1.Analysis of risk factors for postoperative new-onset cardiac complications in patients with esophageal cancer and concomitant coronary heart disease

Objective To investigate the factors affecting the occurrence of new postoperative cardiac complications in patients undergoing esophageal cancer surgery with concomitant coronary heart disease. Methods Clinical data of patients who underwent esophageal cancer surgery with coronary heart disease at the Affiliated Huai'an No.1 People's Hospital of Nanjing Medical University from December 2019 to June 2023 were collected. Patients were divided into two groups based on whether they experienced postoperative cardiac complications. Using the occurrence of cardiac complications as the dependent variable, a multivariate logistic regression model was established to identify related influencing factors. Results A total of 223 patients were included, comprising 148 males and 75 females, with an average age of (71.78±6.31) years (range 53-88 years). Seventy-one (31.84%) patients experienced at least one new cardiac complication postoperatively, including 2 acute coronary syndrome, 13 heart failure, and 59 new-onset postoperative arrhythmias. Univariate analysis showed that age, systemic immune-inflammation index, pulmonary infection, need for invasive mechanical ventilation due to respiratory failure, acute respiratory distress syndrome (ARDS), acute delirium, pleural effusion requiring drainage, and acute renal failure were risk factors for postoperative new-onset cardiac complications (all P<0.05). Multivariate logistic regression analysis identified age, postoperative length of hospital stay, ARDS, and systemic immune-inflammation index as independent risk factors for new cardiac complications in esophageal cancer patients with coronary heart disease. Conclusion Strengthening perioperative management of esophageal cancer patients, ranging from preoperative evaluation to postoperative complication treatment, is crucial. Particular attention should be paid to age, ARDS, and other indicators to improve postoperative prognoses in patients with coronary heart disease complicated by esophageal cancer.
2.Mechanisms of sperm quality decline induced by psychological stress: a review

Abstract
Psychological stress is a significant contributing factor for the decline in male sperm quality. In terms of the psychological mechanism, the decline of sperm quality due to psychological stress is associated with increased perceived stress and different coping strategies. External sources of stress can elevate stress perception, then affect sperm quality, while coping strategies may modulate the impact of stress perception on sperm quality. In terms of the physiological mechanism, psychological stress disrupts the normal functioning of the sympathoadrenal system, hypothalamic-pituitary-adrenal axis and hypothalamic-pituitary-gonadal axis, leading to disturbances in the essential endocrine environment for sperm production. This review synthesizes the psychological and physiological mechanisms through which psychological stress influences male sperm quality, providing insights into the prevention and treatment of male infertility.
3.Spatiotemporal distribution characteristics of polychlorinated dibenzo-p-dioxins and dibenzofurans in PM2.5 in Pudong of Shanghai and population health assessment

Background In the Global Burden of Disease research, it has been found that atmospheric fine particulate matter (PM2.5) pollution significantly harms human health. Currently, there is limited research on polychlorinated dibenzo-p-dioxins and dibenzofurans (PCDD/Fs) that exhibit high toxicity effects in PM2.5 . Objective By studying the spatiotemporal distribution and variation characteristics of PCDD/Fs in PM2.5 in Pudong area of Shanghai, to assess the associated population health risk. Methods This study set up 28 sampling points in Pudong area. One sample of PM2.5 was collected during winter (February 2022) and summer (July 2022) at each site, with a sampling period lasting 24 h. The concentration of PM2.5 was measured by membrane filter method, and the content of 17 kinds of 2,3,7,8-substituted chlorinated PCDD/Fs in the samples was analyzed using isotope dilution. Seasonal variations (winter and summer) in the concentrations of PM2.5 and PCDD/Fs were evaluated, sources of PCDD/Fs pollution were tracing by principal component analysis, and health risks to the population from respiratory exposure to PCDD/Fs were estimated by VLIER-HUMAAN model. Results The PM2.5 concentrations in the 28 samples ranged from 10 to 126 μg·m−3, while the concentrations of PCDD/Fs in PM2.5 ranged from 58 to 2625 fg·m−3. The concentration of PM2.5 during winter (11-126 μg·m−3) was higher than that during summer (10-60 μg·m−3). The concentration range of PCDD/Fs in winter was from 58 to 2625 fg·m−3, which corresponded to a range of toxic equivalent quantity (WHO-TEQ) concentration from 2.99 to 40.97 fg·m−3 when taking World Health Organization's toxic equivalency factor (WHO-TEQ); the concentration range of PCDD/Fs in summer was from 72 to 446 fg·m−3, which corresponded to a range of WHO-TEQ concentration from 2.66 to 16.61 fg·m−3. This range in summer was significantly lower than that observed in winter. The results of principal component analysis revealed that waste incineration was the primary source of PCDD/Fs in winter PM2.5 in the area, whereas traffic emissions emerged as the main source in summer. The assessment of Pudong residents' respiratory exposure to PCDD/Fs in PM2.5 showed significantly higher exposure of children in summer and winter than that of adults, indicating higher susceptibility of children to air pollutants. Both the hazard ratios (HR) for children and adults were below 1, while the cancer risks (CR) ranged from 8.41×10−8 to 2.35×10−6. Notably, during winter, the CR at 4 locations slightly exceeds 1×10−6, indicating a potential carcinogenic risk. Conclusion The overall pollution level of PCDD/Fs in PM2.5 in Pudong area is relatively low, but it shows clear seasonal patterns. Waste incineration and traffic are the main sources of PCDD/Fs in PM2.5 in the area. Although the cancer risk of exposure to PCDD/Fs in PM2.5 for children or adults is relatively low, there is a certain risk at some locations in winter, necessitating additional monitoring and control.
4.A novel PAX9 variant in a Chinese family with non-syndromic oligodontia and genotype-phenotype analysis of PAX9 variants

Objective This study aimed to identify PAX9 variants in non-syndromic tooth agenesis families of Chi-na,as well as to analyze the genotype-phenotype of non-syndromic tooth agenesis caused by PAX9 variants,which can provide a basis for the genetic diagnosis of tooth agenesis.Methods We collected the data of 44 patients with non-syn-dromic oligodontia who underwent treatment at Stomatological Hospital of Hebei Medical University between 2018 and 2023.Whole-exome sequencing was performed on the peripheral blood of the proband and its core family members,and the variants were verified by Sanger sequencing.Pathogenicity analysis and function prediction of the variants were per-formed using bioinformatics tools.The correlation between the genotype of PAX9 variant and its corresponding pheno-type was examined by reviewing 55 publications retrieved from PubMed.The studies involved 232 tooth agenesis pa-tients with PAX9 variants.Results A novel PAX9 c.447delG(p.Pro150Argfs*62)and a reported PAX9 c.406C>T(p.Gln136*)were identified in two Chinese families.Through bioinformatics analysis and three-dimensional structural mod-eling,we postulated that the frameshift variant was pathogenic.The outcome was the premature cessation of PAX9 pro-tein,which caused severe structural and functional deficiencies.Summarizing the PAX9 genotype-phenotype relationship revealed that patients carrying the PAX9 variant commonly led to loss of the second molars.Conclusion We identified the novel PAX9 c.447delG(p.Pro150Argfs*62)in a Chinese family of non-syndromic oligodontia,expanding the known variant spectrum of PAX9.The most susceptible tooth position for PAX9 variants of tooth agenesis was the second mo-lars and the deciduous molars during the deciduous dentition.
5.Clinical observation of Ruyi jinhuang powder combined with ICIs in the treatment of advanced liver cancer complicated with dampness and heat syndrome of liver and gallbladder

OBJECTIVE To explore the clinical efficacy and safety of Ruyi jinhuang powder for external application combined with immune checkpoint inhibitors (ICIs) in the treatment of patients with advanced liver cancer complicated with dampness and heat syndrome of liver and gallbladder. METHODS All patients with advanced liver cancer complicated with dampness and heat syndrome of liver and gallbladder were admitted to our hospital from January 2018 to June 2022 and assigned into observation group and control group according to random number table method. Patients in the control group (n=56) were treated with ICIs (Navulizumab injection/Sintilimab injection/Camrelizumab for injection) 200 mg, ivgtt, 21 days as a treatment cycle. Patients in the observation group (n=56) were additionally treated with Ruyi jinhuang powder for external application, once a day, on the basis of control group. The therapeutic effects of 2 groups were compared after a treatment cycle. The levels of interleukin-6 (IL- 6), matrix metalloproteinase-9 (MMP-9), cyclooxygenase-2 (COX-2), prostaglandin E2 (PGE2), carbohydrate antigen 199 (CA199), alpha-fetoprotein (AFP) and vascular endothelial growth factor (VEGF) in serum were compared between 2 groups before and after treatment. Karnofsky functional status (KPS) score, digital rating scale (NRS) score, total symptom score of traditional Chinese medicine, and the occurrence of adverse reactions were recorded for both groups of patients. RESULTS After treatment, the levels of IL-6, MMP-9, COX-2, PGE2, CA199, AFP, VEGF, NRS score and total symptom score of traditional Chinese medicine in observation group were significantly lower than control group (P<0.05), KPS score was significantly higher than the control group (P<0.05). The zypp-04) total effective rate and remission rate of the observation group were 64.29% and 80.36%, those of control group were 60.71% and 73.21%. There was no statistical significance between two groups (P>0.05). The adverse drug reactions of both groups were mainly nausea and vomiting, liver function injury, fever hlshli@yeah.net and so on; the incidence of adverse reaction in observation group was significantly lower than that of control group (P<0.05). CONCLUSIONS In patients with advanced liver cancer complicated with dampness and heat syndrome of liver and gallbladder, the combination of Ruyi jinhuang powder for external application and ICIs can help inhibit the secretion of pain mediators, regulate vascular endothelial function, reduce the inflammatory response, promote the recovery of cardiopulmonary function, improve clinical efficacy and has good safety.
6.Qualitative study of pain experience in patients with rheumatoid arthritis

Objective:To deeply explore the pain experience of patients with rheumatoid arthritis, so as to provide a basis for the practical interventions in the next step.Methods:Using the phenomenological research method, 18 patients with rheumatoid arthritis who experienced pain in the First Hospital of Soochow University from September 2020 to January 2021 were selected for semi-structured interviews, and the Colaizzi 7-step analysis method was used for data analysis.Results:The pain experience of patients with rheumatoid arthritis were summarized into six themes. Pain was complex and erratic, pain relief strategies were ineffective, pain negatively affected daily life, expected more pain relief, seeking help selectively when pain occurs, and experienced pain brings positive change.Conclusions:Medical staff must pay attention to the real experiences and demands of pain in rheumatoid arthritis patients, use information technology and multidisciplinary collaboration to provide patients with effective pain management strategies and encourage patients to make more positive changes to relieve pain symptoms.
7.99Tc m-DTPA SPECT/CT imaging in the diagnosis of cerebrospinal fluid leakage

Objective:To investigate the diagnostic efficacy and clinical application value of 99Tc m-diethylene triamine pentaacetic acid (DTPA) SPECT/CT imaging in cerebrospinal fluid leakage (CSFL). Methods:A total of 23 patients (11 males, 12 females; age (44.2±15.1) years) who underwent endoscopic repair surgery for suspected CSFL in Shanghai Jiao Tong University Affiliated Sixth People′s Hospital between April 2018 and January 2020 were retrospectively reviewed. All patients performed 99Tc m-DTPA SPECT/CT imaging, paranasal sinus high resolution CT (HRCT) and MRI before surgery. The diagnostic efficacies of 3 imaging techniques were calculated according to the result of surgery regarded as the golden standard. χ2 test was used to compare the qualitative and localized diagnostic efficacies of 3 imaging techniques for CSFL. Results:Of 23 patients, 21 were finally confirmed with CSFL and 24 leak locations were identified according to the results of surgery; the other 2 patients had no obvious CSFL and no leak location was found during the operation. The sensitivity and accuracy of 99Tc m-DTPA SPECT/CT, MRI and HRCT for the diagnosis of CSFL were 100%(21/21) and 95.7%(22/23), 85.7%(18/21) and 82.6%(19/23), 76.2%(16/21) and 69.6%(16/23), respectively. The accuracy of 99Tc m-DTPA SPECT/CT, MRI and HRCT for the diagnosis of leak location was 79.2%(19/24), 50.0%(12/24) and 45.8%(11/24), respectively. There was no statistically significant difference of diagnostic efficacies for CSFL among 3 imaging techniques ( χ2 values: 0.451-3.453, all P>0.05). For leak location, the diagnostic efficacy of 99Tc m-DTPA SPECT/CT was significantly better than that of MRI and HRCT ( χ2 values: 4.463, 5.689, both P<0.05). Conclusion:99Tc m-DTPA SPECT/CT imaging shows an excellent diagnosis efficacy not only for CSFL but also for leak location, which is helpful for guiding surgery.
8.Association between self-reported snoring and hypertension among Chinese Han population aged 30-79 in Chongqing, China.

BACKGROUND:
We aim to explore the association between self-reported snoring and hypertension among adults aged 30-79 in Chongqing, China.
METHODS:
A total of 23,342 individuals aged 30-79 were included at baseline from August 2018 to January 2019, and the final sample size for the analysis was 22,423. Face-to-face interviews and physical examinations were conducted by trained investigators. Logistic regression was performed to study age-specific and gender-specific associations between snoring and hypertension.
RESULTS:
Frequent snoring was associated with the risk of hypertension for each age and gender group, and the frequency of snoring was positively correlated with the risk for hypertension. For the three age groups (< 45, 45-59, ≥ 60), compared with the non-snoring group, those who snore often had a 64.5%, 53.3%, and 24.5% increased risk of hypertension (< 45: OR = 1.65, 95%CI 1.34-2.02; 45-59: OR = 1.53, 95%CI 1.37-1.72; ≥ 60: OR = 1.25, 95%CI 1.09-1.42), respectively. For men and women, those who snore often had a 46.8% and 97.2% increased risk of hypertension, respectively, than the non-snoring group (men: OR = 1.47, 95%CI 1.33-1.63; women: OR = 1.97, 95%CI 1.75-2.23).
CONCLUSIONS
People who snore frequently should pay close attention to their blood pressure levels in order to achieve early prevention of hypertension, particularly for snorers who are female and aged under 45; importance should be attached to their blood pressure control.

Based on the Kraljic model,this paper established the scoring standard system for medical equipment procurement,and determined the weight coefficients of each evaluation factor by the analytic hierarchy process (AHP),then the procurement classification model of medical equipment was successfully established.It is proved that this model can classify medical equipment effectively,which enables the purchasing personnel to adopt corresponding purchasing strategies according to different classification.This model can be extended to all medical equipment procurement,realizing the delicacy and individualized procurement management of the equipment,which has strong operability and guiding significance.

10. Clinical significance of serum level changes of heart-type fatty acid-binding protein and S-100B protein in elderly patients with chronic heart failure

Objective:
To investigate serum level changes of heart-type fatty acid-binding protein(H-FABP)and S100 calcium-binding protein B(S-100B)protein in elderly patients with chronic heart failure and their clinical significance.
Methods:
A total of 160 patients with chronic heart failure treated at our hospital were recruited, and 80 healthy individuals receiving regular check-ups were enrolled as normal controls.Serum levels of H-FABP and S-100B and cardiac function index scores were compared between patients with different cardiac function grades.Correlations of serum H-FABP and S-100B levels with N-terminal pro-B-type natrlure tiepeptide(NT-proBNP)and with cardiac function index scores in heart failure patients were analyzed.The sensitivity, specificity and accuracy of serum H-FABP, S-100B and NT-proBNP for heart failure detection were compared.
Results:
Serum levels of H-FABP, S-100B and NT-proBNP in elderly patients with chronic heart failure were elevated with increased cardiac function grading(
