1.Effect Difference and Mechanisms of Zishenwan Against Chronic Prostatitis Before and After Salt-processing of Anemarrhenae Rhizoma and Phellodendri Chinensis Cortex by Integrating Network Pharmacology and Metabolomics

ObjectiveThis paper aims to systematically reveal the effect difference and mechanisms of Zishenwan against chronic prostatitis (CP) before and after salt-processing of Anemarrhenae rhizoma and Phellodendri chinensis cortex based on an integrated strategy of ultra-high performance liquid chromatography-quadrupole-orbitrap mass spectrometry (UPLC-Q-Orbitrap-MS/MS), network pharmacology, and serum metabolomics. MethodsZishenwan samples before and after salt-processing of Anemarrhenae rhizoma and Phellodendri chinensis cortex were extracted by alcohol-water dual extraction. The chemical components of each sample were detected by UPLC-Q-Orbitrap-MS/MS, and differential components were screened by multivariate statistical analysis. Network pharmacology analysis was performed based on the identified chemical components of Zishenwan to construct a protein-protein interaction (PPI) network of "component, target, and pathway", and the core components, targets, and pathways of Zishenwan against CP were screened. Forty-two male Sprague-Dawley (SD) rats were randomly divided into a blank group, a model group, a Qianliekang group (1.54 g·kg-1), low- and high-dose raw Zishenwan groups (1.8, 5.4 g·kg-1), and low- and high-dose salt-processed Zishenwan groups (1.8, 5.4 g·kg-1). The CP rat model was established by intraprostatic injection of carrageenan. After one week of recovery, the rats were administered the corresponding drugs for 21 days, while those in the blank group and model group received the same volume of normal saline. After the experiment, serum and tissue samples were collected to evaluate pharmacodynamic indicators including organ indices, histopathology, and inflammatory factors in serum. Subsequently, untargeted serum metabolomics technology was used to analyze metabolite changes and perform pathway enrichment analysis. The network pharmacology was used to construct a network of "differential metabolite, reaction, enzyme, and gene". ResultsA total of 76 components were identified in raw and salt-processed Zishenwan, and 34 differential components were screened by multivariate statistical analysis. Among them, the contents of 14 components, including berberine, berberrubine, and phellodendrine, increased after salt-processing, while the contents of 20 components, such as neomangiferin, decreased. The 28 active components and 185 potential targets were screened out by network pharmacology. The core components included berberine, phellodendrine, magnoflorine, and jatrorrhizine, and the core targets included signal transducer and activator of transcription 3 (STAT3), protein kinase B1 (Akt1), and transcription factor AP-1 (JUN). These targets were significantly enriched in pro-inflammatory signaling pathways such as phosphatidylinositol 3-kinase/protein kinase B (PI3K/Akt) and mitogen-activated protein kinase (MAPK). Compared with the model group, all Zishenwan administration groups showed decreased prostate index, reduced levels of interleukin (IL)-1β, IL-18, and B-cell lymphoma-2 (Bcl-2) in serum (P<0.05, P<0.01), as well as varying degrees of alleviation in histopathological damage. At the same dose, compared with the raw Zishenwan groups, the salt-processed Zishenwan groups showed lower prostate index, pathological scores, and IL-1β, IL-18, and Bcl-2 levels in serum, but the differences were not statistically significant. Metabolomics reveals that 38 differential metabolites were reversed after salt-processed Zishenwan administration. Both raw and salt-processed Zishenwan regulated pathways such as β-alanine metabolism and tryptophan metabolism. In addition to the common regulated pathways, the salt-processed group specifically regulated pantothenate and coenzyme A biosynthesis, pyrimidine metabolism, and arginine and proline metabolism. The intersecting pathways between network pharmacology and metabolomics were tryptophan metabolism and arginine and proline metabolism, with overlapping targets including monoamine oxidase A (MAOA) and arginase 1 (ARG1). ConclusionThe increased contents of components such as berberine and phellodendrine in salt-processed Zishenwan may enhance its therapeutic effect on CP by inhibiting the PI3K/Akt and MAPK signaling pathways, along with multi-target regulation of tryptophan, arginine, and pantothenate metabolism pathways to comprehensively regulate inflammatory and immune responses.
2.A Comparative Study of Artificial Intelligence-based Classification Versus Manual Classification of Medical Adverse Events: Taking the DeepSeek Large Language Model As an Example

To analyze the application value of artificial intelligence (AI)-based classification in the categorization of medical adverse events. Medical adverse events reported to the Adverse Event Reporting System of Peking Union Medical College Hospital from September 1, 2023, to August 31, 2024, were retrospectively collected as the study subjects. After de-identification of adverse events meeting the inclusion criteria, conventional manual classification and AI-based classification using a large language model (DeepSeek-R1 Full-Context Internet Edition) were performed. The time required for classification using both methods was recorded, and the consistency and discrepancies between the two methods were compared. Using manual classification as the gold standard, the accuracy of AI-based classification was comprehensively evaluated. A total of 273 medical adverse events were analyzed. Manual classification took 38 838 seconds in total, with an average of 14.22 seconds per event. AI-based classification took 600 seconds in total, with an average of 2.19 seconds per event. The two methods showed consistent classification in 202 events and inconsistent classification in 71 events, yielding an overall agreement rate of 73.99% and a Kappa coefficient of 0.646 (95% CI: 0.575-0.717), with a standard error of 0.0362. Using manual classification as the gold standard, AI-based classification achieved accuracy ranging from 80% to 100%, precision from 30% to 100%, recall from 40% to 100%, F1 scores from 0.46 to 0.79, and specificity from 46% to 98%. Notably, AI-based classification demonstrated balanced and overall excellent performance in the categorization of device-related and drug-related adverse events. The DeepSeek large language model can assist in improving the efficiency of medical adverse event classification, showing promising application potential, particularly in the categorization of device-related and drug-related adverse events.
3.Evaluation on repeatability and accuracy of iCare IC100 tonometer in measuring intraocular pressure

AIM: To evaluate the repeatability and accuracy of iCare IC100 tonometer in measuring intraocular pressure(IOP)by comparing the correlation and difference with Goldmann applanation tonometry(GAT)and non-contact tonometer(NCT), and to compare the correlation of the three types of IOP measurement with the central corneal thickness(CCT).METHODS: Prospective study. A total of 90 outpatients(90 eyes)in Liaoning Aier Eye Hospital from March 2019 to May 2019 were randomly selected as study subjects. All patients were measured IOP using iCare IC100, NCT, and GAT. The interclass correlation coefficient(ICC)was used to evaluate the repeatability of IOP measured 3 times consecutively using an intraocular tonometer. The correlation and consistency of iCare IC100, GAT and NCT were compared by one-way ANOVA, Pearson linear correlation analysis and Bland-Altman analysis. The linear regression analysis was used to analyze the correlation of the three tonometers with CCT.RESULTS: The mean IOP measured with iCare IC100, GAT and NCT was 19.74±6.90, 19.88±7.07 and 18.47±6.31 mmHg, respectively(F=1.180, P=0.309). The measurements of iCare IC100 with GAT, iCare IC100 with NCT and GAT with NCT were all positively correlated(r=0.930, 0.946, 0.918, all P<0.05), the Bland-Altman analysis showed that the mean differences between iCare IC100 and GAT, iCare IC100 and NCT, GAT and NCT were -0.142±2.61, 1.27±2.24, and 1.41±2.81 mmHg, respectively, with 97%(87/90), 96%(86/90), and 97%(87/90)IOP differences distributed within their 95% confidence intervals. The IOP measured with iCare IC100 and CCT, GAT and CCT and NCT and CCT were all positively correlated(r=0.426, 0.353, 0.451, all P<0.01). The linear regression equations between iCare IC100, GAT and NCT measurement and CCT were iCare IC100 IOP=-19.62+0.074×CCT; GAT IOP=-13.54+0.063×CCT; NCT IOP=-19.65+0.072×CCT; that is, for every 10 μm increase in CCT, iCare IC100 measurement increased by 0.74 mmHg, GAT measurement increased by 0.63 mmHg, and NCT measurement increased by 0.72 mmHg.CONCLUSION: The iCare IC100 tonometer has good repeatability and accuracy in measuring IOP, and the CCT has a greater impact on the measurement of iCare IC100 than the GAT and NCT.

Cardiovascular disease is one of the serious health burdens in the world,and atherosclerosis is its impor-tant pathological basis.In recent years,the effect of environmental pollutants on human health has been paid more and more attention.Some new epidemiological studies and experiments have reported that environmental organic pollutants,involved in the occurrence and development of atherosclerosis,is an independent risk factor of cardiovascular diseases.Evidence shows that it is related to oxidative stress response,cell phenotype transformation,dyslipidemia,genetic changes and so on.This review summarizes available studies on the latest epidemiological studies and experiments of typical per-sistent organic pollutants such as polychlorinated biphenyls and per-and polyfluoroalkyl substances so as to provide refer-ences for future research.
6.Remodeling characteristics of super-enhancers in intestinal gastric cancer and their roles in upregulating CEMIP expression

Objective To map the super-enhancers remodeling of intestinal gastric cancer and reveal the tumor biological functions of the super-enhancers and the downstream target genes that may be activated.Methods A total of 31 normal gastric mucosal tissues,23 intestinal gastric cancer tissues and 9 intestinal gastric cancer organoids were collected from the Department of Gastroenterology of Army Medical Center of PLA from January to December 2022.Chromatin targeting histone H3K27ac modified chromatin targeting cleavage under targets and tagmentation(CUT&Tag)sequencing was conducted on above tissues.The remodeling profiles of super-enhancers in intestinal gastric cancer were analyzed and the key target genes were identified based on bioinformation tools.CRISPRi technology was used to intervene with the super-enhancers,the expression of target genes was detected with Western blotting,and the proliferation,migration and invasion abilities were detected by CCK-8 assay and Transwell chambers in the control group and the intervention group.Results There was a significant difference in the signal of super-enhancers between intestinal gastric cancer tissues and normal gastric mucosal tissues(P<0.05),and the active super-enhancers in cancer tissues may be involved in biological processes such as negative regulation of the immune system and cell adhesion.The expression of up-regulated cell migration-inducing protein(CEMIP)in tumor cells was regulated by the super-enhancers,and intervening the super-enhancers down-regulated the expression of CEMIP(P<0.05),and inhibited the cell proliferation,invasion and migration abilities of tumor cells(P<0.05).Conclusion Super-enhancer remodeling is observed in intestinal gastric cancer,and they can up-regulate the expression of CEMIP gene and promote the growth,migration and invasion of cancer cells.
7.Remodeling of enhancers in high-grade epithelial dysplasia of gastric mucosa and its effect on expression of proliferation-related gene CD24

Objective To identify the enhancer profile marked by histone H3K27ac modification in high-grade intraepithelial neoplasia(HGIN)in order to reveal the novel regulatory mechanism of HGIN pathogensis.Methods Gastric tissue samples were collected from Department of Gastroenterology of Army Medical Center of PLA between June 2022 and June 2023,including 14 normal gastric tissues(Nor group),31 HGIN tissues(HGIN group)and 17 gastric cancer tissues(GC group).Cleavage under targets and tagmentation(CUT&Tag)technique was employed to capture enhancer regions modified by histone H3K27ac.Multi-omics analysis was performed to identify HGIN-specific active enhancers and their potentially regulated genes.Immunohistochemical profiling was performed to assess differential expression of the gene of interest across clinically stratified specimens,combined with CRISPR-dCas9-mediated ablation of active enhancers to monitor the gene of interest transcriptional dynamics and validate enhancer-mediated regulatory mechanisms.Results Epigenomic sequencing obtained the data with excellent quality,and indicated that obvious remodeling was observed in H3K27ac enhancers in HGIN and GC groups(P<0.05),though no significant difference in the genome-wide distribution of H3K27ac modification among the 3 groups.Combining transcriptome data revealed that enhancer remodeling may up-regulate the expression of the proliferation-related target gene,CD24,in the HGIN tissue;while,inhibiting enhancer activity can notably reduce CD24 expression level(P<0.05).Immunohistochemical assay displayed a positive correlation between the expression levels of CD24 and Ki-67(P<0.001).Conclusion The remodeling of H3K27ac enhancer represents a significant epigenetic feature of the transformation from normal condition to HGIN.Remodeling of H3K27ac enhancer up-regulates CD24,which may facilitate the abnormal proliferation of gastric epithelial cells.
8.Features of tumor cells and microenvironment associated with recurrence risk of mesenchymal-subtype gastric cancer based on bulk RNA-seq and scRNA-seq

Objective To analyze clinical characteristics of mesenchymal-subtype gastric cancer(Mes-GC)by integrating multi-omics data and explore the characteristics of tumor cells and microenvironment associated with the risk for recurrence.Methods Gastric tumor tissue samples were collected from the patients who visited Department of Gastroenterology of Army Medical Center of PLA from January 2022 to December 2023.Transcriptome and genome sequencing were applied for these tissue samples,including 19 cases of diffuse-type gastric cancer,22 cases of intestinal-type gastric cancer,and 23 cases of mixed-type gastric cancer patients.Bioinformatics analysis was employed to investigate the differences in clinical characteristics and tumor microenvironment between Mes-GC and non-mesenchymal-subtype gastric cancer(non-Mes-GC)by integrating data resources including The Cancer Genome Atlas(TCGA),Gene Expression Omnibus(GEO),and National Genomics Data Center(NGDC).Results Compared to non-Mes-GC patients,Mes-GC ones were characterized by later clinical stages,deeper tumor infiltration,and higher rates of lymph node metastasis.Kaplan-Meier survival analysis confirmed that Mes-GC patients were associated with shorter survival time,poor prognosis as well as increased risk of cancer recurrence(P<0.05).Single-cell RNA sequencing data revealed that tumor cells in Mes-GC showed higher expression levels of the genes related to stemness,metastasis(P<0.05),and epithelial-mesenchymal transition(EMT).And in the tumor microenvironment,there were significant more myeloid cells,smooth muscle cells,endothelial cells and fibroblasts,with the most pronounced elevation in the proportion of fibroblasts(P<0.05).Moreover,the patients with larger proportion of fibroblasts were associated with poorer prognosis.Conclusion Mes-GC tumor cells exhibit higher stemness and EMT characteristics,and stromal cells such as myeloid cells,endothelial cells,and fibroblasts are enriched in the tumor microenvironment.These features may be key factors contributing to poor prognosis and high recurrence rate of Mes-GC.
9.Enhancer remodeling characteristics in diffuse-type gastric cancer and role in upregulating GDF15 expression and promoting cancer cachexia

Objective To identify the enhancer landscape marked by histone H3K27ac modifications in diffuse-type gastric cancer(DGC)tissues,and to elucidate the epigenetic remodeling mechanisms by which active enhancers regulate cachexia-related genes.Methods Gastric mucosal tissue samples were collected from Department of Gastroenterology of Army Medical Center of PLA during January 2022 to March 2023,including 10 normal gastric mucosa tissues(Normal group),10 DGC tissues diagnosed with cachexia(DGC group),and 10 organoids derived from DGC tissues(Organoid group).Using H3K27ac chromatin targeting cleavage and tagmentation(CUT&Tag)technology,genomic modification regions were captured to screen specific active enhancers and their potential target genes in DGC tissues.CRISPR-dCas9 gene editing technology was used to intervene with the enhancers,and the expression of target genes was detected with Western blotting and qRT-PCR.Sixteen female SPF-grade BALB/c Nude mice(6~8 weeks old,weighing 18~21 g)were utilized to establish an orthotopic xenograft tumor model using the human diffuse-type gastric cancer cell line MKN45.Cachexia-related phenotypes were evaluated in 3 groups:normal group(n=4),silencing group(n=6),and control group(n=6).Results Significant differential enhancer regions were identified between DGC and normal gastric mucosa tissues.DGC tissues exhibited a marked increase in enhancer abundance(P<0.05)and signal intensity when compared with the normal counterparts.Integrated analysis of transcriptome data revealed that some of these active enhancers up-regulated the expression of GDF15,a cachexia-associated target gene in DGC.Targeted silencing of the active enhancer of GDF15 using CRISPR/dCas9-KRAB plasmid technology resulted in a significant reduction in GDF15 expression at both mRNA levels(P<0.05)and protein.Results from orthotopic transplantation experiments of DGC demonstrated that silencing of active enhancers alleviated the cachexia phenotype in nude mice(P<0.05).Conclusion DGC exhibits enhancer remodeling,which regulates the expression of the cachexia-associated gene GDF15,and thereby contributes to the pathogenesis and progression of cancer cachexia.
10.Mechanism of lncRNA SNHG14 modulating alveolar epithelial cells damage infected with Streptococcus pneumoniae

Objective To investigate the impacts of long non-coding RNA SNHG14(lncRNA SNHG14)on proliferation,apoptosis,and inflammatory response of alveolar epithelial cells infected with Streptococcus pneumoniae by targeting the miR-17-5p/forkhead box K2(FOXK2)axis.Methods Alveolar epithelial cells A549 were divided into control group,infection group,sh-NC group,sh-SNHG14 group,sh-SNHG14+inhibi-tor NC group,and sh-SNHG14+miR-17-5p inhibitor group.The mRNA expressions of lncRNA SNHG14,miR-17-5p and FOXK2 were detected by RT-qPCR,the proliferation of A549 cells was detected by CCK-8 kit,and the apoptosis of A549 cells was detected by flow cytometry.The levels of IL-1β,TNF-α and IL-6 were de-tected by ELISA.Dual luciferase reporter gene assay was used to detect the binding of lncRNA SNHG14 to miR-17-5p,miR-17-5p and FOXK2.Results Compared with control group,lncRNA SNHG14,FOXK2 mR-NA expression,cell apoptosis rate,IL-6,TNF-α and IL-1β levels were increased in infection group,while miR-17-5p level and absorbance(A)value were decreased,with statistical significance(P<0.05).LncRNA SNHG14 and FOXK2 mRNA expression,cell apoptosis rate,IL-6,TNF-α and IL-1β levels in sh-SNHG14 group were lower than those in infection group and sh-NC group,while miR-17-5p expression level and A val-ue were higher than those in infection group and sh-NC group,and the differences were statistically significant(P<0.05).FOXK2 mRNA expression,apoptosis rate,IL-6,TNF-α and IL-1β levels of sh-SNHG14+miR-17-5p inhibitor group were all increased compared with sh-SNHG14+miR-17-5p inhibitor group,and the miR-17-5p expression and A value were decreased compared with sh-SNHG14+inhibitor NC group,the differences were statistically significant(P<0.05).Dual luciferase reporter gene test results showed that lncRNA SNHG14 and miR-17-5p,as well as miR-17-5p and FOXK2 had a targeting relationship.Conclusion Interfer-ing the expression of lncRNA SNHG14 can up-regulate the expression of miR-17-5p and down-regulate the expression of FOXK2,thus inhibiting the apoptosis and inflammatory response of alveolar epithelial cells in-fected with Streptococcus pneumoniae infected cells and promoting the cell proliferation.
