In recent years,significant breakthroughs have been achieved in the immunotherapy for Alzheimer's disease.In line with global advancements,two anti-amyloid-β monoclonal antibodies have been approved and successfully launched in China for clinical use.Lecanemab and Donanemab were officially used in June 2024 and April 2025 in China,respectively.In order to standardize the rational and safe application of anti-amyloid-β monoclonal antibodies for Alzheimer's disease in China,this article integrates recom-mendations from the clinical trials and real-world experience from the author's team and domestic peers to further update the recom-mendations for the clinical use of anti-amyloid-β monoclonal antibody based on the 2024 version.It includes indications for therapy,pre-treatment evaluation and preparation,administration protocols and safety measures during treatment,and post-treatment monitor-ing strategies.
2.Antiviral Efficacy and Mechanism of BD-77 Against Novel Coronavirus SARS-CoV-2

ObjectiveThe human angiotensin converting enzyme2 (hACE2) transgenic mouse model was used to clarify the antiviral efficacy of BD-77 against a novel coronavirus SARS-CoV-2 and explore the action mechanism of BD-77 against SARS-CoV-2. MethodSARS-CoV-2 Omicron and Delta variant strains-infected VeroE6 cell models were established and administered with BD-77 to observe the antiviral effect of BD-77 in vitro. A kit was used to detect the effect of BD-77 in vitro on the binding of spike S protein of SARS-CoV-2 virus (Delta/Omicron) to angiotensin converting enzyme2 (ACE2). Chromatography was adopted to detect the binding of BD-77 to the S protein and N protein of the novel coronavirus. hACE2 transgenic C57BL/6 mice were divided into a blank control group, SARS-CoV-2 infection group, BD-77 administration groups of 37.5 mg·kg-1 and 75 mg·kg-1, with eight mice in each group. The pneumonia model of SARS-CoV-2-infected hACE2 transgenic mice was built to observe the survival of the mice, detect the virus titer of the lung tissue of the mice, and observe the lesions in the lung tissue. ResultBD-77 had a certain inhibitory effect on Omicron and Delta variant strains in vitro, with median inhibitory concentration (IC50) of 526.3 mg·L-1 and 653.0 mg·L-1, respectively. BD-77 had no significant inhibitory effect on the binding of the S protein of WT, Omicron, and Delta variant strains of SARS-CoV-2 to ACE2 and had no binding effect with the S protein and N protein of the novel coronavirus. No mice in the blank group died, while the mortality rate of SARS-CoV-2-infected mice was 75%. There was a large amount of virus replication in the lung tissue of the mice and large areas of inflammatory infiltration in the lung tissue and interstitium. Compared with the model group, BD-77 administration groups of 37.5 mg·kg-1 and 75 mg·kg-1 could reduce the mortality of mice, significantly lower the virus titer in the lung tissue of mice (P<0.05), and improve lung lesions. ConclusionBD-77 demonstrated significant inhibitory effects against SARS-CoV-2 virus in vitro and in vivo. However, its mechanism of action did not involve direct inhibition of the virus itself or intervention in the virus-host binding process. This finding suggests that the mechanism of action of BD-77 needs to be thoroughly investigated and elucidated by further experiments.

In the process of dealing with epidemic situation, medical laboratories have played an important role in disease diagnosis, treatment, efficacy evaluation and prognosis judgment and so on. Beijing Youan Hospital, as the designated hospital of the coronavirus disease 2019 (COVID-19), has set up an emergency laboratory in the capital for the prevention and control of the COVID-19 by carrying out routine tests and virus nucleic acid tests, it provides timely and effective evidence for clinical diagnosis and treatment. To provide ideas and references for the building of the Emergency Laboratory in infectious hospitals. This article discuss how to set up an independent emergency laboratory efficiently, strengthen the cooperation with the Center for Disease Control and Prevention, make the best use of all resources, and share the enlightenment gained in the process of construction.

6.Clinic significance of fractional exhaled nitric oxide in childhood bronchial asthma

Objective To investigate the clinical significance of fraction exhaled nitric oxide (FeNO) in the diagnosis and monitoring of childhood bronchial asthma (asthma). Methods The FeNO levels of 204 children with acute asthma attack, 148 children with asthma clinical remission, 107 children with cough variant asthma (CVA) and 250 children with pneumonia from March 2016 to March 2017 were retrospectively analyzed and compared. Results The FeNO levels of acute asthma attack, asthma clinical remission, CVA and pneumonia were 18 (10, 37), 16 (10, 38), 18 (9, 31) and 13 (8, 20) nmol/L, and there was statistical difference (P<0.05). The FeNO levels of acute asthma attack, asthma clinical remission and CVA were significantly higher those of pneumonia, and there were statistical differences (P<0.05). There was no statistical difference in the FeNO levels among acute asthma attack, asthma clinical remission and CVA (P>0.05). Conclusions FeNO has clinical significance in the diagnosis of asthma in children, and its clinical significance in monitoring asthma and the prediction of acute attack needs further observations.
7.Expression and significance of neutrophil to lymphocyte ratio and platelet to lymphocyte ratio in patients with chronic obstructive pulmonary disease

Objective To discuss the expression and significance of neutrophil to lymphocyte ratio(NLR) and platelet to lymphocyte ratio(PLR) in peripheral blood in patients with chronic obstructive pulmonary disease(COPD).Methods One hundred and fifty cases COPD patients who were treated in Respiratory Departments of Hai'an People′s Hospital Affiliated to Nantong University from January to December 2015 were selected and divided into stable period group with 60 cases and acute exacerbation period group with 90 cases,and 50 healthy subjects were selected as control group.Clinical data were collected and white blood cell count(WBC),neutrophil count(NC),lymphocyte count(LC),platelet(PLT),NLR,PLR,forced expiratory volume in one second(FEV1%) predicted and C reactive protein(CRP) were tested.Results NLR,PLR and CRP values were significantly higher in patients in acute exacerbation period group than in stable period group and control group(NLR:(10.30±9.93) vs.(6.76±5.90) vs.(5.85±3.67);PLR:(335.88±164.94) vs.(286.80±118.98) vs.(194.11±88.14);CRP: (42.18±24.89) mg/L vs.(23.27±13.59) mg/L vs.(22.49±13.47) mg/L,F=6.637,6.808,11.007,P<0.05),but FEV1% predicted was no difference in stable period group and acute exacerbation period group((58.85±16.83)% vs.(57.47±17.76)%;t=0.228,P>0.05).There was a significant positive correlation between NLR,PLR and CRP in acute exacerbation period group(r=0.374,0.379,P<0.01),there was a positive correlation between NLR and PLR(r=0.482,P<0.01),there was a negative correlation between NLR,PLR and FEV1(r=-0.297,-0.299,P<0.05).There was a significant positive correlation between NLR,PLR and CRP in table period group(r=0.593,0.433,P<0.01),NLR and PLR were also positively correlated(r=0.618,P<0.01),NLR,PLR and FEV1 accounted for the expected value of the negative correlation(r=-0.324,-0.342,P<0.05).Conclusion Both NLR and PLR are inflammatory markers,furthermore they show a significant negative correlation to airflow limitation in patients with COPD.
8.The quantitative measurement of midkine in serum of patients with acute leukemia and its clinical significance

Objective To explore the meaning of midkine (MK) levels in serum in different development stage of acute leukemia,and to explore the relationship between MK and WT1.Methods The levels of the MK in serum of 86 cases of acute leukemia and 30 cases of normal people were detected by ELISA.Real-time quantitative PCR (RQ-PCR) method was used to determine the expression of WT1 at mRNA level in 15 AML patients.Results The MK level in serum in the new diagnosed group was higher than that in the complete remission group and the normal control group [7.52 (5.44,10.55) ng/ml vs 3.52 (1.56,5.20) ng/ml vs 2.44 (1.89,3.12) ng/ml].There' s no statistical difference between midkine level in new diagnosed acute B cell leukemia (B-ALL) group and acute myeloid leukemia (AML) group [7.88 (5.78,15.78) ng/ml vs 6.25 (4.59,16.33) ng/ml].The clear correlation was found between the level of serum MK and quantities of marrow WT1 gene (r =0.529,P =0.043).Conclusions The level of MK in serum of acute leukemia patients is increased at the time of new diagnosis and decreased at complete remission.ELISA may be a way to measure the status of AL.The location of MK gene is adjacent to WT1 gene and MK' s clinical significance is similar to WT1' s,furthermore,there is a clear correlation between MK in serum and WT1 of marrow in quantities.
9.Investigation on feasibility analysis of the training for specialist nurse in operating room under direction of the third-degree subject

Objective To identify the needs of different degree hospitals for specialist nurse in operating room, and to explore the specialist nurse training programs which can meet their actual requirements . Methods We used the self-designed questionnaire to carry out a survey in students which were graduated from the training courses for specialist nurses in operating room in Jiangsu province .Four points of the contents were as below:the general information , the refinement situation of the third-degree subject and subspecialty of one′s own hospital , how their jobs were going after the traing , and the feasibility and plan of the training for specialist nurses in operating room under the direction of third-degree subject .Results 100%of participants thought that it was necessary to hold a training for specialist nurses in operating room , 80.00%of them thought that the current mode belonged to the general nursing , 72.22% of whom from the second-class hospital thought that both the theoretical knowledge and skill learned from the clinical practice could meet their working requirements , and they would continue to take the current training mode .74.19%of whom from the tertiary hospital thought that although it could meet the requirements , it was not strong enough and was short in details for their own direction , they thought specialist nurses should be trained according to the third-degree subject .Conclusions The training mode is combined with centralization and decentralization , and it includes unified entrance , synchronous training , hierarchical culture .It is established based on the actual needs getting from the hospitals in different grades , so it is better to adapt for different needs of different hospitals .
