1.Effect of Traditional Chinese Medicine Monomers and Compounds on Regulating JAK/STAT Signaling Pathway in Rheumatoid Arthritis Treatment: A Review

Rheumatoid arthritis (RA) is a common chronic systemic autoimmune disease with synovitis as the main manifestation, which often causes joint swelling and pain or even deformity. It is considered to be an incurable lifelong disease. Although the current Western medicine treatment can alleviate the progression of the disease, it has the clinical limitations of liver injury, cardiovascular complications, and other adverse reactions, along with easy recurrence. Traditional Chinese medicine (TCM) has a long history and has the advantages of individualized treatment and fewer adverse reactions. It can effectively relieve the symptoms of joint swelling and pain in RA patients and slow down the progression of bone destruction, which has attracted wide concern in the medical community. Janus kinase (JAK)/signal transducer and activator of transcription (STAT) signaling pathway is an important intracellular pathway involved in cell proliferation, differentiation, apoptosis, immune regulation, and other biological behaviors, and plays an important role in the pathophysiological process of RA. In recent years, many studies have confirmed that TCM monomers and compounds can inhibit inflammation and angiogenesis by regulating the JAK/STAT signaling pathway, induce apoptosis and inhibit proliferation of fibroblast-like synoviocytes (FLS), regulate immune response, and thus exert an effect in the treatment of RA. However, there is still a lack of comprehensive and systematic induction and overview. Therefore, by searching the relevant literature in China National Knowledge Infrastructure (CNKI) and PubMed databases from 2009 to 2024, this study described the mechanism of the JAK/STAT signaling pathway in the occurrence and development of RA and summarized the research progress of TCM monomers and compounds in regulating the JAK/STAT signaling pathway in RA intervention. The study aims to provide new ideas and strategies for the clinical treatment of RA with TCM and the research and development of new drugs.
2.The predictive value of advanced lung cancer inflammation index for postoperative pharyngocutaneous fistula,recurrence,metastasis,and prognosis in laryngeal squamous cell carcinoma

OBJECTIVE To investigate the predictive value of the advanced lung cancer inflammation index(ALI)for postoperative pharyngocutaneous fistula,recurrence,metastasis,and prognosis in patients with laryngeal squamous cell carcinoma(LSCC).METHODS This retrospective study analyzed the clinical data of 114 LSCC patients treated at Huangshi Central Hospital between January 2014 and December 2023.The data included tumor staging,ALI scores,age,gender,smoking history,alcohol history,diabetes,hypertension,hyperlipidemia,coronary heart disease,tumor differentiation,HPV infection,and tumor location.Based on the occurrence of pharyngocutaneous fistula,the patients were divided into the fistula group(10 cases)and the non-fistula group(104 cases).Single-factor and multi-factor logistic regression analyses were conducted to identify independent factors associated with postoperative pharyngocutaneous fistula.Patients were also classified into recurrence and metastasis groups(22 cases)and non-recurrence groups(92 cases),and further into poor prognosis(25 cases)and good prognosis groups(89 cases)based on follow-up results.Cox regression analysis was performed to identify factors influencing recurrence,metastasis,and prognosis.RESULTS Multivariate logistic regression analysis showed that tumor stage(stage Ⅰ-Ⅱ)[HR:0.040(95%CI:0.004-0.392)]and ALI score[HR:0.885(95%CI:0.805-0.972)]were independent protective factors for the occurrence of pharyngeal fistula(P<0.05).Multivariate Cox regression analysis further confirmed that tumor stage(stage Ⅰ-Ⅱ)[OR:0.180(95%CI:0.063-0.518)]and ALI score[OR:0.970(95%CI:0.939-0.992)]were independent protective factors for recurrence,metastasis,and poor prognosis(P<0.05).ROC curve analysis revealed that the AUC for ALI in predicting pharyngeal fistula was 0.910(95%CI:0.849-0.970),with a cutoffvalue of 29.877,sensitivity of 0.827,and specificity of 1.000.The AUC for ALI in predicting recurrence and metastasis was 0.804(95%CI:0.698-0.910),with a cutoffvalue of 34.398,sensitivity of 0.920,and specificity of 0.618.CONCLUSION The ALI score is an independent protective factor for the occurrence of postoperative pharyngocutaneous fistula,recurrence,metastasis,and poor prognosis in LSCC patients.It is a valuable predictor for early warning of postoperative complications,recurrence,metastasis,and prognosis,with potential clinical utility in guiding personalized treatment strategies.
3.The Intra-rater and Inter-rater Consistency of GRBAS and CAPE-V in Chinese Context

Objective To evaluate the intra-rater and inter-rater consistency of GRBAS and CAPE-V in Chi-nese context.Methods Five experts used the"Voice Assessment Assistant"APP to evaluate voice samples extrac-ted from The Sixth Medical Center of Chinese PLA General Hospital Pathological Voice Database V1.0.Fifty-two cases were used for inter-rater consistency analysis,and 38 cases were used for intra-rater consistency assessment.Intraclass correlation coefficient(ICC)was used for intra-rater and inter-rater consistency analysis of CAPE-V,while Cohen's Kappa method was applied for intra-rater consistency analysis of the five features of GRBAS.Inter-rater consistency analysis for GRBAS used Fleiss Kappa method,and Spearman's correlation analysis was conducted for the same features in both scales.Results The results of intra-rater consistency analysis showed that all six fea-tures of CAPE-V exhibited high consistency,with ICC coefficients of overall severity(OS)=0.80,roughness(R)=0.69,breathiness(B)=0.77,strain(S)=0.75,pitch(P)=0.74,and loudness(L)=0.78.For GRBAS,ex-cept for the features G(0.48)and S(0.45)which showed weaker intra-rater consistency,the rest of the features had poor intra-rater consistency.The results of inter-rater consistency analysis showed that the ICC correlation coef-ficients for all features of CAPE-V were greater than 0.85,indicating a high degree of consistency in the scoring of each feature by different raters.For GRBAS,except for the feature G(correlation coefficient=0.48),the rest of the features had correlation coefficients less than 0.40,indicating that the inter-rater consistency was poor for all features except G.When evaluating the same sample with both GRBAS and CAPE-V,the Spearman correlation co-efficients for the overall voice quality OS/G,roughness R,breathiness B,and strain S were 0.89,0.85,0.91,and 0.91,respectively,suggesting a high degree of correlation between the scoring results of the four features in the two scales.Conclusion In the auditory perceptual assessment of Chinese voice samples,the CAPE-V scale has higher intra-rater and inter-rater consistency compared to the GRBAS scale and is more suitable for clinical practice.

The National Institutes of Health Stroke Scale (NIHSS) was used to evaluate disease progression. Based on the progression of the disease, the patients were divided into two groups: the progressive group ( n = 38) and the non-progressive group ( n = 109). ADPN level and the hs-CRP/ALB ratio were compared between the two groups. Additionally, multivariate logistic regression analysis was conducted to identify risk factors for disease progression in ACI patients. The predictive value of ADPN and the hs-CRP/ALB ratio for ACI was assessed using receiver operating characteristic curve analysis. Results:In the progressive group, hs-CRP level and hs-CRP/ALB ratio were (14.38 ± 3.27) mg/L and (0.43 ± 0.13), respectively, both of which were significantly higher than those in the non-progressive group [(8.92 ± 2.73) mg/L, (0.20 ± 0.07), t = 9.22, 10.37, both P < 0.001]. In contrast, the levels of ADPN and albumin in the progressive group were (2.71 ± 0.59) mg/L and (33.54 ± 8.66) g/L, respectively, both of which were significantly lower than those in the non-progressive group [(5.36 ± 1.42) mg/L, (45.05 ± 10.42) g/L, t = -15.99, -6.68, both P < 0.001]. The progressive group had higher admission NIHSS scores, a higher proportion of patients with diabetes, higher white blood cell counts, higher hemoglobin A1c (HbA1c) and fibrinogen levels compared with the non-progressive group ( t = 8.43, 2.88, 5.79, 2.77, χ2 = 4.40, all P < 0.05). Multivariate logistic regression analysis results indicated that higher admission NIHSS scores, HbA1c levels, and hs-CRP/ALB ratios were independent risk factors for disease progression in ACI patients ( OR = 2.438, 1.600, 2.971, all P < 0.05), while higher levels of ADPN were identified as a protective factor against disease progression ( OR = 0.321, P < 0.05). The receiver operating characteristic curve analysis showed that the area under the curve for predicting disease progression after admission for ACI patients using serum ADPN and hs-CRP/ALB ratio alone were 0.730 (95% CI: 0.658-0.800) and 0.758 (95% CI: 0.685-0.826), respectively. The area under the curve for the combined prediction of serum ADPN and the hs-CRP/ALB ratio was 0.895 (95% CI: 0.845-0.940), demonstrating higher predictive efficacy ( Z = 1.80, 2.13, both P < 0.05). Conclusions:ADPN level and hs-CRP/ALB ratio are closely related to the progression of ACI and demonstrate good predictive efficacy for disease progression.

7.Effect of Dapagliflozin on cell proliferation,apoptosis and inflammation of podocytes induced by high glucose via regulating p38 MAPK pathway

Objective To investigate the effect of Dapagliflozin on high glucose-induced podocyte proliferation and apoptosis through p38 mitogen-activated protein kinase(p38 MAPK)pathway.Methods Human glomerular podocytes(HGPC)were divided into control(Con)group,low/medium/high D-glucose(Glu 10,Glu 20,Glu 30)group,high glucose(HG)group,low/medium/high concentration Dapagliflozin(HG+Dap 12.5,HG+Dap 25,HG+Dap 50)group,Dapagliflozin(HG+Dap)group,inhibitor(HG+ SB 203580)group,Dapagliflozin + inhibitor(HG+Dap+SB 203580)group and Dapagliflozin + activator(HG+Dap+C16-PAF)group.After 24 hours of intervention,the cell viability,proliferation rate,apoptosis rate and levels of related factors were tested.Results Compared with Con group,IL-1β,TNF-α,apoptosis rate,Caspase-3 mRNA and protein expression,p53,p-p38 MAPK protein expression were increased(P<0.05),while cell proliferation rate,Cyclin D1 mRNA and protein expression were decreased in HG group(P<0.05).Compared with HG group,the proliferation rate,Cyclin D1 mRNA and protein expression were increased(P<0.05),while IL-1β,TNF-α,apoptosis rate,Caspase-3 mRNA and protein expression,p53,p-p38 MAPK protein expression were decreased in the HG+Dap and HG+SB 203580 groups(P<0.05).Compared with HG+Dap group,cell proliferation rate,Cyclin D1 mRNA and protein expression were increased(P<0.05),while IL-1β,TNF-α,apoptosis rate,Caspase-3 mRNA and protein expression,p53,p-p38 MAPK protein expression were decreased in HG+Dap+SB 203580 group(P<0.05).In HG+Dap+C16-PAF group,IL-1β,TNF-α,apoptosis rate,Caspase-3 mRNA and protein expression,p53,p-p38 MAPK protein expression were increased(P<0.05),while cell proliferation rate,Cyclin D1 mRNA and protein expression were decreased(P<0.05).Conclusion Dagagliflozin can promote HGPC proliferation and inhibit apoptosis and inflammation in high D-glucose environment,and its mechanism may be related to the inhibition of p38 MAPK pathway signal transduction.
8.Research progress on metformin in the treatment for malignant tumors

In recent years, the potential anti-tumor effects of metformin have attracted widespread attention in the field of cancer treatment. This article summarizes the research progress of metformin in the treatment of malignant tumors,finding its potential application in the treatment of malignant tumors in the digestive system (biliary tract cancer,gastric cancer,esophagus cancer,colorectal cancer,pancreatic cancer,liver cancer) and reproductive system (prostate cancer,ovarian cancer,breast cancer, cervical cancer),non-small cell lung cancer,renal cell carcinoma,and melanoma. Metformin can inhibit the proliferation of tumor cells and extend the overall survival of patients. Its mechanisms of action include,but are not limited to,inhibiting the activity of mitochondrial complex Ⅰ,activating adenosine monophosphate-activated protein kinase/p53 signaling pathway,and blocking the cell cycle. Additionally,the combined use of metformin with chemotherapy drugs has shown potential for reducing toxicity and enhancing efficacy. It can enhance the sensitivity of biliary tract cancer,ovarian cancer,and melanoma cells to chemotherapy drugs, improve the drug resistance of gastric and colorectal cancer cells to chemotherapy,and reduce the toxic reactions of breast cancer patients during chemotherapy. Metformin is also used as an immunomodulator,applied in the immunotherapy of patients with esophagus cancer,colorectal cancer,cervical cancer,non-small cell lung cancer,and melanoma.

Standardized reporting is a crucial factor affecting the use of patient guidelines (PGs), particularly in the reporting and presentation of recommendations. This paper introduced the current status of PG reporting, including the research on PG content and presentation formats, and provided comprehensive recommendations for PG reporting from aspects such as overall framework, recommendations, presentation format, and readability. First, the presentation of PG recommendations should include clearly defined clinical questions, recommendations and their rationale, and guidance on how patients should implement the interventions; for specific content in the PG, such as level of evidence, level of recommendation, it is recommended to explain in text the reasons for giving different levels of recommendation, i.e., to present the logic behind giving the level of recommendation to the patient; additional information needed in the recommendation framework should be supplemented by tracing references or authoritative textbooks and literature that support the recommendations. Subsequently, the PG text should be written based on the Reporting Checklist for Public Versions of Guidelines (RIGHT-PVG) reporting framework. Finally, to enhance readability and comprehension, it is recommended to refer to the Patient Education Materials Assessment Tool (PEMAT) for translating PG content. To enhance the readability of PGs, it is suggested to present the PG content in a persona-lized and layered manner.

Since the concept of patient versions of guidelines (PVGs) was introduced into China, several PVGs have been published in China, but we found that there is a big difference between the concept of PVG at home and abroad, and the reason for this difference has not been reasonably explained, which has led to ambiguity and even misapplication of the PVG concept by guideline developers. By analyzing the background and purpose of PVGs, and the understanding of the PVG concept by domestic scholars, we proposed the term patient guidelines (PGs). This refers to guidelines developed under the principles of evidence-based medicine, centered on health issues that concern patients, and based on the best available evidence, intended for patient use. Except for the general attribute of providing information or education, which is typical of common health education materials, PGs also provide recommendations and assist in decision-making, so PGs include both the patient versions of guidelines (PVG) as defined by the Guidelines International Network (GIN) and "patient-directed guidelines", i.e. clinical practice guidelines resulting from the adaptation or reformulation of recommendations through clinical practice guidelines.
