1.Exploring Factors Influencing Decisions to Withdraw Treatment in Iranian Patients With Metastatic Cancer: A Qualitative Content Analysis Study

Objectives:
The decision to withdraw treatment in patients with metastatic cancer is a complex and emotionally charged process influenced by multiple interrelated factors. Despite its significant clinical and ethical implications, this phenomenon remains insufficiently explored through qualitative inquiry. This study aimed to explore the factors influencing decisions to withdraw treatment among patients with metastatic cancer.
Methods:
A qualitative study using conventional content analysis was conducted. Fourteen patients with metastatic cancer were purposively selected. Data were collected through in-depth, semi-structured interviews and analyzed using the qualitative content analysis approach proposed by Graneheim and Lundman. Data organization was facilitated using MAXQDA software.
Results:
The analysis yielded 1 main theme (“Finding the meaning of life, and freedom from cycles of suffering”), 4 categories, and 15 subcategories. The overarching theme suggests that understanding life’s meaning enables patients to situate their illness within a broader existential purpose, thereby affirming life’s intrinsic value beyond mere physical survival. The 4 categories were social and family influences, physical health factors, psychological determinants, and divine and spiritual beliefs.
Conclusions
This study outlines a multilevel approach to improving end-of-life care. At the clinical level, holistic nursing assessments are needed to guide transitions to palliative care. At the institutional level, implementing structured protocols with multidisciplinary support and financial counselling is essential. At the national level, policies should ensure universal health coverage and promote cultural acceptance of dignified treatment withdrawal. These findings provide actionable insights for healthcare administrators and policymakers and offer a foundation for strategies aimed at reducing barriers and improving access to supportive care services.

Introduction:
Primary aldosteronism (PA), most commonly caused by aldosterone-producing adenomas (APAs), represents the leading
form of surgically curable secondary hypertension. While genomic studies have elucidated the mutational landscape
of APAs, the spatial organisation and functional role of immune populations across APAs, aldosterone-producing
micronodules (APMs), and adjacent adrenal cortex remain poorly defined at single-cell resolution.
Methodology:
Single-cell RNA sequencing (scRNA-seq) was integrated with spatial transcriptomics in APAs and paired adjacent adrenal
cortex, complemented by immunohistochemical (IHC) validation. Immune populations were spatially mapped using
canonical markers (CD14, CD68, CD163, HLA-DR, CD8A, and CD4) across defined adrenocortical regions.
Results:
The adrenal microenvironment in PA demonstrates structured immune organisation rather than passive infiltration.
CD14+ monocyte-lineage cells localize intraparenchymally within APAs (n = 10), intercalating between CYP11B2+
aldosterone-producing cells and forming a pattern distinct from perivascular immune niches. scRNA-seq further
identified a transcriptionally distinct CD14+ population within the zona reticularis (zR) that co-expresses steroidogenic
markers (CYB5A, SULT2A1, TSPAN12) while lacking canonical monocyte transcripts. IHC supported this observation,
demonstrating CD14 expression within adrenocortical zR parenchymal cells (n = 5). In parallel, CD4 and HLA-DRA
exhibited diffuse cytoplasmic staining within zR parenchymal cells in the absence of classical macrophage marker coexpression (CD68), suggesting non-canonical or context-dependent expression within steroidogenic compartments.
CD68+ and CD163+ macrophages were sparsely distributed across APA, APM and adjacent cortex, consistent with lowdensity tissue-resident populations, while CD8A+ cytotoxic lymphocytes were enriched in APAs and APMs with diffuse
parenchymal cytoplasmic staining of CD8A additionally observed within the zR.
Conclusion
These findings reveal a previously unrecognized spatially organised immune-steroidogenic interface within the adrenal
cortex. The presence of immune-associated transcriptional and protein signatures within zR cells suggests potential
functional plasticity of steroidogenic cells, possibly extending to antigen presentation-related pathways, warranting
further mechanistic investigation

4.Glycemic Control in Adolescents with Type 1 Diabetes Mellitus (T1DM): The Role of Health Literacy and Numeracy

Introduction:
Type I diabetes mellitus (T1DM) is the most common form of diabetes diagnosed in childhood. The Diabetes in Children
and Adolescents Registry (Di-CARE) in Malaysia reported that many of these patients are between 10 and 19 years old.
However, little is known about the association of their health literacy and numeracy with their glycemic control.
Methodology:
A cross-sectional study among T1DM patients aged 12 to 19 years old who attended Paediatric Endocrine Clinic at
Hospital Tunku Ampuan Besar Tuanku Aishah Rohani and Hospital Tunku Azizah was conducted from March 2025 to
March 2026. Participants answered 14 questions on the Diabetes Numeracy Test (DNT-14) and completed a 29-item health
literacy questionnaire (myMOHLAA-Q). Data analysis was performed using SPSS version 30. The relationship of health
literacy and numeracy with glucose control (HbA1c) was analyzed using Spearman’s rank order correlation.
Results:
A total of 43 adolescents with T1DM were recruited with a median age of 15.2 years (IQR: 13.2–17.2 years). There were
22 female (51.2%) and 21 male (48.8%) participants. The majority of the participants (n = 27, 62%) had moderate glycemic
control with a median HbA1c of 8.3% (IQR: 7.5–9.6%). A Spearman’s correlation revealed a negative relationship between
mean HbA1c and knowledge on blood glucose monitoring, rs (43) = −0.34, p <0.05. Additionally, positive relationships were
found of knowledge on blood glucose monitoring and knowledge on insulin with total DNT-14 score, rs (43) = 0.54, p <0.001
and rs (43) = 0.78, p <0.001, respectively. The small sample size may have limited the ability to detect other significant
relationships.
Conclusion
The majority of T1DM adolescents have moderate glycemic control. Active intervention in improving knowledge on blood
glucose monitoring and insulin helps to improve glycemic control, reiterating the importance of diabetes education in
the long-term management and glycemic control in adolescents with type 1 diabetes.

5.Paediatric NODAT Following Kidney Transplant: Clinical Characteristics of Four Cases at Hospital Tunku Azizah

Introduction:
New-onset diabetes after transplantation (NODAT) is a clinically significant complication associated with adverse
metabolic and graft outcomes and affects patients’ quality of life. Data in paediatric populations are limited. This case series
describes the incidence, clinical characteristics, and early outcomes of NODAT in a paediatric kidney transplant cohort.
Cases:
Thirty patients (aged 5–17 years; 16 males, 14 females; 14 Malay, 15 Chinese, 1 Indian) underwent renal transplantation
at Hospital Tunku Azizah between 2019 and 2025.
NODAT was identified using standard diagnostic criteria and occurred in four patients (13.3%). Among patients who
developed NODAT, three were Malay and one Chinese; all were aged 10–17 years, with a predominance of females (n = 3).
All were non-obese (body mass index [BMI]: 11–21.4 kg/m²), and only one had a family history of type 2 diabetes mellitus.
Underlying causes of ESRF in these patients included focal segmental glomerulosclerosis (FSGS, n = 2), CAKUT (n = 1),
and unknown etiology (n = 1). Two patients received kidneys from living donors, and two from cadaveric donors. Two
patients demonstrated reduced C-peptide levels, consistent with impaired insulin secretion. All patients received steroid
induction and maintenance therapy alongside tacrolimus-based immunosuppression, with one patient also receiving
everolimus. Two patients developed post-transplant CMV viremia, which has been associated with NODAT.
The onset of NODAT was early, occurring within 2 weeks post-transplant in three patients and at 1 month in one patient.
Insulin therapy achieved glycemic control in all cases. Two patients were able to discontinue insulin within 3–4 months,
suggesting recovery of endogenous glycemic regulation, while the remaining two required ongoing insulin therapy.
Conclusion
NODAT affects a substantial proportion of paediatric kidney transplant recipients and may present despite the absence
of conventional risk factors. Early surveillance is essential. The observed potential for insulin independence may indicate
a reversible component of β-cell dysfunction, which warrants further investigation in larger cohorts.

Adenotonsillectomy for Paediatric Sleep Disordered Breathing – Post Surgery Placement and Outcome
Adenotonsillectomy (AT) is the treatment of choice for children with sleep disordered breathing (SDB) caused by adenotonsillar hypertrophy. However, there is an increased risk of post operative complications among those with identified risk factors. Therefore, they are electively admitted to critical care wards after surgery. Unfortunately, due to limited resources and availability of beds in critical care wards, the cancellation rate for AT can be significant at some hospitals. It is becoming more difficult to satisfy the heightened demand for close monitoring post AT due to the increasing number of obese children and surge of SDB cases. Delaying surgery may indirectly worsen the child’s co-morbidities. Thus, it is vital to have a flowchart for post AT placement, which can be practical to many centres facing similar logistics concerns. This retrospective chart review done over 6 months included all paediatric patients who had AT for SDB at our centre. The objectives of this review were to identify factors contributing to elective admission to the paediatric intensive care unit (PICU)/paediatric high dependency unit (PHDU) following AT among patients with paediatric SDB, recognize association between obesity and PICU/PHDU admission as well as find the association between severity of allergic rhinitis with outcome post AT. We aim to have a flow chart for preoperative planning of post-surgery nursing placement among patients with SDB who are managed with AT at this centre. Obese patients without significant co-morbidities can be nursed in the general ward, the real admission rate to PHDU/PICU is lesser compared to that was planned pre-operatively and children with moderate- severe AR tend to have residual snoring post AT. A flow chart for preoperative planning of post-surgery nursing placement among patients with SDB can help to reduce unnecessary booking and admission to critical care wards, decreasing the waiting time for AT and backlog of cases at public hospitals.
7.Correlation between the maximum standard uptake value and mean Hounsfield unit on single-photon emission computed tomography-computed tomography to discriminate benign and metastatic lesions among patients with breast cancer

Methods:
In total, 185 lesions were identified on bone and SPECT-CT scans from 32 patients. Lesions were classified as metastatic (109 sclerotic lesions) and benign (76 lesions) morphologically on low-dose CT. Semiquantitative analysis using SUVmax and mean HU was performed on the lesions and compared. To discriminate benign and metastatic lesions, the correlation between SUVmax and mean HU was determined using the intraclass correlation coefficients.
Results:
The SUVmax was higher in metastatic lesions (20.66±14.36) but lower in benign lesions (10.18±12.79) (p<0.001). The mean HU was lower in metastatic lesions (166.62±202.02) but higher in benign lesions (517.65±192.8) (p<0.001). A weak negative correlation was found between the SUVmax and the mean HU for benign lesions, and a weak positive correlation was noted between the SUVmax and the mean HU on malignant lesions with no statistical significance (p=0.394 and 0.312, respectively). The cutoff values obtained were 10.8 for SUVmax (82.6% sensitivity and 84.2% specificity) and 240.86 for the mean HU (98.7% sensitivity and 88.1% specificity) in differentiating benign from malignant bone lesions.
Conclusions
Semiquantitative assessment using SUVmax and HU can complement qualitative analysis. Metastatic lesions had higher SUVmax but lower mean HU than benign lesions, whereas benign lesions demonstrated higher mean HU but lower SUVmax. A weak correlation was found between the SUVmax and the mean HU on malignant and benign lesions. Cutoff values of 10.8 for the SUVmax and 240.86 for the mean HU may differentiate bone metastases from benign lesions.

Malnutrition and poor quality of life (QoL) in older adults are becoming a public health concern. Therefore, this
review article summarised significant studies which investigated the assessment of malnutrition and QoL and its relation. A literature research on academic journals published from 2011 to 2021 was conducted by using electronic
databases, which were Google Scholar, PubMed, Science Direct and SCOPUS. A total of 21 studies with 26,569
participants were identified and included in this review. The prevalence of malnutrition ranged from 1.6% to 26.5%,
and poor QoL was significant amongst malnourished older adults. In conclusion, malnutrition is associated with poor
QoL amongst older adults. Future studies regarding factors that influence malnutrition and its relation towards the
QoL amongst older adults are needed as it will help in assisting the healthcare professions to combat these health
problems.
9.Oral Health Needs and Barriers to Care in Children with Learning Difficulties

Introduction: Children with learning difficulties (LD) have poorer oral health compared to those without LD due to
barriers in maintaining oral care. However, the scarcity of data for this population in Malaysia has left a huge gap in
understanding their problems and how to overcome those barriers. Therefore, this study was conducted to evaluate
the unmet dental needs and barriers to care perceived by the guardians of children with learning difficulties (CWLD)
attending the Special Education Integrated Programmes of a mainstream primary school, in comparison to children
without learning difficulties. Methods: This cross-sectional study surveyed the guardians of CWLD (case) and those
without LD (control), aged 6-12 years old. A total of 225 questionnaires were distributed to the guardians with a
response rate of 40.4% (N=91). Unmet dental needs and barriers in both groups were analysed using the chi-square
test. Barriers with significant Chi-square results were further tested with logistic regression to investigate possible
confounders. Results: Unmet dental needs of 23.1% of CWLD were found. Most of the guardians agreed that regular dental check-ups were the most needed dental treatment (27.1%) compared to other treatments. The child’s
behaviour and the unwillingness of the dentists to treat were among the significant barriers to dental care within
the CWLD group. Conclusion: Despite regular dental visits, guardians of CWLD perceived that their children had
the most unmet dental needs compared to other children without LD, with significant barriers in terms of accessing
professional dental services.
10.Association between Body Mass Index, Physical Activity and Quality of Life amongst Older People in Malaysia during COVID-19: A Cross-Sectional Analysis

Introduction: COVID-19 is a disease caused by a new strain of coronavirus spreading rapidly over the globe, and
the older population has had a larger impact as the fatality rate increases with age. This pandemic caused them to
reduce their physical activity (PA). COVID-19 also resulted in food supply disruption and led to unhealthy dietary
changes, which are usually associated with weight gain and affecting the body mass index (BMI). PA and BMI can
influence the quality of life (QoL) of the older people. Therefore, this study aimed to determine the association between PA, BMI, and QoL amongst the older people in Malaysia during COVID-19. Methods: A cross-sectional study
was conducted amongst community-dwelling older people from all states in Malaysia. An online survey consisting of
self-reported anthropometry, Global Physical Activity Questionnaire- Malay version (GPAQ-M) and Short Form-36
Health (SF-36) Survey were collected. Statistical analysis chi-square test was used to identify the association between
BMI, PA, and QoL. Results: This study included data from 180 individuals with a median age of 64.0 ± 9.8 years. The
results showed that participants aged 60-69 years were significantly associated with better QoL, meanwhile, those
who practice low PA levels were significantly associated with poor QoL. Conclusion: The findings revealed that age
and PA were associated with QoL. Thus, the healthcare profession must emphasise regular PA to accomplish a better
QoL amongst older people in the future.
