1.Effect of elbow-wrist functional orthosis on plantar pressure and balance function in stroke patients with hemiplegia

ObjectiveTo explore the effect of elbow-wrist functional orthosis on plantar pressure distribution and balance function in stroke patients with hemiplegia. MethodsFrom June, 2024 to April, 2025, 60 patients with post-stroke hemiplegia were recruited from Huashan Hospital, Fudan University, and Shanghai Hebin Rehabilitation Hospital. They were randomly divided into control group (n = 30) and intervention group (n = 30). The control group received routine neurological rehabilitation, while the intervention group received additional training with an elbow-wrist functional orthosis on the affected side, for eight weeks. Before and after intervention, the Modified Ashworth Scale (MAS) of the elbow joint, plantar pressure symmetry index (SI), plantar contact area and mean plantar pressure were recorded, and balance and mobility were assessed using the Berg Balance Scale (BBS), Timed Up & Go Test (TUGT) and 10-Meter Walk Test (10MWT). ResultsTwo cases dropped out in the control group. After treatment, MAS grades of the elbow joint, forefoot SI, affected side plantar pressure area, BBS scores, TUGT and 10MWT of both groups improved (|Z| > 3.969, |t| > 3.528, P < 0.01), while the hindfoot SI and average pressure of the affected foot improved in the intervention group (∣t∣ > 4.264, P < 0.001). Except for TUGT and 10MWT, the intervention group was superior to the control group (∣Z∣ > 2.030, ∣t∣ > 2.096, P < 0.05). ConclusionThe elbow-wrist functional orthosis can enhance balance function in stroke patients with hemiplegia by reducing upper-limb spasticity, optimizing center-of-gravity distribution, and improving postural control.
2.Evaluation of transdermal absorption of self-assembled nanoparticles of Huangqin decoction loaded with terbinafine

OBJECTIVE To investigate the effect of Huangqin decoction (HQD)-based self-assembled nanoparticles (SAN) co-loaded with terbinafine (TBF) (TBF-HQD-SAN NPs) on the transdermal absorption of TBF. METHODS High-speed centrifugation combined with dialysis was used to separate HQD-SAN, and TBF-HQD-SAN NPs were obtained by loading TBF using the ultrasound magnetic stirring method; the particle size distribution, Zeta potential and polydispersity index (PDI) of the nanoparticle were characterized, and the encapsulation efficiency (EE) and drug loading (DL) of TBF were determined; using in vitro and in vivo transdermal experiments, the differences in transdermal performance between TBF-HQD-SAN NPs and TBF raw materials, as well as TBF and HQD-SAN physical mixture (TBF-HQD-SAN PM), were compared and analyzed. RESULTS TBF- HQD-SAN NPs were spherical with a particle size of (177.60±2.57) nm, a PDI of 0.197 4±0.007 9, and a Zeta potential of (-14.63±0.85) mV. The EE and DL of TBF were (99.49±0.71)% and (3.22±0.10)% , respectively. In vitro transdermal experiments, compared with TBF raw materials, the steady-state permeation rate (Jss) and skin retention of TBF-HQD-SAN NPs increased by 3.34 times and 27.56 times, respectively (P<0.05); compared with TBF-HQD-SAN PM, its Jss and skinretention were increased by 2.04 times and 7.44 times, respectively (P<0.05). In vivo transdermal experiments 69号) showed that, the area under the drug-time curve and the maximum concentration of TBF-HQD-SAN NPs increased by 2.13 times and 2.06 times respectively compared to TBF raw materials, and increased by 1.59 times and 1.65 times respectively compared to TBF-HQD-SAN PM (P<0.05). CONCLUSIONS TBF-HQD-SAN NPs can significantly enhance the in vitro and in vivo transdermal absorption efficiency and skin retention of TBF.
3.Application of action observation therapy in stroke rehabilitation from 2016 to 2025: a bibliometric analysis

ObjectiveTo analyze the application trends and research hotspots of action observation therapy (AOT) in the field of stroke rehabilitation over the past decade. MethodsLiteratures on AOT in stroke rehabilitation published from January, 2016 to December, 2025 were retrieved from the Web of Science Core Collection database. CiteSpace 6.4.R1 was used for visual analysis. ResultsA total of 463 articles were included. The annual publication volume showed a fluctuating upward trend. The country with the highest number of publications was China, the most productive institution was Chang Gung University and Consiglio Nazionale delle Ricerche, and the most prolific author was Avanzini Pietro. Mirror neuron system, motor imagery, upper limb and facilitation were identified as high-frequency keywords and bursting words. ConclusionIn the past decade, the number of publications on AOT in stroke rehabilitation has generally increased. Researches are focusing on the synergy of sensory-closed-loop multimodal technologies, reconstruction of fine upper limb function and neural facilitation mechanisms.
4.Association between epicardial adipose parameters and microvascular obstruction following acute myocardial infarction based on CMR

ObjectiveTo evaluate the predictive value of epicardial adipose tissue (EAT) parameters for microvascular obstruction (MVO) formation in patients with ST segment elevation myocardial infarction (STEMI) using cardiac magnetic resonance quantification. MethodsA total of 139 STEMI patients were included in this study, and various parameters such as EAT thickness, volume, and mass index were measured utilizing cardiac magnetic resonance. All included patients were divided into MVO group and non-MVO group according to whether MVO occurred. Differences in EAT related parameters between two groups were compared and correlation analysis was applied to evaluate the correlation between quantitative indicators of EAT and indicators such as infarct size and ejection fraction. Logistic regression analysis was used to identify the relevant risk factors for MVO formation. Receiver Operating Characteristic (ROC) curve analysis was performed to assess the predictive value of the epicardial adipose tissue (EAT) quality index and other indicators for the occurrence of MVO. ResultsCompared with non MVO group, patients in MVO group presented with higher peak troponin T levels, increase of neutrophil lymphocyte ratio (NLR) and C-reactive protein (CRP), larger infarct size and compromised left ventricular ejection fraction (LVEF) (P<0.05). Total EAT volume, EAT mass index, left atrioventricular EAT volume, left atrioventricular EAT mass index and thickness of EAT in the left atrioventricular groove were significantly higher in patients with MVO. Multivariate Logistic regression analysis demonstrated that NLR, peak troponin T levels and left atrioventricular EAT mass index were independent predictors of MVO. The ROC curve suggested that the left atrioventricular EAT mass index had the highest predictive power for MVO formation in STEMI patients. ConclusionThe parameters of EAT quantified by cardiac magnetic resonance serve as imaging biomarkers for predicting MVO formation in STEMI patients. These metrics enable risk stratification post-myocardial infarction and facilitate early identification of high-risk individuals, thereby supporting personalized therapeutic decision-making.
5.Development of online-offline integration in internet hospital and its application in thoracic surgery

With the continuous advancement of internet technology and the improvement of internet literacy among the general population, the concept of online-offline integration in internet hospitals has gradually gained acceptance and has been applied and developed both domestically and internationally. In thoracic surgery, the applicability of this model lies in enhancing efficiency and delivering comprehensive, diversified, and personalized medical services to address complex and severe conditions. However, challenges such as hardware limitations and diagnostic/treatment risks persist during the implementation of internet hospitals. Through future in-depth and localized research, the online-offline integration of internet hospitals is expected to undergo further development and refinement. This progress will facilitate its integration into clinical practice in thoracic surgery, ultimately providing patients with improved medical care services.
6.Analysis of risk factors for postoperative new-onset cardiac complications in patients with esophageal cancer and concomitant coronary heart disease

Objective To investigate the factors affecting the occurrence of new postoperative cardiac complications in patients undergoing esophageal cancer surgery with concomitant coronary heart disease. Methods Clinical data of patients who underwent esophageal cancer surgery with coronary heart disease at the Affiliated Huai'an No.1 People's Hospital of Nanjing Medical University from December 2019 to June 2023 were collected. Patients were divided into two groups based on whether they experienced postoperative cardiac complications. Using the occurrence of cardiac complications as the dependent variable, a multivariate logistic regression model was established to identify related influencing factors. Results A total of 223 patients were included, comprising 148 males and 75 females, with an average age of (71.78±6.31) years (range 53-88 years). Seventy-one (31.84%) patients experienced at least one new cardiac complication postoperatively, including 2 acute coronary syndrome, 13 heart failure, and 59 new-onset postoperative arrhythmias. Univariate analysis showed that age, systemic immune-inflammation index, pulmonary infection, need for invasive mechanical ventilation due to respiratory failure, acute respiratory distress syndrome (ARDS), acute delirium, pleural effusion requiring drainage, and acute renal failure were risk factors for postoperative new-onset cardiac complications (all P<0.05). Multivariate logistic regression analysis identified age, postoperative length of hospital stay, ARDS, and systemic immune-inflammation index as independent risk factors for new cardiac complications in esophageal cancer patients with coronary heart disease. Conclusion Strengthening perioperative management of esophageal cancer patients, ranging from preoperative evaluation to postoperative complication treatment, is crucial. Particular attention should be paid to age, ARDS, and other indicators to improve postoperative prognoses in patients with coronary heart disease complicated by esophageal cancer.
7.Correlation of metabolic comorbidities and insulin resistance with CKD in an elderly population taking physical exam

Objective To explore the relationship of metabolic comorbidities and insulin resistance(IR)with chronic kidney disease(CKD)among elderly individuals undergoing health exam.Methods A cross-sectional observational study was conducted on 8358 older adults who took health exam in Chinese PLA General Hospital between December 2009 and May 2021.According to the guidelines for CKD diagnostic criteria,they were divided into CKD group(983 cases)and non-CKD(7375 cases).Clinical data was collected,and the eGDR was calculated.Quasi-Bayesian method was used for causal mediation analysis.Results The prevalence of metabolic comorbidi-ties including hypertension,CHD,DM,hyperlipidemia,and hyperuricemia was significantly higher in the CKD group than the non-CKD group(P<0.01).The eGDR was obviously lower in the CKD group than the non-CKD group[6.88±2.09 mg/(kg·min)vs 8.41±2.12 mg/(kg·min),P<0.01].Logistic regression analysis revealed that,without adjusting covariates,each 1-unit increase in eGDR was associated with a 29%reduction in the risk of developing CKD(OR=0.714,95%CI:0.691-0.738,P<0.01),and after adjusting covariates,eGDR remained signifi-cantly negatively association with the risk of CKD(P<0.01).Mediation analysis indicated that DM and brachial-ankle pulse wave velocity accounted for the highest proportions of the mediating effect in the relationship between eGDR and CKD(14.2%and 12.5%,respectively).Conclusion In the elderly population undergoing health exam,reduced insulin sensitivity is significantly asso-ciated with the development of CKD.Diabetes and arteriosclerosis exert mediating effect in this association.
8.Identification of serum differential metabolic markers in patients with ulcerative colitis

Objective To identify serum metabolic markers served in the clinical diagnosis of ulcerative colitis(UC).Methods Ser-um samples from 29 UC patients,31 Crohn's disease(CD)patients,and 21 matched healthy controls(HC)admitted to Department of Gastroenterology,Nanjing Drum Tower Hospital during September 2022 and March 2024 were collected.The ultra-high-performance liquid chromatography coupled with quadrupole-orbitrap mass spectrometry(UHPLC-Q Exactive HF-X)technology was used to detect and analyze serum metabolites.A partial least squares discrimination analysis(PLS-DA)model was constructed,and the metabolites significantly up-regulated in UC were screened based on the variable importance in projection(VIP)score>1,P value<0.05,and fold change(FC)>1.2.The pathway enrichment analysis was performed using the Kyoto Encyclopedia of Genes and Genomes(KEGG)database to reveal the biological pathways involved in the metabolites.The area under the receiver operating characteristic curve(AUCROC)was calculated to evaluate the diagnostic potential of the differential metabolites.Results A total of 1 522 metabo-lites were identified from the three sample groups.Among them,4 metabolites,namely leucodopachrome(VIP=1.964,P<0.05,FC=1.916),tetrahydrodipicolinate(VIP=1.74,P<0.05,FC=2.65),N-ethylmaleimide(VIP=1.519,P<0.05,FC=1.597),and 5,6-dihydroxyindole(VIP=3.018,P<0.05,FC=1.575),were significantly up-regulated in UC.Their AUCROC values for distinguishing UC from CD were 0.788(95%CI:0.655-0.921),0.773(95%CI:0.639-0.907),0.834(95%CI:0.720-0.949),and 0.899(95%CI:0.821-0.977),respectively,while those for distinguishing UC from HC were 0.966(95%CI:0.924-1.000),0.926(95%CI:0.857-0.995),0.969(95%CI:0.928-1.000),and 0.910(95%CI:0.830-0.990),respectively.KEGG pathway analysis showed that the up-regulated metabolites in UC were primarily enriched in biological pathways such as tyrosine metabolism,glycerophospholipid me-tabolism,and arachidonic acid metabolism.Conclusion The serum metabolic profile of UC patients is significantly changed,and the four differential metabolites mentioned above may serve as effective biomarkers for the differential diagnosis of UC,CD,and HC.
9.Relationship between social media use and internalized homophobia in homosexual and bisexual persons

Objective:To explore the relationship between social media use and internalized homophobia in homosexual and bisexual persons,and the role of self-esteem and perceived social support.Methods:A total of 626 homosexual and bisexual persons were selected through online social media platforms,and the Social Media Use In-tensity Scale(SNSIS),Self-Esteem Scale(SES),Perceived Social Support Scale(PSSS)and Internalized Homo-phobia Scale(IHS)were used for measurement.Use the SPSS macro program PROCESS to test moderated media-tion effects.Results:The average SNSIS score was(3.0±1.0).The SES score played a partial mediating role in the relationship between SNSIS scores and IHS scores,and the value of mediating effect was 21.43%.The PSSS scores moderated the relationship between the SNSIS scores and the SES scores(β=-0.10,P<0.01),and the re-lationship between the SES scores and the IHS scores(β=-0.11,P<0.01).Conclusion:Social media use by ho-mosexual and bisexual persons is associated with internalized homophobia,self-esteem mediated the relationships,and perceived social support plays a moderating role.
10.Correlation between serum uric acid and incident chronic kidney disease in middle-aged and elderly Chinese population and gender differences

Objective To investigate the correlation between serum uric acid(SUA)levels and incident chronic kidney disease(CKD)in middle-aged and elderly Chinese population and gender differences.Methods Based on the longitudinal survey data of China Health and Retirement Longitudinal Study from 2011 to 2015,the CKD-Epidemiology Collaboration cystatin C formula was used to estimate the glomerular filtration rate(eGFR),and 4 119 participants with normal renal function(eGFR≥60 mL·min-1·1.72 m-2)at baseline were included.Incident CKD was defined as eGFR<60 mL·min-1·1.72 m-2 at the follow-up in 2015.Logistic regression analysis was used to analysis the association of SUA levels at baseline and incident CKD among different genders.Restricted cubic spline analysis was used to analyze the dose-response relationship.Results After 4-year follow-up,127 participants developed incident CKD,including 57 males and 70 females.Multivariate Logistic regression analysis showed that elevated SUA levels were independently associated with the risk of incident CKD(OR=1.532,P<0.001).For each 1 mg/dL increase in SUA,the risk of incident CKD increased by 33.6%in males(OR=1.336,P=0.012)and 77.5%in females(OR=1.755,P<0.001).Restricted cubic spline analysis showed a linear positive correlation between SUA levels and incident CKD in both males and females.Participants were divided into four groups according to SUA quartiles(Q1-Q4).Multivariate Logistic regression analysis indicated a significant increase in the risk of incident CKD in Q3 group(3.75 mg/dL
