Background: This study established sex-specific reference intervals (RIs) for hematological parameters, including the monocyte distribution width (MDW), in Korean adults and assessed the need for separate RIs according to sex in older adults. Methods: Hematological parameters were measured using a DxH 900 hematology analyzer (Beckman Coulter, USA) on 328 peripheral blood samples from 124 men and 204 women aged 19–93 years. Results: The RIs for white blood cells, red blood cells (RBCs), Hb, Hct, mean corpuscular hemoglobin, mean corpuscular hemoglobin concentration, monocyte percentage, and neutrophil, eosinophil, lympho­cyte, and monocyte counts differed significantly by sex (P<0.05). Mean corpuscular volume was significantly positively associated with age, whereas RBC and platelet counts were significantly negatively correlated with age in both men and women. Age-based analysis revealed significant shifts in the RBC parameters beginning at 60 years of age in men and 70 years of age in women. The MDW did not show significant differences according to sex or age. Conclusions This study supports the use of sex-specific RIs and highlights the importance of considering age when interpreting results regarding hematological parameters in clinical practice.

The content of the interviews was analyzed using Elo and Kyngäs’s inductive content analysis method. Data collection and analysis were performed iteratively to enable a continuous comparative analysis. Results: : The main category derived from the analysis was the “coexistence of partial consciousness and discomfort.” Three general categories were identified: (1) the state of being neither asleep nor awake, (2) persistent discomfort, and (3) ambivalence toward consciousness. The subcategories included a fragmented memory, disorientation, surreal experiences, brain fog, half-heard noises, physical discomfort, mental distress, preference for sleep, and preference for being awake. Conclusion : Patients in the ICU under light sedation experienced an incomplete state of consciousness while experiencing various types of discomfort. Nurses should understand the patients’ subjective experiences with light sedation to provide patient-centered care. Based on these findings, further research is required to develop and apply nursing interventions for patients under light sedation.

Objective:To investigate the awareness of Chinese gastroenterologists regarding the treatment targets of inflammatory bowel disease (IBD) as defined by the STRIDE-Ⅱ consensus and to identify challenges encountered in clinical practice.Methods:A nationwide cross-sectional survey was conducted via an online questionnaire distributed to gastroenterologists between February 16 and March 31, 2024. The survey encompassed demographic data, recognition and adherence to the STRIDE-Ⅱ consensus, perceived importance of IBD treatment targets, and clinical challenges.Results:A total of 203 valid questionnaires were collected from 28 provinces (autonomous regions/municipalities). The majority of participants (177/203, 87.2%) reported routinely referencing the STRIDE-Ⅱ consensus in IBD management. Over 90% of physicians identified endoscopic mucosal healing as the core treatment target. Additionally, approximately 80% emphasized the importance of clinical remission, endoscopic remission, quality-of-life improvement, disability prevention, and normalization of growth in pediatric patients. The challenges reported by more than half of physicians in acheiving the goals of the treatment process included lack of a standardized definition for mucosal healing (69.5%), management of psychological comorbidities (89.2%), drug failure (74.4%), penetrating Crohn's disease (72.4%), perianal fistulizing Crohn's disease (65.0%), and IBD management during pregnancy (61.1%).Conclusions:Chinese gastroenterologists demonstrate high awareness and adherence to the STRIDE-Ⅱ consensus, prioritizing mucosal healing as the cornerstone of IBD management. However, numerous challenges remain in clinical practice, particularly regarding operational definitions of therapeutic endpoints and complex disease phenotypes.

Objective To investigate the molecular mechanism of aprepitant(Apr)reversing 5-Fluorouracil(5-FU)resistance in colorectal cancer(CRC)mouse model through endoplasmic re-ticulum stress(ERS).Methods Thirty mice were selected as experimental animals.Five mice were randomly assigned to control group,and the remaining 25 mice underwent subcutaneous injection in the back to establish the HCT-116/5-FU CRC mouse model.These mice were then divided into the CRC group,5-FU group,Apr group,Apr+5-FU group and Apr+ERS inhibitor Tauroursodeoxychol-ic acid(TUDCA)group,with five mice in each group.Changes in body weight and tumorigenesis in mice were recorded,and their organ indicators were calculated.Western blotting(WB)was used to detect the protein expression levels of protein kinase R-like ER kinase(PERK),eukaryotic initiation factor 2 subunit α(eIF2α),activating transcription factor 4(ATF4)and C/EBP homologous protein(CHOP)in each group.Results At 5,10,15 and 20 d after medication,there were no statistically significant differences in body weight among CRC,5-FU,Apr,Apr+5-FU and Apr+TUDCA groups,neither in time points nor in interactions(P＞0.05).Two days after the last administration,there was no significant difference in the indexes of thymus,lung,liver,spleen,heart,kidney and other organs among all groups(P＞0.05).Compared with CRC group,the protein expression levels of PERK,P-EIF2α/eIF2α,ATF4 and CHOP in Apr group and Apr+5-FU group were significantly increased,the number of tumors was significantly decreased,the tumor mass was significantly de-creased,and the tumor volume was significantly decreased(P＜0.05),and the improvement of Apr+5-FU group was better than that of other groups(P＜0.05).Conclusion Apr can enhance chemotherapy sensitivity and reverse chemotherapy resistance in CRC mice,which may be mediated by downstream molecules of ERS pathway.

Objective To explore the influencing factors on foot function recovery after distraction device-assisted repositioning of small incisions treatment for Sanders Ⅱ and Ⅲ calcaneal fractures,and to provide guidance for timely clinical interventions.Methods A retrospective analysis was conducted on 80 patients with Sanders type Ⅱ and Ⅲ calcaneal fractures admitted to the People's Hospital of Bozhou from January 2021 to June 2023.Data were collected using a general information questionnaire,the hospital's electronic medical record system,the visual analog scale(VAS),the Marland foot function scale(MFS),the perceived social support scale(PSSS),and the perceived stress scale(PSS).The influencing factors of postoperative foot function recovery were analyzed using univariate factor analysis,multivariate linear regression,and structural equation modeling.Results The MFS scores of 80 patients at 6 months post-operation were significantly higher than preoperative scores(P＜0.001).The rates of excellent,good,fair,and poor foot function at 6 months post-operation were 56.3％,31.3％,10.0％,and 2.5％,respectively.Univariate analysis revealed significant differences in MFS scores based on different ages,time to start full weight-bearing exercise,bone mineral density,surgical timing,body mass index(BMI),postoperative VAS score,PSSS score,PSS score,and the presence of postoperative peroneal muscle spasm and peroneal muscle tendon adhesion(P＜0.05).Multivariate linear regression identified age,time to start full weight-bearing exercise,surgical timing,BMI,postoperative fibular muscle spasm,postoperative fibular muscle tendon adhesion,postoperative VAS score,PSSS score,and PSS score as significant factors affecting foot function recovery(P＜0.05).Structural equation modeling indicated that age,time to start full weight-bearing exercise,bone density,operative timing,BMI,postoperative peroneal muscle spasm,postoperative peroneal tendon adhesion,and pain severity directly influenced postoperative foot functional recovery.Additionally,psychological stress not only directly affected postoperative foot functional recovery but also indirectly influenced it through the mediating effect of social support.Conclusions Foot function recovery after surgery for Sanders type Ⅱ and Ⅲ calcaneal fractures is influenced by multiple factors.Psychological stress not only directly affects the recovery of foot function after surgery,but also mitigates the harm to foot function through social support.
