1.Effects of Huoxue Xiaoyi Formula (活血消异方) on Tfh Cells and the JAK/STAT Pathway in Ectopic Tissues of Ovarian Endometriosis Model Rats

ObjectiveTo explore the potential mechanism of Huoxue Xiaoyi Formula (活血消异方, HXF) in treating ovarian endometriosis (OEM) from the perspective of T follicular helper (Tfh) cells and the Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway. MethodsForty-five female SD rats with normal estrous cycles were randomly divided into three groups, HXF group, model group, and normal group, with 15 rats in each group. A rat model of OEM was established by autologous endometrial tissue implantation. After successful modeling, the treatment group received HXF at 5.85 g/(kg·d) by gavage for 14 consecutive days. The model group and normal group received 1 mL/d of normal saline by gavage. RNA-sequencing data from human proliferative-phase endometriotic and normal endometrial tissues were downloaded from the GEO database. Transcriptomic sequencing was used to analyze gene expression in rat ovarian ectopic tissues and normal uterine tissues, and comparisons were made with human data to verify JAK/STAT pathway activation in proliferative-phase ectopic tissues. Immunohistochemistry was used to detect the positive expression of CXC chemokine receptor 5 (CXCR5) and interleukin-21 (IL-21) in rat ovarian ectopic and normal uterine tissues. Western Blotting was performed to detect the protein levels of IL-21, IL-21 receptor (IL-21R), Janus kinase 1 (JAK1), signal transducer and activator of transcription 6 (STAT6), and B-cell lymphoma 2 (Bcl-2). Tfh cell infiltration was analyzed using immune cell infiltration methods. ResultsGene set enrichment analysis showed that the JAK/STAT pathway was significantly activated in human proliferative-phase endometriotic tissues compared to normal endometrial tissues. Similarly, the JAK/STAT pathway was markedly activated in rat ovarian ectopic tissues in the model group compared to the normal group, but suppressed in the HXF group compared to the model group. Compared with normal uterine tissues, ovarian ectopic tissues in the model group showed increased Tfh cell infiltration scores, higher CXCR5 and IL-21 expression, and elevated levels of IL-21, IL-21R, JAK1, STAT6, and Bcl-2 proteins. Compared with the model group, HXF group showed reduced CXCR5 and IL-21 expression and decreased protein levels of IL-21, IL-21R, JAK1, STAT6, and Bcl-2. ConclusionHXF may suppress activation of the JAK/STAT signaling pathway in ovarian endometriotic tissues by inhibiting IL-21 secretion from Tfh cells.

Objective To investigate the relationship between tooth loss and the occurrence of esophageal cancer in a natural village in Wenfeng District, Anyang City, Henan Province. Methods A prospective cohort study was conducted to observe the occurrence of tooth loss and esophageal cancer among the asymptomatic residents of the natural village for 16 years from January 2008 to July 2024. Data were analyzed by chi-square test, binary logistic regression, and restricted cubic spline. Results Among the total population of 711 cases, 136 cases were lost to follow-up and 575 cases were included in the final statistics, including 45 cases with esophageal cancer. Significant statistical difference was found between esophageal cancer patients with and without tooth loss (P<0.05). Ulcerative colitis (UC) is a refractory disease of the digestive system characterized by diverse etiologies, complex pathogenesis, a prolonged course, and frequent relapses. In recent years, the incidence of UC has been increasing annually, severely impairing patients' quality of life, posing a risk of malignant transformation that may threaten patients' lives, and resulting in a substantial medical burden. Traditional Chinese medicine (TCM) compound formulas, with their advantages of multi-component and multi-target actions, have become a new therapeutic option for UC. The NOD-like receptor pyrin domain-containing 3 (NLRP3) inflammasome is a core component of innate immunity, and its aberrant activation is closely associated with the onset and progression of UC, involving multiple processes such as inflammation and oxidative stress, and exhibiting crosstalk with pathways including nuclear factor-κB (NF-κB), nuclear factor erythroid 2-related factor 2 (Nrf2), and thioredoxin-interacting protein (TXNIP). At present, NLRP3 has become one of the most intensely studied hotspots in UC-related research. Although increasing studies have focused on the regulation of the NLRP3 inflammasome by TCM compound formulas for UC treatment, challenges remain due to the complex pathogenesis of UC and the compositional diversity of TCM, hindering the realization of precision therapy. In this context, by reviewing literature from the past decade, this paper summarizes the activation process of NLRP3 and its relationship with UC, and elucidates the roles and mechanisms by which TCM compound formulas regulate the NLRP3 inflammasome and related signaling pathways, with a view to providing a reference for further research into the pathogenesis of UC, TCM treatment strategies, and their mechanisms of action.

OBJECTIVE:
To observe the effects of moxibustion at "Feishu" (BL13) and "Xinshu" (BL15) on the circular RNA of exon 2-5 of the Pan3 gene (circPAN3), forkhead box O3 (FOXO3), and Bcl-2/adenovirus E1B19kDa-interacting protein 3 (BNIP3) in rats with chronic heart failure (CHF), and explore the potential mechanisms of moxibustion in alleviating myocardial fibrosis.
METHODS:
Ten rats of 60 male SPF-grade SD rats were randomly assigned into a normal group. The remaining rats underwent left anterior descending coronary artery (LAD) ligation to establish the CHF model. Forty successfully modeled rats were randomly divided into a model group, a moxibustion group, a rapamycin (RAPA) group, and a moxibustion+RAPA group, with 10 rats in each group. The moxibustion group received mild moxibustion at bilateral "Feishu" (BL13) and "Xinshu" (BL15), 30 min per session. The RAPA group received intraperitoneal injection of the autophagy activator RAPA (1 mg/kg). The moxibustion+RAPA group first received RAPA injection, followed by mild moxibustion at bilateral "Feishu" (BL13) and "Xinshu" (BL15). All interventions were administered once daily for 4 consecutive weeks. After the intervention, cardiac ultrasound was used to measure ejection fraction (EF) and left ventricular fractional shortening (FS). Serum placental growth factor (PLGF) level was determined by ELISA. Myocardial tissue morphology and collagen volume were assessed using hematoxylin-eosin (HE) staining and Masson's trichrome staining. The expression levels of circPAN3, FOXO3, and BNIP3 mRNA in myocardial tissue were detected by real-time PCR, while FOXO3 and BNIP3 protein expression levels were analyzed by Western blot.
RESULTS:
Compared with the normal group, the model group exhibited myocardial cell disorder, severe fibrosis, and increased collagen volume (P<0.01), along with significantly decreased EF, FS, and circPAN3 mRNA expression in myocardial tissue (P<0.01), and the serum PLGF level, as well as FOXO3 and BNIP3 mRNA and protein expression in myocardial tissue were increased (P<0.01). Compared with the model group, the moxibustion group showed reduced myocardial fibrosis, decreased collagen volume (P<0.01), increased EF, FS, and circPAN3 mRNA expression in myocardial tissue (P<0.01), and decreased serum PLGF level as well as FOXO3 and BNIP3 mRNA and protein expression in myocardial tissue (P<0.01). Compared with the model group, the RAPA group showed further deterioration in these parameters (P<0.01). Compared with the RAPA group, the moxibustion+RAPA group exhibited alleviation of myocardial fibrosis, reduced collagen volume (P<0.01), increased EF, FS, and circPAN3 mRNA expression in myocardial tissue (P<0.01), and decreased serum PLGF level as well as FOXO3 and BNIP3 mRNA and protein expression in myocardial tissue (P<0.01).
CONCLUSION
Moxibustion could alleviate myocardial fibrosis in CHF rats, possibly through upregulation of myocardial circPAN3 expression, downregulation of FOXO3 and BNIP3 expression, and inhibition of excessive myocardial autophagy.

7.Multimorbidity pattern and traditional Chinese medicine syndrome characteristics of patients with adenomyosis:A cross-sectional study

Objective To explore the multimorbidity pattern and traditional Chinese medicine(TCM)syndrome characteristics of patients with adenomyosis(AM)and to provide insights and data support for medical treatment and comprehensive AM management.Methods By retrospectively analyzing multi-center,large-sample,nationwide cross-sectional survey data,patients were divided into a single AM group or a multimorbidity group based on whether they had other diseases.The prevalence rates of specific multimorbidity or a combination of multimorbidities were calculated respectively.Single-factor analysis was used to explore the influence of age,course of disease,education level,labor nature,monthly income,production history,abortion history,operation history,region,and TCM syndromes on multimorbidity in patients with AM.The multimorbidity patterns of patients with AM were explored using Apriori association rules,and the distribution characteristics of TCM syndromes in different multimorbidity patterns were explored.Results Among 1,814 patients with AM,1,429 were in the multimorbidity group(prevalence of 78.78%).Compared with the single AM group,the patients in the multimorbidity group had a long disease course,high academic qualifications,jobs that mostly involved sitting,a history of surgery,and were primarily concentrated in the north(P<0.05).The proportion of patients with qi stagnation and blood stasis syndrome decreased,whereas the proportion of patients with kidney deficiency and blood stasis syndrome increased.Ten multimorbidity association rules were sorted according to the confidence level,and the first place of binary association rules was breast nodule to breast hyperplasia,and the first place of ternary association rules was hysteromyoma,breast nodule to breast hyperplasia.Comprehensive support and confidence level,in the binary association rules,the strong association rule was breast nodule to thyroid nodule,and sub-strong association rule was thyroid nodule to breast hyperplasia;In the ternary association rules,the strong association rule was hysteromyoma,breast nodules to breast hyperplasia,and sub-strong association rule was endometriosis,thyroid nodules to breast hyperplasia.There was no significant difference in the distribution of TCM syndromes in AM patients with different binary and ternary multimorbidity patterns.Conclusion Patients with AM have different multimorbidity patterns,and the TCM syndromes of patients with AM under different multimorbidity patterns have different characteristics.The management system of AM multimorbidity should be further developed to ensure more comprehensive and standardized health management for patients with AM.

Objective To analyze the clinical efficacy and safety of Huoxue Xiaoyi decoction combined with exercise in the treatment of ovarian endometriosis.Methods 36 patients with ovarian endometriosis treated in Guang'anmen Hospital,China Academy of Chinese Medical Sciences from May 2022 to November 2022 were selected and treated with Huoxue Xiaoyi decoction combined with exercise for 3 months.The size of ovarian ectopic cyst,dysmenorrhea VAS score,PBAC score,female hormones,serum CA125 and CA199,catecholamines and cytokines levels were observed to evaluate the clinical efficacy and safety.Results After treatment,the maximum diameter of ovarian ectopic cyst,dysmenorrhea VAS score,PBAC score,cytokines(serum IL-1β,IL-2,IL-4,IL-6,IL-8,IL-10)levels were significantly lower than those before treatment(P<0.05).The levels of SHBG and adrenaline were significantly higher than those before treatment(P<0.05).During the treatment,the patient had no adverse drug reactions,vital signs,liver,and kidney function indexes were normal.Conclusion Huoxue Xiaoyi decoction combined with exercise therapy can reduce the size of ovarian ectopic cyst,relieve dysmenorrhea,regulate menstrual volume,increase the level of adrenaline,increase the level of sex hormone binding globulin,reduce the level of cytokines,and has fewer adverse reactions and good safety.
9.Comparison of clinical efficacy of different doses of rituximab combined with tacrolimus in the treatment of idiopathic membranous nephropathy

Objective To investigate the effect of two treatment regimens combining Tacrolimus(TAC)with different Rituximab(RTX)dosages,and to provide clinical reference for treatment strategies.Methods A retrospective analysis was conducted on patients diagnosed with idiopathic membranous nephropathy(IMN)and treated with RTX combined with TAC regimen(RTX+TAC group and low-dose RTX+TAC group)in The First Affiliated Hospital of Xinxiang Medical University.Propensity score matching(PSM)was performed at a 1:1 ratio,and a total of 60 patients were enrolled,with 30 in each group.In low-dose RTX(375 mg/m2 at the first and fifteenth day respectively)+TAC group,if circulating B cells(CD19?)exceeded 5 cells/μL after 3 months,a 200 mg RTX infusion was administered.In RTX(1g at the first and fifteenth day respectively)+TAC group,if complete remission(CR)was not achieved by 6 months,an additional 1000 mg RTX infusion was administered.The incidence of CR,partial remission,and adverse events were followed up for 12 months after medication in both groups.Results(1)Both groups showed significant reductions in 24-hour proteinuria,with the RTX+TAC group demonstrating a notably higher decrease compared to the low-dose RTX+TAC group.Statistical differences were observed between the two groups at the 1st and 3rd months of treatment(P<0.05).Albumin levels gradually increased,and there were differ-ences between the two groups at both the 1st and 3rd months(P<0.05).The anti-phospholipase A2 antibody levels decreased significantly after one month of treatment[3.45(1.90,22.10)vs.3.28(8.30,23.08)RU/mL],P>0.05.At 3 months of treatment,the overall clinical remission rate was 63.3%for the RTX+TAC group compared to 36.7%for the low-dose RTX+TAC group(P<0.05).At 12 months,the RTX+TAC group achieved an overall remission rate of 86.7%,while the low-dose RTX+TAC group reached 83.3%,showing no statistical significance(P>0.05).After one month of treatment,the RTX+TAC group achieved a complete serological immunological remission rate of 33.3%,significantly higher than the 3.3%in the low-dose RTX+TAC group(P<0.05).(2)The cumulative remission rate of the RTX+TAC group was higher than that of the low-dose RTX+TAC group during the first 6 months of follow-up.The remission rate in the low-dose RTX+TAC group increased significantly after 6 months.Log-rank test showed no statistical difference between the survival curves of the two groups(P=0.37).(3)Based on a multifactorial COX regression analysis of factors related to remission in patients with IMN,for every unit increase in serum immunological remission time,the risk of patients achieving remission decreased by 13.5%(HR=0.87,P=0.016).The risk of remission for patients with high titers of anti-PLA2R antibodies decreased by 60.2%(HR=0.39,P=0.018).Conclusions Different RTX dosages yielded comparable overall clinical remission rates without significantly increasing adverse events.RTX+TAC regimen achieves higher early CR rate.Serological remission time and high titer anti-PLA2R antibodies are associated with clinical outcomes.
10.Comparison of the effects of two different concentrations of mydriatic drugs in Chinese type 2 diabetic patients: a randomized controlled trial

Objective:To compare the mydriatic effects of a combination of 1% tropicamide and 2.5% phenylephrine with a 0.5% tropicamide and 0.5% phenylephrine combination in patients with type 2 diabetes.Methods:A randomized, double-blind, controlled trial was conducted.Ninety Chinese patients (90 eyes) with dark irises and type 2 diabetes who needed mydriasis examination at the Fundus Disease Clinic of Tianjin Medical University Eye Hospital from June to September 2024 were included.The subjects were divided into control group (30 patients 30 eyes), high concentration group (30 patients 30 eyes) and half-dilution group (30 patients 30 eyes) using the random number table method, which received 2 drops of a mixture of 0.5% tropicamide and 0.5% phenylephrine, 2 drops of a mixture of 1% tropicamide and 2.5% phenylephrine, 1 drop of a mixture of 1% tropicamide and 2.5% phenylephrine+ 1 drop of saline respectively.The pupil diameter of the patients was measured with a pupillometer 40 minutes before and after instillation.The study adhered to the Declaration of Helsinki and the study protocol was approved by the Ethics Committee of Tianjin Medical University Eye Hospital (No.2024KY-16).Written informed consent was obtained from all participants.Results:The proportions of patients whose pupil diameters reached 7 mm 40 minutes after the initial administration in the control group, high-concentration group, and half-dilution group were 56.7%(17/30), 86.7%(26/30) and 66.7%(20/30), respectively, with a statistically significant overall difference ( χ2=6.667, P=0.036).The proportion of patients in the high-concentration group whose pupil diameter reached 7 mm 40 minutes after the initial administration was higher than that in the control group, and the difference was statistically significant ( P<0.05).The pupil diameters 40 minutes after the initial administration in the control group, the high-concentration group and the half-dilution group were (7.01±0.86), (7.64±0.61) and (7.49±1.15)mm, respectively, with a statistically significant overall difference ( F=4.019, P=0.021), and the pupil diameter of the high-concentration group was significantly higher than that of the control group ( P=0.024).Changes in pupil diameter 40 minutes after the initial administration in the control group, high-concentration group and half-dilution group were (3.23±0.81), (3.82±0.60) and (3.62±0.75)mm, respectively, with a statistically significant overall difference ( F=5.121, P=0.008), and the change in pupil diameter in the high-concentration group was higher than that in the control group ( P=0.007). Conclusions:The combination of 1% tropicamide and 2.5% phenylephrine has better pupil dilation than the combination of 0.5% tropicamide and 0.5% phenylephrine.It is recommended that pupil dilation be performed with a high-concentration mydriatic drug prior to outpatient fundus examination for diabetic patients.
