1.Association between core symptoms and biological markers in children with autism spectrum disorders

Objective:
To explore the relationship between serum homocysteine (Hcy), interleukin-5 (IL-5), folate and core symptoms in children with autism spectrum disorders, so as to provide potential biomarkers for early diagnosis and intervention of diseases.
Methods:
A total of 106 children with autism spectrum disorders and 106 healthy children with matched sex and age in Lu an People s Hospital were enrolled as autism group and healthy group between May 2020 to December 2023. On the day of admission, levels of serum Hcy, IL-5 and folate were detected. The core symptoms in autism group were evaluated by Autism Behavior Checklist (ABC), Wechsler Preschool and Primary Scale of Intelligence-fourth edition(WPPSI-IV), Childhood Autism Rating Scale (CARS) and Social Responsiveness Scale (SRS). The levels of serum Hcy, IL-5 and folate in the two groups were compared by t- test. The relationship between serum Hcy, IL-5, folate and core symptoms in children with autism spectrum disorders was determined by Pearson correlation analysis.
Results:
The levels of serum Hcy and IL-5 in autism group were (7.48±0.32) μmol/L and (345.77±32.51) pg/mL, higher than those in healthy group [(6.11±0.54) μmol/L, (274.04±25.17) pg/mL], while folate level was lower than that in healthy group [(15.24±3.47) ng/mL, (22.51±4.69) ng/mL], the differences were statistically significant ( t =22.47, 17.96, 12.83, all P < 0.05 ). In autism group, levels of serum Hcy and IL-5 were positively correlated with scores of ABC, CARS and SRS ( r =0.29, 0.53 , 0.54; 0.45, 0.41, 0.50), while negatively correlated with WPPSI-IV score ( r =-0.28, -0.26)(all P <0.05). The folate level was negatively correlated with scores of ABC, CARS and SRS ( r =-0.55, -0.40, -0.25), while positively correlated with WPPSI-IV score ( r =0.41) (all P <0.05).
Conclusion
Children with ASD show elevated serum Hcy and IL-5 alongside decreased folate,and three markers correlate with core symptoms and intellectual level.
2.Antibody levels of diphtheria and tetanus in healthy population in Pudong New Area, Shanghai, 2018-2024

Objective To understand the antibody levels of diphtheria and tetanus among healthy population in Shanghai Pudong New Area, and to provide a scientific basis for improving the vaccine immunization strategy. Methods Random sampling was used to select healthy people of all ages in 16 communities in Shanghai Pudong New Area from 2018 to 2024, and serum samples were collected and tested for serum anti-diphtheria and tetanus toxin IgG antibodies by enzyme-linked immunosorbent assay (ELISA) method to analyze the antibody positivity rate (≥0.1 IU/ml) and the geometric mean concentration (GMC) of antibodies. Results A total of 3 312 serum samples were included, with a male-to-female ratio of 0.76:1, and 53.77% were local residents. The seropositivity rates and geometric mean concentrations (GMC) of both diphtheria and tetanus antibodies generally declined with increasing age, but exhibited a transient rebound in the 7y-. A total of 1 175 individuals (35.48%) were seropositive for diphtheria, with a GMC of 0.054 IU/mL. For tetanus, 988 individuals (29.83%) were seropositive, with a GMC of 0.033 IU/mL. Significant differences in seropositivity rates (χ2diphtheria=950.005,χ2tetanus=1 324.393) and GMC (Hdiphtheria=1027.160,Htetanus=1 142.007) were observed among different age groups (P<0.001). Significant differences in seropositivity rates (χ2diphtheria=950.005,χ2tetanus=1324.393) and GMC (Hdiphtheria=1027.160,Htetanus=1142.007) were also found across different years (P<0.001). Conclusion The prevalence of diphtheria and tetanus antibodies in the healthy population of Pudong New Area is relatively low, particularly among adults over 20 years of age with inadequate immunization. This underscores the need to reinforce the National Immunization Program (NIP) vaccine specifications for children under 6 years of age and implement an immunization strategy for adolescents or adults against diphtheria and tetanus.
3.Measurement of Meso-Mechanical Properties of Rabbit Cortical Bone after Sciatic Neurectomy Based on Resonant Ultrasound Spectroscopy

Objective The effect of sciatic neurectomy(SN)on the meso-mechanical properties of cortical bone was explored by combining animal modeling and resonant ultrasound spectroscopy.Methods A total of five New Zealand White rabbits underwent unilateral SN,and cortical bone specimens were obtained from the tibias on the operated and normal sides at 4th week after SN;multiple elastic constants(C11,C12,C13,C33,and C44),engineering mechanical parameters,and anisotropy ratios of the bone specimens were acquired using irregular resonant ultrasound spectroscopy under assumptions of transverse anisotropy,and the paired t-test was used to assess the differences in mechanical properties of the cortical bone between the two sides.Results Compared with the normal side,the elastic constants in different directions(C11,C12,C13,and C33)of the cortical bone on the operated side showed a decreasing trend,ranging from 8.49%to 32.23%;the axial elastic modulus(E3)and Poisson's ratio(v31)were reduced by 5.85%and 24.07%,respectively,but there were no significant changes in the anisotropic properties.Conclusions The method of cortical bone disuse modeling through SN is feasible.This method can significantly change meso-mechanical properties of the cortical bone,and the elastic constants can more comprehensively reflect the changes in mechanical properties of the cortical bone.
4.Clinical efficacy of hiatal hernia repair combined with tunneled esophagogastric fundoplication and diaphragmatic dome suspension-fixation in the treatment of gastroesophageal reflux disease

Objective:To investigate the clinical efficacy of laparoscopic hiatal hernia repair with tunneled esophagogastric fundoplication and diaphragmatic dome suspension-fixation (HHR-TEF-DDSF) in the treatment of gastroesophageal reflux disease.Methods:The retrospective and descriptive study was conducted. The clinical data of 32 patients with gastroesophageal reflux disease who were admitted to Yifu Hospital Affiliated to Nanjing Medical University from October 2024 to June 2025 were collected. There were 20 males and 12 females, aged (68±7)years. All patients underwent laparoscopic HHR-TEF-DDSF. Observation indicators: (1) surgical and intraoperative conditions; (2) postoperative conditions; (3) follow-up. Measurement data with normal distribution were expre-ssed as Mean± SD, while measurement data with skewed distribution were expressed as M( Q1, Q3) or M(range). Count data were expressed as absolute numbers or percentages. Results:(1) Surgical and intraoperative conditions. All 32 patients successfully underwent laparoscopic HHR-TEF-DDSF. The operation time was (75±10)minutes, and volume of intraoperative blood loss was 50(50,100)mL. Among the 32 patients, there was no conversion to open surgery, no blood transfusion, no intra-operative complications such as unexpected massive hemorrhage or adjacent organ injury, no intra-operative adverse event or death. (2) Postoperative conditions. For the 32 patients, the time to post-operative first flatus was 1(1,2)days, the time to postoperative first defecation was 1(1,3)days, the time to postoperative first intake of liquid food was 1(1,3)days, the duration of postoperative drainage tube indwelling was 3(3,6)days, the postoperative hospital stay was 6(5,14)days, and the time to relief of postoperative dysphagia was 5(5,8)days. No obvious hiccup was observed in any patient in the short term after surgery. (3) Follow-up. All 32 patients were followed up for 7.5(range, 3.0-11.0)months. Among the 32 patients, 26 cases achieved subjective relief of overall postoperative digestive tract symptoms, and 32 cases achieved subjective relief of overall postoperative respiratory tract symptoms. The proton pump inhibitor (PPI) withdrawal rate was 84.4%(27/32), and the treatment satisfaction rate was 87.5%(28/32). The incidences of postoperative complications inclu-ding abdominal distension, dysphagia, diarrhea, and increased flatus were 21.9%(7/32), 18.8%(6/32), 6.3%(2/32), and 3.1%(1/32), respectively. Dysphagia was significantly relieved in all affected patients within 2 months after surgery, and no patient had persistent dysphagia by the end of the follow-up period. There was no death, symptom recurrence, or reoperation.Conclusion:HHR-TEF-DDSF is safe and feasible in the treatment of gastroesophageal reflux disease, with favorable short-term efficacy.
5.Establishment and Validation of a Prognostic Model for Hepatocellular Carcinoma Based on Macrophage-Ferroptosis-Related Genes

Objective:This study aimed to integrate RNA-seq data to identify key genes associated with macrophage-ferroptosis and develop a prognostic model for hepatocellular carcinoma(HCC).Methods:We retrieved a dataset from the GEO database containing transcriptome data and clinical information for 163 samples.Macrophage-related genes were identified using WGCNA and immune infiltration analyses.Ferroptosis-related genes from the FerrDbV2 database were intersected with differentially expressed genes to obtain significant macrophage-ferroptosis-related genes.Hub genes were screened via protein interaction network construction,and key genes were identified using four machine learning algorithms.These genes exhibited significant expression differences between cancer and normal tissues and were closely linked to patient prognosis.ROC curve and KM survival analyses were performed,and expression levels were validated at the transcriptome and proteome levels.Results:Four key genes RRM2,KIF20A,PCK2,and PDK4 were identified and evaluated.RRM2 and KIF20A demonstrated high importance in classification prediction models and reliable performance in ROC and KM analyses(AUC>0.9,P<0.05).These genes regulate cancer cell proliferation/survival and macrophage polarization/function,influencing the tumor microenvironment and HCC progression.Conclusion:RRM2 and KIF20A regulate cancer cell proliferation and survival,modulate macrophage polarization and function,and influence the immune response in the tumor microenvironment.They can serve as prognostic biomarkers and potential immunotherapy targets for hepatocellular carcinoma.
6.Analysis Method of Empagliflozin and Related Substances in Empagliflozin Bulk Drug and Tablets

Objective To establish a high-performance liquid chromatography method to detect empagliflozin and related substances in empagliflozin bulk drug and tablets,and to provide technical support for quality control and unified monitoring of empagliflozin bulk drug and its tablets.Methods A liquid chromatography development system with the full factorial design of experiments and the Box-Behnken model was used to screen and optimize the chromatographic parameters.Related substances were detected in empagliflozin API and empagliflozin tablets from different companies with the optimized chromatographic parameters.Results The optimized chromatographic parameters were obtained:Shim-pack GIST C18-AQ column(250 mm×4.6 mm,5 μm)was used,column temperature was 15 ℃,gradient elution with water-acetonitrile as mobile phase was as below,flow rate was 1.2 mL·min-1,detection wavelength was set at 224 nm.The specificity of the method is good,with recoveries ranging from 94.8%to 101.7%,and RSD ranging from 0.5%to 3.1%.The known single impurity in APIs and tablets is less than 0.05%,any other unknown single impurity is less than 0.06%,and the total amount of impurities are less than 0.3%.The related substances of supervised sampling are under good control.Conclusion The method is reliable and robust for determining related substances of empagliflozin and its tablets.

Guidelines for Digital Ancient Books of TCM Indexing(T/CIATCM 119-2024)is based on the theoretical knowledge,disciplinary methods,and practical applications of TCM classical cataloging.Taking digital ancient books of TCM as the object,it systematically reveals the content of TCM knowledge,which is an essential indexing processing standard for building an intelligent retrieval system for TCM ancient books,and can provide support for the deep development and innovative utilization of TCM knowledge.It can not only promote the co-construction and sharing of ancient book resources in the TCM industry,but also promote the standardization construction and application of TCM information.This standard specifies the principles,methods,and examples of free indexing of digital ancient books of TCM based on their original content.It is applicable to the indexing and processing of digital ancient books of TCM for TCM professional libraries and related institutions,and to the data processing and construction of various types of TCM ancient book databases.
8.Application of virtual reality technology in preoperative anxiety of patients with hand trauma

Objective:To explore the application effect of virtual reality technology in preoperative anxiety of patients with hand trauma, aiming to improve the negative psychological state of patients before surgery.Methods:A prospective non-randomized controlled trial was conducted. A convenience sampling method was used to select 116 patients with hand trauma who underwent surgical treatment at Wuxi Ninth People′s Hospital from January 2022 to December 2023 as the research subjects. They were divided into an intervention group and a control group according to the treatment time, with 58 cases in each group. The control group received routine nursing intervention, while the intervention group received nursing intervention based on virtual reality technology in addition to the routine care. The preoperative anxiety, uncertainty of illness and subjective well-being of the two groups were compared before and after the intervention.Results:Three cases dropped out in the control group and six in the intervention group, with 55 and 52 patients completing the study in the control and intervention groups, respectively. In the control group, there were 29 males and 26 females, with an age of (40.60 ± 2.09) years. In the intervention group, there were 32 males and 20 females, with an age of (41.27 ± 10.55) years. After the intervention, the scores of the Self-Rating Anxiety Scale and the Uncertainty in Illness Scale in the intervention group were 52.81 ± 5.47 and 62.87 ± 4.26, respectively, which were lower than those in the control group 58.20 ± 7.29 and 67.41 ± 4.15, with statistically significant differences ( t=-4.31, -5.60, both P<0.05). The Index Well-Being score of the intervention group was 7.17 ± 1.15, higher than that of the control group 6.09 ± 0.57, with a statistically significant difference ( t=6.19, P<0.05). Conclusions:Virtual reality technology can effectively alleviate preoperative anxiety, reduce the uncertainty of illness and enhance subjective well-being in patients with hand trauma, which is beneficial to the physical and mental health of patients.

Currently,there are practical and technical difficulties in the construction of clinical ques-tions in the development of clinical practice guidelines.Clinicians or guideline developers seldom construct clin-ical questions based the actual case scenario,leading to some information loss between structured and actual clinical connotation.To overcome this challenge,we proposed a case-guided questions construction approach,and carried out case research and verification in the formulation of the guideline.We found that this method could more efficiently and scientifically assist the formulation of clinical questions,and provide reference for clinicians or guideline developers.
