[摘 要] 目的：探讨干扰素刺激基因家族成员黏病毒抵抗蛋白2（MX2）在调控肿瘤细胞对呼肠孤病毒（Reo）溶瘤敏感性中的作用及其机制。方法：选取4株具有不同耐药特征的人源肿瘤细胞，通过CCK-8法评估其对Reo的溶瘤敏感性；通过转录组测序筛选出差异表达基因MX2，qPCR法及WB法验证MX2在4株人源肿瘤细胞中的表达；使用siRNA敲低溶瘤低敏感的COC1/DDP细胞中的MX2基因。在细胞感染Reo病毒后，通过CCK-8法检测细胞存活率；qPCR法检测细胞中Reo病毒S1基因表达；免疫荧光法检测细胞内Reo病毒蛋白的积累；半数组织培养感染剂量（TCID50）法测定病毒滴度；流式细胞术分别检测细胞内Reo病毒dsRNA、活性氧（ROS）水平以及细胞凋亡率；透射电镜观察细胞内质网形态变化并采用WB法检测内质网应激相关蛋白（JNK、p-JNK、eIF2α、p-eIF2α、CHOP、PERK）的表达。结果：在4株肿瘤细胞中，SKOV3细胞对Reo溶瘤作用高度敏感，而COC1/DDP、HuH-7SRB及SNU-398细胞均为溶瘤低敏感性。转录组测序结果显示，MX2在溶瘤低敏感肿瘤细胞中的表达水平显著高于溶瘤高敏感细胞（P < 0.01）；在溶瘤低敏感性的COC1/DDP细胞中，敲低MX2显著促进Reo病毒复制、诱导细胞凋亡增加，并升高细胞内活性氧水平（均P < 0.001）。透射电镜观察显示，敲低MX2的COC1/DDP细胞感染Reo病毒后出现内质网肿胀、扩张及断裂等典型内质网应激超微结构改变。WB结果显示，内质网应激关键标志物eIF2α/p-eIF2α、PERK、CHOP及凋亡相关调节蛋白JNK/p-JNK的表达均显著上调（P < 0.05或P < 0.01）。结论：肿瘤细胞对Reo的溶瘤敏感性与其细胞内MX2表达水平密切相关。敲低MX2可显著增强Reo在细胞内的复制，进而促进ROS积累，触发内质网应激并促进凋亡。病毒复制增加与细胞凋亡激活的双重作用，最终协同增强Reo的溶瘤作用。

ObjectiveTo analyze the epidemiological characteristics of 8 major respiratory pathogens in influenza-like illness (ILI) cases with acute respiratory infections at fever clinics in Huangpu District, Shanghai from 2015 to 2024, and to provide a scientific basis for the prevention and treatment of respiratory diseases. MethodsA retrospective study was conducted in Huangpu District. Individuals meeting the case definition of ILI from 2015 to 2024 was registered. Their nasopharyngeal swabs were collected for pathogen detection. A total of 8 respiratory viruses were tested, including Influenza A virus (Flu A), Influenza B virus (Flu B), adenovirus (ADV), enterovirus/human rhinovirus (EV/HRV), human parainfluenza virus (HPIV), human coronavirus (HCoV), respiratory syncytial virus (RSV), and human metapneumovirus (HMPV). ResultsFrom 2015 to 2019, a total of 344 ILI cases were tested, of which 192 out of 344 cases (55.81%) were tested positive for single respiratory pathogen. From 2023 to 2024, 1 557 ILI cases were tested, with 572 out of 1 557 cases (36.74%) being positive for single pathogen. From 2023 to 2024, the positive rate of single pathogen in ILI cases was significantly lower than that in 2015‒2019 (χ²=42.66, P<0.001). Specifically, the positive rate of Flu A (χ²=74.43, P<0.001) decreased, while that of HPIV (χ²=8.66, P=0.003) increased, both with statistically significant differences. According to the seasonal pattern, the epidemic intensity of Flu A decreased in summer, while that of HPIV increased in summer and autumn. Demographic results showed statistically significant differences in the positive rates of EV/HRV between genders (χ²=22.38, P<0.001), with males exhibiting a higher positive rate than females. No statistically significant differences were identified in the positive rates of single pathogen among different age groups (χ²=4.42, P=0.110). Nevertheless, statistically significant differences were noted when comparing the positive rates of EV/HRV, Flu A, Flu B and HPIV across different age groups (P<0.05). EV/HRV was more commonly detected in the 15‒<25 age group (10.93%), while Flu A and HPIV had the highest positive rates in the ≥60 age group (21.24% and 4.77%). Flu B had the highest positive rate in the 25‒<60 age group (11.26%). 52.63% of cases with co-infections occurred during winter, with the primary pathogens involved being EV/HRV (9 cases) and HCoV (6 cases). The most prevalent combination of co-infection was Flu A with EV/HRV. ConclusionThe prevalence of respiratory pathogens among ILI cases from 2023 to 2024 exhibited notable fluctuations compared to that from 2015 to 2019. Therefore, influenza surveillance should be strengthened, and attention should also be paid to the prevalence of respiratory pathogens such as HPIV. These findings have profound implications for future research, surveillance, vaccine planning, and public health policy making.

ObjectiveTo explore the effect of elbow-wrist functional orthosis on plantar pressure distribution and balance function in stroke patients with hemiplegia. MethodsFrom June, 2024 to April, 2025, 60 patients with post-stroke hemiplegia were recruited from Huashan Hospital, Fudan University, and Shanghai Hebin Rehabilitation Hospital. They were randomly divided into control group (n = 30) and intervention group (n = 30). The control group received routine neurological rehabilitation, while the intervention group received additional training with an elbow-wrist functional orthosis on the affected side, for eight weeks. Before and after intervention, the Modified Ashworth Scale (MAS) of the elbow joint, plantar pressure symmetry index (SI), plantar contact area and mean plantar pressure were recorded, and balance and mobility were assessed using the Berg Balance Scale (BBS), Timed Up & Go Test (TUGT) and 10-Meter Walk Test (10MWT). ResultsTwo cases dropped out in the control group. After treatment, MAS grades of the elbow joint, forefoot SI, affected side plantar pressure area, BBS scores, TUGT and 10MWT of both groups improved (|Z| > 3.969, |t| > 3.528, P < 0.01), while the hindfoot SI and average pressure of the affected foot improved in the intervention group (∣t∣ > 4.264, P < 0.001). Except for TUGT and 10MWT, the intervention group was superior to the control group (∣Z∣ > 2.030, ∣t∣ > 2.096, P < 0.05). ConclusionThe elbow-wrist functional orthosis can enhance balance function in stroke patients with hemiplegia by reducing upper-limb spasticity, optimizing center-of-gravity distribution, and improving postural control.

Objective: To investigate the factors influencing intraoperative blood transfusion in patients with hip fractures and to develop a machine learning (ML) model for predicting this risk. Methods: A total of 424 patients with hip fractures who underwent surgical treatment between November 2022 and March 2025 in our hospital were selected. Key feature variables of intraoperative blood transfusion risk were identified using the Boruta algorithm. Four different ML algorithms—support vector machine (SVM), linear discriminant analysis (LDA), mixed discriminant analysis (MDA), and extreme gradient boosting (XGBoost)—were used to develop predictive models for intraoperative blood transfusion risk. The predictive performance of the four ML models were evaluated using accuracy, precision, receiver operating characteristic (ROC) curves, precision-recall curves (PRC), precision-recall gain curves (PRGC), and F1 scores. Shapley additive interpretation (SHAP) was used to interpret the final model. Results: Among the 424 patients, 77(18.2%) received intraoperative blood transfusion. The Boruta algorithm identified albumin (ALB), activated partial thromboplastin time (APTT), types of anesthesia, types of fracture, and hemoglobin (Hb) as key feature variables for predicting intraoperative blood transfusion risk. In model evaluation, the SVM model outperforms the other three models across multiple metrics, including the area under the receiver operating characteristic curve (AUC), recall, recall gain, accuracy, precision, F1 score, and the area under the precision-recall curve (PRC-AUC). The SVM model, interpreted and visualized based on SHAP values, effectively predicted intraoperative blood transfusion risk in patients with hip fracture. A visual online application was developed based on the SVM model (https://pbo-nomogram.shinyapps.io/blood/). Conclusion: Preoperative low ALB and Hb levels, prolonged APTT, general anesthesia, and intertrochanteric fractures are risk factors for intraoperative blood transfusion in hip fracture patients. The risk prediction model for intraoperative blood transfusion constructed based on the SVM algorithm has optimal performance, which provides new ideas and methods for the clinical early identification of hip fracture patients with high transfusion risk and the implementation of targeted interventions.

Background Benzothiazoles and benzotriazoles are ubiquitously detected in drinking water, posing potential health risks. Developing a reliable and sensitive analytical method is critical for assessing their exposure levels and associated risks. Objective To establish an automated solid-phase extraction (SPE) coupled with liquid chromatography-tandem mass spectrometry method for the simultaneous determination of benzothiazole, 2-aminobenzothiazole, benzotriazole, and 5-chlorobenzotriazole in drinking water. Methods Key parameters were systematically optimized. Three mobile phase systems were compared to evaluate their effects on chromatographic peak shape and separation; three SPE cartridges were tested for extraction efficiency; the influences of water pH on extraction recoveries and matrix effects were investigated; the contributions of pretreatment steps to benzothiazole blank were analyzed, and control measures were established; nitrogen evaporation temperatures were tested to ensure analyte stability. The optimized parameters were used to develop the method, validate its performance, and analyze drinking water samples. Results Drinking water samples were first adjusted to pH 6.0 with formic acid, and then extracted using HLB cartridges. After elution with 6 mL methanol, the eluate was concentrated under nitrogen evaporation at 45 °C, followed by separation on a C18 column using a gradient elution of acetonitrile-0.1% formic acid solution, and tandem mass spectrometry detection. Avoiding nitrogen blow-drying during SPE and eliminating the use of polypropylene materials during nitrogen evaporation can effectively minimize benzothiazole contamination. Good calibration linearity was obtained for the target analytes in the concentration range of 5.0-250 μg·L⁻¹, with a correlation coefficient of r > 0.995. The method detection limits were in the range of 1.0-5.0 ng·L⁻¹. The recoveries of the target analytes in pure water and tap water were 80.2%-119.5% and 72.2%-115.6%, respectively, with relative standard deviations of were 3.2%-12.8% and 2.3%-11.6%, respectively. When applying this method to actual water samples, benzotriazole was detected in 100% of treated and tap water samples, with median concentrations of 79.4 ng·L⁻¹ and 114 ng·L⁻¹, respectively. Benzothiazole was detected in 83.3% of treated water samples and 100% of tap water samples, with median concentrations of 48.3 ng·L⁻¹ and 65.4 ng·L⁻¹, respectively. In addition, 5-chloro-benzotriazole exhibited low detection rates and concentrations, while 2-amino-benzothiazole was undetected. Conclusion The developed method demonstrates high accuracy, reliability, and sensitivity, making it suitable for the analysis of trace levels of benzothiazoles and benzotriazoles in drinking water.

Objective To explore the safety and feasibility of 5G remote robot-assisted thoracoscopic anatomic lobectomy and segmentectomy. Methods A retrospective analysis was conducted on patients who underwent 5G remote robot-assisted thoracoscopic anatomic lobectomy or segmentectomy between July and September 2024. The surgeries were performed collaboratively by Shanghai Chest Hospital, Affiliated Hospital of Yangzhou University, and The First Affiliated Hospital of Nanchang University. Perioperative outcomes were recorded and analyzed. Results Ten patients were included, comprising 4 anatomic lobectomies and 6 segmentectomies. The median age was 61.0 (56.0, 72.0) years. The median intraoperative bi-directional network latency was 52.5 (39.0, 54.0) ms. There were no instances of network interruption, robot-related adverse events, or conversions to open thoracotomy. The median postoperative chest tube drainage volume was 500.0 (375.0, 600.0) mL, and the median hospital stay was 5.5 (4.0, 6.0) days. No complications of Clavien-Dindo grade Ⅱor higher occurred. All patients were recurrence-free and alive during the 6-month postoperative follow-up period. Conclusion 5G remote robot-assisted thoracoscopic surgery demonstrates high safety and operational stability. This technology shows promising potential for clinical application and is worthy of further development and utilization.

Although video-assisted thoracoscopic surgery (VATS) offers clear advantages, including minimal trauma and rapid recovery, stable intraoperative visualization still depends heavily on the camera assistant. Traditional manual endoscope holding is therefore susceptible to several limitations, including image instability, assistant fatigue, and poor coordination with the primary surgeon. Operating in complex and confined anatomical spaces with a rigid thoracoscope further increases the demands on hand-eye coordination. Thoracoscopic camera-holding technology has progressively evolved from passive support systems to active camera holders and robot-assisted thoracic surgery (RATS) platforms, and is now advancing toward artificial intelligence-driven tracking and intelligent control. Based on this, a series of devices, including multifunctional intelligent assistant arms and active camera-holding arms for thoracic surgery, have been developed and have attracted increasing attention. This article reviews the development of intelligent camera-holding assistant arms and discusses related advances and future directions in the field.

Objective To investigate the impact of ultra-high-frequency radiation(UHFR) on the mental arithmetic ability in exposed workers. Methods A total of 79 workers engaged in high-frequency heat-sealing operations in a shoe factory were selected as the exposure group, and 121 workers from the same factory without such exposure were selected as the control group using the judgment sampling method. The intensity of UHFR from the high-frequency heat-sealing machines at the workplaces was measured on-site. Mental arithmetic performance of the participants of the two groups was assessed using the neurobehavioral ability index (NAI) from Neurobehavioral Evaluation System Chinese 3rd Generation. Results The NAI of the participants was lower in the exposure group than that in the control group [(1.17 ± 0.48) vs (1.39 ± 0.59), P<0.01], as was the NAI of the high-exposure subgroup within the exposure group [ (1.10 ± 0.43) vs (1.39 ± 0.59), P<0.01]. Females in the exposed group had lower NAI than that in the control group [(1.14 ± 0.49) vs (1.39 ± 0.60), P<0.01]. Multivariable logistic regression analysis indicated that exposure to high intensity electric field was a risk factor for abnormal NAI (odds ratio=2.04, 95% confidence interval was 1.00-4.19, P<0.05). Conclusion Occupational exposure to UHFR from high-frequency heat-sealing machines may negatively affect the mental arithmetic ability of workers. Occupational health protection measures, such as reducing UHFR exposure levels, enhancing individual protection, and strengthening occupational health surveillance, should be implemented.

Chronic atrophic gastritis （CAG）， a premalignant gastric condition， represents a critical stage in inflammation-cancer transformation. In this pathological process， mitochondrial damage serves as a key mediator in the inflammation-driven transformation of normal gastric mucosal epithelial cells into gastric cancer cells， while the dysregulation of the reactive oxygen species （ROS）/NOD-like receptor protein 3 （NLRP3） axis plays a key role in perpetuating the vicious cycle of inflammation-mitochondrial damage. The classical theory of traditional Chinese medicine （TCM）， vigorous fire consuming Qi， aptly encapsulates this pathological feature. The essence of this theory lies in the struggle between pathogenic and vital factors， wherein vigorous fire shares similarities with chronic inflammation and may be regarded as a driving force behind inflammation-cancer transformation. Qi reflects the mitochondrial function， and consuming Qi signifies structural and functional impairment of mitochondria. Thus， this theory accurately summarizes the pathological characteristics of inflammation-mitochondrial damage during CAG progression. Building upon this theoretical framework， the theory of vigorous fire consuming Qi provides a holistic understanding of CAG pathogenesis. Reviewing the clinical expertise of TCM master YANG Chunbo in treating spleen-stomach disorders， this paper further elucidates its therapeutic implications： modulating systemic inflammatory responses to restore gastric mucosal homeostasis and targeting mitochondrial damage as a pivotal intervention to delay CAG progression. Clinically， achieving dynamic equilibrium between fire and Qi is essential， mitigating excessive inflammation without compromising vital Qi， while reinforcing Qi without exacerbating pathological fire. This approach offers novel insights for developing preventive and therapeutic strategies against CAG.

ObjectiveTo investigate the epidemiological characteristics of respiratory adenovirus (AdV) infection among hospitalized patients under 18 years of age with pneumonia in Yancheng City of Jiangsu Province from 2023 to 2024, and to provide a basis for the prevention and control of adenovirus infection. MethodsNasopharyngeal swab samples were collected from hospitalized patients under 18 years of age diagnosed with pneumonia at two national influenza sentinel hospitals in Yancheng City from February 2023 to June 2024. A respiratory pathogen nucleic acid detection kit was used to test for 28 pathogens, including influenza A virus, influenza B virus, respiratory syncytial virus, human rhinovirus, adenovirus, Mycoplasma pneumoniae, Chlamydia pneumoniae, human metapneumovirus, human bocavirus, enterovirus, polyomavirus K1, polyomavirus WU, parainfluenza viruses 1‒4, coronavirus 229E, HKU1, OC43, and NL63, Haemophilus influenzae, Bordetella pertussis, Group A Streptococcus, Staphylococcus aureus, Streptococcus pneumoniae, Legionella pneumophila, Klebsiella pneumoniae, and the novel coronavirus, and to analyze patients’ mixed infections. Adenovirus-positive samples were subjected to polymerase chain reaction (PCR) amplification using Hexon and Fibre as target genes, and the products were analyzed by sequencing. Whole-genome sequencing analyses were performed on positive samples with Ct values less than 22. ResultsAmong 368 hospitalized pneumonia patients under 18 years of age in Yancheng City from February 2023 to July 2024, most were male (51.63%, 190/368), with preschool-aged children (42.66%, 157/368) and primary school-aged children (40.22%, 148/368) comprising the majority. A total of 44 cases tested positive for adenovirus, yielding a positivity rate of 11.96%. Single infections, dual infections, and multiple infections accounted for 61.36% (27/44), 36.36% (16/44), and 2.27% (1/44), respectively. From November 2023, the adenovirus positivity rate rose sharply, peaking at 51.61% in December. Following PCR amplification and sequencing, valid sequences were successfully obtained for 37 strains, comprising 36 AdV-3 strains and 1 AdV-C strain. Eight adenovirus-positive samples with Ct values below 22 were inoculated into HEp-2 cell lines for virus isolation, and whole-genome sequencing analyses were performed on the five successfully isolated strains. Phylogenetic analyses revealed that the AdV-3 strains collectively belonged to the same evolutionary clade. Whole-genome sequencing results indicated that four AdV-3 strains (HAdV-B3) showed no evidence of genetic recombination, whereas one AdV-C strain (HAdV-89) might have undergone genetic recombination. ConclusionThe adenovirus positivity rate among hospitalized pneumonia patients aged under 18 years of age in Yancheng from 2023 to 2024 was slightly higher than that reported in previous studies. Adenovirus infection exhibited a pattern of high incidence in winter, with the AdV-3 type being the predominant pathogen; one AdV-C strain (HAdV-89) might have undergone genetic recombination. It is recommended to strengthen surveillance of respiratory adenovirus type AdV-3 and its variants during the winter season, promote the use of multiplex pathogen detection techniques, and explore the clinical application of an AdV-3 vaccine.
