1.Association between sleep and myopia in Tianjin schoolchildren aged 6-15 years

Objective:To explore the association between sleep and myopia among students aged 6-15 years in Tianjin.Methods:A cross-sectional study was conducted.A total of 218 864 primary and secondary school students aged 6-15 years in Tianjin were recruited from January 2023 to May 2023.Basic information and responses to the Children's Sleep Habits Questionnaire (CSHQ) were collected.Logistic regression models were used to assess the association between myopia and sleep.The study followed the Declaration of Helsinki, and the research protocol was approved by the Medical Ethics Committee of Tianjin Medical University Eye Hospital (No.ChiCTR2200065710). All questionnaires and demographic information were collected with parental consent.Results:It was found that 68 121(31.12%) students were myopic and 178 514(81.56%) had sleep disorders.The prevalence of myopia among students with average daily sleep durations of ≤8 hours, >8-9 hours, >9-10 hours, and >10 hours was 52.17%(9 288/17 803), 35.35%(31 037/87 787), 25.18%(24 481/97 216), and 20.64% (3 315/16 058), respectively, and the difference was statistically significant ( χ2=6 835.649, P<0.001). After adjusting for sex, age, body mass index, and potential confounding factors, compared with students with average daily sleep duration of >10 hours, students with average daily sleep durations of ≤8 hours ( OR=1.496, 95% CI: 1.415-1.581, P<0.001), >8-9 hours ( OR=1.364, 95% CI: 1.383-1.447, P<0.001), and >9-10 hours ( OR=1.257, 95% CI: 1.202-1.316, P<0.001) had a higher risk of myopia.Students with sleep disorders, bedtime resistance, sleep-onset delay, irregular sleep duration, sleep anxiety, night wakings, parasomnias, sleep-disordered breathing, and daytime sleepiness were more likely to be myopic. Conclusions:Sleep is a key factor influencing myopia among schoolchildren aged 6-15 years in Tianjin.
2.Intratumoral and peritumoral CT radiomics for predicting Ki-67 expression level of lung adenocarcinoma with mixed ground glass nodules

Objective To observe the value of intratumoral and peritumoral CT radiomics for predicting Ki-67 expression level of lung adenocarcinoma with mixed ground glass nodules.Methods Totally 284 cases of pathologically confirmed lung adenocarcinoma with mixed ground glass nodules were retrospectively enrolled,among them 197 cases were taken as training set(54 cases with high and 143 cases with low Ki-67 expression)and 87 cases as validation set(27 cases with high and 60 cases with low Ki-67 expression).Intratumoral and peritumoral radiomic features were obtained from non-contrast chest CT,and radiomic models for predicting Ki-67 expression in lung adenocarcinoma with mixed ground glass nodules were established using adaptive boosting,light gradient boosting machine(LightGBM)and multilayer perceptron algorithms based on intratumoral features,peritumoral features,as well as intratumoral+peritumoral features,respectively,and the optimal radiomics signature was selected according to the area under the receiver operating characteristic curve(AUC).Univariate and multivariate logistic regression analysis were performed to identify independent impact factors of Ki-67 expression level,and a clinical model was constructed,and the efficacy of the models were evaluated.Results Among radiomics models,LightGBMintratumoral+peritumoral model had the highest AUC(0.934 in training set and 0.845 in validation set),which were superior to that of clinical model(0.616 in training set and 0.684 in validation set)(both P<0.05)Conclusion Intratumoral and peritumoral CT radiomics had good efficacy for predicting Ki-67 expression level of lung adenocarcinoma with mixed ground glass nodules.
3.Construction and performance study of biological treatment experimental system for space wastewater

Objective To address the bottlenecks in the application of wastewater biological treatment technology under space conditions,an experimental system for the biological treatment of space wastewater was constructed and its biochemical performance examined.The findings of this study will provide technical support for the biological treatment of space wastewater.Methods Based on the Membrane Aerated Biofilm Reactor(MABR)process,a biological treatment experimental system for space wastewater was constructed and conducted the continuous flow test for 77 days to investigate the performance of PVDF and PP membrane modules in the treatment of simulated air condensate.Results The results demonstrated that both membrane modules exhibited an average TOC removal rate of 90%,indicative of their effective organic matter removal capacity.In the air supply mode,the ammonia oxidation capacity was observed to be comparatively lower,whereas in the oxygen source without bubbling mode,the nitrogen oxidation rate and total nitrogen removal rate could be attained above 90%,indicating a notable degree of simultaneous nitrification and denitrification.The results demonstrated that the mode of gas supply had a significant impact on the nitrogen conversion performance.The abundance of nitrogen-converting bacteria in PP membrane module is higher than that in PVDF membrane module,indicating a better nitrogen-converting performance in PP membrane module.Conclusion The constructed wastewater biological treatment system is optimally suited for the treatment of air condensate,thereby offering a novel technical approach for space wastewater treatment.
4.Clinical and pathological features and prognostic analysis of early-onset intrahepatic cholangiocarcinoma

Objective:To explore the clinical and pathological features and survival outcomes of patients with early-onset intrahepatic cholangiocarcinoma (EOICC).Methods:This is a multicenter, retrospective cohort study. Data of 1 160 intrahepatic cholangiocarcinoma patients undergoing radical resection in 14 tertiary Grade A hospitals in China from January 2010 to November 2021 were retrospectively collected. The cohort included 632 males and 528 females, aged( M (IQR)) 61 (14) years (range: 22 to 93 years). ICC aged ≤50 years at the time of diagnosis was defined as EOICC and >50 years as late-onset intrahepatic cholangiocarcinoma (LOICC). Of these, there were 247 cases in the EOICC group and 913 cases in the LOICC. The clinical and pathological characteristics of both groups were analyzed and compared using the independent sample t-test, Mann-Whitney U test or Kaplan-Meier method. Univariate and multivariate Cox regression models for patient outcomes were constructed and forest graphed. Results:Compared with the patients in the LOICC group, patients in the EOICC group had lower carcinoembryonic antigen levels (2.5(4.0) μg/L vs. 3.1(5.2)μg/L, U=124 899, P=0.009) and CA19-9 level (63.4(524.7)U/ml vs. 77.9(611.3)U/ml, U=120 320, P=0.013), higher levels of ALT (29(35)U/L vs. 24(26)U/L, U=101 214, P=0.013), a lower score of the Eastern US Cooperative Oncology Group (0 score patients: 54.7% vs. 44.1%, χ2=12.472, P=0.014), higher TNM stage ( χ2=11.807, P=0.038), and proportion of lymph node dissection (62.3% vs. 54.1%, χ2=5.355, P=0.021). Patients in the two groups in sex, first diagnosis symptoms, intrahepatic bile duct stone history, nail protein, albumin, total bilirubin, transaminase, liver function Child-Pugh grade, T stage, stage, N stage, preoperative laparoscopic exploration proportion, tumor diameter, vascular invasion proportion, differentiation, margin, intraoperative bleeding, postoperative complications, postoperative hospital days were no statistical significance (all P>0.05). Patients in the EOICC group had better outcomes than the LOICC group (median survival time: 29.7 months vs. 25.0 months, 3-year overall survival: 45.1% vs. 37.8%, P=0.027). Conclusion:EOICC patients are better than LOICC patients in carcinoembryonic antigen, CA19-9, ALT, physical strength status and TNM stage, and the long-term prognosis is also better than LOICC patients.

To summarize LI Qiangou's clinical experience in treating laryngopharyngeal reflux disease (LPRD) based on the theory of "yang transforming qi while yin constituting form". It is believed that "insufficiency of yang transforming qi and excess of yin constituting form" is the key mechanism of LPRD, in which yang deficiency of the lungs, spleen and kidneys is an important factor causing "insufficiency of yang transforming qi", while phlegm, dampness and other tangible yin turbidities are the pathological products of "excess of yin constituting form". The key treatment principle is to reinforce yang qi and dispel yin turbidity. Starting from regulating the qi transformation of the sanjiao, the treatment is based on the different pathologies of lung yang insufficiency and phlegm congestion in the upper jiao, spleen yang failing to ascend and dampness accumulation in the middle jiao, and kidney yang deficiency and phlegm-fluid retention in the lower jiao, prescribed by modified Suzi Jiangqi Decoction (苏子降气汤), Shengyang Chushi Decoction (升阳除湿汤), and Jingui Shenqi Pill (金匮肾气丸), the prescriptions could also combine with Banxia Houpo Decoction (半夏厚朴汤) to dissolve phlegm and relieve pharyngeal pains, and regulate qi to direct counterflow downward.
9.Associations of human intestinal Dorea and glucose intolerance in people with obesity

Objective:To evaluate the role of Dorea in glucose intolerance of people with obesity. Methods:This study recruited 113 young individuals with obesity and varying degree of glucose tolerance [body mass index(BMI)≥30 kg/m 2] and 105 controls, comparing the metabolic phenotypes and Dorea abundance. Correlation analysis and ROC analysis were performed to assess the association between Dorea and clinical parameters and its predictive role in predicting glucose intolerance. Results:(1) Metabolic parameters were higher in obesity group than the control group. There was no difference in body weight, BMI and WHR among subgroups classified by glucose tolerance in people with obesity. (2) The abundance of Dorea, Dorea formicigenerans were higher in obese individuals, however showing a downward trend in accordance with glucose intolerance. The abundance was inversely associated with OGTT-2 h plasma glucose and HbA 1C, while positively associated with HOMA-β. Logistic regression demonstrated that Dorea formicigenerans was an independent protective factor after adjusting confounders such as age and gender in the prevention of glucose intolerance. (3) ROC analysis exhibited that the AUC values of Dorea formicigenerans was 0.73 in the total population. Conclusion:Dorea and Dorea formicigenerans exert protective effect on glucose metabolism in obese subjects. The abundance of Dorea and Dorea formicigenerans can be used as predictors of glucose intolerance risk in obese subjects, which facilitate the early screening and monitoring.
10.Dissecting Causal Relationships Between Gut Microbiota, Blood Metabolites, and Stroke: A Mendelian Randomization Study

Background:
and Purpose We investigated the causal relationships between the gut microbiota (GM), stroke, and potential metabolite mediators using Mendelian randomization (MR).
Methods:
We leveraged the summary statistics of GM (n=18,340 in the MiBioGen consortium), blood metabolites (n=115,078 in the UK Biobank), and stroke (cases n=60,176 and controls n=1,310,725 in the Global Biobank Meta-Analysis Initiative) from the largest genome-wide association studies to date. We performed bidirectional MR analyses to explore the causal relationships between the GM and stroke, and two mediation analyses, two-step MR and multivariable MR, to discover potential mediating metabolites.
Results:
Ten taxa were causally associated with stroke, and stroke led to changes in 27 taxa. In the two-step MR, Bifidobacteriales order, Bifidobacteriaceae family, Desulfovibrio genus, apolipoprotein A1 (ApoA1), phospholipids in high-density lipoprotein (HDL_PL), and the ratio of apolipoprotein B to ApoA1 (ApoB/ApoA1) were causally associated with stroke (all P<0.044). The causal associations between Bifidobacteriales order, Bifidobacteriaceae family and stroke were validated using the weighted median method in an independent cohort. The three GM taxa were all positively associated with ApoA1 and HDL_PL, whereas Desulfovibrio genus was negatively associated with ApoB/ApoA1 (all P<0.010). Additionally, the causal associations between the three GM taxa and ApoA1 remained significant after correcting for the false discovery rate (all q-values <0.027). Multivariable MR showed that the associations between Bifidobacteriales order, Bifidobacteriaceae family and stroke were mediated by ApoA1 and HDL_PL, each accounting for 6.5% (P=0.028) and 4.6% (P=0.033); the association between Desulfovibrio genus and stroke was mediated by ApoA1, HDL_PL, and ApoB/ApoA1, with mediated proportions of 7.6% (P=0.019), 4.2% (P=0.035), and 9.1% (P=0.013), respectively.
Conclusion
The current MR study provides evidence supporting the causal relationships between several specific GM taxa and stroke and potential mediating metabolites.
