1.Advances in perioperative nutritional management for patients with esophageal cancer

Esophageal cancer is a prevalent malignant tumor of the digestive tract in China, and radical surgery remains the cornerstone of its comprehensive treatment. However, multifactorial challenges such as postoperative gastrointestinal tract reconstruction, traumatic stress, and tumor-related metabolic disturbances render esophageal cancer patients highly susceptible to malnutrition. Perioperative nutritional support therapy plays a crucial role in enhancing surgical safety, improving clinical outcomes, and elevating patients' quality of life by regulating metabolic homeostasis, preserving organ function, and optimizing the immune microenvironment. This article reviews the mechanisms underlying malnutrition in esophageal cancer, methods for nutritional status assessment, and precision intervention pathways based on multi-omics evaluations. The aim is to strengthen clinicians' awareness of standardized perioperative nutritional management for esophageal cancer patients and promote its clinical implementation, thereby facilitating postoperative recovery and improving long-term quality of life.
2.Research Advances in Traditional Chinese Medicine Regulation of Pyroptosis for Lung Cancer Prevention and Treatment

Lung cancer remains one of the leading causes of cancer-related morbidity and mortality worldwide, and its treatment continues to face major challenges such as therapeutic resistance and tumor recurrence. Pyroptosis, a newly characterized form of programmed cell death, induces tumor cell death through gasdermin-mediated membrane pore formation and is accompanied by the release of inflammatory mediators, thereby playing complex roles in lung cancer initiation, progression, and modulation of the tumor microenvironment. Active components and herbal formulas derived from traditional Chinese medicine can modulate pyroptosis-related signaling pathways through multi-target mechanisms, showing potential advantages in inducing lung cancer cell death, inhibiting proliferation and migration, and reversing chemoresistance. This review systematically summarizes relevant studies from domestic and international sources, focusing on the molecular mechanisms of pyroptosis, its roles in lung cancer development and tumor microenvironment remodeling, and the current research progress on traditional Chinese medicine-based interventions targeting pyroptosis, with the aim of providing references for the prevention and treatment of lung cancer using traditional Chinese medicine.
3.Genetic analysis of four individuals harboring a 16q22 fragile site.

OBJECTIVE:
To analyze four patients with a 16q22 fragile site with miscarriage or infertility by using cytogenetic methods.
METHODS:
Four patients presented at Northwest Women's and Children's Hospital between January 2022 and December 2024 were selected as the study subjects. Peripheral blood samples were collected from the patients and subjected to G-banded chromosomal karyotyping, among whom two were also subjected to copy number variation (CNV) sequencing. This study has been approved by the Ethics Committee of the Hospital (Ethics No. 2020-022).
RESULTS:
The chromosomal karyotypes of the patients were mos 46,XX,fra(16)(q22)[26]/47,XX,del(16)(q22),+chrb(16)(q22)[4]/46,XX,del(16)(q22)[3]/46,XX[91], mos 46,XY,fra(16)(q22)[21]/46,XY,del(16)(q22)[3]/46,XY[76], mos 46,XX,fra(16)(q22)[21]/ 46,XX,del(16)(q22)[4]/46,XX[75] and mos 46,XX,fra(16)(q22)[16]/46,XX,del(16)(q22)[7]/47,XX,del(16)(q22),+chrb(16)(q22)[6]/47,XX,fra(16)(q22),+chrb(16)(q22)[3]/46,XX[68], respectively. CNV sequencing of patients 2 and 4 revealed no deletion or duplication on chromosome 16.
CONCLUSION
Identification of the 16q22 fragile site has facilitated genetic counseling for these patients.

5.Treating Type 2 Diabetic Nephropathy by Down-regulating NOX4 to Inhibit the Oxidative Stress Pathway in Mesenchymal Stem Cells

Diabetic nephropathy(DN)is a serious complication of diabetes mellitus and a leading cause of end-stage renal diseases.In DN patients,key pathological mechanisms include proteinuria,glomerulo-sclerosis,and fibrosis,largely driven by poor glycemic control and oxidative stress caused by prolonged hyperglycemia.This stress damages renal podocytes and triggers inflammatory mesenchymal infiltration of renal tubular cells,exacerbating the progression of proteinuria and fibrosis.Human umbilical cord-de-rived mesenchymal stem cells(hUC-MSCs)offer promising potential for treating DN due to their strong anti-oxidative properties.In this study,we developed a DN mouse model and treated the mouse via tail vein injections of hUC-MSCs(1×106 cells/mouse).The results indicated that hUC-MSCs significantly lowered fasting blood glucose levels(22.5±3.0 vs 14.7±1.1,P<0.01)and improved glucose toler-ance,as shown by intraperitoneal glucose tolerance test(IPGTT)results(P<0.05).Additionally,the renal function improved in hUC-MSCs-treated mice,with marked reductions in oxidative stress markers,including blood urea nitrogen(BUN),urinary creatinine(Ucr),urinary protein(PRO),superoxide dismutase(SOD),and malondialdehyde(MDA)(P<0.05).Histological analyses through hematoxy-lin-eosin(H&E),Periodic Acid-Schiff(PAS),and Sirius red staining demonstrated alleviation of glo-merular mesangial hyperplasia,glomerular hypertrophy,and tubular inflammation.Furthermore,hUC-MSCs treatment downregulated the expression of oxidative stress-related proteins,such as NADPH oxi-dase 4(NOX4)and thioredoxin-interacting protein(TXNIP),and reduced reactive oxygen species(ROS)production(P<0.05).Meanwhile,human renal cortical proximal tubule epithelial cells(HK-2 cells)were selected for validation in vitro experiments using high glucose treatment followed by super-natants of hUC-MSCs(MSC-CM),and Western blotting showed that the expression of both NOX4 and TXNIP was inhibited(P<0.05)and ROS expression was reduced.In conclusion,hUC-MSC treatment effectively lowered blood glucose levels and improved renal function in DN mice,likely through the sup-pression of NOX4 expression and TXNIP-mediated oxidative stress.
6.Role of loneliness and physical activity in relationship between childhood emotional maltreatment and problematic social media use among college students

Objective:To explore the relationship between childhood emotional maltreatment and problematic social media use among college students,and to examine the role of loneliness and physical activity in their relation-ship.Methods:A total of 1 186 college students completed the Childhood Abuse Questionnaire(CTQ-SF)emotion-al maltreatment subscale,Bergen Social Media Addiction Scale(BSMAS),UCLA Loneliness Scale(ULS),and Physical Activity Rating Scale(PARS-3).Results:Childhood emotional maltreatment scores were positively corre-lated with the BSMAS scores(r=0.34,P<0.01).The loneliness partially mediated the relationship between child-hood emotional maltreatment and problematic social media use,with the mediating effect accounting for 30.75%of the total effect.Physical activity moderated the association between childhood emotional maltreatment and prob-lematic social media use(β=-0.10,P<0.01).Conclusion:Childhood emotional maltreatment is associated with problematic social media use among college students,mediated by loneliness and moderated by physical activity.
7.Mechanisms of sesamin on the prevention and treatment of fatty liv-er disease in hypertensive rats with dyslipidemia based on mRNA-seq

AIM:To investigate the preventive and therapeutic effects of sesamin(SES)on fatty liver disease in rats with hypertension combined with dyslipidemia,and to explore the potential mecha-nisms based on mRNA-seq.METHODS:Spontane-ously hypertensive rats(SHRs)were fed a high-fat,high-cholesterol diet to establish a rat model of hy-pertension combined with dyslipidemia,and then treated with SES for 16 weeks continuously.The ex-periment was divided into four groups:WKY,SHR,Model,and Model+SES(160 mg·kg-1·d-1).Blood pressure was measured using the tail-cuff method.Body weight was monitored,and body mass index was calculated.Liver morphology was detected by ultrasound,and liver thickness was measured.Liver wet weight was weighed,and liver index was calcu-lated.Liver volume was detected by the water dis-placement method.Serum triglycerides(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),alanine aminotransferase(ALT),aspartate amino-transferase(AST),and total bile acids(TBA)were de-tected by ELISA.Liver sequencing analysis was per-formed using mRNA-seq.Liver histomorphological changes were observed by HE staining.The degree of hepatic steatosis was observed by Oil Red O stain-ing,and the degree of hepatic fibrosis was observed by MASSON staining.The mRNA expression of Al-dh1a7,Nnmt,Irs2,Pltp,and Scd was detected by q-PCR.The protein expression of Scd,Nnmt,AMPK,p-AMPK,PPARα,and PPARγ was detected by Western blotting.RESULTS:After 16 weeks of continuous SES administration to rats with hypertension combined with dyslipidemia,blood pressure was significantly reduced(P<0.01),and body weight was decreased.Serum TG,TC,and LDL-C levels were decreased,while HDL-C levels were increased.Serum ALT and AST levels were decreased.Liver weight,organ in-dex,liver thickness,and liver volume were de-creased.The degree of hepatic steatosis and hepat-ic fibrosis was improved.A total of 545 differentially expressed mRNAs were identified in the livers of rats in each group,of which 278 were upregulated and 267 were downregulated.Among the 27 com-monly differentially expressed mRNAs,five mRNAs related to lipid metabolism were screened,namely Aldh1a7,Nnmt,Irs2,Pltp,and Scd.KEGG enrich-ment analysis showed that the enriched pathways were AMPK and PPAR.Further validation revealed that in the SES-treated group,the mRNA expression of Scd in the liver was decreased,while the mRNA expression of Nnmt was increased.The protein ex-pression of Scd was decreased,while the protein ex-pression of Nnmt,AMPK,p-AMPK,PPARα,and PPARγ was increased.CONCLUSION:SES has preven-tive and therapeutic effects on fatty liver disease in rats with hypertension combined with dyslipidemia,and its mechanism of action may be related to the reduction of Scd expression levels in the liver and the increase in the expression of Nnmt,AMPK,p-AMPK,PPARα,and PPARγ.
8.Application of Multimodal Analgesia in the Postoperative Period of Hypospadias Surgery

Objective:To explore the application of multimodal analgesia after hypospadias.Methods:Ninety children undergoing elective hypospadias,12~47 kg from April 2021 to December 2023,American Society of Anesthesiologists(ASA)grades Ⅰ~Ⅱ.Thirty patients were randomly assigned to each of the three groups that were used to create the random number table method.The three groups consisted of a control group,an experimental group 1,and an experimental group 2.The measures of postoperative pain level,changes in vital signs,complications,sleep quality,and satisfaction were compared in the three groups.Results:There were no significant differences in body quality,operation time and age among the three groups,P>0.05;Compared with the group A,There were significant differences in postoperative pain scores between groups B and C at T2,T3,T4,T5,T6,and T7 time points,P<0.05,B and C showed no significant,P>0.05;no difference in SpO2 in the three groups,respiratory circulation between group B and C was relatively stable,compared with group A;group A was significantly more than group B and C,P<0.05,no difference between group B and C;100%of the families of group B and C were better and better with the postoperative sleep quality,group A accounted for 30%and 70%.Conclusions:The application of multimodal analgesia after hypospadias can better maintain the vital signs of children,with significant analgesic effect and less complications,and the family members are satisfied with the sleep quality of children.
9.Influence of Preoperative Visit in Operating Room Based on"Internet Plus"on Perioperative Negative Emotion,Sleep Quality and Stress Response of Elderly Patients

Objective:To investigate the effects of preoperative visit in operating room based on"internet plus"on perioperative negative emotion,sleep quality and stress response of elderly patients.Methods:142 elderly patients who underwent surgery in the Yancheng No.1 People's Hospital from From February 2022 to February 2024 were selected,the patients were randomly divided into observation group(n=71,preoperative visit in operating room based on"internet plus")and control group(n=71,traditional preoper ative visit).The perioperative indicators(heart rate,systolic blood pressure,diastolic blood pressure),negative emotions[Self Rating Anxiety Scale(SAS),Self Rating Depression Scale(SDS)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)score],stress response indicators[C-reactive protein(CRP),cortisol(Cor),norepinephrine(NE),epinephrine(E)]were compared between the two groups.Results:Heart rate after intervention increased,systolic blood pressure,diastolic blood pressure,SAS,SDS scores,PSQI scores,CRP,Cor,NE and E decreased,and the change range of observation group was greater than that of control group(P<0.05).Conclusion:Preoperative visit in operating room based on"internet plus"can effectively reduce the negative emotions of elderly patients,improve the quality of sleep,and alleviate the stress reaction through early communication,rich information,convenient,efficient and personalized service.
10.Effects of key molecules in m6A methylation modification on the replication and proliferation of Japanese encephalitis virus

This study was aimed at investigating the effects of demethylase fat mass and obesity-associated protein(FTO)and methyltransferase methyltransferase like protein 3(METTL3),key molecules in N6-methyladenosine(m6A)modification,on the replication and proliferation of Japanese encephalitis virus(JEV).Recombinant lentiviruses were generated by packaging the FTO and green fluorescent protein into lentiviral vectors.Neuro2a cells,a mouse neuroblastoma cell line,were infected with the lentivirus,and stable FTO-expressing cell lines were obtained through puromycin selection.Successful overexpression of FTO was confirmed through fluorescence microscopy,real-time quantitative PCR,and western blot analysis.When Neuro2a cells overexpressing FTO were infected with JEV,the overexpression of FTO decreased JEV replication in the cells,and increased the expression of interferon(IFN)and related molecules.Additionally,treatment of JEV-infected Neuro2a cells with the METTL3-specific inhibitor STM2457 resulted in a dose-dependent decrease in JEV replication and viral protein expression.These findings suggested that lowering m6A methylation levels inhibits JEV replication,thus shedding light on the regulatory role of methylation modification in JEV replication.
