. 82%),and loW serum albumin in 22 cases(100. 00%). High serum ferritin levels Were detected in 20 cases(90. 91%);the detection of natural killer(NK)cell activity shoWed nor﹣mal activity( active > 15%) in 7 cases( 31. 82%),and decreased activity( activity ≤ 15%) in 15 cases (68. 18%). The genes associated With hemophagocytic syndrome Were detected in 22 cases of patients,and 12 of them Were associated With mutations related to hemophagocytic syndrome,accounting for 54. 55%. LYST,ITK and UNC13D gene Were common. There Was no statistical difference in Which ages of onset,symptoms of the nervous system,and labo﹣ratory data of leukocyte count,red blood cell count,hemoglobin,platelet count,NK cell activities,prognosis,hemophago﹣cytic phenomena shoWed in bone marroW smears,alanine aminotransferase,albumin,triglyceride,ferritin and fibrinogen betWeen mutation group and non-mutation group(all P>0. 05). Conclusions Pediatric hemophagocytic syndrome is mostly accompanied by fever,liver,spleen and lymph node enlargement,and most of them are accompanied by gene mu﹣tations. LYST,ITK and UNC13D gene are commonly seen. But there is no significant correlation betWeen gene mutation and general condition,biochemical index and severity of the disease.
8.Serum levels of miR‐21 and miR‐155 in T2DM + CHD patients and their relationship with lipid me‐tabolism/

To explore serum levels of miR‐21 and miR‐155 in patients with T2DM complicated CHD and their relationship with lipid metabolism .Methods : A total of 134 T2DM patients treated in our hospital from 2016 to 2017 , were divided into T2DM + CHD group (n=60) and pure T2DM group (n=74).Blood glucose and blood lipid levels and serum miR‐21 and miR‐155 levels were measured and were compared between two groups .Results :There were no significant difference in general data , blood pressure , body mass index (BMI) , glycosylated hemo‐globin A1c (HbA1c) , plasma glucose and total cholesterol (TC) between two groups , P>0. 05 all.Compared with pure T2DM group , there were significant rise in levels of triglyceride (TG) [ (1. 89 ± 0.92) mmol/L vs.(2. 75 ± 1.61) mmol/L] , LDL‐C [ (2.83 ± 0.79) mmol/L vs.(3. 52 ± 1.24) mmol/L] and serum miR‐21 [ (0. 93 ± 0. 15) vs.(1. 86 ± 0.24 )] , and significant reductions in levels of HDL‐C [ (1.35 ± 0. 34 ) mmol/L vs.(0. 94 ± 0.31 ) mmol/L] and serum miR‐155 [ (0. 95 ± 0.19) vs.(0. 27 ± 0. 10)] in T2DM + CHD group , P=0.001 all.Multiva‐riate Logistic regression analysis indicated that TG , LDL‐C and miR‐21 were independent risk factors for T2DM +CHD (OR=2. 800~4. 986 , P<0.05 all) , while HDL‐C and miR‐155 were its independent protective factors (OR=0.314 , 0.327 , P< 0.05 both).Pearson correlation analysis indicated that serum miR‐21 level was significant positively correlated with TG and LDL‐C levels ( r=0. 415 , 0.506 , P<0.05 or <0. 01) , and serum miR‐155 level was significant inversely correlated with TG and LDL‐C levels ( r= -0. 397 ,-0. 526 , P<0.05 or <0. 01 ).Con‐clusion : Serum miR‐21 level was significant positively correlated with TG and LDL‐C levels , but serum miR‐155 level was significant inversely correlated with TG and LDL‐C levels ,
9.Impact of flash glucose monitoring on glycemic control in type 1 diabetic patients

Objective The application of flash glucose monitoring ( FGM) in type 1 diabetes could diminish the frequency of hypoglycaemia, however, how FGM improving the glycaemic control target (e.g. HbA1C) is waited to be investigated. Methods Seventy type 1 diabetic patients were recruited from December 2017 to July 2018 in the First Affiliated Hospital of Nanjing Medical University. Their demographic data, use of FGM and HbA1C level were collected. Results We observed that type 1 diabetic patients wearing the FGM showed lower insulin dose [(0.562± 0.059)U/kg vs (0.772±0.049)U/kg, P=0.008)], shorter diabetic duration[2.0(0.8-6.0) vs 6.0(2.5-10), P=0.013], lower HbA1C value [6.8%(6.2%-7.7%) vs 8.0%(6.9%-9.4%), P=0.001]. Nevertheless, after adjusting the potential confounders, the level of HbA1C in patients wearing FGM was significantly lower than those without FGM. Moreover, it was independent of other factors, such as the dose of insulin and the insulin delivery method. Conclusion Type 1 diabetic patients wearing FGM had lower HbA1C value but it needs further follow-up to confirm the improvement of glycaemic control in type 1 diabetes with the use of FGM.

In this study, we proposed an approach to interpret the classification of body constitution based on the Japanese Version of Constitution in Chinese Medicine Questionnaire (CCMQ-J) in terms of an augmented questionnaire combining seven independent questionnaires. The augmented questionnaire consists of 254 questions in terms of seven categories of attributes, which are the (i) basic information (BI), (ii) disease (DI), (iii) social factors (SO), (iv) mental factors (ME), (v) dietary habits (DH), (vi) sleeping state (SL), and (vii) sub-health (SH). The partial least square (PLS) regression has been adopted to model the correlations between the scores of body constitutions and the questions, and their results show that the body constitution can be represented by the linear combination of the questions substantially (correlation coefficients between the true and predicted constitutions are all above 0.7). Moreover, by using the crowdsourcing technique in data collection, a total of 851 samples (350 males and 501 females between 20 and 85 years old) samples with diverse geographical backgrounds in Japan have been collected, from which new medical implications have been extracted through the discussion in a Traditional Chinese Medicine standpoint. This study serves as a crucial step for validating the philosophy of ancient Chinese medicine by the state-of-the-art information science techniques and facilitating the use of the CCMQ-J in public healthcare.
